Two front end loaders, one on each side, were being used to position an 80 foot section of 18 inch diameter pipe in the waters edge. When the loader on the right was about 25-30 feet from the lake, the bank started to cave in. Within 3 minutes the loader was submerged in about 20 feet of water.

The miner was repositioning themselves in the back seat of the site pick-up truck when they placed their right hand on an oscillating tool, causing the tool to puncture the miners glove and lacerating the right palm with a 1/2 inch cut.

WHILE DOING PLANT REPAIRS THE EE WAS WALKING AROUND THE BACK OF THE MAINTENANCE TRUCK WHEN HESLIPPED ON WET SLUDGE ON THE CONCRETE PAVEMENT. HE TRIED TO CATCH HIMSELF WITH HIS LEFT LEG & STRAINED HIS LEFT KNEE.
