Two front end loaders, one on each side, were being used to position an 80 foot section of 18 inch diameter pipe in the waters edge. When the loader on the right was about 25-30 feet from the lake, the bank started to cave in. Within 3 minutes the loader was submerged in about 20 feet of water.
The miner was repositioning themselves in the back seat of the site pick-up truck when they placed their right hand on an oscillating tool, causing the tool to puncture the miners glove and lacerating the right palm with a 1/2 inch cut.
When standing by the back of the service truck, the wind blew sand and dirt into his left eye, that resulted in a scratch to the cornea that required an antibiotic perscription.
THE EMPLOYEE WAS PERFORMING MAINTENANCE ON THE TRUCK SCALE PEDESTAL, WHEN IT SLIPPED OUT OF HIS HAND PINCHING & LACERATING HIS LEFT RING FINGER & LEFT LITTLE FINGER. EE REQUIRED STITCHES.
THE EMPLOYEE WAS CUTTING METAL ON THE UNDERSIDE OF THE OVERSIZED ROCK CHUTE THAT WAS LINED ON THE TOP SIDE WITH NEOPRENE RUBBER LINER. THE TORCH MELTED SOME OF THE LINER CAUSING THE EMPLOYEE TO BREATHE THE FUMES.
EE WAS LOOSENING BOLTS ON A FRAME THAT WERE VERY TIGHT AND HE STRAINED HIS RIGHT SHOULDER.
THE EMPLOYEE WAS CHECKING MATERIAL IN A PUGMILL BIN, WHILE ON AN EXTENSION LADDER ABOUT 12 FEET HIGH, HE STARTED DOWN THE LADDER, TOOK 2 STEPS & THEN LET GO & FELL ABOUT 9 FEET ONTO CONCRETE. HE THOUGHT HE WAS CLOSER TO THE GROUND.
WHILE DOING PLANT REPAIRS THE EE WAS WALKING AROUND THE BACK OF THE MAINTENANCE TRUCK WHEN HESLIPPED ON WET SLUDGE ON THE CONCRETE PAVEMENT. HE TRIED TO CATCH HIMSELF WITH HIS LEFT LEG & STRAINED HIS LEFT KNEE.
THE EMPLOYEE WAS ASSISTING WITH SPLICING A CONVEYOR BELT. HE WAS PULLING ON A COME-ALONG, TIGHTENING & STRETCHING THE BELT WHEN THE COME-ALONG CABLE BROKE, CAUSING HIM TO LOSE HIS BALANCE & FA LL OFF THE LADDER, FROM ABOUT 6 FEET. HE HAD SOME MINOR CONTUSIONS & ABRASIONS & STRAINED HIS LEFT WRIST. RETURNED TO WORK ON 4/10/01 WITH NO RESTRICT. RETURNED TO DR ON 5/10 & NOW HAS RESTRI
THE EE WAS CLIMBING DOWN FROM A TANK THAT HE WAS WORKING ON WHEN HE SLIPPED HE TRIED TO GRAB THE SIDE OF THE TANK, HE FELL ABOUT 6' TO THE FLOOR RESULTING IN CONTUSIONS TO HIS LEFT SIDE. 2 DAY S RESTRICTED WORK ACTIVITY.
