An Employee was removing the sample prep baghouse blower to replace bearings that had gone bad. As the employee was using an Allen wrench to loosen the Allen bolt in the bushing, the wrench slipped causing the employee's hand to hit a bolt resulting in a laceration that required medical treatment.

Employee was fueling equipment and tripped over a 2"" fuel hose. He fell and caught himself with his right arm and heard a pop in the shoulder area. ER physician diagnosed with muscle strain and the arm was placed in a sling. He is to follow up with his physician the week of 6/4/12. He is on modified duty until he sees another physician.

Driver was opening 4"" product valve on pheunmatic trailer to off load bulk crushed lime. Valve was stuck and he jerked on valve handle and said he felt pain in right sholder.Doctor said he had pulled muscles in right sholder and restricted him to no pulling or pushing of 20 pounds or more

While pushing on a wrench, employee felt a pop in his right elbow. Employee was evaluated and returned to work at full duty. After several follow ups and referrals, employee was diagnosed with cubital tunnel syndrome and surgery was performed to correct the ulneral nerve on 1/20/10. Employee continued on full duty until 1/20/10.

While changing the auto lube piping on LD023, the fitting next to the pump would not break loose. Employee braced a small wrench against grease tank for leverage, used a second wrench to break the fitting loose and his right shoulder popped three times.
Employee states 5 gallon bucket was being lowered from top of rougher and came and fell on nose.

Employee was sharpening a chain on a chain saw using a hand file with his left hand. He pushed the file and it caught, the saw twisted, fell off of the table and pivoted the blade upward catching the employee on the wrist causing a laceration.
While conducting repairs on a lube truck, the employee stepped off the lube truck onto a ladder and lost his balance falling approximately 3 feet to the floor. Caused a laceration above his eye and bruising with swelling to his knee.
Employee was working on a derrick screen boot and standing on some grating next to screen. The narrow grating buckled causing the employee to loose his balance and twist his body which strained his back.
Employee was leaning against hand rail to disconnect an air hose to a chicago fitting. Employee heard a pop while applying pressure on the hose fitting and later felt pain in rib area on chest.
Employee was attempting to make a hole in a clipboard larger with a pocket knife. The knife closed on his right thumb causing a laceration.
Employee was drilling bolt holes in a flange with a drill press. The drill bit caught in the flange causing the flange to spin. The employee's hand got caught in between the flange and the height adjustment pipe. Employee was taken to BM General hospital for evaluation.
Employee states he was pulling filter cloths and felt pain in his lower back/leg.
Employee was walking along leach pad, stepped on a piece of ice and fell into a hole tearing the soft tissue in her left calf.
While unspooling the last wrap of 2"" poly pipe off a spool, the end of the pipe sprang back and hit the employee on the left ring finger resulting in a fracture to the tip of the finger.

Employee states he stepped off of a service truck onto a rock causing him to ""twist"" his ankle resulting in a sprain/strain.
Employee slipped on some slurry and injured his knee. He was taken to a medical facility, evaluated, and returned to work. After some continuing problems with his knee, he got an MRI. On 12-18-06 it was determined that employee would need surgery (to be scheduled) but could return to work.
Employee was checking a loose fan belt on a forklift. He grabbed the belt, pulling upward twice, and felt something slice across his fingers. The employee caught his hand between the belt and pulling causing lacerations on his fingers.
EE was hosing down CIL containment for clean up work. The sump pump went down, causing the containment to back up with milk of lime solution. The solution was at or near the top of the EE's boots, dampening his pants.
While pulling hose from CIL containment tank, employee received contact burn from lime after stepping into containment tank and getting lime into work boots.
Employee was pulling the slack out of a cable to keep it out from under the tires of haul equipment. The cable started to feed itself out and the employee was struck in the legs by the cable.
Shovel was loading haul truck on the blind side when a 3 foot (approx) rock rolled out of the shovel and struck the bed of the truck behind the cab causing the truck to bounce. This caused the employee of the truck to jar his neck and bang his kness on the steering column.
Employee and 2 contractors were pulling a sump pump to repair. Employee got a Come-a-long and was hooking it up to move the pump so it could be lifted out of the area. The lifting strap broke at that time and the pump fell hitting the employee in the shoulder area.
Employee caught his finger in a pin lock while installing a bed cable.
Employee was walking from the drill to the truck. He stepped on a rock and twisted his right knee and felt a little pain.

Two employees were lifting wheelbarrow to dump contents into a dumpster when one of them felt a sharp pain in his lower back. Employee was taken to a medical facility and placed on light duty.
Employee felt a pop in his back while bending over to pick up locks for shovel teeth. Employee complained of pain and was taken to a medical facility. He was placed on light duty.
Employee was using a come-a-long and a hook to reconnect a mast chain for a drill. While tightening the come-a-long, the other hook started straightening and the come-a-long hook slipped off hitting the employee in the collar-bone. He was taken to a medical facility and diagnosed with a broken collar-bone.
Employee was dismounting uniloader and slipped on a bolt that was in the bottom of the bucket. He fell hitting his face and cut his right eyebrow with the edge of the safety glasses. Employee was taken to a medical facility and received stitches. He was released to full duty.
Employee was using a hammer to remove lip protector when a piece of steel flew up and stuck into his leg. He was taken to a medical facility where the metal was removed, and the employee received 2 stitches. He was released back to full duty.
Employee had placed a Duff Norton Jack under a haul truck and jacked it up. He then had a stand placed under the truck. When he pushed the lever to lower the jack, it popped loose hitting him around the mouth cutting his face. Employee also broke his ankle, but unsure how. He was taken to a medical facility and received stitches.
Crew was changing a drill bit and employee's left pinky finger was pinched between drill bit and drill table. Emlpoyee received a laceration to the tip of his finger that required stitches.
Picking up a compressor motor - strained low back.
Employee was climbing off a loader when he felt a pain in his right knee. He said the knee had been hurting from a prior, unreported incident that occurred while getting out of a van. Employee was taken to a medical facility, given pain meds. & placed on restricted duty. Follow up appointment scheduled.
Cleaning rocks and debris in tunnel. He reached into a moving conveyor belt to remove a rock and the rock shifted and pinched his hand between the rock and a bolt. He was taken to the doctor and was placed on restricted duty.
While working on a leaking line, solution sprayed under employee's glasses and got into his eyes. He was taken to a medical facility and released to full duty. At follow up appt. on 4/25/2005, he was given a prescription for antibiotic eyedrops and released to full duty. Seen again on 5/3/05 with no problems.
Employee removed a pump from back of truck by reaching over side and lifting with one arm causing pain in shoulder. He reported incident to supervisor, but did not want medical attention. He went on regular days off, returned to normal shift on 4/25/05, and asked for medical attention due to pain in shoulder. He was taken to a medical facility, and was placed on restricted duty.
Employee was preparing to go to work. While changing his boots, he stepped on a rock, shifted his weight, and hit his left foot on a bench while trying to catch his balance. He proceeded to his designated work area, but later in his shift felt that he might need to see a doctor. He as taken to the medical facitlity and then placed on restricted duty.

On 12/15/04 employee was lifting small lifters into place in the sag mill. He feels he hurt his back at this time. On 12/29/04 he informed his supervisor of the incident. At this time we took him to the doctor and he was placed on restricted duty.
While taking out an old liner a piece fell on his right foot. The employee did not want to seek medical attention at that time. After going home and seeing that his foot was swollen, he was taken to the doctor and was placed on restricted duty for one day.
Unscrewing drill pipe and pinched tip of pinkie finger between wrench and pipe.
Employee was grinding and some of the dust came off of the grinder shield and hard hat and entered employee's eye. He did not report the incident until 12/6/04 when he was taken to the doctor and a foreign body was removed from his left eye. He was released to full duty.
Putting out signs and cones to alert of heavy equipment in area and slipped through cattle guard causing a laceration on left shin. Employee was taken to medical facility and received stitches.
Latching on to 5' sub with elevators on well pulling rig. Left ring finger was between the elevators and the bails. When the sub lifted off the ground it pinched the end of the finger. He was taken to the doctor and received stitches.
Using a hammer to align coupling halves in tight quarters. He swung at the coupling and hit a come-along chain with the handle of hammer, missed the mark and struck left wrist with hammer head. He was taken to the doctor and treated for a bruised bone and placed on restricted duty.
Employee was loosening the spring retainer nut on the new brake can. THe wrench slipped and hit employee in the chin causing a cut in front of chin and another cut under his chin. He was taken to the doctor and was given 5 stiches.
Employee was climbing down from water tank on ladder. She was reaching for the engine deck area with her right foot when her left ankle turned over on the bottom step. She was taken to the hospital and treated. She was placed on restricted duty.
On 6/16/04 the employee was bending over from his drill to pick up a drill sample when he felt a pain in his lower back. He was taken to the hospital, treated and released to full duty. On 6/24/04 he was taken to the doctor for a follow-up visit and at that time he was placed on restricted duty.
Employee dislocated his right shoulder while loostening a c-clamp on the mud tub. He was not aware of any previous damage to his shoulder.
WHILE REMOVING THE OLD GASKET FROM THE COOLER ON THE DOZER, THE GASKET SCRAPER SLIPPED AND CUT THE RIGHT HAND BETWEEN THE INDEX FINGER AND THUMB REQUIRING TWO STITCHES.

AT THE END OF SHIFT ON 9/30 THE EMPLOYEE NOTICED A SMALL SORE ON HIS FINGER. HE REPORTED IT THE NEXT DAY AND WAS TAKEN TO THE DOCTOR. HE WAS TREATED FOR A SUSPECTED SPIDER BITE AND RELEASED. O N 10/7 HE WAS SENT FOR A FOLLOW-UP VISIT AND THE SORE HAD BECOME INFECTED. ON 10/14 HE WAS SEEN AND GIVEN A FULLY DUTY RELEASE.
THE EMPLOYEE WAS ATTACHING A ROTARY BIT TO HIS DRILL. HE TWISTED HIS BODY TO REACH FOR THE BIT ON THE WINCH CABLE AND FELT A PAIN IN HIS BACK. HE WAS TAKEN TO THE DOCTOR AND PLACED ON RESTRICT IVE DUTY.
AS THE EMPLOYEE WAS BEING LOADED, A LARGE ROCK HIDDEN IN THE FIRST BUCKET LANDED IN THE TRUCK. WITHOUT BEDDING, THE TRUCK WAS JOLTED CAUSING THE WINDSHIELD TO SHATTER AND HIS BACK TO STRAIN.
AS THE EE WAS SETTING THE PARKING BRAKE, SHE PUSHED DOWN WITH BOTH HANDS AND FELT A TINGE TRAVEL ACROSS HER LEFT SHOULDER.
EE HAD TURNED THE ROUTER DOWN TO ADJUST THE BLADE DEPTH WHEN HE ACCIDENTALLY HIT THE ON/OFF TOGGLE SWITCH, TURNED THE ROUTER ON. HE RECEIVED A LACERATION ON THE OUTSIDE OF HIS RIGHT LITTLE FI NGER AND A HALF MOON LACERATION ON THE PALM OF HIS LEFT HAND.
WHILE DIGGING INTO HARD TOE, HE HIT A HARD SPOT CAUSING THE LOADER TO JAR. THIS CAUSED HIM TO CLENCH HIS TEETH TOGETHER AND CHIP HIS FRONT TOOTH.
WHILE DISMOUNTING TD 753, HE STEPPED OFF A LADDER ONTO A ROUND ROCK, TWISTING HIS RIGHT ANKLE.
WHILE SHOWING SKIP SEPARATION IN LEFT OUTSIDE REAR TIRE, HE TRIPPED OVER CHOCK BLOCK, HITTING RIGHT LOWER LEG.
EE WAS MAKING AN ANTENNA COVER OUT OF PVC WHEN THE KNIFE SLIPPED CUTTING THE PALM OF HER HAND.

EE WAS IN PROCESS OF DISMOUNTING A DRILL WHEN HIS FOOT SLIPPED ON A RUNG OF THE LADDER. HE LANDED AKWARDLY AND TWISTED HIS KNEE.
WHILE CLIMBING LADDER ON HAUL TRUCK EMPLOYEE LOST HIS FOOTING AND SLIPPED. TRYING TO AVOID INJURING HIS BACK, EMPLOYEE JUMPED.
EE HAS SUFFERED FROM CARPAL TUNNEL WITH EE REPORTING IT AS OCCUPATIONAL INJURY ON 10-6-98. EE HAS HAD NO RESTRICTED DUTY OR LOST TIME UNTIL RECENTLY WHEN HE WAS SEEN BY PHYSICIAN WHO SCHEDULED HIM FOR SURGERY TO CORRECT. THIS SURGERY WAS ON 7-19-02 WHICH WAS ALSO EE'S FIRST DAY MISSED.
WHILE GETTING OUT OF PICK UP SMASHED FINGER BETWEEN DOOR AND JAM.
EE SUFFERED A HEART ATTACK ON MINE PROPERTY AND DIED AS A RESULT. THE HEART ATTACK WAS NOT CAUSED BY AN ACCIDENT OR ANY EXPOSURE IN THE WORK ENVIRONMENT, NOR WAS IT WORK-RELATED OR RELATED TO WORK ACTIVITY.
BACK INJURY (MUSCLE) WORKING ON DRY LEVEL DIRT GROUND, EE BENT OVER TO PICK UP A SMALL 9/16"" WRENCH LAYING ON THE GROUND. HE FELT A SHARP PAIN IN LEFT LOWER BACK. HE STOPPED WORKING IMME DIATELY. LOCATION: HAUL TRUCK READY LINE.
WHILE REMOVING A SOCKET HEAD BOLT USING IMPACT WRENCH THE PIN THAT HOLDS THE SOCKET TOGETHER CAME OUT CUTTING EE'S HAND.
EE WAS PUTTING LIDS ON CLEANER CELLS. WHILE CARRYING THE LID HE DROPPED IT ON HIS FOOT.
WHILE DOING REGULAR MAINTENANCE DUTIES ON TRUCK, EE WAS MOVING A HOSE FROM ONE SIDE OF THE TRUCK TO THE OTHER. HIS HEAD STRUCK THE LEFT REAR HOIST CYLINDER JAMMING HIS NECK.
EE WAS RELEASING BRAKE ON GRADER WHEN HE FELT A POP IN HIS THUMB. HE DID NOT SEEK MEDICAL ATTENTION AT THAT TIME. HE CONTINUED TO EXPERIENCE PAIN SO SAW DOCTOR ON 4/10/02 AND WAS PLACED ON RES TRICTED DUTY.
EE FELT POP IN SHOULDER WHEN LIFTING CHOCKS INTO RACK ON FRONT OF TRUCK. HE WAS SEEN BY PHYSIAN AND WAS RELEASED TO FULL WORK DUTY-NO RESTRICTIONS. HE CONTINUED TO HAVE PAIN IN HIS SHOULDER, T HOUGHT AND AT A FOLLOW UP VISIT IT WAS DETERMINED HE WOULD NEED SURGERY. HE HAD THIS DONE 4-16-02.
EE WAS HELPING REMOVE A 10 HP MOTOR ON THE ACID AREA SUMP PUMP. THE MOTOR CAME LOOSE CATCHING EMPLOYEE'S HAND BETWEEN MOTOR & WALL.
EE WAS ATTEMPTING TO REMOVE AN 0-RING FROM A 785HAUL TRUCK TIRE & WHEEL ASSEMBLY. THE WHEEL ASSEMBLY WAS STILL SEATED TO THE TIRE BEAD ON THE BOTTOM SIDE OF THE TIRE. WHEN THE WEEL BROKE LOOSE FROM THE BEAD IT FELL CATCHING EE'S FINGERS BETWEEN WHEEL & BEAD RING ON TOP SIDE OF WHEEL CAUSING TIPS OF FINGERS TO BE AMPUTATED.
EMPLOYEE WAS WORKING IN THE FILTER PRESS CHANGING CLOTHS. HE HAD PULLED THE CLOTH, LIFTED IT OUT OF THE PRESS AND WAS THROWING IT OVER THE SIDE TO THE DECK WHEN HE FELT A POP IN HIS BACK.
EE WAS LIFTING THE PIPE OUT OF THE BOTTOM OF PIPE RACK AND SMASHED HIS THUMB BETWEEN PIPE AND UPPER RACK.

EE WAS CLEANING UNDERNEATH THE SAG FEED BELT IN THE AUTOCLAVE RECLAIM TUNNEL WHEN HE NOTICED A PIECE OF WIRE THAT HAD COME IN WITH THE MATERIAL. WHEN THE EE TRIED TOR EMOVE THE WIRE, HE DID NO T NOTICE THAT PART OF IT HAD GOTTEN HOOKED TO THE CONVEYOR FRAME. WHEN HE PULLED ON IT, THE WIRE SPRANG CAUSING A CUT ON ARM.
EMPLOYEE WAS WASHING SAND AND GRAVEL OUT OF #2 ROUGHER CELL WITH A FIRE HOSE. WATER AND SAND SPRAYED BACK IN HIS FACE GETTING BEHIND HIS SAFETY GLASSED AND INTO HIS EYES.
EMPLOYEE WAS CARRYING A 5 GALLON WATER BOTTLE AND FELT A POP IN HIS FOOT FOLLOWED BY INCREASING PAIN WHEN HE TRIED TO WALK.
EE WAS HANDING A STEAM CLEANER WAND TO ANOTHER EE. WHEN THE OTHER EETOOK THE WAND BY THE HANDLE, HE WRAPPED HIS FINGERS AROUND THE PISTOL GRIP, SQUEEZING HARD ENOUGHT TO OPEN THE WATER FLOW. T HIS CAUSED ENOUGH PRESSURE THAT THE WAND SWUNG AROUND STRIKING THE FIRST EE IN THE MOUTH.
EMPLOYEE WAS HELPING INSTALL A RUBBER BOLT ON A VIBRATING SCREEN. WHILE THE MECHANIC WAS PLACING THE BOOT EMPLOYEE WAS PRYING ON THE SCREEN BODYWITH A 5' BAR. HE HAD THE END OF THE BAR ON HIS RIGHT SHOULDER AND BOTH HANDS AROUND THE BAR. AS HE LIFTED THE BAR. HE FELT A POP IN HIS SHOULDER.
EE WAS REMOVING A ROTARY SUB TO PUT THE HAMMER SUB ONTO DRILL. WHEN HE WAS WINCHING SUB OUT, THE SUB CAUGHT THE END OF THE DRILL STEEL AND TIGHTENED THE WINCH LINE UP CAUSING ROTARY SUB TO POP OUT OF DECK, HITTING THE EE SMASHING HIS LEFT HAND AGAINST THE CAROUSEL ON DRILL.
EMPLOYEE WAS PERFORMING GRINDING DUTIES WITH FULL PPE INCLUDING SAFETY GLASSES AND FACE SHIELD. A PIECE OF METAL EMBEDDED IN HIS EYE.
EE ATTENDING ON-SITE MINE RESCUE TRAINING CLASS BEING TAUGHT BY STATE OF NEVADA AT LONE TREE MINE. WHILE PERFORMING A LOW ANGLE RAPPEL, EE TWISTED HIS ANKLE.
3-20-01 EMPLOYEE UNLOADING A 4 WHLR FROM TRAILER WHEN THE UNLOADING RAMP SLIPPED AND EMPLOYEE FELL, LANDING ON HIS ELBOW AND SHOULDER. AT THE TIME OF ACCIDENT, EMPLOYEE DID NOT WISH TO SEEK ME DICAL ATTENTION. THEN ON 5-31-01, THE EMPLOYEE EXPERIENCED DISCOMFORT, WANTED TO SEE DR. AT THAT TIME, EMPLOYEE WAS PLACED ON RESTRICTED DUTY.
ORIGINAL ACCIDENT HAPPENED ON 3/14/01 WHEN EE MISJUDGED HIS STEP WHEN HE WAS CROSSING SOME PIPE.HIS FOOT SLIPPED & HE TWISTED HIS KNEE. HE DID NOT WANT TO SEEK MEDICAL ATTENTION AT THAT TIME. ON AUG. 31 HE HAD SURGERY ON THE KNEE.
THE EE WAS WALKING FROM THE VAN TO THE CIC BUILDING WHEN HE SLIPPED ON SOME ICE. HE FELL BACKWARDS AND HIT HIS HEAD. HE WAS SEEN BY A DR. AND WAS RELEASED TO FULL DUTY. ON 1-24-01 HE COMPLAINE D OF HAVING PAIN AND WANTED TO SEE A DR. AGAIN. AT THAT TIME THE DOCTOR RELEASED HIM TO LIGHT DUTY.

ON 11-28, THE EE WAS CHANGING BITS ON A DRILL WHEN HE LEANED OVER AND FELT A SHARP PAIN IN HIS GROIN AREA. HE DID NOT WANT TO SEE A DOCTOR UNTIL DECEMBER 11. AT THAT TIME HE WAS DIAGNOSED WITH A SMALL REDUCEABLE INGROINAL HERNIA AND WAS PLACED ON LIGHT DUTY.
EE WAS WASHING OFF HAUL TRK BEFORE IT WAS TO BE PM'ED SOME MUD SPLASHED IN HIS RIGHT EYE. HE WASTAKEN TO DR TO HAVE IT REMOVED & WAS PUT ON LIGHT DUTY FOR 24 HRS
EE WAS CUTTING SOME ELECTRICAL WIRE THE KNIFE SLIPPED & HE CUT HIS LEFT INDEX FINGER. THIS RESULTED IN 3 SUTURES.
TWO EMPLOYEES WERE INSTALLING A 4"" ELBOW. ONE EE DROPPED HIS END OF THE ELBOW AND HIT THE OTHER EE ON THE NOSE. THIS RESULTED IN A BROKEN NOSE.
THE EE WAS CHIPPING FLUX OFF OF A WELD & AP IECEOF THE FLUX WENT INTO HIS RIGHT EYE
THE EE WAS REMOVING THE PUMP CASING ON THE FLOT GRIND SAG MILL. HIS FINGER GOT CAUGHT BETWEEN THE CASING AND ONE OF THE BOLTS CAUSING IT TO PINCH. THIS RESULTED IN A LACERATION AND A TUFT FRAC TURE REQUIRING 4 STITCHES. THE EE RETURNED TO WORK FULL DUTY.
JUST FINISHED OUTLING A CORE TUBE, WHEN RIG FLOAT PREMATURELY RELEASED DOWN BETWEEN WORKERS, IT HIT THE EE'S BICEP AND FOREHEAD.
EMPLOYEE WAS LIFTING TWO PEOPLE TO THE ROOF IN ABOOM TRUCK. AS HE WAS DOING THIS, HE FELT SOME-THING IN HIS LEFT EYE; HE WAS WEARING HIS SAFETYGLASSES.
THE EMPLOYEE WAS MOVING A TIRE RING WHEN HE SLIPPED ON SOME MUD. AS HE WAS GOING DOWN, HE GRABBED THE RING AND PULLED HIS LEFT BICEP. THIS RESULTED IN A TORN BICEP AND LIGHT DUTY.
THE EMPLOYEE WAS ATTEMPTING TO SIT DOWN IN THE CAB OF THE DRILL. THE SEAT SHEARED OFF FROM THE SWIVEL STATION, THROWING THE EMPLOYEE INTO THE CAB DASH BOARD. THIS RESULTED IN MUSCLE STRAIN A ND LIGHT DUTY.
WHILE FUELING A DRILL, HE CLIMBED DOWN THE SIDE LADDER AND STEPPED IN A DRILL HOLE AND TWISTED HIS LEFT ANKLE RESULTING IN ONE DAY RESTRICTED DUTY.
INJURED AND ANOTHER EMPLOYEE HAD BEEN WELDING AND GRINDING. THERE WERE SPARKS FLYING AND HE WAS WEARING HIS PPE. THE NEXT MORNING HE WOKE UP AND HIS EYE WAS HURTING. HE WAS SEEN AT BATTLE MT G ENERAL HOSPITAL IN THE ER. DOCTOR REMOVED A SLIVER OF METAL FROM RIGHT EYE.
EMPLOYEE WAS CHECKING PUMPS IN THE FLOTATION BUILDING WHEN HE SLIPPED ON SOME CONCENTRATE ANDTWISTED HIS RIGHT KNEE. HE WAS REFERRED TO A SPECIALIST 0N 3-15-00. AT THAT TIME HE WAS PUT ON RESTRICTED DUTY.
EE WAS INSTALLING A NEW SEAT IN A HAUL TRUCK WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK. AT THAT TIME, HE DID NOT WANT TO SEE A DR AND RETURNED TO WORK. ON 3/13/00 HE WAS FEELING A LITTLE STI FF. WE TOOK HIM TO THE DR AT THAT TIME AND HE WAS PLACED ON RESTRICTED DUTY.
EE HAD JUST COMPLETED WASHING THE LUBE TRUCK IN THE WASH BAY. AS HE WAS DESCENDING THE STAIRS, HE SLIPPED ON ONE OF THE STEPS. HE HIT HIS ELBOW AND CAUSED A LACERATION. THIS REQUIRED SUTURES A ND ONE DAY OF RESTRICTED DUTY.
THE WIND WAS BLOWING VERY HARD. EMPLOYEE WAS OPERATING THE BOOM TRUCK WHEN A PIECE OF DIRT BLEW OVER HIS SAFETY GLASSES AND INTO HIS EYE. THIS RESULTED IN RESTRICTED DUTY.
EE WAS GETTING NUMBERS OFF OF THE SAMPLE BAGS. SHE BENT OVER TO GET SAMPLES AND FELT SHARP PAIN IN HER LOWER BACK.
