Strain to lower arm requiring splint for 2 weeks. EE was lifting bags of anfo picking bags up off of the ledge. Grabbing bags with EE's left hand to load the holes. Noticed it about 6pm at home after work. Couldn't get comfortable when EE tried to go to sleep. This morning EE couldn't pick up an orange. Has a hard time opening EE's hand

Employee was ascending a set of stairs leading up and into a trailer. EE tripped at the top step and fell onto the hand rail. The end of the hand rail had an exposed burr that cut into the employees arm requiring 5 stitches.

Employee first reported on Oct. 25 2016 that EE has been having pain in EE's elbows (joint and muscle) for a while (more constant since around Oct. 3) from EE's Haul Truck position. The injury was evaluated and on December 1st, was determined to be workers comp.
Employee was working with the crew changing out v-belts and hand slipped resulting in one broken finger and one fractured finger on right hand.

EE reported that while running the dozer he backed over a frozen dirt clod which caused him to jar his back. Health Bridge was contacted. EE was informed by Health Bridge that home care was all that was needed. Ice and Ibuprofen was recommended. January 5th EE saw a doctor and was given a restriction of not operating the Dozer for 2 weeks.
