Over-exertion in lifting objects

EE reports on 10/8/18 EE was carrying 5 gal. buckets of sand all week and EE's back began to feel sore but EE continued to perform job all week. EE did not report to Supervisor until 10/12/18, Health Bridge notified, self care measures were provided. EE came to work 10/15/18 complaining back still hurt & was sent to clinic. EE was placed on restricted duty.

The slurry water supply hose appeared to be leaking, it was under ice so we attempted to raise it's pump by cranking the hand crank. I felt a sharp pain, pull or tear in right shoulder.

Employee was pulling cables off from the shackles on a crane when his pinky finger to the right hand got smashed in between the pin and the shackles. He noted that he was wearing gloves when the incident happened this morning. Did not become reportable until 10/13/14 when he went to the doctor and his finger was splinted.

The employee was working as a Deck Hand, pulling a cable, hooking up a barge when he felt a pull in his abdomen. The injury was diagnosed as a Symptomatic Umbilical Hernia, reducable. He was referred to a surgeon and is scheduled to have outpatient surgery on Friday, November 13, 2009.

Miner was injured when his finger was smashed between the parts of a pump he was working on. He sustained a broken finger and 10 stitches were used to close an 8 cm laceration.

Employee was climbing over wall, slipped and caught himself causing a strained shoulder. This is now reportable because he has to have therapy. Before, he was on regular duty and waiting to see if shoulder improved. Reportable as of 7/24/07.

Employee was climbing stairs on his front-end loader. There was a bolt under the handrail, he caught his right thumb on the bolt which resulted in a laceration that required four stitches.

Employee was cuting rubber belting. The blade came out of the box cutter and cut employee's right thumb. It required 4 stitches.

Employee's eye became inflamed. Possible foreign body in eye. He will be having surgery on July 10 to remove scar tissue. Until then he is on full duty but was given a prescription for his eye. Will amend report when he has surgery.

Employee was at this plant location to change out a printer. He is our computer analyst and is not a miner. He had a massive heart attack, was transported to North Kansas City Hospital, where he passed away. He is considered office personnel and his regular location is at our main office.

Employee was carrying a spool of cable up a catwalk. He felt pain in his left side. He has not yet been diagnosed but could have a hernia. Employee is going to the doctor on 10/26/06.

Employee was lifting a headwire off the barge and strained his lower back.

Employee was swinging a sledge hammer and strained his lower back. He went to his family Chiropractor and he is now fine and back to work full duty. No lost time, no restricted duty.

This accident has just become reportable. Employee hurt his knee in January, but doctor thought it was only a slight strain. He was on full duty and was taking over the counter Aleve. It has been determined after an MRI that he has a slight tear in his right knee. He is continuing to work full duty but may have to have surgery in the future.

Employee was breaking ice off of the tipple shack, slipped and fell on stairway at the dredge. He hurt his left knee. There were no fractures. This is lost time.

Employee was working on a highloader, he lit a torch and burnt his right arm. He has second degree burns on his right arm.

Employee was jumbing down from a barge and hit foot on a timberhead. He was wearing his steel toed shoes. He has a slight fracture of the small toe on the left foot.

Employee was checking rollers on the conveyor when he tripped and hit his right elbow on the catwalk. This accident just became reportable because employee is going to therapy to improve his forearm and shoulder.

Employee was working on our dry dock. He was pushing on a lever and it flew back and struck him in the palm of his left hand. This injury required 19 stitches. This is not a lost time accident.

Struck by flying object

Employee was standing on the dock barge when the wind blew a piece of metal in left eye. EE did have safety glasses on at the time. The piece of metal went between the glass and INTO eye.

This accident just recently became reportable. EE was thought to have a bruised shoulder. Missed no time and was not under restrictions. However, EE is now going to therapy which makes it reportable. EE was using a pry bar and strained left shoulder.

EE REACHED UNDER A PIECE OF FOIL ON A PLATE AND PICKED UP WHAT HE THOUGHT WAS FOOD AND IT WAS A ROCK. HE BIT DOWN AND BROKE HIS DENTURE A TOOTH OUT OF HIS DENTURE AND DAMAGED TWO LOWER TEETH. APPARENTLY SOME OF THE MEN TOOK HIM A PLATE OF ROCKS AS A JOKE FOR HIS CHRISTMAS DINNER.
