Forklift turned over - The miner was operating a forklift, traveling on a decline toward the mine entrance, when the forklift went out of control. The forklift struck a concrete support for the belt conveyor and overturned, killing him.

Employee was injured when they fell walking to the troop carrier. Injury became reportable on 11-18-25 when an orthopedic specialist diagnosed a left shoulder partial rotator cuff tear and left shoulder proximal humerus fracture. Physical therapy was also ordered.
Employee received a laceration from a screwdriver while reaching into a toolbox, The laceration required stitches.

Employee was reeling up a hose on a reel when employee felt a pop in elbow. After seeing an Orthopedic Specialist, physical therapy and restrictions were prescribed on 02/05/2025 which made it reportable.
A break in a slurry line caused an inundation of water in the mine. This condition was discovered on a regular workplace exam before the beginning of the shift. Only two miners were in the mine performing the workplace exam and they safely evacuated to the surface. There were no injuries.
Installing panels using a clip gun. When the clip gun misfired while employee had their hand in the wrong place.
Employee contacted the left elbow on the windowsill of the haul truck while getting loaded by a loader. Determined to be reportable on 12/04/2024.
Employee strained left side trapezius while advancing carousel on Fletcher bolter.

Employee climbed ladder to enter loader and felt pain in right knee. Injury became reportable 18 Sept 2023.
Employee was opening a door on a pickup, when a gust of wind caught the door. The employee held onto the door straining their left shoulder. Employee reported the injury 03-17-2023.

While operating the scaler, employee stated that the wrist was injured due to jarring the machine. Following an investigation, with company management and our work comp provider, it became reportable on 8-13-21.

During a pre-shift of the Gradall scaler, the employee was leaning against a chain handrail. The chain broke and the employee fell backwards approximately six feet to the ground.
While lifting lunchbox up onto loader catwalk, employee felt a pop in left knee. The injury did not become reportable until 6-3-21 when the investigation and MRI showed that the injury was work related.
Employee's fingers became pinned between the bearing and tub of the log washer while doing maintenance. Weight shifted of the bearing during task and fractured the two outer fingers on the left hand.
Employee was operating a scaler underground. The scaler pick broke free dropping the scaler boom and jarring EE's back. Became a ""Restricted Duty"" injury 8-13-20.

Employee was assisting a mechanic reattach a dust collector hose for the bolter when employee noticed pain in shoulder and stopped the task. Employee went to a physician who prescribed anti-inflammatory and physical therapy for a strained shoulder.
While changing screens another employee was handing a screen off to the injured, when it slipped striking injured employee in the elbow injuring the elbow. Employee sought medical treatment and was given light duty work for a week and medication to allow the elbow to heal.
Employee states that on or about July 12th, 2019 employee was operating a loader with a bad left armrest that caused injury to arm. On November 21st employee saw a doctor who gave off duty restriction and MRI as a followup due to work related injury. MRI shows torn rotator cuff.

Employee threw paper in the trash and missed. EE rolled over with the chair to pick it up. When reaching down to pick it up the chair went out from underneath EE. EE fell on shoulder injuring it. Initially evaluated by physician. Recently rotator cuff surgery scheduled for 12/26/18 triggering a lost time reportable

Employee was using a scaling bar to scale down loose material. The loose material then caught on the scaling bar causing the bar to hit the employees leg/knee requiring 6 stitches to their leg.
After exiting the cab of the underground drill the operator walked on uneven ground and rolled their left ankle. Ice was applied on site. The next day the operator went to the doctor and was placed on light duty for a sprained ankle.
EE was operating a 272 CAT skid loader to remove snow from customer scales and contacted hard hat on the inside of the cab, resulting in the hard hat contacting above left eye. This caused a laceration requiring stitches.

At the end of employee's shift, morning of 9/16/16, of operating a loader, employee parked the loader and set wheel chocks. After placing last chock, employee took 2 steps and experienced the pain in back. Employee stated had to stop multiple times due to the pain and wasn't sure was going to make it to the break room. Employee clocked out and went home.

At approximately 9:30 AM employee started the pump up and the hose was kinked, while he was getting the kinks out of the hose it fell out of the truck. Employee walked down the ramp to the pump to shut it off, he was in hurry to shut the pump off. He tripped over a small rock and landed on his left knee.

TECHNICIAN WAS LIFTING AN ENGINE FLYWHEEL OUT OF A FLYWHEEL HOUSING. OVERCOMPENSATED FOR THE WEIGHT OF THE FLYWHEEL AND LACERATED THE TOP OF HIS RIGHT HAND ON THE FLYWHEEL HOUSING, REQUIRING 3 STITCHES.

Employee was walking, looked up for a moment and fell down, resulting in a broken left wrist.
EE was loading boxes of resin to roof bolter when he felt a ""pop"" in shoulder. EE was treated on site but continued to experience pain. EE was seen on 11/19/13 at Occ Med clinic diagnosis was possible rotator cuff tear. MRI was completed on 11/21. No tear was found. EE was prescribed 6 sessions of PT starting 11/20/13. EE continues to work regularly scheduled hours.
EE was kneeling while making repairs to a BTI powder machine. The EE attempted to shift from his knees to a sitting position and felt something ""pop"" in his knee. Initial diagnosis was knee sprain. MRI later confirmed a tear.
EE was attempting to throw a rock approx. 2' x 1'x 4"" behind him from the face when the rock caught on his glove causing his right hand to contact another rock on the ground breaking his right ring finger.

EE was preparing to burn empty explosives boxes and was moving a cover over a small burn pit. EE caught his thumb between the cover and the pit resulting in a laceration requiring 5 stitches.
At approx. 1:00a.m. an area of shotcrete fell from a rib at CC75 between room 24 and 25. The area is approx. 25'x50'. No EE's were present when fall occurred. The affected area was barricaded to prevent entry and an outside contractor was called to determine corrective measures.

EE was catching a sample and reported a pain in his left elbow. EE was terminated on 9/6/11. During separation from company ee received physical therapy, ee was reinstated on 9/21/11 and made whole.
Was operating a haul unit in mine transporting a load of rock from the face to the primary crusher. Drove over 2 dips in the road causing the seat to bounce and allowing ee to strike his head on cab. Ordinal diagnosis was neck strain and returned with no restriction. EE continued med evaluation & was seen by a specialist on 7/14/11 with recommendation for 3-4 PT treatment.
Was working on Gradall Scalar installing cab mount bolts. EE was standing under the cab tightening bolts. Socket needed to be slip past some hoses that ran through the area were the bolt was located. After ee had tightened the bolt he was attempting to remove the socket when it was caught between the hoses and dislodge from the extension striking ee on front tooth.

EE was fixing an electrical control wire for the manlift on the BTI powder truck. EE dropped an electrical connector to the ground & crouched down to retrieve it. When he stood up his knee popped. Initial diagnosis was torn cartilage. MRI later revealed a torn miniscus. EE continued to work until 06/28/10 when he had surgery. EE returned to light duty on 06/29/10.

EE reported he was having problems with his wrist and wanted to have it looked at. EE was seen by doctor and received surgery on 02/12/2010. Employee returned to work on 02/15/2010 to light duty.
Employee lowered the manlift of the roof bolter to the mine floor. Employee stepped out of the basket and felt his knee pop. EE reported it to his supervisor aproximately 3 hours later. Diagnosed as strained ligament. EE stating his knee had been bothering him for about a month prior to this.

Employee was helping another employee work on drill and was holding the brackets up when he felt a tingling in his left arm and was diagnosed with a left shoulder strain needing physical therapy. See attached interview document. Incident was not reported to mine management until the evening of 12/02/2008. Claim is being disputed.
While operating a scaler, the employee received a hard jolt that caused the seat to bottom out resulting in a lumbar strain.
EE was descending ladder on 5110 Gradall Scaler. When his right foot stepped on the mine floor his Achilles tendon broke.

EE was putting a spacer on the scaler. The spacer fell and struck the EE on the finger. EE reported injury to mgmt on 10/01/2007 but refused medical treatment. On 10/11/07 EE requested to see a doctor. Initial x-ray was inconclusive. Read by radiologist and determined to be a minor fracture.
Employee was scaling using a Getman scaler. A hydraulic suction hose ruptured causing the boom to suddenly drop jarring the employee's neck.
Subsidence that occurred in XC7, R6 of the west side of the mine. A sinkhole was found by the land owner and mine management was notified. The hole was approximately 25' x 40' and went to the mine floor.
Employee fell off engine compartment of Getman scaler while servicing unit. Employee struck his leg on the catwalk and landed on the ground. Diagnosis was POSSIBLE rib fracture. Released to full duty. On 04/02/2007 employee heard a pop and felt increased discomfort in rib area while operting a haul truck. Treating physician then diagnosed rib fracture.
Employee was loading a box of stick powder weighing 55 lbs onto the powder truck. EE used his right elbow to push open the door. Later on in the day he began to feel pain. Diagnosis was tennis elbow, temporarily reassigned and given home therapy exercises. Pain continued and was referred to PT on 02/02/2007. EMPLOYER knew about PT on 02/16/2007.

Using chop saw to cut all-thread, blade bound kicking material back and pinching right thumb in between all-thread and more material to be cut. Resulting in a broken right thumb with two fractures and 6 to 7 stitches.
Employee was helping set a 10"" pump hose, he slipped and fell backwards and the hose strainer landed on his left hand breaking the tips of fingers 3&4.
On the night of 1-27-06 we had our mine evacuation drill. I went to pick up 2 co-workers to take them out of the mine. One of them went to get out of the drill and stumbled and fell to the ground. He got back up and into the pickup. I asked him if he was alright and he said yes. Nothing was mentioned again until 3-12-06. He had surgey on 5-4-06.

THE SECONDARY RETURN CHUTE WAS PLUGGED WITH A BIG ROCK. THE EMPLOYEE WAS USING A 3 LB HAMMER TO BREAK THE ROCK OUTSIDE OF CHUTE. HE QUIT AND WAS DIGGING OUT WITH HIS HAND AND I WAS ON THE OTHER SIDE OF THE CHUTE DIGGING ROCKS OUT. HE PICKED UP THE HAMMER AND SWUNG NOT KNOWING I HAD A HOLD OF THE ROCK HE WAS GOING TO HIT AND SMASHED THE MIDDLE FINGER OF THE RIGHT HAND.
EE rolling up fuel nozzle on fuel truck when nozzle got hung up on the hook, reeled it up and it shot off hook slammed his finger in door causing 7 stitches to left middle finger.
SHOULDER SPRAIN AS A RESULT OF USING GREASE GUN ON EQUIPMENT. SURGERY EXPECTED ON 1/21/05
