On 1/6/2026 the employee was issued restrictions following an appointment to provider for arm pain. Employee was issued restriction and schedule for MRI. Initial information arm pain is caused by repetitive motion of lifting drill steel while bolting.

Injured employee was holding a truck part while another employee used Sawzall to cut the mounting bolts. The sawblade slipped and cut employee across back of Left hand, through gloves. The employee was seen by on-site medic and referred to further care. EE received suture to close the laceration.

Struck against stationary object

While operating a Club Car the employee reached out to push some vent line out of the way. In the process they received a laceration to the forearm. The laceration required 5 sutures to close. Employee was issued a full release.

Struck by powered moving object

Miner was assisting partner in loading a pallet with a power cable on it. While guiding the 2yd operator up to the pallet, the bucket of the loader contacted the Miner's ankle, resulting in a fracture.

Over-exertion in lifting objects

Miner was attempting to move a unit of wire panels by hand. Afterwards felt discomfort in back.

Upon performing a post blast inspection at the start of night shift, a miner identified material that had fallen approximately 50' from blast area. Miner notified supervisor, incident was confirmed to meet the criteria of immediately reportable. Area was barricaded to prevent entry.

Employee reports EE was drilling a reamer hole when the steel shanked. EE was contacted by an object in the shoulder but thought nothing of it. Upon changing out EE noticed a small laceration that required one suture to close.

Employee inspecting shotcrete coverage on new air door install, & as EE grabbed the west de-energized air door, while pulling it open to inspect behind it, in doing so the the direction of air flow was east to west causing the east door to close resulting in EE's right hand to be pinched between the doors.

A miner was mucking a recently blasted round. While mucking, the mucker bucket contact and initiated a primer (blasting cap). The miner was uninjuried and barricaded the area to prevent injury.

Employee reports was changing steering valve assembly (approx 50 lbs). Set unit on mucker hood to continue work. It fell off the hood contacting employee's right pointer finger resting on mucker frame, causing a blow out type injury and fracturing the tip.

A FOG occurred in the 72 740 +280 west panel heading. The miner was remote mucking at the time when the back fell in. There was no injury.

While stepping out of pick-up employee slipped on small frozen puddle. Employee fell congtacting pick-up causing fracture to left forearm. Employee was given light duty restrictions and returned to work on next scheduled shift.

The employee reports EE contacted EE's head while entering the rail mantrip. The contact jarred EE's neck causing pain. The employee was seen by a doctor and issued restrictions.

Employee states EE was using a safety knife to cut suction hose to length. The hose and knife slipped and contacted the miner in the leg. The contact resulted in a ~2 inch laceration requiring suture to close. The employee returned to EE's next scheduled shift.

The employee reports was traveling down the 75 fwl in a haul truck on good roads. The parking brake set up on the truck while in motion causing the employee to twist lower back. The employee was wearing seat belt at the time. The employee was seen by a doctor and issued restrictions.

The employee reports that EE pinched their finger between a 4"" steel pipe and a rock while EE was setting the pipe on the ground.

The employee reports that while using a scaling bar in a chute area, EE's finger was pinched between the scaling bar and a handrail. A rock moved contacting the scaling bar and pushed the bar back causing the injury.

The employee reported that while assisting to re-position a steel wedge EE was contacted on the leg by the wedge. The contact caused a laceration to EE's thigh requiring sutures to close.

Employee reports they were using a scaling bar to clear debris from the surface crusher when they pinched their thumb between the handrail and scaling bar. The contact resulted in a fracture to their right thumb.

7/8/24 n/s miners identified HW rib concerns. They used a mucker to scrape off rock and ground support to aid in rehab bolting rib. Info was not adequately passed onto d/s. D/s miners reported heading condition to supervisor/GF who had MSHA inspector in office. Out of abundance of caution it was report as FOG but all ground support and rock was removed by miners. No FOG occurred.

While pinning screen the miner was contacted by rocks falling from the back. EE was contacted by the rock causing fractures to EE's left leg, right ankle, and spine. The ankle injury required surgery.

Employee reported EE was sitting in a mantrip, the train jerked forward and stopped unexpectedly causing neck pain. Employee was seen by site medic and referred to Occ Health. On 7/3/2024 EE was issued light duty restrictions.

Employee reports a piece of collar pipe contacted EE's eye while EE was installing the pipe in a lifter-hole. The contact required surgery to repair the damage caused to the eye. Employee reports was wearing safety glasses at the time of the incident and the piece of pipe came around the glasses.

Inhalation of radiations, caustics, toxic and noxious substances

The company was made aware of the mercury exposure diagnosis on 7/18/24. The release form provided to work comp and the company on 5/21/24 did not have a diagnosis. The treating physicians have also not made a determination as to the source of the mercury exposure, at work or personal life. This 7000-1 is being filed as directed from MSHA, per citation 9906316.

Struck by falling object

A tech. expert on site to advise during mill re-line was injured when EE operated a liner trolley with a liner on the cart. The cart stopped causing the liner to rock and fall. The injured reached to stop it but liner fell on EE's arm breaking forearm bones. Out of an abundance of caution following a conversation with MSHA we are reporting on behalf of the Canadian Company PolyCorp

Employee reports utilizing a ""micky"" bolt to bend hog wire in preparation to hang in the back. Employee reports pushing down on the bolt to bend the wire when the bolt slipped backwards contacting their bicep and causing a laceration. Employee was taken to ER to have injury cared for.

While holding a metal punch, 6lbs hammer glanced off and contacted left middle finger resulting in crush injury. Employee was holding punch with wrench in an attempt to keep hands free of hazard but glancing blow from hammer contacted hand. Employee was seen by medic and transported to ER for care.

The exhaust hose on the Drifter started to come apart and the Driller used a safety knife to cut the remaining hose in order to replace it. The blade of the knife did not retract back in completely after making the cut and struck the palm of the Driller's left hand causing a laceration.

The employee reports slipped and reached to catch self on the mucker bucket. In the process EE's fingers were caught between bucket and roll back cylinder of a 2 yard mucker pinching two fingers.

Employee was prepping the engine compartment head of LHD for removal. While removing the last bolt, the hood accidentally fell striking the employee on the finger causing injury.

Employee reports ongoing back issues. Stated on 12/1 it has progressed to the point EE no longer can take the pain. Employee referred to levels of care and MRI revealed a herniated disk at Doctors appointment on 12/20/2023.

Initial report was employee was contacted by a small rock that fell through hog wire on left shoulder. Employee worked EE's way through levels of care and was finally diagnosed with a partial tear of EE's rotator cuff on 12/22/23.

An over the road truck driver suffered a fatal personal medical incident while on property. Emergency medical care was rendered but resuscitation was not successful. MSHA was notified via 1-800 number and the incident was investigated. The driver had been to the mine site on a very limited basis in the past 3 years.

Laborer was walking on screen at MSE wall and caught EE's left boot on the metal screen and tripped and fell. The employee hit EE's left knee on the ground causing a laceration.

While manually removing a 6"" Reamer and Hex Rod due to completion of the drill hole, the injured employee's left pointer finger was smashed between the Hex Rod and a guard in place over the jack controls of the drill.

Over-exertion in lifting objects

Employee states: was removing cable hanging on the rib approximately 6' above the sill. With hands over head, lifting the cable EE's foot slipped causing a ""pop"" in right shoulder. The employee required an MRI and a follow up appointment with an ortho dr. on 10/3/23 and was diagnosed with torn shoulder ligaments. EE was taken off work to rehab the shoulder.

Employee was attempting to lift a wire spool onto the jack stand without help. When EE lifted the spool, EE saw their right bicep move up the arm. EE felt discomfort but no pain. EE stopped and reported to on-site supervisor. EE was transported to the hospital and seen by a doctor and diagnosed with a ruptured bicep.

Employee was handling an airline (hose) for a jack leg and was attempting to clear air out of the line. While handling the hose, EE felt pain in EE's bicep.

Utilizing a forklift around rail flat, one fork was inadvertently under a 1"" lip of the flat. The uninjured employee applied lift to the forklift at the same time the injured employee reached across the forks. The fork under the lip slipped off springing upwards, contacting the employee in the chin causing a laceration and a broken mandible.

During the bolting cycle a rock contacted the miner in the left knee causing ruptured ligaments. The miner was seen at the site medic and referred to an orthopedic provider. Due to the swelling the MRI was not completed and read until 3/15/23. On that date the diagnosis of the ruptured ligaments was determined, and surgery scheduled.

Employee was emptying a mop bucket in sink and strained EE's back. Was seen at medics and referred to ED where EE was again referred to Occ Health. Occ Health apt on 2/28/203 restrictions were issued and taken off work.

Miner hanging 6"" HDP airline pipe in the 85-655 development ramp. Caught left ring finger between pipe and cab rail of Young scissor deck USD04 causing laceration requiring sutures and fingertip fracture.

The employee states was helping to stand a booster pump up when EE felt pain in the left hip. EE was seen by on site medic and taken to the ER. EE was referred to ortho and an MRI was conducted on 9/12/2022 and was diagnosed with a herniated disk in the back and kept off work.

Employee was bolting up a round utilizing a jackleg when EE was contacted by small rock causing laceration to lip. The laceration required sutures to close and the employee was issued a full release.

While changing drill steel in a CMAC the employee pinched left middle finger tip between drill steel and drifter. Employee reported incident was evaluated by medic and transported to St Vincent Hospital in Billings for further evaluation. X-rays were positive for a fracture to EE's left middle finger tip.

Employee stepped on spade of a 5foot steel fence post which punctured employee's boot causing a laceration to right foot. Employee received 2 sutures and prescribed antibiotics. Employee was released to work without restrictions

Over-exertion in pulling or pushing objects

The mechanic was tightening a bearing on an S83 72"" drill leg, using a 24"" pipe wrench, when EE felt a pop in left shoulder area. EE saw a medic at the end of shift and was given an ice pack and a sling and instructed to follow up with EE's personal physician. EE was instructed by EE's physician to get an MRI to determine the damage. No report on the determination yet.

Employee was stabilizing the Hydraulic Torqueing tool while also activating the tool to a point where it would apply pressure to the reactionary arm. When activated, the reactionary arm pinched the employee's hand causing injury to the ring finger of the left hand. The injury required 5 sutures.

While sitting on the bench to remove EE's sock the employee's foot slipped out from underneath EE. The motion caused EE to slip off the bench. EE caught self with EE's left arm on the bench causing injury to EE's shoulder. Evaluated at PMC and issued a full release with a shoulder strain. Follow up on 3/16/22 w/ Ortho. Diagnosed with torn shoulder muscles. Issued restrictions.

EE was working on a haul truck when they were over exposed to the diesel exhaust. EE reports they parked a buggy near the exhaust and was looking for a tool when they began to feel ill. EE went to fresh air and reported the incident. EE was evaluated by the medics and transported to the hospital where they were kept overnight for observation monitoring the effects of CO exposure.

IE was manually adding an HM38 drill rod to the shank. EE knocked it against the drill to remove mud in the end and smashed the right ring finger between the drill rod and the drill resulting in a fractured fingertip (tuft) and open wound.

Employee was utilizing a 4 yard mucker to tram muck down a ramp. The mucker contacted the rib with the cab of the mucker. The impacted caused the employee to strike head on cab resulting in a laceration to left side. Employee was evaluated at a clinic and required 4 staples to close laceration and issued a full release.

Employee was checking supply flats on surface for specific parts. While walking near supply flats employee slipped on the ice and struck head on the ground. The impact caused a laceration requiring sutures to close. The employee was issued a full release and returned the following shift.

Struck by flying object

While hanging pipe to 4"" drisco pipe to plumb in the stationary air compressor. IE tried to open the valve to check for pressure but the valve was stuck and IE had to use force to open the valve. When the valve opened, the pipe jumped forward from pressure shock and struck the IE in the mouth causing a laceration to the lip and two broken teeth.

The miner III trainee was utilizing a jackleg to install a low bolt. The trainee was applying pressure to the cob to keep the leg from pushing the machine up. The crows foot of the jackleg kicked out causing the machine to fall pinching the trainee's index finger between the cob and the leg. The incident resulted in a laceration requiring sutures to close, full release issued.

Struck by flying object

Mill Operator was attempting to unplug a circuit with an air-lance when the fittings contacted structural steel and separated from the compressed air line. While the operator was attempting to shut off the air, the compressed air hose disconnected from the whip-check and struck the operator in the back of the head causing a laceration. Employee returned to work the following day.

The Mechanic was utilizing a torque multiplier to torque lug nuts on haul truck when the tool shifted pinching the mechanics pinky finger between tool and the rim.

Employee was conducting post blast inspection, wetting down a muck pile. Employee tripped and fell injuring left shoulder. EE was seen by site medic, referred to ER and then to Occ Health where EE was given restrictions.

Miner jackleg bolting 82-655 +100W production stope. Drilling into rock and rock fell. Jackleg machine pitched sideways pinching the miners right little finger between drill and right rib causing a laceration (5 sutures) and fracture to the tip of right little finger.

While conducting a post blast inspection the employee was scaling HW rib. EE was working a bowling ball size rock from behind the screen when a rock fell contacting the employee in the right foot.

Dayshift miners cut out wire to perform rehab of the area. Nightshift miners were to continue this work but during the end of dayshift blasts it is assumed the fall of ground occurred. The fall was discovered and reported on nightshift of 6/18/21.

The employee was operating a jackleg when EE was contacted in the head while wearing a hardhat. EE was seen by the medics and transported to the Medical Center where EE was diagnosed with a concussion. EE was admitted for observation and not given a release.

The employee was operating a jackleg machine and a finger was pinched in-between the machine and the leg. This caused a fracture to the finger tip and a laceration requiring stitches. Employee was wearing gloves but not impact gloves.

While pulling the jackleg out of a low bolt hole on the rib, the employee stumbled and fell. The jackleg also fell contacting the employee on EE's ankle. The result of the incident was a chip/fracture to the distal end of the tibia.

Employee was utilizing an air powered impact tool to tighten an anchor bolt. The bolt broke causing the plate, bolt, or tool to fall striking the employee in the face. The contact caused a small laceration to EE's cheek. EE was seen by medic and referred to PMC for sutures. Issued a full release.

The employees were removing a steel plate attached (welded) to the floor using a cutting torch and a fork lift. The fork lift was under the plate and applying lift. The other employee utilized a cutting torch to cut the final weld and when the weld was cut the plate ""sprung"" up and landed on EE's foot causing two fractures in EE's left foot.

An electrical disruption caused the raise fan stopped working during the shift which is used to supply main vent to the ramp. The employees were unaware and continued to work. Employees began to feel ill and left the area and reported it to supervisors. Employees was transported to Livingston Health care and issued full releases.

Employee was installing a joint bar (fish plate) utilizing hammer to drive the bolt and contacted the finger of the hand holding the bolt in place. EE was seen by site medic and referred to Medical Center where EE was diagnosed with a fracture to the finger.

EE was seen at site medic and eventually transported to a clinic where EE had surgery to treat injuries to two middle fingers.

Upon post blast inspection of the 82 655 +30 East heading, a fall of ground was identified in the area a slab round had been drilled and shot the shift before. The area was barricaded and reported to management. A call to MSHA followed.

Fall down stairs

Employee was descending a set of stairs. EE was carrying a bucket in one hand and holding the hand rail with the other when EE's left foot slipped on the step causing EE to fall into a sitting position with the right foot under EE. EE was seen by site medic and referred to the ED. EE was diagnosed with a fracture to the rt fibula. Referred to Ortho for follow up.

8"" cyclone feed pipe was being rotated when the employees hand was contacted by pipe while removing rock from under the pipe. The employee was seen by site medic and refereed to ER. X-rays were negative for fractures but employee was restricted from using hand.

Miner felt symptoms of exposure while underground operating the muck train. The tester carried by partner reported 59 ppm of CO for 130 sec. EE was seen by medics and transported to hospital where EE was restricted from work for one day out of precautions.

Employee was waiting at the 65 780 laydown for the man trip to pick them up after the rounds had been initiated at the end of shift. The train was late due to a muck-car with a BO axle. This delay exposed the miners to blast smoke at the laydown for a period of time resulting in a gas exposure. Employee was transported to the hospital and was taken off of work.

Driller was adding a reamer to the centralizer while the drill was in the ""Helac Under"" position. The rod and striker got into a bind causing the rod to kick out and pinch Driller's left thumb between the rod and slide resulting in a distal fracture to left thumb.

Employee was operating a 4 yrd, side loading own truck. After dumping a bucket into the the truck the mucker hit a pothole\rock causing the seat to compress and spring up. The operator contacted head on the cab of the mucker. Operator was wearing seat belt. Employee was transported to Livingston hospital and diagnosed with a cervical strain. Taken off work for 2 days.

The miner was collaring a hole at about the 5 foot line of the last ring of bolts on the HW rib when some loose fell off of the face striking miner on rt fore arm and rt knee. Miner was seen at Medical Clinic and referred to ortho who requested an MRI. Results from MRI on 6/24/20 were positive for MCL tear and taken off work.

Employee was operating a jack leg and the leg was extended. Lost balance and fell. EE's hand was still holding on to jackleg handle and was caught between the handle and a rebar bolt that was stood on the rib. Employee was seen at ED. X-ray negative but did require 3 sutures to close laceration.

The Fall of Ground occurred in the mucking cycle of the advancing round, causing the long support to come out at the brow. The location was in the area of 72 740 ramp. The condition was reported to a supervisor and then to the safety department who called the hotline. The area was barricaded to prevent entry.

Employee was dismounting Toyota Man Carrier when EE believes stepped on a loose rock causing leg to fall out from under EE. EE fell to the ground on knees. EE was referred to ortho and was diagnosed with a torn meniscus in the left knee.

Over-exertion in pulling or pushing objects

A CAPRA employee attempted to push a metal catwalk frame into position in the disc filter building at the East Boulder mine. The employee pushed against the frame with 60% of their power and felt a pop in ankle/calf area. EE was seen by an orthopedic doctor and diagnosed with a torn Achilles tendon and had to have surgery to repair it.

Over-exertion in lifting objects

Employee was attempting to hang a number of cable bolts on an ""S"" hook on the rib. The ""S"" hook fell off the rib and employee picked the hook with all the cable bolts still on it and attempted to re-hang it on the rib. In doing so employee felt a ""pop"" in their hand followed by a burning sensation. Employee was seen by medic and referred to ER for care.

Employee was riding in the back of a man carrier when the transport hit a bump causing a jarring motion. This motion caused a back strain to the employee.

Walking down excavator platform steps, mis-stepped, ""rolled"" right ankle. This accident occurred on the last step of the platform. EE was seen by EB Medics and referred to the ED where EE was diagnosed with a fractured RT ankle/foot.

Employee was using a CMAC to install a bolt when a finger was contacted by the drifter. Employee was attempting to line up drifter and driver. Employee was seen by Medic and referred to ED where restrictions were issued for ligament damage to finger.

Employee was pulling a rolled up jump cable into a mucker bucket. Their hands slipped off the cable and they stepped on a rock and felt some back pain. EE was seen by EB medics and referred to the ER for evaluation. EE was issued restrictions and taken off of work.

Employee was bolting up a round using the CMAC. Employee went to change the 6 foot steel when it slipped from EE's grasp and contacted right foot causing a contusion.

Employee was working on mucker bucket hydraulic hose when oil sprayed. EE stepped back and contacted EE's knee on the bucket causing a knee injury. EE was seen by site medics and transported to PMC for treatment.

Employee bent to pick up jackleg machine off of the rack and felt a sharp pain in right leg. Employee was seen by site medic and referred for a follow up from personal provider. EE's provider then issued restrictions for strained leg muscle.

After completing EE's bolt up cycle the miner returned to EE's mucker and turned to get in. While turning the employee caught EE's arm on a piece of hog wire sticking out from the rib. EE was treated at the site medic and referred to PMC to have the wire removed and 3 sutures to close the incision.

While walking back to transportation from a sump the employee fell landing on right buttock and strained groin in the same leg. The employee was seen by the site medic and taken to a Medical Center where employee was issued restrictions.

Stinger Drill Rig operators Kevlar sleeve caught the feed control lever and energized the drifter which moved and pinched the operators right hand between the shank and the drill rod.

The miner was loading round when a small rock fell and caused a laceration to left cheek. EE was seen by the site medic and referred to Pioneer Medical Center to have cheek evaluated. EE received sutures and was issued a full release.

The mining trainer was attempting to help another miner hang wire while bolting a round when the miner operating the jackleg shut leg feed air off to grab another driver and left the machine up in the split-set. The machine fell from the bolt striking the mining trainer in the thumb. The mining trainer was seen a PMC and diagnosed with a fractured thumb.

While bolting with a jackleg, the employee reached out to grab the machine as it was falling. This motion caused discomfort in right shoulder. EE was seen by the site medic and referred to a doctor. EE was issued restriction and taken off of work.

Fall down stairs

The employee was reporting to work on night shift. As EE reach the top step to the office and dry area EE stumbled, catching EE's fall with EE's right hand. In so doing EE dislocated EE's rt. small (5th) finger on the grating.

Employee states has had dull ache neck pain the last 2 years, worsening over that time. Was seen by a Dr. Ended up with ""Full reconstruction"" of Right shoulder in December of 17. The neck pain persisted, and employee saw the Dr. for a shoulder follow up, who x-rayed employee, and states sees signs of arthritis in employee's neck.

Employee stated about 10 years ago the employee sustained a knee injury and did not receive care. Employee's knee began to hurt again and went to have it evaluated. On 04/03/2019 Workers comp accepted the claim as an Occupation illness.

While bolting up last ring of the round a small rock came through the screen and bounced off the jackleg muffler. The rock contacted the miner in the nose and lip causing a laceration. Miner was seen by site medic and referred to a medical center where the miner received cosmetic sutures to close the laceration.

Tire tractor pulled into a cross cut to allow tomcar to pass by. While attempting to back further out of the way the tire tractor moved forward contacting the tomcar operator as EE drove past. The operator received a laceration to the arm and was seen by the on-site medic. EE was transported to St.Vincent Hospital and diagnosed with forearm laceration.

Employee was moving 4"" schedule 40 pipe over about 4"" to hook it up to drisco. Upon lifting the end of the pipe EE felt a ""pop"" in left elbow. EE was seen by the on site medic and referred to a Medical Center to be evaluated. EE was then referred to a specialist and diagnosed with a ruptured tendon.

The miner was bolting up last ring with a jackleg when EE's right hand was struck by a rock from the footwall/ back contact. EE was seen by the on-site medic and transported to Livingston Medical Center to have EE's hand evaluated. EE was diagnosed with fracture to a metacarpal and issued restrictions.

While performing a workplace inspection, employee stumbled while looking the back over. EE landed on left side and received a laceration to left buttock. EE was seen by site medic and referred to Pioneer Medical Center. EE received three sutures and a full release.

Employee reported soreness in left shoulder due to the repeated motion of hanging pipe to the site medic. The shoulder was evaluated and the employee was released back to work missing no time. On 7/23/19 the employee had surgery on shoulder and missed the first shift on 7/27/19.

Struck by falling object

Employee was loading hog wire panels from bundles leaning on rib. After 8 panels had been removed the bundles fell, pushing miner away. Employee was seen by on site medic and referred to E.D. Employee was seen at the E.D. and referred to Occ Health on 11/26/18 and again referred back to Occ Health. On 11/29/18 employee was issued restrictions.

On 10/12/18 an employee was cleaning to drill a slab round on the rib with a mucker. When ee rolled the mucker bucket back a small pop was heard. No injuries or damage was reported. The employee did not report the incident to management until 10/15/18.

Employee finger was contacted when they closed the door of the haul truck. Employee was seen by the on-site medic and referred to the ER. Employee received stitches to close a finger laceration and received a full release.

Employee went to perform a muck check when a rock rolled down the pile and contacted pinkie. EE was evaluated by site medic and referred to Pioneer Medical Center for further evaluation, receiving stitches and a full release.

EE was barring down on the 65-695 ramp when they felt a pop in the right elbow. EE went to see the Medic onsite and was referred to the Livingston hospital. The hospital determined EE had soft tissue damage and was given restrictions.

Struck by falling object

EE was bolting third row of bolts in the back when a rock came out of the back and bounced off the ground contacting EE's foot. EE was transported to Pioneer medical Center and diagnosed with a bruised foot and given restrictions causing a lost time accident.

EE bent down to sort caps and lost footing and struck right knee on the loader bucket. EE did not report the incident until the 13th and went to see a doctor on the 14th and restrictions were given turning bruised knee into a lost time accident.

EE was pulling in to the 79 LOM with a loaded truck and struck a rock with front right tire causing EE's head to contact the roof of the cab. EE was transported to the hospital and diagnosed with a concussion and neck strain.

While cleaning out the louvers on the fan located at Graham Creek vent raise EE received a laceration on EE's left ring finger. The laceration was closed with 6 sutures making the accident a medical reportable.

On 4/14/18 ee went to get into a running 4 yard loader and knee hit the controls causing the loader to articulate. EE was pinched in the articulation resulting in a hip injury.

While getting off the train ee rolled ankle. EE was sent to the medic and then sent to the hospital. EE was told to go to Ortho specialist and was diagnosed with a chip in ankle and given restrictions.

A miner was putting hard line ventilation together and felt like they pulled a muscle. Employee went to see the medic who sent them to the hospital where they were diagnosed with a distal bicep tendon rupture. This requires surgery to fix, causing lost time injury.

Tripping rods out of a 90 degree up hole, when the bell of the rods caught the wire and a piece of loose rock hit the back of EE's hard hat and then another hitting EE in the face causing a laceration under left side of nose on upper lip. The laceration is approximately 1 inch long.

Employee was discarding used oil in the oil shed outside the surface shop. EE opened the doors to the shed, while entering the doorway, the wind blow the left door into EE and contacted EE's left lower hip/back. Seen by onsite medic referred to next level of care. Given restrictions.

On 2/4/18 employee fell while getting on a scissor lift truck causing a contusion to EE's left hip, did not report the incident & on 2/5 EE was noticed limping by supervision & asked what happened. EE explained & was alright, on days off was not better so EE doctored on 2/9 not mentioning it was work related. 2/11 doctored again & was given restrictions making this reportable.

Miner and partner were changing a feed cable on a CMAC DHS drill and while lowering the feed beam to provide easier access to complete the job the cradle that attaches the drill to the feed beam fell off of and landed on the edge of the working deck striking an object that flipped up and struck the employee on the cheek.

EE was stepping and felt discomfort in right knee. EE reported the incident the following work rotation on 11/2/17. EE was evaluated by a doctor and an MRI was performed and EE was diagnosed with a bone spur and arthritis in the knee. On 12/4/17, workers' comp determined this was an occupational related disorder making this a reportable illness at this time.

A geologist went into the 82 670+100W to blue line for sublevel stoping. EE removed a barricade & upon entering EE ID'd a hanging wall fallout that pulled past the anchorage zone. EE left the area, rebarricaded it & reported the incident. At the time it was determined the area was inactive. Based upon an MSHA investigation, it was determined it should've been immediately reported.

Employee said they stepped to grab a 6-foot jackleg drill steel and experienced some knee discomfort. Employee worked the rest of the shift. Employee went to the doctor the following day and an MRI was taken. Employee had an appointment on December 19 to review the MRI and was diagnosed with a small meniscus tear.

Employee was bolting with a jackleg drill and while was bent down picking up the drill a 6-foot drill steel fell and struck the employee across the back of the head. The employee called off on last shift of the rotation. Upon further investigation when the employee returned on the next rotation (4 on 4 off rotation) it was determined that EE called off for a headache.

Employee was rehab bolting with a jackleg and while attempting to drive a 5-foot split set in the upper hanging wall rib the jackleg pushed back pinching arm between the footwall rib and the jackleg feed control handle resulting in a fracture to the right ulna.

Employee tripped over his own feet while walking. He tried to catch himself on the canopy post and his right hand struck the post. Went to medic on site, then to ER. X-rays resulted in a metatarsal fracture. He was given a full release. On 1/27/2015 Orthopedic doctor issued restrictions~ then it became a Lost Time injury.

While attempting to bundle several sections of pipe together so they could be secured to the rib with chain, the employee felt a ""pop"" in his right forearm followed by some discomfort to his forearm and lower triceps area. The incident cause was an arcing starter cable which melted a hydraulic hose & oil sprayed near the arc causing an ignition. It was determined that it was extinguished immediately & didn't meet immediately reportable criteria.

Employee was removing a 6"" bit and 4' drill rod from the drill feed on an up hole. When the employee lifted the bit and rod from the feed, he lost grip of the rod and forced his hand into the side of the drill feed causing a boxer fracture on his right hand.

75 670+300E panel was shot at end of day shift 11/4. Supv went to the area on 11/4 night shift & saw there was a block failure so the area was signed off until it could be investigated. Upon investigation it was determined the fall was above the anchorage point of the 5' split sets & 8' bolts although cable bolts were present in the back, so it was immediately reported to MSHA.

Employee was bending the keeper latch (on a mucker) that holds the air cleaner housing cover in place, while doing so with a pair of pliers, the pliers slipped off the latch resulting in the top of his left hand being struck against the hook on the end of the latch resulting in laceration.

A co-worker was using an air chisel working on the cutter head of the TBM, when a flying object struck the injured employee in the arm pit (right side) causing a puncture. Injured employee went to medic on site, then down to ER room.
