Fall to the walkway or working surface

Maintenance employee tripped on a catwalk toe board. Employee fell and struck the edge of the catwalk grating resulting in fracture to the tip (tuft) of EE's left thumb.

Struck against a moving object

Dump truck employee was driving, crossed a berm and rolled over resulting in a fracture to the posterior 12th rib.

Fall from machine

While standing at an elevated position, employee lost balance when a tight bolt on the air filter housing assembly loosened abruptly. Employee fell backwards approximately 2 feet onto the concrete floor, bracing self with left hand resulting in a fracture to EE's left wrist.

Employee tripped on a railcar hatch while atop the car. Hatch lip lacerated the employee's left shin resulting in 7 sutures.

Employee was changing belt on a locked out elevator motor. Employee lifted up weight on the belt and had the weight reset. Right index fingertip was lacerated when caught between belt and pulley, requiring 5 sutures.

Struck against stationary object

An employee was working on a Hammer Mill performing a rebuild. While EE was tightening a bolt, the wrench on the right slipped off the bolt resulting in EE's right hand moving upwards and contacting the sharp edge of the liner that was adjacent to the access door. The employee received a laceration on the top of EE's right hand.

Employee was troubleshooting a palletizer and put EE's hand on a motor terminal box to see if brake was engaging. Motor box jumped and caught in between top of motor box and metal guard. Employee suffered broken hand needing medical treatment.

Employee was using a shovel to push a piece of ore. The top edge of the shovel slipped off of the ore and the employee lost balance and fell, jamming arm/shoulder.

Employee was lifting the RSV in a team lift with another employee (to pallet height) when ee strained back.

Struck by falling object

Employee was performing post-op on Peterbilt truck. Employee opened hood and let hood free fall to resting position. Safety cable failed and hood struck employee on head and shoulder and hip resulting in a lower back strain.

Troubleshooting and Repairing 2F27 Blower - after making repairs and filling blower with oil stood up and backed away from blower while doing so fell backwards, while trying to grab nearby handrail to prevent fall, bent back or jammed right thumb.

Lifting/removing Rotex Screens to change screens. Strained back, did not report injury to shift or department supervisors. Reported to Safety Manager on 1/4/2011 did not seek immediate medical treatment. On 1/17/2011(15 days after incident) he then requested treatment. At that time physical therapy was administered thereby making this MSHA Reportable due to Medical Treatment Only.

Accident became reportable when found out from corporate office via insurance company that employee had received physical therapy. injured employee was clearing a plastic film jam up from the handlemaker on the Hamer Form Fill & Seal Automated Packaging Machine. Somehow his gloved right hand became caught in the top seal belt assembly causing crushing/cutting off injury to his right thumb.

Employee was to change bags in our A15 Baghouse. While taking hold of cage being handed to him, the injured got the cage too close to right earlobe. His earring became caught, when he released the cage to place in position it tore his earring out of earlobe. Medical Treatment Only Accident. Stitches/Sutures to reattach earlobe. NO Lost Time

Employee was collecting sample while holding bag on spout became distracted. In process of looking away finger was pinched by the bag holder clamp on packaging machine. This reflex action caused finger to hit a bolt resulting in laceration. Employee did not seek medical treatment at time of injury. Employee went for treatment 24 hours later which resulted in Restricted Duty.

Injured employee was filling 50 pound bags of diatomaceous earth powder then lifting & stacking onto a pallet for storage. While lifting employee felt shoulder muscle pull/strain. Reportable due to modified duty restrictions given.

Injured employee was checking the tension on the film feed belt of the Hamer automated packaging system while it was running. His left middle finger was caught in the tensioner pulley thus causing the injury. This was a violation of company policy/procedure.

Employee was standing in her office and blacked out due to a personal illness. She fell to the floor and hit her head on a piece of furniture and cut the back of her head and received six stitches.

Fall down stairs

EMPLOYEE WAS WALKING AROUND THE PLANT INSPECTING EQUIPMENT AND MAKING ADJUSTMENT TO F-20 FAN FOR PRODUCTION PURPOSES WHEN HE SOMEHOW STEPPED IN SOME GREASE. DURING THE ABOVE PROCESS HE THEN WE NT DOWN A FLIGHT OF STAIRS TO RETURN TO THE CONTROL ROOM WHEN HE SLIPPED ON THE STEP AND FELL APPROXIMATELY 6 STAIRS BEFORE CATCHING HIMSELF AND COMING TO A STOP.

EE WAS INITIALLY INJURED ON 5/22/02 BUT DID NOT SEEK MEDICAL ATTENTION. HE WAS CARRYING PINCH VALVES UP THREE DECKS OF STAIRS TO DO MAINTENANCE WORK. EE THEN SAUGHT MEDICAL ATTENTION ON 6/19/0 2 AND WAS GIVEN A 3 WEEK LIGHT DUTY RESTRICTION.HE HAS SINCE BEEN RETURNED TO FULL DUTY AS OF 7/15/02. EE WAS ADVISED TO USE OTHER MEANS FOR LIFTING.

Fall from headframe, derrick or tower

EE WAS ASSIGNED TO SUPERVACUUM FLOOR DRY FROM THE KILN FINISH TOWER. DURING THE COURSE OF VACUUMING THE HOSE LINE PLUGGED. THE PRODUCTION SUPERINTENDENT NOTICED THIS AND WENT TO GIVE ADVICE ON HOW TO PREVENT. IT WAS NOTICED THAT THE HOSE WAS TWISTED SO THEY BOTH WORKED TOGETHER TO STRAIGHTEN OUT HOSE AND HE WALKED OFF ROOF BACKWARDS.

EMPLOYEE WAS CLIMBING UP A LADDER TO INSTALL ELECTRICAL WIRING. THE LADDER SLIPPED OUT FROM UNDERNEATH HIM, CAUSING HIM TO FALL FROM APPROX 7 FTHIGH. EMPLOYEE LANDED ON HIS RIGHT SIDE INJURING HIS HAND AND HIP BUTTOCK AREA.

WHILE PACKING FLOOR DRY (DIATOMACEOUS EARTH), REMOVED BAG FROM PACKER AND SOME FLOOR DRY BLEW UNDER MY GLASSES (SAFETY) AND GOT INTO MY RIGHT EYE. USED EYE WASH FOR APPROX 20 MINUTES. THEN ANO THER 20 MINUTES WITH SUPERVISORS ASSISTANCE. DECIDED TO GO TO HOSPITAL DUE TO IRRITATION. **PRESCRIPTION MEDICINE/ANTIBIOTIC GIVEN**MEDICAL TREATMENT ONLY**NO LOST TIME**NO RESTRICTED DUTY**

REMOVING BOLTS FROM AIR LIFT IN ORDER TO CHANGE THE LINER. USING 1/2 INCH IMPACT WRENCH, WHEN IT SLIPPED SMASHING MY FINGER BETWEEN THE WRENCH AND THE AIRLIFT. FOR PREVENTATIVE/DIAGNOSTIC EVAL UATION MEASURES EMPLOYEE WAS SENT TO CLINIC FOR XRAYS. RESULT - NOT BROKEN. **RESTRICTED DUTY ONLY** INJURY REPORT.

EMPLOYEE WAS ASSIGNED TO GET ON TOP OF A RAILROAD HOPPER CAR IN ORDER TO HOOK UP LIDS AND BULK HOSES. ONCE LIDS AND HOSES ARE HOOKED UP WE PUMP FULL OF DIATOMACEOUS EARTH POWDER PRODUCT. IT AP PEARS THAT EMPLOYEE FELL FROM THE LADDER ATTACHED TO RAIL CAR A HEIGHT OF APPROX 6 FT TO THE CONCRETE GROUND.
