Employee was operating a jumbo on an incline in the main haulage. The operator drifted to the right rib and the spare drill steel in the drill steel rack came in contact with the right rib. The drill steel gained pressure breaking the drill steel rack striking employee with the drill steel in the head causing a fatal blow.

Employee was putting a new bolter steel on and pinched EE's left finger.
EE was walking from parking a haul truck on the ready line and slipped and fell, landing on EE's right shoulder and elbow. EE reported the incident upon occurrence. EE refused medical treatment and thought the strain would heal. After a period of time with no improvement EE sought medical attention.
Employee was carrying a 12' bolt to the front of the bolter when EE lost footing and fell. When EE fell EE's finger was smashed between the bolt EE was carrying and a rock.

Employee was closing the rock guard on the front windshield with EE's left hand and had EE's right hand placed on a pinch point. When the guard shut it cut EE's thumb causing a laceration.

Employee went to get a water hose to water the ribs. EE turned the hose on and underestimated the pressure of the hose causing EE to lose grip. The hose whipped around and struck the employee on the head.
Employee was hanging a 480 volt cable with a cab hook. The cable slipped pulling the cab hook off the wire causing the cab hook to come into contact with the employees' right hand causing a laceration.
The employee was plugging in the power cable from the bolter to the power source. There was resistance when EE was trying to plug it in so EE pushed the cable up and felt a pop in the abdomen.

Employee tipped a underground haul truck while making a turn. Employee hit EE's head while exiting the vehicle resulting in a laceration that was stapled at the hospital.

Mechanics were removing a hoist cylinder from a bolter. As they were removing a collet one mechanic was tapping on the boss with a hammer and the collet relieved itself and flew approximately 10 feet and hit the mechanic standing on the opposite side in the face. As a result the individual received (5) five stitches above EE's left eye at the eyebrow area.

While the employees where traveling out of the mine the floor mat wadded up in such a way that it caused the throttle to stick and maintain speed when braking was necessary. The vehicle collided with the rib and resulted in a shoulder injury.
Employee was hanging electrical cable from a man basket off of the forks on a forklift. Employee bent over to get under a 42"" ventilation bag and twisted employee's left knee.

While tramming the mucker (L-35) the brakes set up. This caused his head to slam into front window causing his glasses to push into his eyebrow. It cut his eyebrow and required 4 stitches.
Employee was connecting a 2"" drisco line. As he was lifting the line in place to install the clamp, he felt a burning sensation in his left biceps area.

The mechanic was changing flushing head on Bolter 16. While putting the last piece on something caused the inner piece to shoot out. The mechanics finger was caught in between the two pieces. As a result, his finger was cut and so was the tendon. He had to get surgery to fix it.
MINER WAS CARRYING JACKLEG SLIPPED AND FELL PINCHING HIS MIDDLE FINGER.
Miner was operating haul truck, in second gear, hit a bump. The radio came loose striking miner in nose.
Sandy ground where a slab round was loaded too heavy and timed poorly.
Miner was carrying 2"" Bull Hose on his shoulder. When miner attempted to load the hose onto a trailer he moved incorrectly and injured his back.

While completing a vehicle pre-op on haul truck, miner was attempting to loosen a frozen chock from the wheel. Chock came loose & smashed left index finger between chock an pump cylinder of the truck.
Was hooking up air hose on powder truck and lost footing on loose ground. Miner then rolled ankle.
Located at Zone #1 6740 crosscut 5, a 15'X15' section of the back fell to the floor. A pile of material approximately 10' high was deposited onto the floor. No miners present at the time of incident.
Injury reported on 5/27/2013 @ 9:30. No medical treatment wanted or needed by employee. Employee reported on 5/31/2013 that he was still injured went to hospital for diagnosis. Injury reported on 5-27-2013 was employee was getting into equipment and struck his head and jammed neck on ROPS.
Miner was hanging cable on a hook running along a rib. As he was hanging the cable, the hook failed, spun around, and lacerated the miners left hand and thumb.
The employee was trying to get the locking coupler on the core tube latch-head unstuck, when his hand slipped down the drill rod causing his right thumb to stick between the drill rod and the core tube.

Miner resupplying heading, was carrying bundle of bolts on his shoulder. He observed that a nose pillar was collapsing, turned to run while throwing the bundle of bolts away. Miner sustained a strain to muscle in upper torso.

Fall of ground from the back onto the boom of machinery. Area was bolted and shotcreted at the time of the incident.
Miner was climbing down a ladder way with a metal lid covering it. As he was closing the lid he lost his grip in the lid and it fell on his left hand that was resting on the edge of the railing. The lid caught the thumb between the lid and the railing.
Miner was attempting to drive a split set bolt from the deck of a scissor deck, leg slipped on floor and leg gave out.

Employee was attempting to attach a cable to an RTD starter and could not fit his hard hat into the space, so removed hard hat and reached into the engine compartment. Employee hit his head on a bracket resulting in a laceration. Seven staples were required to close the laceration, released to resume work next shift.

Employee was in the basket long-bolting. He was changing drill steel when the drifter moved and came down on his hand therefore pinching it between the boom and man basket.
There was a ground fall at SSX Z5- 7045 XC17. The fall was approx 15'Long, 8' high above oringinal back and 12' wide. This occurred at the face, there was no one trapped or hurt in this incident. MSHA was called at 10:45 AM on the 1-800-746-1553 number.

We had a Long Bolt Crew attempting to bolt, when we had a ground fall occur pulling past the anchor point of our exsisting split bolts and wire. This was an immediate report to MSHA, which was done and the area was signed off.
Was barring down on one rock and another one came down and landed on his left foot. (Instep)
While changing clothes in a dry, a hardhat fell out of a basket and hit employee in the head.
Employee was working on Sissor Deck lift ajusting the Vent bag and turning on the air and water, when either he was knocked off or fell off of the sissor deck approximately 10-15 feet and landed on his head. He was not wearing fall protection.
Was working on the boot wash with a screw driver and his screw driver slipped and he poked himself in the left hand.
Working on a Long hole drill. Was changing drill rods above his shoulders and pulled his left lower groin area. Has a Hernia. Didn't have a good platform to work from.
Climbing down off of a haul truck his foot slipped and he caught himself with his left arm and pulled his shoulder out.
Was remote mucking with loader and lost control of the loader, the loader came back and pinned him between the rib and loader.

SSX Mine Z1-6945-XC3. Operator was bolting up the area when he noticed the ground starting to work. He called his supervisor and told him that he was going to pull out because of the ground working and they need to shotcrete this area before they could bolt it. Operator was pulling out of area and stopped to roll water hose up, when the back came in on the Boom of the bolter.
Had stepped off of the drill to take a sample and heard a noise and looked up while a drill steel came down and hit on top of instep of his right foot and broke it.
EE was stepping off of a drill and stepped on to a rock and rolled his right ankle and broke it.

The employee was taking his chalk block off of the lube truck when he started to loose his grip and tried to catch it and twisted his right wrist. He was taken in for X-rays and released for full duty with a brace. He went back in on the to see the doctor 07/23/2006 because his hand was not getting any better. He then was put on light duty, which turn this into a reportable.
Was putting in a pin to hook up the Cubex Drill and a Teledyne Boom truck and the Teledyne moved and pinched his fingers on his right hand and had 8-10 stiches put in it.
Walking and ankle rolled on him and he heard a snap. He hurt his ankle about 5 years ago. This was a re-injury to his injury from before.
The injured miner was struck by falling rock while he was bolting and screening in a heading. The loose rock came through /around previously screened and scaled ground. The previous screen had collected some loose rock and appears to have given way.
Employee slipped while pulling water hose from bolter and landed on left side, causing neck pain. Saw doctor on 3/8/06 where physical therapy was prescribed, making this MSHA reportable.

The prell hose failed and blew prell into eye, causing minor burns to skin around eye and small laceration on eye.
Manbasket employee was in stuck on rib while lowering to floor. When basket came undone, it fell approximately 2 feet, jarring his knee.
While working on drill, poked finger on frayed boom cable. By the following day, finger showed signs of infection.
While operating underground haultruck, hit hole in road and hit head on top of truck.
Employee was attempting to lift the seat on the blasting truck to get to the batting. As he pulled very hard his shoulder became very sore. He had pulled his rt shoulder joint out 7 months ago working for another company.
EE was hammering on a bar to push a large pin out. Once the pin was out the bar was forced to the floor striking his foot.
Employee was attempting to pull a bearing out of a motor with a bearing puller. He caught his finger in the puller.
Had new rod trialer supended from hoist installing axles. The trialer slid and hit him, cutting his head. 10 stiches.

Scored a rock bolt with a hacksaw and was bending bolt to break bolt when it flip and hit arm.
A fall of ground above the anction point in an active stope of the ssx mine, in zone 2-6920 sb-21, the fall of ground also prevented travel past that point.
EE STATES THAT APPARENTLY A ROCK FELL OUT OF THE BACK AND ROLLED DOWN THE BOOM OF HIS BOLTER AND WENT THROUGH THE WINDOW INTO THE CAB. (HE WAS CLIMBING UP INTO THE CAB OF THE BOLTER.)
EE WAS REMOVING RICKID VENT TUBE THAT WAS SUPPORT BY A CHAIN AND BOLT ONCE HE REMOVED THE BOLT THE TUBE WAS TO HEAVY AND FELL DOWN ON HIM AND STRUCK HIS KNEE.

PUT TUBE IN RODS CAUGHT BOUNCED BACK HITTING HAND.
EE WAS WORKING ON ONE SIDE OF A SMALL METAL SHED AN OPERATION IN A TRUCK ON THE OTHER SIDE OF THE SHED WAS AT TO BACK UP INSTEAD HE WENT FORWARD AND PUSHED THE SHED INTO HIM PINCHING HIM UP AG AINST A LOADER.
EMPLOYEE WAS USING A PIECE OF WIRE TO UNPLUG ANFO FROM AUGER. THE WIRE APPARENTLY CAUGHT IN THE MOVING AUGER AND PULLED THE END OF HIS FINGER OFF.
EE WAS ATTEMPTING TO CLIMB UP ONTO A UNDERGROUNDTRUCK WHICH WAS ON GUIDE, HE WAS UNABLE TO PUT HIS FOOT IN THE TOE HOLE SO HE PUT HIS KNEE IN AND PULLING HIMSELF UP WHICH CAUSED PAIN TO HIS RIGHT SHIN.
EE WAS ASKED TO DRIVE VAN 3269 DOWN TO THE MAINT SHOP. SHE SWUNG AROUND A CURVE & LOST CONTROL OF THE VAN AFTER APPARENTLY SPOTTING A DEER IN THE HAUL ROAD. THE VAN SLID SIDEWAYS AND TIPPED OV ER ON ITS TOP AFTER HITTING THE BERM.
EE WAS ATTEMPTING TO LIFT A PANEL OF WIRE UP OFF THE GROUND WITH 2 BY 4, THE WIRE FELL ONTO THE 2 BY 4 THE WIRE RIPPED HIS LEFT PALM, EE WAS WEARING GLOVES.
THE RIGHT FRONT HYDRAULIC JACK ON THE TELEDTNE FAILED AND TIPPED TO ONE SIDE. THIS ACTION TOSSED EMPLOYEE OVER THE RIGHT SIDE OF THE ELEVATED DECK (APPROX 10') OFF THE GROUND TO THE GROUND.

APPARENTLY EE WAS MOVING DOWN A DRIFT WHEN A FIBER VENT TUBE EITHER FELL OR WAS HANGING LOW ENOUGH TO STRIKE THE CAB AREA OF THE LOADER. THERE WAS MUCK IN THE TUBING WHICH MAY HAVE CAUSED A WRIGHT BUILD UP IN THE TUBE.
EE STATES THE MUCKER SHOT WAS OPERATING REMTILY DID NOT STOP WHEN SHE WANTED TO SHE ALSO STARTED SHE RAN TO THE RIGHT OF THE MUCKER AND WAS PINNED AGAINST THE RIB. AND GOLD AS OF THIS DATE IS STILL INVESTING THIS ACCIDENT AND HAS NOT YET DETERMINED THE AU CAUSES FOR THIS ACCIDENT.
EE STATES HE WAS STANDING WIRE AGAINST THE RIB. THE WIRE FELL, STRIKING HIM ON THE LEFT CHEEK (FACE).
EE WAS REMOVING CABLE OFF OF THE RIB. A CABLE HANGER FLEW BACK & HOOKED HIS LIP. HE DROPPED THE CABLE WHICH PULLED THE HANGER THROUGH HIS LIP.
EE WAS ATTEMPTING TO HANG CABLE WHEN THE HANGER SLIPPED & DROPPED CAUSING THE HANGER WIRE TO RIP THROUGH HIS GLOVE & TWO FINGERS.
EE STATES THAT HE WAS WORKING UNDER A HAUL TRUCK WHEN HE STOOD UP AND TRIED TO TWIST HIS BODY AND FELT PAIN IN HIS KNEE.

THE FRONT WHEEL WAS ON A HIGH SPOT INCREAS ING THE DISTANCE TO THE FIRST STEP. EE SLIPPED &FALL BACKWARD HITTING THE RIB OF THE DRIFT FRACTURING HIS T-5 RIB.

GROUND FALL INTERSECTION 6966-72 SSX (X-CUT #19) SECTION OF BACK FELL OUT 500 TON APPROX. THE MATERIAL FELL ON TOP OF A KABOTA THAT HAD BEEN LEFT IN THE INTERSECTION.
EE WAS PLACING BROOMS AND SHOVEL NEXT TO THE WALL. THE BROOM FELL TO THE FLOOR. AS EE LEANED OVER TO PICK IT UP, THE SHOVEL FELL AND STRUCK HIM IN THE MOUTH.
