While changing a wear liner on the T-400 crusher, EE was struck in the arm by a wear plate that took an unexpected fall during removal.

While scooping a bucket of stemming rock EE's knee bent a little sideways and gave out and made a loud pop. EE sprained EE's right knee.

Employee moving steel, pinched middle ring finger. Bruised finger and resulted in blood build up behind nail. Middle ring finger subungual hematoma was drained (blood drained from under finger nail), three day work restriction.

Prepping plant for production. Released a discharge gate and caught hand in gate. Cut/Abrasion to hand resulting in 3 stitches.

Helping grind and something (dust or irritant) got behind safety equipment and irritated eye. Classified as Right eye irritation/abrasion.

Employee removing drain plug from machine, leveraged self for additional force and bruised right chest rib. Diagnoses was Right Chest Contusion.

While in man lift, employee caused lift to bump him into beam, bruising chest

Brushed hand on excavator track and cut right 4th finger on chert or track pad. 4 sutures

Repairing conveyor belt, cranking on ratchet hoist, hoist failed, EE lost balance and fell backwards landing on tailbone and right hand.

Exiting dump truck, wind gust blew door and struck hand.

EE pinched his hand in the door resulting in a laceration.

Stepped on rock, twisted left knee.

Employee aggravated pre-existing knee injury (hockey injury). When applying pressure to raise up, knee popped.

Moving steel plate, plate slipped and pinched end of finger, resulting in rupture of skin approx. 1/4"".

Stepped out of truck and stepped on rock and twisted ankle.

Moving oil barrel, strained back.

Reparing primary crusher, strained back while lifting part/componet.

Employee working on conveyor belt, hand pinched in v-belt drive.

Picking up shims to a water pump during repair, shim cut left ring finger.

Employee was tarping equipment prior to move, tarp strap broke, employee lost balance and fell.

EMPLOYEE ATTEMPTED TO LIFT STEEL DOOR TO INSTALL ON CRUSHER AND STRAINED BACK. NOTIFIED OF ACCIDENT / STRAIN ON 8/11/05.

Employee was cutting flashing with box cutter. It slipped and cut right hand.

EMPLOYEE WAS ASSISTING WITH THE CUTTING OF TIMBERS, ONE TIMBER SHIFTED & MOVED WHILE RELOCATING ANOTHER TIMBER, EE'S ARM WAS STRUCK & BROKE.

Employee using a grinder, slipped and cut left leg (thigh).

EE WAS USING A PIPE TO EXTEND A WRENCH, WRENCH EXTENSION BROKE, PIPE STRUCK TOE.

EE WAS INSTALLING BELT ON SECONDARY CRUSHER. PINCHED PINKY FINGER BETWEEN BELT AND PULLEY.

EMPLOYEE WENT TO LIFT DRILL STEEL, FELT PAIN IN BACK.

EMPLOYEE WAS REPAIRING CONVEYOR BELT, KNIFE SLIPPED AND CUT THUMB.

EE WAS CLEANING OUT SECONDARY CRUSHER THAT HAD PLUGGED UP. ROTOR ROLLED AND PINCHED RIGHT FOOT.

EMPLOYEE SLIPPED WHILE IN THE BED OF MECHANICS TRUCK, FELL AND LANDED ON PIECE OF STEEL, LACERATION ON LEFT PALM.
