EE WAS CLIMBING STAIR SCAFFOLDING TO THE 108 FT LEVEL TO FLY EQUIPMENT INTO PLACE WHEN HE FELL FROM THE STEAL STRUCTURE TO THE 45 FT LEVEL FLOOR BLEOW WAS WEARING BEAR HANESS AND DOUBLE LANYAR D WHEN FATALITY OCCURRED. MULTIPLE TIE OFF POINTS WERE PROVIDED.

A Mobile Mechanic boarded the underground mine service cage to leave the mine at the end of the shift. As the man/material hoist started to raise the employee from the mine level, the hoist went into fault and abruptly stopped, throwing the employee to the floor. Once on surface the employee felt pain in lower back and right leg.

EE's were installing the manhead (door) in a mill with the use of an overhead crane. As the door was raised in to position it got hung up overloading the rigging breaking the come along, striking and lacerating the EE's forehead

Accident type, without injuries

Skip bucket over loaded and went in to fault at the collar. When this happened the hoist rope bounced and set the safety dogs. The hoist controls had to be reset and the skip dogs had to be released. Equipment checked out and put back in to service

Struck by flying object

Employee was cleaning the preheater tower. During cleaning activities dust got on employees personal protective equipment. The employee opened EE's face shield to investigate issues with EE's tower cleaning apparatus and had dust come off EE's helmet helmet and in to EE's left eye

Mtce Supvr found in office chair breathing & unresponsive by office mate. Safety Mgnr & Miners rep arrived and began check compressions/AED applied as per dispatch(no shock). EMT and fire department arrived and gave NARCAN as per Supvr's wife's direction VIA speaker phone. Narcan woke up Supvr immediately like nothing happened.

Cutting loose temporary frame. In an attempt to safely lower a piece, a partial cut was made. Upon making final cut, piece was heavier than expected; causing a piece of metal to fall, striking EE in the left hand. This prompted the need to visit an ER where stitches were applied. EE returned to work the same day.

West end of raw fuel storage pile caught on fire. Unknown cause of fire. No one working in the area and no one affected by the fire. Heat warped tin roof and burned paint from support columns.

While walking on ground level to get a lab sample, employee tripped on a wheel chock that was sticking into the walkway. Injured employee fell to the ground landing on right knee, left hand, and left shoulder.

Employee was getting down from SV-26 scaler and rolled EE's right ankle on a used bolt that was laying on the ground where EE had parked.

Worker was attempting to unplug a dust collector hopper when material flushed out door. Material from collector got in employees eyes

Employee was bench testing a radiator cooling fan. The fan became unstable and the employee went to catch the fan. The employees hand came in contact with the rotating blades and lacerated 4 fingers.

EE rolled ankle as EE stepped off north access ramp to pump room. EE stepping off a concrete ramp to the ground & rolled ankle. On 5/26/20 EE went to urgent care, x-ray showed sprained ankle. Follow up visit on 5/28/20 showed improvement requiring continued OTC pain meds & ice packs. Follow up on 6/5/20 showed improvement & dr recommended phys therapy. On 6/8/20 EE started PT.

Absorption of radiations, caustics, toxic and noxious substances

Employee opened up a cement silo port to clear material that was not flowing. The material let loose and dusted employee in face and eyes. The gate isolating the product was not fully closed which allowed material to flow out inspection port at employee. Employee was wearing dust mask and safety glasses at the time of the injury.

Electrician was working inside an electrical box above a cement loading scale. A gust of wind blew across the scale moving particles of dust through the grating up into the electricians left eye. The eye was flushed with water several times after the incident. No permanent damage to eye.

Worker was pulling a scaffold pole from bottom of pile of 10 or so poles when a pole above the one EE pulled on slid down and pinched EE's left hand middle finger near the end.

Descaling in 3rd Stage with Jack Hammer when 2 pieces fell from roof. Hit headache deck busting through and hitting scaffolding before striking employee at bottom of cone. EE climbed out of unit on EE's own power, was transported by ambulance to Hospital ER. Examined, nine (9) stitches to parietal area at top of head, released with instructions for care of wound.

Employee was working to clear a blockage of a bin. Employee sat down to cool off. As he got up he became disoriented and needed assistance from his co-worker to stand. As a precaution the employee was taken to the ER where he was given an IV. The employee returned to work after being released from the ER.

Employee was cleaning on the preheat tower, when a large chunk from above fell and struck the lance the employee was using to clean. The lance inadvertently struck the employee on the right side of his face. This resulted in an orbital bone fracture.

Employee placed his hand on the mask of a forklift unknown to the forklift operator, the forklift operator inadvertently moved the forks and pinched the employee's fingers on his right hand.

Employee was stepping down off of a piece of equipment and felt a pop in his left knee. Employee stated he did not slip, twist, or trip on anything and maintained three points of contact as he descended the steps. Employee sought treatment at Occupational Health Services in Independence and has been diagnosed with a medial meniscus tear.

EE was repositioning right fork on Terex forklift. Standing between two forks, left hand holding up weight of fork, right hand on back for pushing. Left hand slipped moving fork into position and fork dropped. Right ring finger pinched between back of fork and frame. EE transported to Hospital. Plastic surgery to remove 1/2"" of finger.

Employee detected roof fall that exceeded the bolt length anchor points. Mine Manager, phoned the Rolla office with no response. He then notified the One Call line to report this incident. MSHA investigated the accident on 2/24/2011.

Employee and contractor using jack stand to work on 988 loader tire chains. Stand was slick, covered in grease. Contractor was holding top of rod on jack, employee was lining up holes on rod so he could insert pin. Employee's thumb slipped into one hole. Rod slipped from contractor's hand, employee's thumb was cut. Treatment at Occupation Health-4 sutures, returned to full duty.

Employee was working on a trenching project on surface. He slipped on debris from the trench twisting his ankle resulting in a sprain. He is on modified duty and PT.

Employee was working in man basket. He was hanging netting in the underground mine, work was above his head. He tried to straighten the netting out so that there was no slack and one side fell. Material was heavier than he anticipated and it jerked his arm and strained his shoulder. He sought treatment and is on 20 lb lifting restrictions. No Rx meds were prescribed.

Employee welding on Fletcher scaler pick rotation standing on bottom rung of ladder. He was wearing rubber boots with pant legs tucked inside. Spark fell into boot and burned top of left foot. Employee removed boot and had red area size of quarter on top of foot. Pain ceased, put boot back on, placed pants over boots to prevent a reoccurence. Did not report to spv til 7/9/07.

Employee was changing hydraulic hose on single boom drill and stepped off ladder. His right knee gave way as he stepped on floor, but didn't slip or fall. He did not seek treatment at that time. He began experiencing pain again 10/3 when he slipped moving a jack. He sought treatment 10/4, was diagnosed with torn cartillage. Placed on light duty pending MRI and possible surgery.

While trying to remove a piece of metal from the magnet while magnet was still on the metal was pulled back up to magnet crushing finger between magnet surface and metal, resulting in fractured bone.

Mine lost power 8/23-9:35 AM. Compromises two ways out. Employees stopped work, assembled in lunch room. Problem with KCPL-no info on duration of outage. Evacuation ordered 9:40 using backup power supply, called MSHA. Evacuation recorded 9:40. 9:47-employees out of mine and accounted for. Phoned MSHA later in day-employees sent home, will be back 8/24-6:00 AM shift.

Hoist Brake accumulator failed on the production shaft at 11:25 AM. The mine was evacuated at 11:55 AM. Repairs were completed at 1:00 PM.

Employee was working on piece of mobile equipment when he felt his knee pop. Employee reported injury to supervisor on 6/24/04, but did not seek treatment at that time. Employee was off on long weekend and iced knee to help condition. Assessment at OHS on 7/6/04, no restriction. Return appointment 7/8, placed on modified duty.

46""X 14""X 3/4"" PIECE OF METAL WAS WEDGED AGAINST RESTRICTOR PLATE ON THE REVERSING FEEDER BELT. AFTER LOCKING OUT, EE HIT THE PLATE TWICE WITH HIS HAND. THE PLATE THEN FELL DOWN ON HIS HAND ON THE SECOND BLOW, PINCHING HIS HAND BETWEEN THE PLATE AND THE CONVEYOR BELT.
