Cutting band on bolts when the end of the band flipped and hit his forearm, cutting him.
Hooking coiled air hose to connection when it when it came apart and hit him in the mouth and broke tooth.

The drill crew were in the process of securing the drill to the face. As they were moving the foot plate to the face plate, driller placed his hand in the pinch point between the face plate and dui-dag. Right Middle and ring finger pinched. Stitches to tip of right middle finger and amputation of ring finger tip (distal).
While replacing the seat from a piece of equipment, employee pinched his finger between the seat and the peice of equipment.
Employee had bumped his elbow earlier in the shift and did not notice any inflammation. Near the end of his shift the employee noticed a large contusion on his elbow.
A ground fall occurred in the 4020 - 3950 escape raise estimated 30' long by 20' wide by 15' high at 750 tons. No persons were invovled in the fall. The ground fall would not allow escape raise to be used. A temporary secondary escapeway was implimented.

While lifting his tool box into tractor, employee heard aloud pop in his back as he slid the tool box in the tractor. Employee was treated at medical facility and given a resticted duty release.
While lifting a flammable cabinet, employee lost grip with left hand, causing a strain to right elbow. Employee sought medical attention and was placed on restricted duty as of 9/2/2008.
Employee walked into a piece of steel protruding from the buggy. He hit his chin and chipped his 2 front teeth.
While pushing a pump with a two wheel hand truck, employee slipped and fell, injuring his right side of lower back and hip. He has been taken off work at this time.
Employee smashed finger while he was pulling the door of his mucker open. His finger was smashed between the door and the frame of the mucker. He received 5 sutures.
While operating a jackleg drill installing bolts in the left rib, employee picked up drill and spun to the left when he felt his knee pop. He sought medical attention and was placed on restricted duty.
While carrying some parts, employee tripped over a rock that was buried in mud and he fell on his right shoulder and right knee. He is currently off work.

Employee slipped and fell into the rib, injuring his left shoulder and lower back. He was seen by a physican and placed on restricted duty on 12/25/2007.
Employee was struck on the hard hat and knocked to the ground by a rock which peeled from the face while loading a round in the 4080. Employee was released full duty, but then had a follow up with a Chiroprator.
Employee was stepping down from his truck when he slipped on the step twisting his right knee. He sought medical attention and was released full duty with a knee brace.
While changing foam filled tire on UG Jumbo, injured was attempting to push bottom of tire in to align lugs for installation, back began to tighten and radiate pain down leg.
While stepping down from bolter, employee stepped on a rock causing his left ankle to twist. Employee is currently on restricted duty.
While preparing to bolt, employee leaned a wire panel on the rib and as he turned to get another panel the panel leaning on the rib fell striking his face causing a laceration which required sutures.
EMployee was picking up a barrel for pump when he felt a strain on his left shoulder. Employee was originally given full duty release, but shoulder has not gotten any better. He had surgery on 1/14/2008 and is currently off work.
Employee was looking up for phone cable when a drop of liquid fell into his right eye.
Employee was washing off his boots with a high pressure washer and accidently washed above the boot line causing a second degree burn on the inside calf of his left leg. He did not seek medical attention until 06/6/07. He was treated for infection of the burn.
Employee had completed drilling out his round when he got off the jumbo to clean out muck from his lifters. While in the process of doing this a rock fell out of the face and struck him on the top of his right foot. The x-ray was negative for a fracture but he had contusions that placed him on resticted duty.
Employee was getting out of his haul truck at the UG batch plant when he stepped on a rock and rolled his right ankle. This resulted in a sprain. He originally went to medical attention on 4/2/07 but then returned to the Dr. on 4/4/07 and was placed on restricted duty.
Employee was stepping up into his underground haul truck when his foot slipped off of the step. This resulted in a muscle strain of the upper left leg/hip. He was taken off work on 03/23/07
Employee was checking a hydraulic fitting on a jumbo when the electrical plug shorted out and made a ""pop"". This startled him and when he rose up he hit his head on the boom of the machine. The impact resulted in a neck strain with 2 days restricted duty.
The employee just finish filling the hydraulic tank on a 6 yard loader to take to the shop for a hose repair, the driver of the loader started the loader up and drove backward not knowing the mechanic foot was between the bucket and the rib. the loader had been articulated inward toward the rib. the mechanic foot was bruised.
The employee slipped in the mud while walking toward a piece of mobile equipment, the location was 4150 E. The employee had a follow up doctors visit on 1/17/2007, at that time per part 50 reporting this became a medical reportable.

The employee was attempting to unplug a shot crete discharge hose, the employee had pressured up the hose doing so, the hose had flexed upward striking him in the forehead and nose. There was NO hose connection in this to warrant a whip check or restraint.
While loading a bolt into the carousel of the bolter, the bolt slipped and the plate hit the right forearm. Laceration with 7 sutures.
While entering a loader the employee became caught between cab and door of 3 1/2 yard loader. He was taken to medical attension and given a full duty release on the day of the event. On 10/16/06 employee requested to return to the doctor where he was placed on light duty pending results of a MRI.
While performing a PM on a locked-out de-energized mixer, employee was hand adjusting the paddles and got caught between the pulley and belt. 10 sutures and a fracture to the right distal bone of the pinky.
While moving a bucket on a pallet of backhoe teeth, the metal banding snapped back and lacerated the right hand. 5 Sutures. Full-Duty release. MSHA Reportable.
Groundfall at the 4080 level. MSHA contacted and inspected.
Employee tripped over floor drain causing him to lose balance, he fell froward striking his shoulder on a bench. Initial diagnosis shoulder strain/contusion with full duty release. Re-evaluation showed a rotator cuff tear requiring surgery to repair. Surgery performed on 10/10/06.
Groundfall at the 4080 level. MSHA contacted and inspected.
Employee was getting off of a piece of equipment and simply twisted his left knee and felt it pop. The incident occurred on 8/24/06 but he did not seek medical attention until 8/29/06. At this time he was diagnosed with a tendon strain and was placed on light duty.
While in the process of bolting the 4020 west ""E"" R-8 heading a large boulder came out of the back. The boulder went beyond the anchor points of the support bolts. It was estimated to weigh between 8 and 10 tons. There were no injuries and minimal damage to the bolter.
Employee was fabricating int he u/g shop while grinding he got a flake of metal embedded in his right eye. He was taken to NENRH where it was removed. A full duty release was given.
At approximently 3:30 pm on June 30th supervisor was on the 4020 level west sump area. He noticed a mist coming from the acid wash totes in the back of the drift. When he entered the wash bay he noticed a mild throat and eye irritation and became nauseous. He was examined by a Dr. and given a full duty release.
Employee was backing up his loader when it hit the rib of the drift. When the loader came to a stop he struck his face on the instrument panel. He has four broken teeth and a laceration to his upper lip requiring 5 sutures.
Walking down stairs and missed the second to last stair.
At approximently 7:00 am on May 7, 2006 a Dynatec employee noticed a ground fall on the 4835 level of the Deep Star area of the Deep Post Underground mine. It was located in the B-101 heading. This area began working a couple of days earlier but at this time it was still in an inactive area. After it was secured the roof fall continued out into the active work area.
Employee was holding the lid of a powder box open on a mobile powder truck. Another employee in the manbasket swung the basket to the side pinching the injured employees left hand between the boom and the door of the powder box. There was 14 sutures placed in the laceration and he was released full duty.
Approximately 80' past the Deep Star cross cut wash bay, in the direction of the Deep Star Mine the left rib (east rib) springline area sloughed in. The ground fall was approximately 20' long and 18' high. Utilities were dropped to the sill with minor damage. The ground fall resulted in blockage of the Deep Post secondary escapeway, loss of ventilation, and loss of water.
Employee was in the process of installing ground support. He was putting a 4' X 8"" screen on the bolter when he accidently knocked some loose ground down with the screen. A sharp rock struck him on the right forearm causing a laceration which required 5 sutures to close.
Employee was walking in a tight area between a mantrip vehicle and the rib of the drift. There was a sharp edge on the fuel tank. As he was walking by he ran his left forearm over the burr which caused a laceration. The laceration requied 21 sutures and he was released full duty for his next shift.
Employee was in the process of helping another employee install a rock bolt when a rock fell out of the face and struck him on the right foot. The impact resulted in fractures of all five toes on his right foot.
Employee was pulling on a bull hose that was stuck in the mud. The hose suddenly released reulting in a lower back strain. The employee was on restricted duty for 4 days. His first Dr. visit was 03/06/06 and was released for full duty on 03/12/06
Employee was in the process of cleaning out the cab of his UG haul truck. He went to throw garbage away and lacerated his left index finger on a sharp edge of the garbage can lid. He was transported to the hospital and has four (4) sutures placed in his finger.

Employee was working on the bed of a underground haul truck and ran his hand down the sharp side rail of the bed resulting in a laceration of the palm of his right hand. The laceration required 6 sutures. He was released full duty.
There was a ground fall at the Deep Post mine, Deep Star side of the mine on the 4570 level. This ground fall impeded passage to the work area. MSHA mine inspector was notified and went to the area.
Employee started the tractor from the side it jumped forward pinning him between the stairwell and the tractor.
Employee states while walking from drill deck to control panel his knee gave out.
While performing regular maintenance work the employee was swinging a hammer. His hand slipped and he struck his left index finger with the hammer. X-rays showed a fracture in the tip of the finger.
Employee was stepping down off of the bus that was delivering him from town. He stepped on some uneven ground spraining his right ankle. At this time he was not a medical reportable. on July 15th he went for a follow up and was placed in a walking boot and placed on light duty.
Employee was loading a face round when the rib/back fell in. The rock struck him on the head/hard hat. He was taken to the hospital the next day and the Dr. placed him in a neck collar for precautionary reasons.
Employee lost footing off explosives truck and twisted his ankle. (Fell off truck while loading a round.)
At approximatly 12:30 pm a ground fall was discovered at the Deep Post mine in the Deep Star Decline. It was located 75 feet above the second set of steel sets going up the ramp. The demensions were 9 high 12 wide 14 feet long. This was reported to MSHA. Rehab of the area has begun per MSHA approval.
While running a jammer employee hit some backfill and some dust got in eyes. He went to the ER and got a pescription eye medication and two restricted days which made this medical reportable.
EE was driving an underground truck and due to EE inattention, tipped the truck on its side resulting in a right shoulder injury. The emergency room Dr gave EE a restricted work release that resulted in this medical reportable accident.
Employee was walking by a rib and a rock fell out of the rib and struck his right shoulder. He went to the Emergency room on 3-22-05 and got a full duty release. His follow up visit with a orthopedic physician on 3-25-05 where he received a restricted duty release. That made this accident a medical reportable.
This employee had a gang box lid fall and it caused a laceration. on 3/4/05. The Employee did not report this to the company, and went on days off. When he returned to work we were told by him that he went and got stitches. This made this accident reportable. We were not notifed for 4 days and there was a break down in reporting that is the reason for this being subbmitted late
Employee twisted his left ankle on1-15-05 and on 1-16-05 he was seen at the ER and had a tore his acchellies tendon in his left ankle. This resulted in surgery and restricted work. As of 1-17-05 this was reportable.

Employee was working on a belly pan with a pry bar and overstressed his right shoulder. He was given a full duty release to work. On going pain caused him to see another doctor. and On april 22, 2005 he was seen by a Doctor who gave him restricted duty release and made this accident a medical reportable as of 4-22-05
This employee stepped on a rock while getting out of loader and twisted his left ankle. It resulted in a small tear in his ligament in his ankle. This resulted in one restricted work day.
Employee was bolting and a rock fell from the back and struck his hard hat. Staples were used to close the wound.
The machined edge of the oil seal retaining ring fell from a temporary 2"" x 4"" wood support while operator was replacing oil seal. Operator's right forearm was cut and required 12 stitches.
Employee was taking apart a 2"" water line and the pipe hit him in the mouth and lacerated his lip and broke two of his teeth. Making this medical reportable.
EMPLOYEE SPRAINED HIS KNEE ON 6-26-04 AND ON 8-31-04 HE HAD TO HAVE ORTHSCOPIC KNEE SURGERY TO REPAIR THE INJURY SUSTAINED ON 6-26-04.
Employee was using a scaling bar and it slipped and hit him in the mouth and he fell to the ground. This resulted in stitches to the mouth and hand.
Employee was going under a loader and hit his head on the loader bucket causing a strained neck. Restricted work days were the result.
Employee was mucking and his elbow got bumped on the mucker door. He was taken to ER and they made an attempt to drain the wound, which made this reportable
EE HAD A SLING BREAK AND THE HOOK HIT HIM IN THE MOUTH CAUSING A LACERATION TO HIS UPPER LIP. IT REQUIRED SUTURES.
EE WAS WORKING ON A TRUCK AND LOST HIS BALANCE AND FELL. HIS KNEES WERE INJURED; INJURY RESULTED IN LOST WORK DAYS.
EMPLOYEE STEPPED OFF AKUBOTA AND TWISTED HIS RIGHT ANKLE. THIS RESULTED IN TWO RESTRICTED WORK DAYS.
EE WAS PULLING ON A 4"" BULL HOSE AND STRAINED A MUSCLE IN HIS LWOER BACK RESULTING 4 RESTRICTED WORK SHIFTS.
EE SLIPPED AND TWISTED HIS RIGHT KNEE ON 1/15/04 AND ON 2/19/04 IT BECAME REPORTABLE AS HE HAD TO HAVE ORTHOSCOPIC SURGERY TO REPAIR THE DAMAGE.
EE WAS GETTING OFF THE CREW BUS AND SLIPPED AND FELL ON HIS RIGHT SHOULDER. THIS RESULTED IN TWORESTRICTED WORK SHIFTS.
A SUBSIDENCE OCCURRED AT THE 4020 DRIFT A SMALL HOLE OPENED UP IN THE FLOOR. THE HOLE WAS THREE FEET WIDE, FIVE FEET LONG AND FIVE FEET DEEP. THE AREA WAS FLAGGED OFF AND THE APPROPRIATE AGENC IES WERE NOTIFIED AND WE WERE TOLD IT WAS OK TO FILL IT WITH MUCK AND MOVE ON. NO OTHER PROBLEMS WERE ENCOUNTERED.

EMPLOYEE WAS GETTING OFF A DRILL AND HE SLIPPED AND GRABBED THE CHAIN LINK WIRE ON THE RIB AND A WIRE WENT THROUGH HIS THUMB. IT REQUIRED REMOVAL OF THE WIRE & STITCHES.
EMPLOYEE GOT HIS FINGER PINCHED IN A JACKLEG CHUCK TENDER AND REQUIRED STITCHES.
ON 10-28-03 AT APPROX. 3:30 PM A GROUND FALL OCCURED AT THE 415 WB-10 GOING ABOVE GROUND SUPPORT. WE WERE BACKFILLING IN THE HEADING AT THE TIME. APPROX. 50 TONS FELL, AND AT THE BACK OF THE F ALL SOME OF THE BOLTS WERE STILL ANCHORED.
EMPLOYEE WAS LIFTING ROLLS OF WIRE AND FELT A PAIN IN HIS LOWER BACK HE SOUGHT MEDICAL ATTENTION AND IT CAUSED LOST WORK DAYS.

ON 3/29/03 EE SLIPPED & TWISTED HIS LEFT KNEE. ON 6/19/03 HE HAD KNEE SURGERY TO FIX HIS LEFT KNEE WHICH WILL RESULT IN LOST WORK DAYS.
THE MINER RECEIVED A COMPRESSION FRACTURE IN HIS BACK & A BROKEN FOOT WHEN THE BOLTER HE WAS OPERATING WAS STRUCK BY A PIECE OF CEMENTED BACKFILL.
EMPLOYEE WAS WALKING TO THE PERSONNEL CARRIER AND HE TRIPPED AND BROKE HIS ARM.
WHILE GETTING CORE OUT OF TUBE, THE WRENCH SLIPPED CAUSING EE TO SLIP. HE TRIED TO CATCH HIMSELF WITH HIS LEFT HAND.
EE WAS GETTING OFF A WATER TRUCK AND SLIPPED, TWISTING HIS ANKLE. THIS RESULTED IN RESTRICTED WORK DAYS.

MECHANIC WAS USING A HAMMER TO STRAIGHTEN OUT A GUIDE ON A BOLTER. HE STRUCKHIS RIGHT INDEX FINGER BETWEEN THE HAMMER AND THE METAL. CAUSING A LACERATION AND A BROKEN FINGER.
EMPLOYEE WAS STEPPING OFF A MANTRIP. HE STEPPED ON A ROCK AND BROKE HIS RIGHT ANKLE.
EMPLOYEE WAS EXITING A MUCKER AND GOT PINNED IN BETWEEN THE DOOR AND THE CAB OF THE MUCKER.
EE WAS STEPPING OFF AN UNDERGROUND HAUL TRUCK AND STEPPED ON A FIST SIZE ROCK. THIS RESULTED IN A FRACTURED RIGHT ANKLE.
A GROUNG FALL OCCURRED ON 5/31/02 BETWEEN 2 AM AND 4 AM IN THE 4080 INTERSECTION IN THE MAIN SPIRAL. APPROX 150 TONS FELL. THE AREA HAS BEEN BOLTED, AND SHOTCRETED.
EMPLOYEE WAS TIEING IN A ROUND. SOME ROCKS FELL FROM THE FACE AND STRUCK HIM HITTING HIS LEFT SHOULDER AND KNEE.
EE WAS PUTTING A ROCK BOLT IN THE BOLT GRABBED ON A BOLTER AND CUT HIS FINGER.
EE WAS LOADING UTILITY PIPE AND GOT HIS RIGHT THUMB HIT BETWEEN THE EXHAUST GUARD AND THE POLY PIPE.
EE WAS RIDING ON A TRACTOR THAT STRUCK THE RIB AND MADE HIM HAVE A SORE NECK. THIS ACCIDENT BECAME REPORTABLE ON MAY 5TH OF 2002.
MINER WAS INJURED ON 1-22-02 HE WAS GIVEN A FULL RELEASE FROM THE ER ON 1-22-02. ON 10-10-02 HE HAD AN OPERATION TO REMOVE A MASS THAT DEVELOPEDAND WILL RESULT IN 10 RESTRICTED DAYS.

EE WAS RIDING A KUBOTA TRACTOR & THE TRACTOR STRUCK SOME UTILITY PIPE. THE PIPE GOT BENT & IT STRUCK EE ON THE BACK & RIGHT SHOULDER
EE WAS USING SPUD WRENCH TO TIGHTEN BOLTS WHEN IT SLIPPED OFF AND STRUCK HIM IN THE FACE CUTTINGNOSE AND REQUIRING STITCHES.
AT 1230 ON 8-6-01 A GROUND FALL OCCURRED AT THE MS SPIRAL RAMP. ABOUT 60 TONS FELL FROM THE LEFT RIB AND WENT HIGH IN THE LEFT RIB. NO PERSONS WERE AFFECTED. THE AREA HAS BEEN REMEDIATED & SHO TCRETED.
EE WAS INSTALLING DEWIDAG BOLTS & THE BOLT SLIPPED HITTING HIS LEFT TEMPLE AREA (18 STITCHES WERE REQUIRED).
WAS USING SHORT PRY BAR TO ALIGN BOLT HOLES. THE HOPPER MOVED 3""-6"" AND SLID OVER THE END OF HIS STEEL TOE BOOTS.
MINER WAS BOLTING AND THE DRIVER STEEL BROKE AND HIT HIM IN THE MOUTH. 14 STITCHES WERE REQUIRED.
EMPLOYEE OPERATING JACKLEG WAS CHANGING FROM BIT TO BOLT WHEN DRILL SLIPPED AND SMASHED LEFT RING FINGER AGAINST MAN BASKET. FINGER WAS BROKE AND REQUIRED STITCHES.
THE EMPLOYEE WAS LIFTING A CEMENT BAG AND TWISTED DURING THE LIFT STRAINING HIS BACK.

WHILE HAMMERING THE WING NUT TO REMOVE THE LEG FROM A GARDNER DENVER 83 JACKLEG DRILL, A SMALL SPLINTER OF STEEL CAME OFF STRIKING THE EE IN THE LEFT WRIST CAUSING A SMALL PUNCTURE WOUND.
EMPLOYEE WAS PULLING THE JACKLEG OUT OF THE HOLE AND SLIPPED CAUSING HIM TO FALL ON A ROCK, CUTTING HIS LEFT FOREARM.
EE WAS HIT BY A ROCK AND KNOCKED TO THE GROUND DISLOCATING/FRACTURING HIS LEFT HIP. EE HAD SURGERY TO REPAIR THE DAMAGE AND IS CURRENTLY OFF WORK.
WHILE BACKING BOLTER OUT OF HEADING, THE WIRE MESH GOT JARRED OFF RACK WHEN ONE OF THE TIRES ROLLED OFF A ROCK. A SHARP EDGE ON THE MESH STRUCK HIS UPPER LIP CAUSING A HALF INCH LACERATION WHICJ REQUIRED SEVERAL STITCHES.
EE GRABBED D-HANDLE OF JACK LEG TO DRAG IT AWAY FROM FACE, IN DOING SO HE KNPOCKED A ROCK LOOSE WHICH PINCHED HIS FINGER BETWEEN THE HANDLE. EE RECEIVED SEVERAL SUTURES AND WAS RELEASED FOR FU LL DUTY.
PLACING HAND ON LOOKING POINT OF SHOE CLAMP, RELEASING TRIMING ARM CATCHING SHOE ARM CAUSE SHOE TO FALL AND IMPACT FINGER. FINGER BROKEN IN 3 PLACES.
SMALL ROCK 4-5 LBS FELL FROM BACK, HIT VICTIM ON BACK OF RIGHT HAND.
LOOSE ROCK BEHIND 6'X9'X4""WELDED WIRE PANELS IN THE DRIFT BACK WAS BEING RELOADED. MEN WERE WORKING FROM A MAN DECK WITH A GUARD RAIL. A ROCK FELL THRU THE CUT OPENING IN THE WIRE AND BOUNCED OFF THE RAIL POST STRIKING THE MINER BEHIND THE STEEL TOE ON THE R FOOT CAUSING A LACERATION REQUIRING 5 STITCHES.
THE MINER WAS BENT OVER PUTTING TOGETHER PIPE ONCONCRETE LINE OF THE SHOTCRETE TRUCK WHEN HE FELT A STING IN HIS LOWER BACK.
A GROUND FALL OCCURRED IN THE CRANE BAY #3 AFTERA SLAB ROUND HAD BEEN BLASTED AND BOLTED. THE FALL WAS APPROX 500 TONS, WITH AN AVERAGE HEIGHT OF 10 FT. WIDTH 10 FT AND LENGTH 60 FT. NO PERSO NS WERE AFFECTED, HOWEVER A CANNON BOLTER WAS PARKED IN THE AREA AND IS STILL BURIED.
THE MINER WAS IN THE PROCESS OF LOADING A ROUND. WHEN HE BENT OVER TO PICK UP A PRIMER, A SLAB FELL OUT OF THE BACK AND STRUCK HIM ON THE RIGHT SIDE OF HIS LOWER BACK.
EMPLOYEE WAS IN THE PROCESS OF SPOONING OUT A KNEE HOLE WHEN A SLAB PEELED OFF THE FACE, STRKINGTHE MINER IN THE LOWER BACK AND HIP AREA AND FORCING THE MINER'S HEAD INTO THE FACE.
