Pit #3 West CullomMetal/Nonmetal

Employee was lifting an auger bit on the rack and dropped it. It pinched/crushed the tip of left ring finger.

Employee was washing the pad under and around the plant. EE stood up and hit head on the frame of the plant. The angle iron support caused a laceration to the right eye/forehead.

Getting off the haul truck and stepped down on a 4"" X 4"" piece of wood twisting his right ankle. Declined medical attention until 3/15/12. X-rays negative, 2 days restricted work activity.

The plant was shut down at 3:30 pm because of a rock in the impeller on the dredge. After this task, 2 miners were at the controls in the plant electrical trailer to start the plant back up at 4:00 pm. It was within a few seconds of pushing the slurry pump start button, that the 2 miners heard what sounded like an arcing sound & then a loud bang

The EE slipped while climbing down the dozer steps bumping his head & right leg on the dozer ladder, resulting in a bruise, received medical attention as a precaution.

The employee was walking through the plant area and stepped on ice covered with snow. He slipped and fell on his upper back causing a contusion and strain. He was given a RX.

Employee states he was in a dusty area when debris flew into his left eye. He does not know when it happened, he was wearing safety glasses. Scratched cornea, Rx.
