Operator on lunch saw EE's truck on pile of rocks. When contact was made, EE unresponsive, truck running, doors locked. Supervisor broke window to get to EE, EE remained unresponsive. CPR started and continued while EE transported to Hospital. This appears to be a possible cardiac event. Investigation ongoing.

Worker's kneecap popped out of place when trying to sweep an electrical cable to the side with EE's foot
EE was getting numbers off a piece of equipment when coworker approached them from behind. Saw them and was startled, causing them to jump, landing awkwardly on the grating, and rolling right ankle. Seen at medical, diagnosed with a sprained right ankle, and released to restricted duty which employee is working.
Ee was dispatched to assist w/reagents offload delivery. Ee arrived on scene & had difficulty breathing. Contractor that was waiting to be offloaded recognized ee was having trouble & asked if everything was ok. During the interaction, ee used their asthma inhaler, was unable to take deep breaths, & asked contractor to get help. Contractor helped ee to ground & called a MAYDAY.
EE was stepping up onto a concrete pad approximately 4 inches high and felt pop in knee. Reported verbally to safety that EE had been having pain in that knee for about 2 weeks prior to the 13th. No slip/trip/fall, no uneven ground. Does not appear to be a hazard associated with work. Diagnosed with soft tissue injury, put off work.
Ee reports walking to blue room and felt pop in left knee. Reports immediate pain and inability to bear weight. Seen at ER, diagnosed w/knee pop and possible ligament injury, and put off work pending an orthopedic consult. Does not appear to be an injury by accident. No uneven ground, no slip/trip/fall.

The employee was lifting a scaffold plank overhead to set for upper level. the employee felt a pinch in stomach, but did not think it was serious. Later it was determined to be a small umbilical hernia. Late reporting from employee. Work Status: May perform regular duties with attention not to aggravate.
EE reports getting out of truck, slipping on ice, and feeling pain in knee. On 12/2/24 EE saw a provider at the clinic who diagnosed EE with pain in the knee, subscribed physical therapy, and released EE to restricted duty.
EE was changing cutting edges on a loader bucket. EE reports a beam weighing 140 pounds dropped from about two feet above onto EE's right foot. X-rays negative for fracture, referred for ortho evaluation. On restrictions site can accommodate temporarily.
Ee was using a pry bar to help guide a cylinder into place while standing on the third rung of a 6 foot step ladder. As another operator moved the cylinder into place, it caused the pry bar to ""kick back"", causing the ee to lose balance and fall from the ladder, landing on left hip. Ee transported by ground ambulance to hospital, orthopedic surgery done on pelvis.
Employee was conducting welding operations on work platform. The hand rail post broke and the employee fell to the lower level. Miner cut knee requiring sutures to close the wound.
Ee was walking along pump alley when EE stepped on a rock and rolled EE's left ankle. Seen at clinic, diagnosed with pain in EE's left ankle, given a boot, and released to modified duty that site can provide.
Ee was dropping the 3rd belly pan on a piece of equipment in order to gain better access to inspect a leaking hose. The pan dropped onto EE's left hand. Seen at Golden Valley, diagnosed with fractures and laceration, and referred to ortho. Released to restricted duty.
EE was using the fuel reel on the back of the truck. As EE was rolling it up, it was going fast, and popped back and broke EE's tooth. Diagnosed with a broken front tooth, seen at dentist, released to full duty.
Ee was walking to the work area when a ""burst of sneezes"" caught EE off guard. EE reports hunching EE's back and pulling a muscle. EE was seen at Golden Valley and diagnosed with a non-occupational back strain. There was no environmental factor or work-related condition identified that caused EE to sneeze. Released with restrictions.
Ee was tightening bolts with an impact gun when the bolt spun against ee's glove, cutting the glove and cutting ee's palm. Seen at ER, laceration closed with steri-strips and glue, and on follow-up ee placed on restrictions site can accommodate.

Ee was torquing nuts on a valve. Ee was adjusting the open-end wrench and when ee began to torque, the wrench moved and pinched ee's pinky finger between the torque and the open-end wrench. Seen at clinic, diagnosed with distal fingertip fracture, released to full duty, referred to hand specialist.
Ee was working with another ee on a Geho pump. Removing diaphragm door, took last nut off, door popped off, and hot slurry struck the ee in the face. Seen at ER, transported by air to a Medical Center in Vegas for burn consult. Treated and released, able to RTW full duty 8/2 per a Dr. at burn center.

A crew were conducting sonic sampling. The injured miner was holding a bag to collect soil sample while second minor was striking the core barrel to release sample. The injured miner's hand slipped into the path of a 2lb sledge hammer and was struck. The injury was diagnosed as a laceration to right, middle finger. Follow up appointment, 1/9/2023, reveled non-displaced fracture.
Ee was filling propane tanks. Ee accidentally hit the exhaust valve, and liquid propane came out, striking ee on the right hand and wrist. Seen at ER, diagnosed with 2nd degree burns of hand and wrist. Released to modified duty at next scheduled shift.
EE was lifting an impeller that weighed approximately 80 lbs. Within minutes after lifting the impeller, the EE felt pain in lower back. Seen at clinic, diagnosed with a lumbar strain, and released with restrictions the site can accommodate.
Ee removing a door that had glycol behind it. End cap door on the Geho had pressure behind it, and the pressure moved the door off the studs, causing the door to fall on the ee's foot. Seen at ER, diagnosed with fracture. On follow-up with ortho, ee released with restrictions site can accommodate.
EE hooking up grease hose to loader, and coupler was not going on. Cleaned coupler and tried again. used left arm to put pressure on hose and coupler, and felt ""something"" in left elbow. Felt pain following. Seen at medical, diagnosed with a sprain.
Ee was getting on the bus at the Twin Creeks core shed after work. Ee reports a trip/fall. Ee injured shoulder, chin, shin, and lower back. Seen at a medical facility, diagnosed with shoulder strain, left shin abrasion, left lower back strain. Referred for MRI of right shoulder and orthopedic consult. Released to modified duty.
EE was team lifting a small trailer. EE ran out of room to maneuver and moved their body, extending arms. EE felt a strain in the low back. EE reported the incident and sought treatment at the clinic. Diagnosed with a low back strain and released with restrictions site can accommodate.
While preparing to start maintenance task and receiving instructions stepped in slurry which got in boots. Employee did not remove boots until later in the day and discovered burns to bi-lateral feet/ankles. Employee off work 2 weeks with elevation of lower extremities and monitoring of chemical burns.
Ee was using a sledge hammer to remove a back tire from a lube truck. EE swung the hammer, connected with a wheel stud, and crushed EE's right index finger. Seen at Humboldt General Hospital, diagnosed with right index finger distal phalanx fracture and laceration. Sutures, splinted, released to modified duty.
Two process employees were working to clear a plugged drain with an air lance. The plug cleared and sulfuric acid sprayed injured employee in face, arms, and wrist. Transferred to burn center; diagnosis chemical burns. Employee released on 5/8. Lost time.

Ee was troubleshooting a PLC at the autoclave. Ee was kneeling and when ee stood up, knee locked up and ee felt pain. No injury by accident and claim was denied by workers' compensation. Ee released to restricted duty.
Ee welded on 2/13, became sick with flu-like symptoms. Felt better after ee went home. Welded again on 2/17, became sick again with flu-like symptoms. Seen at Golden Valley, testing ruled out other diseases, ee diagnosed with metal fume fever. Seen again on 2/22 and released to full duty 2/23.
Ee was holding a piece of pipe in place for a mechanic when a piece of carbon fell into ee's eye. Ee continued to work and did not report the incident. The next day, 1/23, ee felt pain in the eye and reported the incident. Seen at the ER, given an exam and prescribed eyedrops for inflammation. Released to full duty.
Ee slipped on berm and fell, striking hand on rock. Seen at ER, diagnosed with laceration to the left hand. Sutures needed to close the wound. Ee released to full duty.
EE was in sample prep cleaning out a reject chute. EE stepped back, leg slipped, and they felt a pop in knee. Seen at medical on 1/17/22, diagnosed with knee pain and instability, released to restricted duty, no prolonged standing and no stairs.
Ee was cleaning out carbon screen discharge chute. Another pipe ""y'd"" into drop pipe off chute. Ee pushed on the pipe while ee's hand was on the handrail. The line came apart and landed on ee's thumb. Xrays negative for fracture, laceration required stitches. Ee released to restricted duty.
Ee rigging a mill liner and it slipped. EE reports pushing self away from the liner when EE slipped and collided with the wall. Seen at Golden Valley, diagnosed with a rib contusion, and put off work pending follow-up on 1/10/22.
EE was flying a bin down from charging SAG mill, coming down the second flight of stairs to the bottom floor, the EE's left ankle gave out causing EE to fall on the last step to a small landing. EE was holding hand rail with one hand and had crane remote in the other. Diagnosed with inversion sprain, released to light duty.

Ee was using a chisel to remove a broken bolt from the base of a pump when a piece of bolt broke off and went into EE's left eye. Seen onsite by emergency response and requested treatment on 12/30. Diagnosed with a corneal abrasion and referred to optometrist. Released to full duty.
While performing a stack test, Business Partner employees raised the Genie man-lift to approximately 60' and began to have control problem. Inside the bucket, they tried to get the controls to work, unable to do so, the supervisor tried to get them down from the ground controls. They were unable to get the bucket lowered, NGM employees used a crane bucket to get them down safely.
Pre-check of manlift performed before operation; functioned normally. After raising basket to operating height, basket controls and ground controls stopped functioning. Mechanic tried to repair but that didn't work so employees were rescued from manlift using a crane. No injuries, illnesses or damage occurred.
Ee was attempting to move a table by pulling on a vise. The vise was not mounted and was pulled off the table, landing on the ee's left foot. Seen at a med. center, xrays negative for fracture, ee put off work and referred for consultation with a foot surgeon.
EE was climbing a ladder to reach some steel balls in the sage mill hopper. On 3rd trip down the ladder, they stumbled over a rubber mat that was left on the ground at the bottom of the ladder. EE twisted right knee. Seen at clinic, diagnosed with a strain, subsequently put off work effective 8/27/21.
NGM Employee, touring the refinery, reported they felt dizzy. The employee assisted to the ground by area personnel and it is believed they temporarily lost consciousness. The employee was evaluated by on-site paramedics and released to a supervisor. This event was related to a personal medical condition.
A power loss was experienced at approximately 0251 am. At 0538 am, after a let up in a snow storm the source of the power loss was discovered. A power pole was found on fire. Mine rescue was dispatched to assess and extinguish fire. Fire was safely extinguished at 0715 am.
EE was walking into control room. As the door was closing, EE's finger got pinched, causing a laceration. EE was seen at the ER and stitches were required to close the wound. EE released to full duty.

Ee was at Vista Pit performing walk around on RTD021 after break. Ee was walking on back of counterweight while holding the handrail. The handrail broke loose from the mounting bracket and the ee fell about 5 feet to the ground, injuring right leg/hip, and low back. Fracture right hip.
Ee was chocking truck. Ee slipped on a rock and fell on right side. Seen at a hospital, diagnosed with a right hip fracture, and transferred by ambulance to a hospital in Reno for surgery. Lost time.
Ee was changing drill steel; had drill steel bale in hand, turned, tripped, and fell against handrail of deck. Ee admitted to hospital with two fractured ribs and blood in chest. Put off work.
Ee was cleaning material from a chute and a rock rolled, down, struck EE in the upper left front tooth, and scraped EE's lower lip. EE reported the incident and is scheduling an appointment with the dentist.
An EE was loading brick from a pallet to a brick basket. Material was being transported by crane from one compartment of an autoclave to another compartment of the autoclave. Employees were notified of the overhead hazard & the EE moved to clear the area for the crane. As EE went to clear the area, EE's foot caught the edge of the basket which caused EE to trip & fall.
Ee was dismounting haul truck. Ee's glove slipped off the handrail of the ladder and ee struck left wrist on the top of the ladder. Seen at the ER, diagnosed with a fracture to the left distal radius, referred to an orthopedist, and released to restricted duty.

Ee was replacing oxygen/acetylene tanks and the crane hook came down on their left thumb. EE was seen at Renown ER and surgery was required on the thumb. EE is off work pending surgery follow-up.
EE was running a 1/2 inch impact gun and screwed the thread into their right palm. Xrays negative for foreign body in hand, stitches required. EE released to full duty.
EE was tacking a liner on a loader bucket when a piece of weld broke off and struck EE on the right side of the forehead. Seen at clinic and stitches were required to close the laceration. Released to full duty.
Employee was working overhead tightening bolts with impact gun. When moving to the next bolt the locking mechanism failed allowing the socket to detach from the impact gun. The socket then fell from the bolt striking the employee on the bottom lip.
While swinging hammer, employee felt a sharp pain in middle to lower back.
The employee was removing scale from an autoclave. During the demolition, a piece of scale broke away and fell onto employee's forearm area.
6/9/2019 temporary laborer was reaching over to lift an assay lab sample bag when they felt a pain in the back/shoulders. On 6/13/2019 they sought further medical treatment and was then placed on light duty by the physician

When the bus arrived to drop off the crew for shift, employee stepped off the bus and twisted ankle.
On 12/23/2018 employee slipped and fell on left side of body and was initially given full duty release. After additional evaluation of the employee's shoulder in April 2019, the Dr. determined surgery was required on employee's left shoulder. Surgery was performed on 4/22/2019
Employee was using a 4"" angle grinder when the grinder kicked back and caused a laceration on left thumb.
While exiting the bed of a service truck the employee slipped on the step falling 18"" to the ground landing on right side.
While walking in established walkway, employee stumbled and fell to the ground resulting in a laceration to the left knee. Employee received 9 sutures to close the wound.
EE injured on 3/8/2018 and felt EE did not need medical treatment at the time. After experiencing pain, further evaluation on 4/24/2018 discovered EE had a fracture. EE was using a come along to pull down on pipe to align the flanges. The chain on the come along failed causing the EE's hand to hit the flange bolts resulting in a small cut on right index finger

Employee was cleaning mud buildup on the ball charging conveyer with a hammer. The hammer became lodged between the belt and the tail pulley, pulling the hammer into the belt. The employee's left ring finger was caught between the hammer and the zero speed sensor causing injury to the finger.
There was a sharp wire on the back side of a hose. Operator went to adjust the hose and did not identify the hazard resulting in the wire piercing the glove and cutting and tearing the skin.
Demoing brick from an autoclave with a titanium baffle. Titanium baffle was supported from side to side movement. The employee reached to brush rubble from under the baffle and the vibration from use of the rivet buster caused the baffle to shift, pinching the right middle fingertip of the employee.
EE was helping another laborer move a negative air machine from one vessel to another. The EE aggravated a pinched nerve from a previous injury. The EE requested to be laid-off. On 3/21/2017, we were made aware that EE went to a chiropractor for treatment.
Two mechanics were assembling the back case of low temp heater pump using overhead crane. Mechanics had pump case started on the pump shaft and as they were pushing on the front to align casing, mechanic hoisted up the pump case with crane, when doing so the lifting eye on pump casing broke. Pump casing rotated on pump shaft to the east, pinching one of the mechanics' left hand.
EMPLOYEE WAS SETTING TORQUE FOR WHEEL GROUP MOUNTING BOLTS USING THE RAD GUN WITH A DOUBLE SOCKET REACTION ARM. WHEN EMPLOYEE WAS ON THE LAST BOLT THEY FLIPPED THE GUN AROUND AND WHILE SETTING TORQUE THE GUN POPPED OFF AND THE REACATION ARM AND HIT THEIR FACE.
Contract employee delivered load of mill concentrate with OTR truck and side dump trailers. Emp dumped pup, pulled away from pile to re-position main trailer. Emp traveling to re-position. Emp continued past pile stopping on berm. Loader Oper made May Day call, ERT responded, found emp unresponsive. Emp transported off site by mine emerg. vehicle, transferred to county ambulance.

LD-237 (Caterpillar 992) loader was going down for mechanical reasons, LD-245 (Caterpillar 994) loader was called to support Sage mill feed. While tramming with a full bucket, the operator drove up on one of the stockpiles which resulted in the loader tipping over and coming to rest on its right hand side. Following Work Care's recommendations operator was released back to work
Welder was blend grinding a repair on the hard nose of LD245 when the grinding wheel of the 41/2"" Dewalt grinder grabbed and ran up the steel plate contacting the employees left inside forearm causing a laceration. Employee was wearing all proper PPE and all guarding was in place on tooling at time of accident.
Employee was helping pulling equipment out of DW 69 when hand came into contact between the power tongs and the column pipe, smashing two fingers on left hand
Maint techs were changing out sub adapter as retainer was broken at the hex head. Welder cut old sub adapter off and techs proceeded to remove pin with pin punch and sledge hammer. The tech held the pin punch with left hand on the end of handle and right hand on the handle near the punch. The sledge hammer glanced off of the punch striking employee's right index finger and knuckle.
Employee had a 16 penny nail in EE's right hand when EE heard EE's name called. When EE turned to see who had called EE, EE tripped over an air line and fell to the floor. The nail cut EE's left wrist requiring 7 stitches to close. Employee was released with no restrictions and returned to work.
Mechanic was tightening cutting edges bolts on RT022 with a 1"" impact. As EE was tightening a bolt, the impact slipped off of the nut and the side handle of the impact hit EE in the mouth, chipping EE's left front tooth
Employee sat down to close the bottoms of EE's trailers in water that was contaminated with lime. Causing a chemical burn.

While mounting track dozer employee lost his footing and slipped off the steps resulting in skinning up his shins.
Employee was climbing up a 7ft 6 inch stationary ladder and missed grabbing second to the top ladder rung. Employee fell approximately 3 ft. 6 inches to the grating deck.
While spraying polyuria employee got resin in right eye.
Windy conditions, blowing dust, goggles that did not fit properly on the sides. Driver was bottom off loading in windy conditions. A small amount of lime dust entered thru the side of his goggles getting into his left eye.
Using a pry bar it slipped causing Laceration to nose requiring sutures.

The mechanic was using a pry bar to remove a trip lever when the bar slipped and the mechanic fell to the ground onto his left shoulder.
When transferring some UNR829 Rudy inhaled some vapors.
An employee was using a hand drill when it caught and twisted his right hand. He returned to work but on 12-1-14 reported that it was still hurt. An X-ray on 12-2-14 showed he had broken a bone in his hand.
When stepping out of a pickup the employee stepped on a rock, turning his ankle.
Employee was cutting free a closed tarp from a dumped, side dump trailer. When the tarp was cut free the tarp bar sprung upward striking the employee in the nose. The employee sustained a broken nose.
EE were loading a flange & valve into the back of a p/u tk. Emp. stood on tailgate & moved the valve so tailgate would close. Valve was positioned between the side of the truck & flange. As he moved it his finger was pinched between valve & flange resulting in a cut finger. Dr took x-ray, the tip of finger was fractured & 15 sutures required. EE placed on restricted duty.
Mechanic was lifting a tire jack onto the flat bed of a truck. He felt a pain in his left shoulder and returned to work. He did not report the injury until the following day. He was seen by a doctor on 7-31-14 and was given a full duty release. On 9-5-14 he was given a light duty release in response to an MRI indicating a torn rotator cuff.
Employee was loosening a bolt with a crescent wrench on discharge boot on CIL #1. While loosening bolt employee felt a pop in his lower back which turned into a sharp pain that went through his lower back, hips and legs. Employee was taken to the Clinic and put on restricted duty.
2 Employees were installing hydraulic fittings and hoses on some valves for dump leach. The hose came out of the hands of Employee and hit him in the mouth. Employee was seen by the dentist.

Mechanic & welder were in the JLG man basket removing a leveling cylinder pin. Welder was repositioning the basket the machine was shut off & the boom started to settle resulting in the arm sliding forward. As the boom lowered it pinned the mechanic between the upper bucket & the back railing of the man basket resulting in an injury to the mech left ear. Seen at ER received sutures
Employee was placing bladder in laundry chute of #7 CIL and hurt lower back. Restrictive Duty effective 8/1/13.
While leaning over neutral header to pick up scale employee felt his ribs flex and a small twinge of pain that went away after about 30 minutes. Employee went on 4 days off. Reported it on 7/28/13 when he came back to work his ribs hurt when he tried to pick up or pull anything. Restricted duty starting 8/1/13.
Employee was tightening screw in starter bucket, screwdriver slipped off screw and he hit his hand on metal edge of mcc. Laceration to right finger - sutures required.
Employee was using an impact pun to remove nuts from studs on vessel piping. Stud spun & gun backed up pinning Rt thumb between gun & bolt on flange. Resulting in small cuts to thumb & some swelling. EE iced & returned to work. EE sought medical treatment on 7/15/13. Xray showed non displaced fracture. Restricted duty 7/15/13
Employee's foot slipped off wet step on grader while attempting to mount grader. EE fell downwards, hit her RT hip against the edge of grader blade, feet slipped in the mud and she continued to slide grazing her Rt shoulder against the grader blade. EE taken to clinic for x-rays.
Troubleshooting discharge line & bleeding gas to the pressure relief box. Flow was intermittent so he continued troubleshooting. Tech tapped desiccant tube when the tube failed. Supervisors responded to the noise. Tech was exiting the autoclave room removing his PPE. EE had lacerations on RT forearm & small cut on RT finger & RT side of abdomen. Tech reported ringing in his ears.
Employee was pulling up the cell 3 barge pumps and the cable being used snapped and hit employee in left arm. Contusions from bicep to forearm.

Mechanics were doing an outer feed end liner change out. They started knocking out liners & lifter from the outside through the bolt hole. After the liners & lifters were unbolted they used the steel rod to knock out stuck liner. While knocking out bottom liner with steel rod the center liner came loose & fell on steel rod, bent it & rod struck employee on nose causing a laceration
See Attached Was assisting other EE's in grinding on end of a spool. Co-EE heard a popping noise and the end of the pipe he was working on shifted. When he looked to the IEE he saw EE's head pinned to the concrete by the end of the spool (approx 2,700 lbs.) Co-EE's used forklift to hoist the spool off of the EE. 911 called rescue team transported EE to hospital.
Was moving railroad ties and cutting blades. About 45 minutes later employee was operating a backhoe and experienced extreme pain in his abdomen and upper leg.
Employee smashed his left thumb with 3 pound sledge hammer while driving a bushing into scam on differential. Employee did not leave work - first aid was provided. Emp. called supervisor at approx. 7:30pm and stated that his thumb didn't look good and wanted to go to the dr. Emp. was met at ER. Diagnosis was small fracture. Emp. was given RX and released to full duty.
Employee says he hurt his back while shoveling on Saturday on rig LK-40, there was no specific incident his back just became sore throughout the day.
Employee was working on TD850 with co-worker, they were tightening a bulk head fitting which required 2 people. While kneeling down to hold the bottom of the bulk head fitting his co-worker was tightening the top the crescent wrench slipped off the bulk head and chipped employees top front left tooth. Dentist capped damaged tooth.
Employee went to hit a center punch with a hammer, he missed the center punch and hit his left middle finger with the hammer. Employee was seen by the Dr at the clinic. XRay showed small chip on tip of finger, dr cauterized nail, tetanus was given & antibiotic shot. Employee was given a full release to return to work.
EE and co-worker were lifting a reel of 3/4"" hose off of the hose rack. EE felt a pop in his back and when he rose up he felt a pain going down his right leg and up the right side of his back. EE was seen at Northeastern Nevada Regional Hospital in Elko Nevada.
Emp was cleaning filter parts with grinder w/wire brush attachment. Someone called him he looked away. Grinder jumped causing the wire brush to travel across his left hand causing abrasion & punture to his left thumb knuckle. Emp examined by Dr Agbasi @ Golden Valley. Emp received Tetanus shot & couple sutures. Emp given instruction on keeping wound clean. Full duty release given.

While moving wear iron around in the back of FR393 service truck the mechanic got his ring finger on his left hand pinched between the iron and the left side of the bed.
Employee was loading lumber into the back of a pickup when he felt a pain in this right arm. Employee placed on restricted duty.
Hanging chock blocks on chock holder. Employees left thumb got caught between the chain attached to the chock blocks and the chock holder resulting in a fracture to left thumb. Employee placed on restricted duty.

Employee was walking from Midway building to the ready line, where his truck was parked. In between the South side of the midway and the ready line the employee slipped on a patch of snow covered ice causing him to fall on his left shoulder. Employee was placed on restricted duty.
Employee was exiting the cab of a shovel and caught his left middle finger in between the door and door jam which resulted in a fracture.
Employee lost his footing stepping over a containment wall, reached out to steady himself and received a puncture wound to his left hand from sharp corner of angle iron. Employee received sutures.
The employee was attempting to hitch a light plant to a pick up truck. The jack broke dropping the tongue of the light plant on top of the employee's right foot. The employee was evaluated by a physician and returned to his normal job duties. On April 14, 2010 after a follow up MRI it was determined that one of his metatarsals had a fracture.

While exiting a dozer the employee hit the upper portion of his face against the left door grab handle which pushed his safety glasses into the bridge of his nose causing a laceration requiring sutures.
Employee was stepping down off crane ladder and when he stepped to the ground he twisted his knee.
Employee was drilling out bolt holes in the frame of a dozer using a mag drill. Approx 20 seconds after positioning the drill magnet the drill demagnetized for no apparent reason and fell 12"" landing on the employee's left middle finger.
Employee was closing a valve under a tank (pulling up on the handle) when his left shoulder popped and he hit his head on a brace that was overhead.
Two employees were installing a temporary flange. One employee was holding the pipe up so that the other employee could bolt the flange. The pipe slipped catching the employees finger between the temporary flange and the original flange causing a laceration that required sutures.
Employee was operating a haul truck, when the dipper of the shovel near by made contact with the dove tail of the haul truck, causing the truck to shake. The haul truck driver complained of neck pain afterwards.
Two employees were unloading a 2 piece metal step from a truck. One piece of the step slid off and struck the employee in the mouth, resulting in a chipped lower front tooth. The tooth required dental repair.
Employee slipped on icy ground while exiting a van on the mine site. She strained her back and sought care from a chiropractor on 2/4/09.
Employee was walking across the bumper to get off the truck when he slipped on the VIMS lamp that was not bolted down he lost his balance and fell to the shop floor, he broke his right arm between the shoulder and the elbow.

Employee was working on the blast crew directing a scraper to the pattern and slipped on the icy ground. In the process of catching himself before he fell he twisted his lower back.
Employee was using a pry bar to separate the differential from the differential housing on a haul truck when the pry bar slipped resulting in a fracture of the left index finger.
During shift change employee was dismounting the bus and slipped and fell on the bus stairs. He landed on his elbow which ""jammed"" his shoulder.
The injured employee had pulled the electro-winning sump pump and cleaned the sump. When finished, he walked in front of the sump, as he did so, his right foot slipped causing him to fall backwards. He fell to the floor and in doing so, twisted his left knww. He was taken to to the doctor for further examination and was placed on restricted duty.
Welder was sliding two pieces of iron and his finger was in a hole on the top piece of iron. When the two edges came together the tip of his finger was caught in a pinch point.
Employee was washing a haul truck with a high pressure hose, when he tripped over a chock block and fell to the ground. Employee struck his left elbow on the ground, causing a laceration, requiring sutures.
Employee was cleaning the burrs off of a metal wedge with a bench grinder. The knuckle of his left index finger came into contact with the grinder causing a laceration that required sutures.
Employee was using a sledge hammer to pin the tracks on a piece of equipment. During the swinging motion he felt a sharp pain in his lower back.
Employee brushed up against a wheeled cart in the refinery cutting his leg on some slag at the front edge of the cart. This caused a laceration which required sutures to his upper left thigh.

Employee was operating a grader when his right foot went numb. He stopped the piece of equipment to get out and try and walk off the numbness. When he stepped with his right foot it ""rolled"" forward causing his toes to bend backwards resulting in a fracture to his #2 metatarsal.
Employee was using a grinder to prepare casing to be welded and a foreign body went into right eye.
The employee was doing a pre-op inspection of a shovel and slipped on the walkway while stepping over a cable. He hit his left shoulder on the radiator. He didn't seek medical treatment until 2/28/08 for the pain in his shoulder.

Employee was walking on a pond liner when he slipped and fell. He was carrying a lifting device which landed on his right hand when he fell fracturing his right ring and pinky fingers.
Helper was running an impact wrench to tighten a dog collar on pipe. Driller attemped to flip a toggle switch to turn on air compressor, but hit the rotation lever by mistake. The pipe rotated and pinched helper's right ring finger between impact wrench and wrench stop on the drill table. Fracture and laceration to tip of finger below the last knuckle.
Employee was performing maintenance on the circle driver of a grader. He was using an impact wrench to remove the lower shaft support from the main housing. He was removing bolts when a piece of the cast housing broke off and cut employee on the top of his left hand behind the index finger. The laceration required sutures and surgery.
Employee was unloading angle iron from a truck when his thumb got pinched between two pieces of iron.
Employee was doing maintenance on a pump when he got his finger caught between sheave and drive belt. This resulted in an open fracture requiring sutures.

He had scheduled days off and while on days off he called on 06/18/2007 and his shoulder still hurt so he went to get it looked at.

EE was cutting a 2"" plastic pipe with a chainsaw. He didn't cut all the way through the pipe, which put the saw in a bind, stopping the saw. EE tried kicking the pipe and his left foot slipped off, twisting his right knee and falling to the ground.

Employee was operating a scraper and was told to go help dozer operator who was covering 4 inch drain pipe. The pipe pulled away from the 8 inch connector. They removed about 2 wheelbarrows of dirt to make the connection. Employee was digging with a truck cleaning shovel. A co-worker stated that he did not see employee pick up the 4 inch pipe.
The employee was holding on to the edge of the door of a pickup speaking with the driver of another pickup who also had his pickup door open. A gust of wind blew catching one door blowing it into the other door. The employee's pinky finger was caught between the two doors. The employee suffered a laceration which required stitching.
Employee was installing structual iron deck, it hung up on mounting bolts, He was prying into place and caught hand under the iron.
Employee was using a 2# hammer and screwdriver attempting to unplug lime silo feeder. Hammer bounced of end of screwdriver, striking employee's right thumb against housing on feeder.

Employee pinched her arm between the arm rest and truck door. She didn't wish to seek medical attention at time of accident. It continued to bother her and in July was treated and released to full duty pending surgery date. On 9/12/06 employee had surgery to correct neuropathy and was taken off of work. It became reportable at this time.
While climbing on truck EE felt pain in lower back. He was treated and placed on restricted duty.
EE tripped over 3' waterhose after filling water tank straining his upper back. EE received vicoden from Dr making this reportable.
Employee was loading ore samples into bin on contractor's truck when he felt a ""pop"" in his shoulder. He was treated and released to Restricted Duty.

Employee was pulverizing samples and was turning the bowl over to dump out the samples and the bowl slipped out of her hand. She reached for the bowl to keep it from rolling off the table and in the process twisted her wrist. She was placed on restricted duty.
EMPLOYEE WAS IN CAB OF DOZER USING HAND SIGNALS TO DIRECT TRUCK TRAFFIC. HE HIT THE XM RADIO MOUNT WHICH HAD A ROUGH EDGE CUTTING THE END OF HIS FINGER ON IT. THE CUT REQUIRED 2 STITCHES.
Operating a Case farm tractor & disk, disking clag. Turned out of the clay raising the disk, turned in the seat to look back at the disk & drove over a rough area causing him to turn & twist in the seat. *Note: Seatbelt was being used.
WHILE USING T-POST DRIVER TO DRIVE IN GROUND ROD, EMPLOYEE LIFTED THE DRIVER MISSED THE ROD AND THE TPOST SLIPPED OUT OF HIS HAND & LANDED ON TOP OF HIS FOOT.
Employee knelt on floor to help position forks on a pallet lift. Later in the morning the employee noticed blood on the knee of his pants and found he had a laceration. Employee did not know what he might have knelt on. The laceration did require stitches.
