Fall to the walkway or working surface

After cutting emitter tube line employee attempted to grab the line. As EE bent down, EE lost balance and tried to brace self with right arm and felt pain in the right shoulder.

while closing a barb wire perimeter gate, part of the closing mechanism slipped on the post causing hand to contact top strand of barb wire causing a wound to middle knuckle. Miner received a tetanus shot.

Struck against a moving object

Repairs to man basket, turn buckle, basket would no go into latch mechanism so used crane to moved into place, technician unhooked crane then got into basket to free latch and basket free 4' then stopped in mid air causing right knee to hit rail in basket and left ear hit rail in basket.

Affected employee was experiencing physical discomfort and disorientation while leaving the open pit, notified his Supervisor that he required attention.

Employee was assisting with moving a roller frame and his fingers were caught in a pinch point. He was diagnosed with two tuft fractures and released to restricted duty.

Employee was struck in the chin by a cap from a hydraulic tank. Seen at the Eureka clinic and 4 sutures were required to close the wound. Full duty release. Received the documents 03/14/2012 confirming that sutures were required.

Over-exertion in lifting objects

While getting the rig ready to move, the drill assistant was removing the blocks from under the jacks. When he went to pick up the block, he felt something pop in his lower back.

Employee was changing a drill hammer and smashed his finger between the hammer and the drill pipe. Diagnosed with an avulsion fracture. Released to full duty.

Employee was removing impact slides from conveyor belt/chute. His left middle finger was pinched between the slides' handle and a 5"" channel beam. He was given 3 stitches to close the wound and put on restricted duty for 2 days.

Employee was installing 6"" X 10'steel pipe. His left hand was placed on the bracket, as they were fitting pipe together the pipe slipped off the shim resulting in a laceration to the left index finger requiring stitches. The employee returned to work with no lost or modified work days.

Employee was pulling himself up onto haul truck step using both his arms and felt pain in his stomach. Seen at the ER, diagnosed with a hernia which was repaired on 11/22/2010.

Employee was using a pocket knife to cut a zip tie. When the knife cut through it slipped and cut the employee's left thumb. He was seen at urgent care and stitches were required to close the cut. Released to full duty.

Employee was holding a loader tooth while another employee was using a sledge hammer to pound in the pin. The hammer glaced off the pin, striking the employee's left hand.

Employee was changing hammer bit on drill. The bit fell, smashing the finger against the frame.

Fall from ladders

EE was working on the deck of a Haul Truck.The task involved the removal of the panel on the front of the cab.This panel has a hand-hold as part of its makeup.The employee began to descend the ladder & grabbed hold of the unsecure hand hold.In doing so, the hand hold slipped, resulting in the EE falling approximately 8 ft,landing on his feet.

Ruby Hill requests that this injury not be charged as a mine related accident because employee was in non-compliance with drug policy. The employee received a grease injection at the top of his left index finger between the second and third knuckle. He received medical treatment on 9/1/08. He admitted use before and after the incident.

While using pulverinzing machine, I felt my left arm muscles pull from my hand, up my shoulder. Felt numbness, tingle sensation in hands. I then proceeded to inform my supervisor of the pain I was feeling.

Fall from machine

Truck driver (785C Cat truck) was climbing the ladder to the cab, lost his grip falling to the ground. He suffered an open dislocation of his right elbow and fracture of his right femur.

Employee said he was performing maintenance on a haul truck. The air dryer cartridge he was removing started to fall and he tried to catch it and felt pain in his right shoulder.

637E Scraper began to slide while turning corner out of stockpile. Operator turned into berm to stop slide. Tire marks show that scraper traveled approx. 50-75 feet on top of berm. When operator attempted to turn the scraper down hill and drive off outside slope of the ramp(approx. 12')scraper rolled landed on its top. EE diagnosed lower back strain.

THE ILLNESS IS LATERAL EPICONDYLITIS; RIGHT ELBOW. THE ILLNESS DID NOT MEET REPORTABLE CRITERIA UNTIL A CORTIZONE INJECTION ON 11/29/04. THE EMPLOYEE'S OFFICE WORKSTATION WAS REDESIGNED TO MINIMIZE STRESS DURING REPETITIVE ACTIONS.

EE WASREMOVING WORN BUCKET TEETH FROM 992 LOADER. A WORN TOOTH WAS HIT WITH A 5 POUND HAMMER. AS A RESULT A DIME SIZE PIECE OF METAL BROKE OFF TOOTH AND EMBEDDED IN EE'S RIGHT INNER LEG ABOVE KNEE.

EE WAS USING A METAL JIG SAW TO CUT 16"" METAL. MOTOR FAN ON JIG SAW BLEW SMALL PIECES OF METAL INTO RIGHT EYE. SAFETY GLASSES WERE IN USE AT TIME OF INCIDENT.
