Struck against stationary object

Crew was spooling in the dredge anchor cable using the winch. The cable started to become off track of the spool when the employee placed their right foot into the cable to prevent it coming off the spool completely. The employee reports their foot slipping off the cable driving their heel into the decking/floor of the dredge causing immediate discomfort to the right ankle.

Employee was cutting some rubber and the knife slipped off the rubber hitting his hand. Knife cut approximately 1 inch laceration in right hand near the thumb.

Struck by flying object

The EE had finished welding in the shop, when the wind blew a particle into his left eye. He could not flush it out. Medical facility had to remove a small piece of metal, given RX.

A customer drove his pickup truck onto scale to get an empty weight. He got out of the cab and walked around the pickup instead of walking to the end of the scale as the signs say to do. He tried stepping over the scale side rail, it was snowing and he slipped and fell on the rail.

The employee was welding an ibeam the steel i beam was rolled over to access the other side, the beam came loose from a chain and fell, about 2 feet onto his left foot. His left big toe was fractured behind the steel toe in his boot. This resulted in a lost time injury

WHILE EMPLOYEE WAS WORKING ON SOME DISASSEMBLED CRUSHER EQUIPMENT BROUGHT INTO THE SHOP, HE SLIPPED AND STRUCK HIS LEFT ELBOW ON THE BOWL THREADS, CAUSING A LACERATION AND CONTUSION.

THE EMPLOYEE WAS ASSISTING WITH THE INSTALLATION OF A REPLACEMENT TAIL PULLEY ON A CONVEYOR. HE REACHED DOWN TO GRAB THE SHAFT TO POSITION IT WHEN IT SLID OVER AND PINCHED HIS LEFT HAND.

Struck against a moving object

THE EE WAS SITTING IN DUMP TRUCK BEING LOADED WITH SAND/GRAVEL. THE LOADER OPERATOR DROPPED THE MATERIAL INTO THE DUMP BOX TOO HARD, WHICH CAUSED THE EE'S BACK TO POP AND LOSE SOME FEELING IN HIS LEFT LEG FOR A LITTLE WHILE. EE WENT TO THE DOCTOR ON 6/3/04. HE RECEIVED MEDICATION TO RELAX HIS BACK; NO RESTRICTIONS, RETURN TO DRIVE DUMP TRUCK.

IN THE EE WAS AT AN MSHA ANNUAL REFRESHER TRAINING CLASS WHILE SITTING AT A TABLE HE STARTED TO FEEL LIGHT HEADED AND DIZZY. HE STOOD UP AND WAS GOING INTO THE RESTROOM HE FELT LIKE HE WAS GOI NG TO COLLAPSE AN AMBULANCE WAS CALLED, HIS VITAL SIGNS WERE OK-HOSPITAL DIAGNOSIS WAS DEHYDRATION AND ELEVATED BLOOD SUGAR-NOT EATING AND CONSUMING ALCHOL THE NIGHT BEFORE.

THE EMPLOYEE WAS USING A STEEL BAR TO KNOCK LOOSE MATERIAL INTO THE ROCK WASHER. AFTER SEVERAL DAYS HIS SHOULDER MUSCLES WERE STRAINED, WAS NOT REPORTABLE UNTIL 3/24/00.
