Caught in, under or between running or meshing objects

Employee failed to lock out, tag out, test out a belt-driven piece of equipment and used right hand to get the energized belt to move. Belt engaged and right hand fingertip was cut off by pulley/belt assembly. Fingertip was amputated.

Employee was lubricating a shaft that was outside the line automated packaging area when at the same time, they grabbed a bag for no reason near a moving roller. Their hand reached really high and the roller pulled the tip of middle finger into the roller chipping the tip of the middle finger bone.

Employee lowering landing gear on portable welder trailer. Trailer hooked up to forklift. Putting stabilizer leg of trailer in place, aprox 15 lbs. Pulled pin with dominant left hand, held leg with right hand. Leg slipped out of hand, reached for it awkwardly, hyperextended elbow. Received first aid. Injury reportable on 2-11-19 when employee received work restrictions.

Employee went to get some additional tools and when returning to work area ee stubbed the toe of steel toed boot on an anchor that was bolted to the floor.

Employee was making repairs inside the kiln and a dust particle got in his eye. Doctor prescribed eye antibiotic to prevent infection.

During a severe thunderstorm a miner was closing the shop door. The wind caught the door and slammed it in to his shoulder.

Employee was cleaning up debris in the kiln and got something in his eye. The employee was not wearing his goggles. His eye was washed out but he had scratched his eye.

Employee was moving cinder blocks that had wood attached to them. When he picked up the cinder block the wood attached scissored closed and smashed his ring finger between the cinder block and the wood.

On 10/2/2012 EE came into work using crutches and stated that he had injured his back on 09/18/12 while operating a loader. On 9/18/12, he left work around 3:00AM without notifying anyone of his injury. He called-in sick on 09/19/12. He came back and worked the following days; 9/20, 9/23, 9/25, 9/26, 9/27 and 9/28. He then called in 9/30/12 and 10/1/12.

It had snowed over night giving us about 3 in of snow. Most of the walkways had been cleared and the snow had started to melt. The two employees were walking in an area that had not been clean by the loader yet. One employee slipped and fell.

EE was helping install safety chains on the presip catwalk. he was holding a latch for EE White to weld. The welding rod accidentally while showing another ee were he should hold the part, and touched the part. a spark came off and went over EE safety glasses into his left eye lid.

Employee was climbing up # 52 elevator on cooling side of silos with an MSHA inspector. when he got to the top he felt his left butt cheek start to hurt. Turned out to be a pulled muscle.

Employee was cleaning under 5 pound line when a dust shield came off. He tried to put it back on and got his left hand caught in belt.

Unplugging crusher, when pry bar got caught in crusher wheels, bar then came up and hit EE under neck. Knocked EE out, chipped teeth, hit head and hurt back.

Employee was stacking 25# bags onto a pallet, when he said his lower back started to hurt. He then said it was starting to hurt worse so we put him on a stretcher and called 911.

Employee was trying to remove a piece of D.E. from the crusher, when a piece fell on his right hand.

Jackhammering inside Kiln, when he stepped on a metal pin. It went through his right shoe and poked his foot.

EE was stacking bags on pallet, when he hurt his lower back Monday , did not ask to go to the doctor till Thursday 8-16-07.

Walking backwards, he tripped over oxygen hose, fell backwards and tried to stop the fall with his right hand and hurt his right shoulder.

Was trying to unplug crusher from platform with pry bar, when one rock fell out of crusher tray on top of right foot.

Employee was sitting in a chair at the valve-bagger station. You push foot pedal then a bag fills up, slide the bag off, slide the next one on, then press the foot pedal down. He said he pushed the foot pedal down then he felt a pain go from the bottom of his left leg up to his lower back.

EE was tightening vice to work on nozzel and the wrench broke. It came up and hit him right between his eyes, breaking his glasses. It also hit the left side above his eye. He was given first aid and went to concentra for stitches above his eye.

Two employees were putting a brake drum on a drive axle of a dump truck. One employee picked drum up, turned it around on a table so it would face axle hub. He picked it up and turned around, bent over to place drum on hub and let out a cry. He was put on a stretcher and sent to hospital. He suffered a herniated disc in his back.

Employee was installing metal plate on shrink wrap machine inside warehouse. Plate was being held in the air by a chain connected to a bolt in the middle of the plate. When the bolt came out the plate fell towards the ground, landing on his foot.
