Surface Mine Engineering Sampler was backing up his flatbed pickup on a a drill pattern. A Drill Operator had dismounted his drill and was walking on the pattern. The drill operator walked behind the pickup and the sampler was not aware the the driller was there. The driller stopped walking and was run over by the flatbed pickup, resulting in fatal injuries.

On 10/17 ee working with other ee unloading cement chute from pickup. While moving it, ee felt cramp in lt arm, and coworker reports ee ""exclaimed loudly"" and let go of chute. Ee said EE felt pain in EE's arm. Ee did not report this to site until 10/23. Seen at clinic, diagnosed with possible biceps tendon tear, referred to ortho.
EE reported reaching up and pulling bag of rags weighing approximately 25 pounds and felt pain in shoulder. Seen at clinic, diagnosed with strain, released with restrictions. After medical and factual investigation, workers' comp accepted claim on 4/17.

Individual was in the process of moving the hammer assembly from the truck bed to the rod rack. When doing this individual fell same level to the ground and the hammer inter change landed on their leg. Change of status 05/11/2023.
Ee was opening cab door on equipment. EE had to push hard on the button, and EE's thumb slipped, jamming against the door with force. Ee reports feeling pop in EE's thumb. Seen at clinic, diagnosed with a flexor tendon injury to right thumb, released to restricted duty.
Ee was getting fuel and coolant put in machine. Walking to hand fueler the radio, slipped and fell on back and shoulder. Seen at ER, diagnosed with lumbar fractures, referred for a spine consult. Ee put off work.

Ee reports that ee spotted incorrectly into lime silo, then rubbed eye with lime on it from the handrail, resulting in exposure to lime in eye. Seen at an eye clinic, diagnosed with a corneal abrasion to the eye, prescribed meds for the eye and released with restriction that site can accommodate.
Ee reports driving truck through the dump, hit a pothole, and the air seat ""went out"". Ee reports shooting pain in right hip, numbness to outside of right leg to calf, and tingling in right foot. Seen at clinic, diagnosis lumbar disc disease with radiculopathy. Released to modified duty that site can accommodate. Maintenance confirmed seat did not malfunction.
Ee was jostled in the cab of EE's haul truck when shovel operator dropped a load. Seen on 8/31 for initial visit, diagnosed with a lumbar and thoracic strain, released to full duty. On 9/12/22, ee referred for physical therapy, making this reportable for treatment.
EE reported stepping off platform step of blue room, tweaking their left knee. Claim initially denied as work-related. EE appealed denial and following medical investigation, claim accepted by workers' compensation on 10/27/22. EE lost time following surgery.
Ee was sitting in a pickup on site. A rubber-tired dozer backed into the pickup, jarring the ee. Seen at Golden Health, diagnosed with back pain, and released to restrictions for days off. On follow-up prior to EE's next scheduled shift, 5/26, EE was diagnosed with PTSD, prompting this report for the illness.
EE was getting lime at North Area lime silo. When driving away from the silo, they noticed a burning sensation in eyes and started to cough. EE reported the issue and was seen at health. Diagnosed with chemical conjunctivitis to left eye, scheduled to follow up 4/29. On follow-up EE was released to full duty effective 4/29. Prescription for eyes given.
EE lifted an 80 lb penco into back of pick-up when EE felt a pop in EE's right calf. Slid the object further into back of truck, and was unable to walk. Fell on the ground and had a small bruise. Did not request treatment until 4/26. MRI on 4/27 identified tendon rupture resulting in this report.

Ee lifting track pin to wedge in place to keep track from rotating while setting pressures on pump. Pin started to fall, ee let go and smashed tip of right pinky finger between the pin and track. Seen at hospital, diagnosed with right distal tuft fracture. Splinted, released to restricted duty.
Ee reported being jostled in cab of haul truck when rock hit the headache rack. Seen at clinic and diagnosed with thoracic and lumbar strain. On 2/12 investigation complete and claim accepted as occupational making this reportable. Ee released to restricted duty.
Ee was working with another ee to install a transmission guard. Guard caught on grease lines; ee reached in with left hand to clear grease lines out of the way and the guard slid forward, pinching the ee's left thumb between the guard and steel plate. Seen at ER, referred to ortho. Diagnosed with a fracture and returned to light duty that site can accommodate.

Employee stepped up with the right leg to access a drill at the start of shift. The first step was about 20 inches from the floor. When the ee stepped up, they hyperextended right knee and had immediate pain. Seen at ER, released to light duty.
The EE was helping at the table to trip out drill pipe. The EE grabbed the wrench at the table and the driller moved the hydraulic cylinder pinching the finger between the table bushing and the wrench handle.
Driller and helper removed rotation guard to straighten it. While removing it the top half of the guard folded down and pinched Driller's right thumb. Driller had cut-5 gloves on. X-Rays showed no damage to right thumb. The doctor cleaned and wrapped the wound and prescribed an antibiotic. Employee returned to work with no restrictions.

CAT 16m motor grader made contact with coach bus resulting in bus passenger sustaining neck strain. Company was notified on 03/19/2019 that the injured employee had attended physical therapy that was recommended by the Dr. on 2/27/19 making this reportable.
While walking to hot change van, Employee slipped on ice striking head on ground.

On 11/15/2018 the employee stepped out of the service truck and felt a pop in left knee. This injury became reportable on 01/18/2019 when the knee became painful and could not bend it without more pain. The Dr. put the employee off work.
An employee was checking for air flow in the HVAC system of a CAT grader. EE reached into the fan housing while the fan was running and contacted it with EE's right thumb.

Operator reported pain in right index finger. Initial medical evaluation on 4/12/2017 indicated right extensor tendonitis. Operator was released full duty on 4/12/2017. Operator went in for surgery 8/8/2017 missing 2 scheduled work days which made this a reportable illness. Upon internal audit 7/2018 it was found that a 7000-1 was never completed for the illness.
Driller went to inspect drill rod removed from drill string that was cut and reaching finger inside pin end, cut left index finger.
Tech got finger caught between a belly pan and bolt. Pinching the left index finger. The tech in an attempt to remove the nut from the bolt pressed on the washer with their fingers to bind the bolt, while removing the nut form the bolt. Causing damage to the index finger just above the first knuckle.

While tripping pipe onto the Rolagon, employee was on the Rolagon sliding the 10 foot RC rods into place. When the employee reached down and pulled on the pipe ee felt a pull / strain in right shoulder.
At approximately 2308 on Wednesday, October 5, CAT793D HT953 right rear inside tire caught fire. HT953 was traveling loaded South on the North/South Haul Road just north of the Oasis water stand.
Employee was trying to loosen up a bucket tooth on LD466 (992 Caterpillar front end loader) when a piece of metal 3/8 inch by 1/8 inch came off Snap-on steel faced hammer and struck employee near right collar bone. Transported to medical facility where piece of metal was surgically removed.
Haul truck 930 traveling from the 45 stockpile, section 3 area towards the North/South haul road went through the water bar and slowing down for a stop sign, the operator and trainer heard a loud bang which startled the Operator. The Operator reported some pain in lower left back after the event had occurred. Operator was seen by a Nurse Practitioner and placed EE off work.
Operator on TD856 called and reported that they had pulled a muscle in arm. Employee was lifting a 70 lb drill bit to place on drill rod. This injury became reportable on May 24, 2016 when employee received a restricted duty release during a follow up Dr. visit.

The 13,800 to 4,160 volt transformer failed causing it to catch fire in the primary side cabinet. The feed to the transformer tripped but, the fire continued to burn the insulation on the wires. Once it was confirmed the power was isolated the fire was extinguished by our MERT team.

While changing filters on WT029 the mech. stepped off the boarding ladder and felt a pain in his left knee. Employee worked full duty until 3/23/15. At which time he received restrictions.
Employee was repositioning a 4 inch hose and felt pain in his lower back. He was subsequently seen by a doctor and taken off work.
Employee was conducting normal sampling duties and felt pain in his shoulder. He went to the doctor 07/17/14 and was prescribed physical therapy.
As an employee got up to exit the cab he was turning to the right to walk around the pilot control shut off lever, he felt a sharp pain in the back of his knee.

EE was turning truck off @ the truck wash @ North Area Newmont when he turned to walk from the truck when his right knee gave out.

While walking on a steep grade of the descending ramp,the employee pulled a muscle in his left calf.
employee was taking the left track off on a D10N and had removed the master pad and bolt. he grabbed a hammer with his right hand and put his left hand on a track pad above the master link and when it came apart the track pad came together pinching his left index finger,causeing the employee to received 3 sutures
Drill assistant put the breakout wrench on the table to adjust it. When he did this, the driller went out with the breakout wrench and pinched the driller assistant finger between the handle of the wrench and the table.

Haul truck operator reported feeling some pain in his lower abdomen after boarding his haul truck and that he had a similar pain the previous shift while exiting his haul truck. Worker was fueling a CAT 777D Haul Truck The wiggins kicked off indicating the tank was full. The worker went to remove the fuel hose and was sprayed with diesel fuel. He was wearing full wrap safety glasses, but the fuel made contact with his eyes and clothing. The sup. flushed his eyes with saline solution.
While walking through the line out room the employee(custodian)went to go down the steps in which she missed the steps and landed on the ground and fractured her ankle.

While working on a rubber tire dozer I stepped from the top of the transmission platform to the left side deck. When the employee placed his right foot down he twisted his knee & fell down on the deck. Injured was loading drills onto the truck, the drill slipped and he caught his middle finger between the drill and the ground. It smashed the top of the middle finger, it did not require stiches but was cleaned and bandaged up.
Crew was tightening plumbing on the tophead with a 60 inch pipe wrench and a chain come-along. As helper was tightening the come-along, the handle of the pipe wrench broke and struck the helper on the hardhat. The blow to the head caused the hardhat suspension to cut his head. The laceration required 8 stitches. Laceration and mild concussion.
Employee (Mechanic) was attempting to free the power step on a hydraulic shovel due to it being damaged. The employee was standing in the basket when it was freed and the step free fell to the ground. As a result of the free fall the employee struck his face on the basket requiring sutures to close the cut.
While closing the door on a track drill the operator inadvertantly closed his finger in the door. The finger required sutures to close the wound.

Employee was holding the door on a van for exiting coworkers. Another employee, not knowing EE's hand was in the way, closed the door and smashed EE's pinky. Eight sutures were applied.

Haul Truck #1 staged to pull under loader (bed facing loader); Haul Truck #1 began backing to the loader, and contacted the bed of Haul Truck #2. No damage to the haul truck; minor injury to the driver of Haul Truck #1 (two stitches to left side of scalp).

WORKER EXITED HAUL TRUCK TO PERFORM AN ""AFTER LUNCH"" EQUIPMENT INSPECTION. WHILE PERFORMING THE INSPECTION, HE STEPPED ON A SMALL ROCK,""TWISTING"" RIGHT ANKLE. HE REPORTED THE INJURY TO HIS IMMEDIATE SUPERVISOR, BUT CHOSE NOT TO SEEK IMMEDIATE TREATMENT. APPROX 10 DAYS LATER, HE CHOSE TO SEEK TREATMENT FOR THE INJURY RESULTING IN THE APPLICATION OF A SPLINT.
Employee was climbing down the ladder on the the side of the pipe truck. He slipped and fell from bottom step. He strained his back muscle when he fell. Employee worked the remainder of shift and 3 more days. Went to doctor on 4th day because soreness was not getting any better.

Crew was tripping back into the hole after a bit change. As a piece of drill pipe was being picked up the metal on the clamshell broke. When the clamshell broke the tension on the cable caused it to kick back and hit employee in the left cheek, causing laceration to edge of nose. Stitches required.
Employee was tripping a drill hole and strained lower back muscle while handling drill pipe.
Employee was on the deck of the drill performing routine maintenance -greasing the traveling shieves. His right foot slipped and he fell backwards and to the right and landed on his right hand.
Employee stepped down off of a dozer and missed the bottom step because his bifocal lenses appeared to offset the step. While breaking his fall with his right arm he strained his shoulder. He sought medical attention on the morning of 10/04/04 and was given a shoulder sling and given one day light duty.
Employee was removing a rear hub from a pick up with a pry bar and strained his right shoulder. He sought medical attention on 11/03/04 and was given one day light duty.

Employee was hooking a track into position on a dozer when the fork of the forklift slipped hitting the handle of a hammer. The handle flipped up and hit the employee on the nose.
Employee was in the process of handling a 54lb. box of blasting boosters. As the box was falling off the side of the truck, employee attempted to catch the box which resulted in torn bicep muscle tendon. He did not seek medical attention until 05/24/04 and is scheduled for surgery on 05/28/04. He will become a lost time beginning 06/01/04.
Emloyee was stepping down off a piece of equipment and slipped on the bottom step straining his knee. He was placed in immobilization device for three of his scheduled days off. He was released to full duty at that time. He returned to work on his next regular schedule shift not requiring the brace.
EE WAS STEPPING DOWN OFF A DOZER AND STEPPED ON A ROCK TWISTING HIS RIGHT ANKLE. HE WAS GIVEN A WALKING BOOT FOR IMMOBILIZATION AND 2 DAYS LIGHT DUTY. HE WAS INSTRUCTED TO WEAR THE WALKING BOOT FOR THE 2 DAYS OF LIGHT DUTY.
Employee was cutting off the end of a 42"" plastic pipe. A small piece was left and holding the pipe together. When it broke free the pipe twisted and pinched and cut his thumb. He received sutures to close the wound.
EMPLOYEE WAS SHOVELING SOME MUD OFF A LINER, FOR A POND TO REPAIR. HE SLIPPED AND EXTENDED HIS RIGHT HAND OUT TO CATCH HIMSELF.
Employee was moving a control wire from one terminal to another. The wire somehow touched the top of a breaker and flashed over.
EE SLIPPED ON THE ICE AND FELL ON HIS RIGHT KNEE. EE MISSED ONE DAY OF WORK AND WAS RELEASED TO FULL DUTY BY A PHYSICIAN.

While removing a bed pin on a haul truck, struck the pin with a hammer and the pin flew out unexpectedly. Employee attempted to deflect the pin so that it would not strike him or his partner. In doing so resulted in a shoulder strain. This injury occured on 11/02/03, but was taken off work for surgery on 05/25/04.

SOMETIME DURING THE SHIFT, WHILE THE EE WAS GRINDING , A SMALL PIECE OF METAL GOT LODGED IN THE PUPIL OF HIS RIGHT EYE. OPTOMETRIST REMOVED THE OBJECT AND GROUND AWAY THE RUST RING.
WHEN REMOVING FROMS FROM A WALL, HIS POSITION CHAIN SLIPPED & CAUSED HIS ELBOW TO STRIKE CONCRETE WALL. REP'D ACCIDENT TO SUPERVISOR 3/25 REQUESTED TO SEE A DR. XRAYS SHOWDED NO PROBLEMS DR CONCLUDED FORM OF CARPAL TUNNEL. ISSUED FULL WORK RELEASE RECOMMENDED NO REPETITIVE MOVE WITHARM. 3/14 EE SAW DR. NOT RELEASED FOR WORK

EE WAS ASSIGNED TO A TASK REQUIRING HIM TO PERIODICALLY WORK IN A CROUCHED POSITION OVER SEVERALDAYS. HE DEVELOPED PAIN IN HIS LOWER BACK THAT COULD HAVE POSSIBLY BEEN RELATED TO THIS TASK. THIS RESULTED IN LIGHT DUTY.
WHILE SCRAPING OFF A LABEL, THE SCRAPER SLIPPED CUTTING HER HAND REQUIRING FOUR SUTURES TO CLOSE THE WOUND.

EMPLOYEE WAS FILLING 60 WT OIL BIN IN THE ALMP BAY, THE OIL OVER FILLED AND RAN ONTO THE FLOOR. EMPLOYEE PUT FLOOR DRY AROUND THE SPILL, AS THE EMPLOYEE WAS CLEANING UP HE STEPPED IN OIL AND SLIPPED TWISTING HIS BACK. LATER IN THE SHIFT EMPLOYEE FELT A MUSCLE SPASM IN HIS BACK. EE WAS SEEN BY A PHYSICIAN AND LATER BY A CHIROPRACTOR.
EMPLOYEE HAD SURGERY ON BOTH WRISTS. THE MEDICAL FINDINGS CONCLUDED THAT IT WAS ATTRIBUTED TO REPETITIVE WORK AS A MECHANIC. THESE FINDINGS WERE NOT AVAILABLE UNTIL RECENTLY. EMPLOYEE HAS NOT RETURNED TO WORK AND HAS BEEN OFF SINCE 10/10/01. EMPLOYEE HAS BEEN A MECHANIC SINCE 3/7/88.

CONCRETE WAS LODGED IN THE RIPPER CYLINDER MOUNT AND THEE USING A PRY BAR AND A 12# HAMMER TO DISLODGE THE CONCRETE. WHILE STRIKING THE BAR WITH THE HAMMER THE HEAD OF THE HAMMER STRUCK THE CY LINDER DEFLECTING THE HANDLE INTO THE RIPPER SHANK CAUSING THE LACERATION TO THE FINGER. SUTURES WERE REQUIRED.

ACCIDENT COULD HAVE TRANSPIRED WHILE WORKING ON DOZER TRACK FRAME. EE WAS USING HEAVY HAMMERS AND PRY BARS. THE EE DID NOT EXPERIENCE THE PAIN UNTIL AFTER THE SHIFT WAS COMPLEAT AND HE WAS ON HIS WAY HOME. ACCIDENT DID NOT BECOME REPORTABLEUNTIL JULY 11 WHEN EE HAD SURGERY..
