Employee was driving an explosive truck and struck left elbow on the ROPS.

Pulling off an old swellex bolt and as employee pulled the bolt took a step back to get balance and slipped and fell backwards.

The employee was struck above the left eye by a wrench while trying to get a stuck steel out of the hole.

While drilling up holes with jackleg cuttings got under glasses and enter eye. The Doctor could not find anything in eye, but prescribed an antibiotic.

Installing cylinder in Bolter when it jammed and a piece fly off hitting him in the wrist.

Employee was riding out of the mine on a lube truck when he felt some discomfort in his abdomen. He did have a pre-existing abdominal hernia, however it was determined by three different physicians he had an abdominal muscle strain which was the issue, not the hernia. The determination of the muscle strain was determined on 11/3/2010.

Employee was exiting his loader, when he went to open the door his hand slipped off of the door allowing it to close on his right knee.

While placing boards in slot on back of drill rod trailer, board got stuck, employee applied more force. When board freed, employees hand contacted drill rod end and resulted in laceration.

Was taking a plate off bottom of haul truck Plate Fell hit his hand on ground cutting finger.

While working on loader in engine compartment he slipped and contacted operating fan blade with hand.

While pulling sample tube, tube came to top of rods with overshot intact. Driller stopped pulling tube as soon as ball cage cleared end of rods. Helper grabbed tube with both hands and then driller removed overshot from tube assembly setting it to the side. Helper lifted tube. He attempted to stop tube from slipping down rods, causing his finger to become pinched.

Employee was underneath a haul truck changing a hydraulic hose, when the wrench slipped down and hit employee in upper lip and broke a tooth.

Employee struck on back of hand right by 4"" x 5"" x 1/2"" sharp rock causing a nasty laceration.

Employee states while attempting to slam tube down barrel his glove became entrapped between tube nut and barrel. Partial amputation and fracture right index finger.

Crew were pulling 5' tube. Employee grabbed the tube with left hand and released the overshot with right hand. When overshot release employee lost balance. Regained balance and pulled teh tube out of drill string. Later complained of pain in lower back.

Struck by falling object

EE was installing resin bolts in back, pushing resin bolt up into hole with jack leg, spinner came off of nut. 10' resin bolt fell out of back, struck EE on right wrist. It hurt, it hurt bad.

Struck by rolling or sliding object

Employee was lifting timbers under the drill mast while the driller was positioning the mast to set up a 45 degree drill. A deck timber slid when the mast was being moved.

Caught in, under or between a moving and a stationary object

Disassembling axle hub for brake repair, putting a 5 1/4"" socket on impact wrench, he accidentally touched trigger. Socket came off. His finger came between axle hub's sharp edge and the heavy socket.

LID TO TOOL BOX FELL ON EMPLOYEE'S LEFT THUMB CAUSING LACERATION REQUIRING STITCHES TO CLOSE.

Ee was tripping rods on the LM75 core drill at Chukar, when adding a 5 foot drill rod. While adding the rod ee ran his right hand over the box end of the rod cutting the heel of his right hand.

EE OPERATING 26 TON HAUL TRUCK T-31 IN THE 251 HEADING. EE'S LEFT EYE STARTED TO HURT . EE FELT NOTHING STRIKE HIS EYE. EMERGENCY ROOM DR FOUND A SMALL FLECK OF METAL IN HIS LEFT EYE RESULTING IN AN ABRASION OF THE CORNEA. EE REPORTS HE WASWEARING HIS SAFETY GLASSES.

EE OPERATING LOADER L-21, LEANING FORWARD TO SEE AROUND CORNER HE WAS TURNING INTO. RAN LOADER BUCKET INTO RIB CAUSING EE TO STRIKE CHIN ON METAL OPERATORS COMPARTMENT. EE CHIPPED ONE FRONT TO OTH AND CUT CHIN BELOW LIP REQUIRING ON STITCH.

WHILE ASSISTING IN THE SPRAYING OF SHOTCRETE THEDISCHARGE LINE BECAME PLUGGED. WHILE RECLAMPING DISCHARGE LINE UPON REMOVAL OF BLOCKAGE THE MECHANIC PINCHED HIS HAND ON THE CLAMP. THE RESULTIN G LACERATION REQUIRED 3 SUTURES.
