While descending a portable ladder, an employee missed the last steps and landed awkwardly on the ground, resulting in a knee injury. The employee was promptly attended to and received medical evaluation.
On 5/7/2025, an employee was injured while attempting to remove an Easy Skirt from the Feed Belt of Finish Mill #4. The worker was using a pry bar to detach the skirting from its mounting plate. During the task, the bar slipped while being forced downward, and the sharp end punctured the employee's right palm. The injury required medical attention, including three stitches.
An employee sustained an injury to left thumb. The injury occurred when employee was using a pry bar to remove a rock lodged between the conveyor belt and its supporting structure.
Employee was getting out of haul truck to load production and experienced spasms in their back. Called supervisor for assistance and was taken for medical attention. Placed off work for 1 week.

Miner was working in the quarry, exited excavator. While walking back, miner tripped over a rock. Miner landed on shoulder injuring the shoulder and thumb. Miner was treated and released to work restrictions within the miner's normal work duties. On October 4th, miner had surgery to repair the shoulder injury and placed off work.
Miner struck mill liner with sledge hammer. A piece of metal chipped off the liner and struck miner in the right leg just below the knee, embedding itself approximately 2"" into the leg.
A softball sized rock broke loose from the screen side opening and fell onto the miner's hand while the miner was barring into the oversize chute resulting in a broken pinky finger.

Miner had pipe on sawhorses to weld on flange. As EE was turning pipe to weld on flange, the flange fell off the pipe and dropped on the edge of EE's foot. The flange sliced through the boot and lacerated the edge of EE's foot. The miner received 6 stitches and modified work duties.
Miner was walking around the corner of the mill building, slipped on ice and caught themself awkwardly. Miner incurred knee strain and declined initial medical treatment until 11/7/23. Clinic prescribed work restrictions and physical therapy.

Employee was making a gasket with gum rubber and was cutting the pattern with a utility knife on a table in the carpenter shop. When the knife caught on the material and more pressure was applied, the blade came free and went across the glove on the left thumb. Which resulted in employee's thumb being cut requiring 3 stitches.
Miner broke right ring finger when the coal system baghouse door slammed shut, catching EE's fingers between the door and frame.

Employee was using a skid steer to clean up core sample bags. While exiting the equipment to load bags into the bucket, EE stepped on the edge of one of the sample bags and twisted ankle. The employee was placed on modified work duties.
High CO reading indicated a Coal smolder in Coal mill baghouse. CO2 and Fire Suppression required more than 30 minutes to extinguish smolder. CO & heat source identified as out at approx. noon. See Root Cause Analysis for full details.

Lowering hand rail on scissor lift and a hand was in the pinch point.
The employee was using a cordless impact gun with a hobble head and 3/4"" deep socket, the socket popped off and hit employee in the mouth.
Employee was scooping a sample out of the crusher bowl and the bowl slipped and landed on EE's finger.
This incident was not reported to the company until July 7, 2020. The employee was helping lift the core drill in place when EE felt a sting in the groin area.
Employee was lifting the bag cage, while changing bags in polishing bag house.
During the startup of the kiln, we believe that the flame was lost for a short period of time and raw natural gas spread across the kiln system. When the fuel re-ignited, the blast damaged the precipitator casing causing $65,000 in repairs.

A fork lift with a barrel dumper attachment with a band and clasp around the barrel was being used to move barrels of balls. One employee reached down to unclasp the barrel before the forklift had come to a complete stop. The forklift rolled forward and the employee caught EE's finger between the barrel dumper on the forklift and another barrel on the ground.
Smashed finger between pipe and motor while removing the guard of main clinker conveyor.
An employee parked a truck underneath the muck chute to load kiln brick. The steps of the truck were even with the edge of a concrete pad with a two inch difference to the ground. Employee stepped out of the truck, foot was not squarely on the even surface. As they put full weight on that foot, it rolled to the lower surface causing a sprain to the ankle.
Cable was bound up on the CKD load out spout, the employees finger/glove was pulled into a pinch point.

An external truck driver was attempting to go up the driver platform to shut the hatches, upon attempting to climb the steps they slipped and fell on the stairs. Since this is not an employee I am not sure of the exact start time of the shift.
EE smashed EE's fingers while bricking. While using the ram to put pressure on the bricks, the tube broke and smashed EE's fingers against the shell.
Employee was attempting to weld a plate on the crusher, the plate slipped, EE tried to catch it and left a sharp pain in EE's right shoulder.
Employee was helping forklift driver by pushing the two forks closer together. When the employee was pushing the forks in, EE felt a pop in EE's knee/calve.

Employee was in # 2 finish mill replacing shell liners. Employee was guiding the liner in place & lost footing on a corner of another liner that was already installed. The momentum moving forward caused the liner to drop and the right ring finger to be smashed between the mill shell and the liner. The supervisor did receive stitches to right ring finger.

Employee was using a wrench to loosen bolts on a valve on top of the cement silos. As employee was pushing down to loosen the bolt felt a pain in right shoulder. There was no damage to the shoulder but was issued muscle relaxers.
Employee was chipping the slag off a weld with a hammer. A piece of slag flew into left eye. The employee was wearing prescription safety glasses with side shields. The employee was working over the weld and the slag flew up and underneath the glasses. There was no damage to the eye. Antibiotic eye drops were prescribed for prevention.
Employee was using a 4"" hand grinder and the grinder kicked back while in use and the employee was cut on the left side of the neck.

EE climbed into the cab of the water truck to use a portable radio. As the employee was stepping down he stepped into a small hole, lost his balance, fell backward to the ground landing on his left elbow causing a scrape. The injured employee was on the 3rd level platform trying to free the hoist that got caught on the handrail of the platform. The injured employee pushed on the I-Beam and at the same time 2 employees pushed on the roller and the hoist became free and jerked the trolley wheel against the injured employee hand and caused a laceration to his pinky finger.
Contractor from Westate was exiting out of the passenger side of the service truck. Contractor came out facing forward and slipped on the top step and fell to the ground. Contractor then fell and struck his head on the left upper side on an unknown object. Contractor was escorted to Belgrade Urgent care by his supervisor. Contractor received 4 staples to his wound on his head
Three employees were lifting a 100 lb crated bearing. The injured employee tripped on a pallet and fell to the ground. The bearing fell out of the box and broke the left index finger and middle finger. The employee received 15 stitches and was released back to work to full duty.

Employee was torquing head liner bolts on the raw mill. As employee was torquing the ratchet come loose causing employee to loose his balance. As employee step back to catch his balance he rolled his ankle and his lower leg absorbed all the weight, fracturing his lower leg.

The employee was working in the chain section of the kiln, when a soft piece of dust fell from the chains. The dust glanced off the side of the employee face allowing dust to get behind employees safety eye wear and into the right eye.
EE was adjusting a kiln shell cooling fan. As the ee started moving the fan, the fan housing rotated and pinched his finger between the fan housing and the frame of the fan.
EE was taking measurements of a guard when he dropped his measuring tape. He bent over to pick up the tape measure and struck his bottom lip on a bearing guard on the end of the screw conveyor, splitting his lower lip.
While getting out of the car for work EE slipped and fell on ice in the gravel parking lot. Due to freezing rain conditions the parking lot was slick. The parking lot had sanding stations and had been sanded.

While sliding a piece of steel off of the welding table one of the iron supporting blocks under the plate steel fell from the table and struck the EE just behind the steel toe on his right foot.
The employee was using a plasma cutter to remove liners from the inside of the kiln. A piece of slag popped bouncing off the kiln shell and the employees welding hood landing in his right ear.
EE was repositioning the slings to release the pan feeder head shaft when one of the slings slipped slightly causing the shaft to move pinching the EE's left thumb between the shaft and the drive sprocket.
Employee was assisting to rebrick the kiln. While handing a brik to another employee another brick fell strking the employee's small finger on the left hand.
EE had soreness in right elbow & reported it to his supervisor. The ee was taken to clinic & diagnosed with tennis elbow. Elastic band & cold compresses used to control inflammation. EE returned to clinic for follow up on 10/21, referred to a physical therapist for ultra sound therapy. EE was given his 1st ultra sound treatment on 10/27.

There was a large push of material into the clinker cooler. The hot clinker is transported to an elevator & to the outside bins. A trip by the motion switch on the elevator that caused the clinker to go to the main clinker elevator where it is dispensed onto the belt. The emergency water sprays were immediatley activated but the hot clinker ignited the main clinker belt.
Employee slipped on ice while entering the plant for work. On 5/1/08 the employee was referred to chiropractic treatment making the accident reportable.
EE was replacing a rubber boot on a inlet spout, while streching boot EE felt pain in right forearm. Went to DR 4/22/08 and EE was enrolled on physical therapy. Making injury reporatable.

EE was changing out acetylene bottle. Smashed his finger between bottle and wall. After ee went home at night finger started to bother him. He went to clinic and had finger nail drained.

THE SHOVEL BOUNCED OFF HER HEAD STR IKING HER ON HER RIGHT THUMB. SHE DID NOT REPORTTHE INCIDENT UNTIL A MONTH LATER. SHE THEN REPORTED SWELLING AND SORENESS IN HER THUMB AND WAS ENCOUNGED TO SEE MEDICAL ATTENTION.

WHILE WORKING ON A CRUSHER JOB BUT D ID NOT FILE A REPORT. THE TINGLING AND NUMBNESS IS MOST NOTICIBLE IN HIS RIGHT ARM AND HAND AND IS WORSE AT NIGHT. HE WENT TO A DOCTOR ON MAR. 8 & HAS BEEN OFF WORK DUE TO UNRELATED ILLNESS.

HE WAS HELPING TO LIFT THE BARRELS INTO A GARBAGE DUMPSTER AND AT TIMES HE WAS LIFTING BY HIMSELF. THE EE WENT TO THE DR. ON THE 4TH DAY AFTER THE INCIDENT AND WAS TOLD IT COULD BE A HERNIA OR AN INFECTION. ON 9-6, HE WAS DIAGNOSED W/ A MUISCLE TO THE LOWER ABDOME & GIVEN MEDIC
EE WAS OPERATING THE TRACKMOBILE TO MOVE RAIL CARS. AFTER CHANGING A SWITCH ON THE TRACKS, HE STARTED DOWN A TRACK AND THOUGHT HE HAD ENOUGH CLEARANCE TO PASS BY A RAIL CAR ON THE ADJACENT TRA CK. EE WAS OPERATING THE KOMATSU LOADER IN THE QUARRY. HE HAD PICKED UP A LARGE BOULDER IN THE BUCKET WHICH WAS TOO HEAVY FOR THE LOADER TO LIFT. THE LOADER TIPPED FORWARD AND ALTHOUGH HE HAD HIS SEATBELT ON, HIS LEFT KNEE WAS FORCED INTO THE STEERING COLUMN, CAUSING A DEEP BRUISE NEAR THE KNEECAP. THE DR. THE EMPLOYEE WAS MOVING THE QUARRY DRILL TO A NEW LOCATION. THE EMPLOYEE WAS STANDING WHILE MOVING THE DRILL IN ORDER TO SEE BETTER. THE DRILL DROPPED UNEXPECTEDLY AND CAUSED HIS BODY WEIGHT T O BE SHIFTED TO THE LEFT LEG. THE EMPLOYEE NOTICED PAIN AFTERWARD AND REPORTED THE INCIDENT ON 5/30/01. HE STARTED SEEING A CHIROPRACTOR ON JUNE 6TH BUT THE CO. WAS NOT INFORMED UNTIL JUNE 20
EE HAS BEEN WORKING ON THE #9 WABCO HAUL TRUCK REPLACING A ROCK BAR. AFTER COMPLETING THE JOB, WHICH INVOLVED LIFTING AND USING TOOLS TO REPLACE THE BAR, HE NOTICED PAIN IN THE RIGHT GROIN ARE A. HE VISITED A DR. A COUPLE OF DAYS LATER AND WAS DIAGNOSED WITH A HERNIA. NO MEDICAL TREATMENT WAS PROVIDED BEYOND THE DIAGNOSIS. THE EE REPORTED SURGERY SCHEDULED FOR JUNE 5TH.
EMPLOYEE WAS REARRANGING CLINKER COOLER SIDE FORMS AND PLATES ON A WOOD PALLET. ONE OF THE UNITSSLIPPED AND CAUGHT HER MIDDLE RIGHT AND RING FINGER. THE MIDDLE FINGER WAS SMASHED AND LACERATED . AN X-RAY REVEALED A FRACTURE IN THE BONE AT THE END OF THE FINGER. THE LACERATION REQUIRED STITCHES.
EE REPORTED ON 5/22/01 THAT HE HAD BEEN EXPERIENCING NUMBNESS IN FINGERTIPS & TINGLING IN WRISTS& HANDS RIGHT & LEFT. THIS HAS BEEN OCCURRING SINCE 4/30/01 HE CHOSE AT THAT TIME TO NOT SEEK MEDICAL TREATMENT. ON 1/23/02 HE REQUESTED MED ATTENTION SURGERY WAS PERFORMED ON 1 WRIST 3/25 & THE OTHER OTHER WRIST WILL OPERATED ON SOMETIME IN MAY 2002
EE HAD CARRIED APPROX 35 LB OF TOOLS AND EQUIPMENT UP SEVERAL FLIGHTS OF STAIRS TO WORK ON THE KILN EXIT ANALYZER. WHEN HE CAME IN FOR LUNCH HE REPORTED HIS BACK HURT BUT DID NOT REPORT IT AS WORK RELATED. THE EE HAD SURGERY SEVERAL DAYS LATER FOR A RUPTURED DISK. THE EE WAS IN THE WELDING TRUCK AND HAD MOVED IT TO THE BACK OF THE SHOP BUILDING. AS HE WAS GETTING OUT OF THE VEHICLE, HIS FOOT SLIPPED ON SOMETHING AND HE TWISTED HIS BACK. HE RECEIVED CHIR OPRACTIC TREATMENT THE FOLLOWING DAY.
THE EMPLOYEE AND A CO-WORKER WERE CHANGING OUT ADRIVE BELT ON A DUST COLLECTOR FAN. ALL EQUIPMENT WAS PROPERLY LOCKED OUT AND THEY WERE TURNING THE FAN BY HAND TO SEE IF BELT LOOKED OK. THE EM PLOYEE WAS NOT AWARE OF WHERE HIS HAND WAS AND CAUGHT HIS FINGER IN THE BELT PULLEY. HE VISITEDTHE DOCTOR THE NEXT DAY AND FOUND HE HAD BROKEN THE FINGER. HE IS CURRENTLY ON RESTRICTED DUTY.
EE WAS DISMOUNTING FROM THE WA500 KOMATSU LOADER. HE HAD ONLY ONE HAND ON THE LADDER AND ONE OF HIS FEET SLIPPED OFF A LADDER RUNG. HE CAUGHT HIMSELF WITH HIS RIGHT HAND AND HEARD SOMETHING PO P IN HIS WRIST. THE EE REPORTED TH INCIDENT ON THE 18TH AND RECEIVED MEDICAL TREATMENT ON THE 19TH. THE DOCTOR GAVE HIM A WRIST BRACE TO WEAR TO AID IN HEALING.
EMPLOYEE HAD BEEN DRIVING TRUCK IN THE QUARRY AND DUMPING ROCK INTO THE CRUSHER. WHEN THE CRUSHER PLUGGED, HE ATTEMPTED TO BAR OUT LARGE ROCK. THE ROCK SLIPPED AND PULLED DOEN HARD ON THE BAR. HE HUNG ON TO PICKER HANDLE AND WAS PULLED FORWARD. THE FORCE EXERTED ON THE HANDLE DISLOCATED HIS SHOULDER.

EE WAS HELPING TO FIX A BROKEN HAND RAIL ON THE KILN ACCESS RAMP. THE METAL PIPE RAIL SLIPPED AND THE BROKEN END OF THE PIPE HIT THE BACK OF HIS LEFT HAND CAUSING A LACERATION APPROX. 2 INCHES IN LENGTH. THE EE WAS TAKEN TO A NEARBY CLINIC AND RECEIVED 7 STITCHES TO CLOSE THE WOUND. NO GLOVES WERE BEING WORN AT THE TIME OF THE INCIDENT.
ON DEC 15, 2000 EMPLOYEE REPORTED THAT HIS RIGHT SHOULDER HAD BEEN BOTHERING HIM ON AND OFF SINCE SUMMER. THIS WAS THE FIRST REPORT OF ANY PROBLEM. HE ALSO STATED IT SEEMED TO BOTHER HIM MORE AFTER STACKING CEMENT SACKS. THE EMPLOYEE VISITED A DOCTOR ON DECEMBER 27, 2000 AND WAS GIVEN EXERCISES TO DO TO RELIEVE HIS SHOULDER DISCOMFORT.
EE IS NOT SURE OF WHEN OR HOW METAL FRAGMENT GOT INTO EYE. HE FELT DISCOMFORT ON AND OFF AND DID NOT EXPERIENCE ANY REDNESS. ONLY BLURRED VISION IN THE RIGHT EYE. IT IS BELIEVED THAT THE INCID ENT OCCURRED ON DEC 11,2000. IT WAS REPORTED ON DEC 14TH. THE EE WENT TO A DR ON DEC 15TH AND HAD A METAL FRAGMENT REMOVED. THE VISIT TO THE DR WAS REPORTED ON DEC 20, 2000.
EE WAS WALKING TO HIS CAR IN THE EE PARKING LOT WHEN HE SLIPPED AND FELL ON ICE AND SNOW THAT COVERED THE DIRT PARKING LOT. HE FELL A SECOND TIME WHEN HE FIRST ATTEMPTED TO GET UP. THE EE SAW A CHIROPRACTOR LATER THE SAME DAY FOR TREATMENT.THE EE FELL ON HIS BACK THE FIRST TIME AND FELL ON HIS KNEES AND ELBOWS THE SECOND TIME.
EE DID NOT REPORT UNTIL DEC. 6TH THAT HE CONSIDERED THE 11/16/00 INCIDENT TO BE WORK RELATED. ON 11/16/00 THE EE REPORTED FEELING SOMETHING IN HIS EYE AND COULD NOT WASH IT OUT AND COULD NOT R ECALL ANY SPECIFIC TIME WHEN SOMETHING WENT IN THE EYE. THE OBJECT APPEARED TO BE A WHITE ROUND OBJECT ON THE EDGE OF THE EYELID. ON 12/7/00 EE SOMETHING ENTERED HIS EYE WALKING PAST COOLER.
EE WAS ORGANIZING PARTS & SUPPLIES IN THE OLD DRYER BUILDING. SHE WAS STANDING ON A PALLET THAT WAS SITTING ON THE FLOOR. ONE OF THE BOARDS IN THE PALLET BROKE & SHE FELL TO THE GROUND. SHE REPORTED BRUISING & MINOR CONTUSIONS WITH DISCOMFORT IN HER LEFT FOOT, WRIST, LOWER BACK, RIGHT SHOULDER & LEFT HAND. AN X-RAY SHOWED A CRACKED BONE IN HER LEFT FOOT WHICH WILL REQUIRE A CAST.
EE WAS WORKING WITH HIS CREW TO REPAIR THE MAIN CLINKER ELEVATOR. SOMETIME WHILE USING A 3-TON CHAIN HOIST. HE APPARENTLY STRAINED HIS UPPER BACK. BETWEEN THE SHOULDER BLADES. HE HAD BEEN REAC HING OVERHEAD AND PULLING HARD. THE INCIDENT OCCURRED ON 10-28-00. THE EE DID NOT SEE A DR UNTIL 11-6-00 AND MISSED HIS FIRST SHIFT OF WORK ON 11-8-00.
EE HAD BEEN ASSIGNED TO PERFORM CLEAN-UP WORK IN THE PLANT. THIS INVOLVED SHOVELING PILES OF CEMENT. CLINKER AND OTHER LOOSE MATERIAL ON TOP OF THE CLINKER SILO FOR ABOUT 4 HOURS ON OCT 23RD. HE REPORTED THAT HIS LEFT ARM WAS SWOLLEN ON MONDAY AND BECAME MORE PAINFUL ON TURESDAY WHEN HE DID MORE SHOVELING. HE INDICATED THAT HE WAS REACHING OUT FURTHER THAN NORMAL WITH A LOADED SHOV
EE WAS BLOWING OFF MATERIAL FROM THE #2 CEMENT PUMP. APPARENTLY, MATERIAL FROM THE PUMP WAS BLOWN INTO HIS RIGHT EYE. THE EE WENT TOTHE DOCTOR AFTER WORK AND HAD A FOREIGN BODY REMOVED FROM TH E RIGHT EYE.
EMPLOYEE WAS WORKING NEAR ONE OF THE FINISH MILLS AND HAD JUST CHANGED A SCREEN FILTER; WHEN SHE STOOD UP, SHE HIT HER LOWER BACK ON SOME OVERHEAD CONDUIT. SHE REPORTED FEELING PAIN THAT EVEN ING (9/11/00) AND REQUESTED MEDICAL TREATMENT ON 9/14/00. ANTICIPATED TIME OFF IS TWO WORKING DAYS.
AT 845 AN UNPLANNED EXPLOSION OCCURRED IN THE COAL SILO DUST COLLECTOR. THERE WERE NO INJURIES. DAMAGE TO EQUIPMENT INCLUDED. A ACCESS DOOR WAS BLOWN OFF OF THE COAL SCREW AND DAMAGED A HANDRA IL WHEN IT LANDED ON A LOWER LEVEL ROOF: THE BAGS OF THE DUST COLLECTOR WERE BURNED AS WERE SEVERAL CARDBOARD BOXES STORED NEAR THE DUST COLLECTOR.
EMPLOYEE WAS WORKING IN THE MACHINE SHOP, LIFTING A HYDRAULIC CYLINDER THAT WAS NEEDED FOR CRUSHER REPAIR. THE CYLINDER WAS BEING LIFTED FROM THE FLOOR TO A TABLE APPROX WAIST HIGH. THE EMPLOY EE TWISTED HIS BACK AS HE LIFTED.
AN UNPLANNED EXPLOSION OCCURRED AT 0835 IN THE COAL ELEVATOR. ABOUT 3 MIN. PRIOR TO THE EXPLOSION , THE DUST COLLECTOR WAS DUMPED (ROUTINE PROCEDURE) INTO THE ELEVATOR. IT IS BELIEVED THAT SMO LDERING SYNCOAL FINES CREATED ENOUGH HEAT THAT ALONG WITH OTHER CONDITIONS BEING JUST RIGHT AN IGNITION OF THE SYNCOAL DUST OCCURED IN THE ELEGVATOR SHAFT. THERE WAS STRUCTURAL DAMAGE, BUT NO
EE WAS HELPING TO LOAD A RAIL CAR WITH MILL GRINIDNG BALLS. THE BARRELS OF BALLS WERE ATTACHED TO A FORK LIFT WITH A BARREL CLAMP. HE ATTEMPTED TO PUSH A CLAMP CLOSED WITH HIS FOOT AND SLIPPED OFF THE CLAMP. HE FELL AGAINST THE 50 GALLON BARREL AND FRACTURED THE LEFT FIBULA. HE DID NOT SEEK MEDICAL TREATMENT OR REPORT THE INCIDENT UNTIL 6-29-00. THE DR TOLD HIM HE COULD RETURN IMME
EMPLOYEE WAS IN HIS WELDING SHOP USING A GRINDERON THE DSM SCREEN HOUSING; SPARKS FROM GRINDING LANDED ON HIS JEANS & MATERIAL CAUGHT FIRE. HE IMMEDIATELY EXTINGUISHED FLAME & NOTICED A BURN THE SIZE OF A QUARTER TO 50 CENT PIECE; NO TREATMENT WAS GIVEN AT THE TIME OF INCIDENT. ANINFECTION DEVELOPED AND MEDICAL ATTENTION WAS OBTAINED ON 6/1/2000.
EE WAS USING AN AIR CHISEL IN THE DUST COLLECTOR TO BREAK UP A MATERIAL BUILDUP. ALTHOUGH THE EE WAS WEARING DUST GOGLES, FINE DUST PARTICLE PASSED THROUGH LEAKS IN THE GOGGLES AND SCRATCHED T HE CORNEA OF THE RIGHT EYE. THE EE VISITED THE DOCTOR ON 5/9/00 AND WAS GIVEN MEDICATED DRIPS. SHE MISSED ONE SHIFT OF WORK.
ON APRIL 10, 2000. EMPLOYEE REPORTED THAT HIS RIGHT ARM AND ELBOW WERE SORE. THE EMPLOYEE VISITED A DR ON 4-14-2000 AND WAS DIAGNOSED WITH TENNIS ELBOW. HE HAS BEEN GIVEN ANTI-INFLAMMATORY MEDICATION, EXERCISES, AND AN ELBOW BRACE. THERE ARE NO RESTRICTIONS ON WORK DUTIES. THE LIKELY CAUSE IS FROM REPETITIVE LIFTING AND PLACING OF CEMENT SACKS ONTO WOOD PALLETS.
EE WAS ASSISTING A WELDER ON THE #2 COMPARTMENT FAN PROJECT. HE WAS BENT OVER THE FAN TO HELP HODL A PART IN PLACE AND APPARENTLY TWISTED WRONG,. HE FELT PAIN IN HIS BACK A SHORT TIME LATER AN D TAKEN TO DR. HE WAS DIAGNOSED WITH A MUSCLE SPASM AND GIVEN PRESCRIPTION MEDICATION FOR PAIN AND A MUSCLE RELAXANT.
