The weather was after the ice storm, several inches of ice. We had ramps and roads covered in dust. The EE got out of equipment, walked over where the edge was not coated and he fell.

THE PLANT WAS SHUT DOWN FOR MAINTENANCE THE EE WAS MANUALLY TURNING A PULLEY WHEN HIS FINGER GOT CAUGHT BETWEEN THE BELT & THE SHIVE
EMPLOYEE WAS PREPARING TO SERVICE THE 980 LOADER TO GET TO THE ENGINE YOU HAVE TO PULL OPEN FENDERS. IN PULLING ON ONE OF THE FENDERS HE HURT HIS BACK. 2 WEEKS LATER HE COULD NOT WORK.
THE EMPLOYEE WAS SHOVELING. HE LAID THE SHOVEL ON THE GROUND TO CLIMB OUT OF THE HOLE. HE TRIPPED AND FELL. HIS HAND HIT THE SHOVEL.
EE WAS UNPLUGGING THE CRUSHER. REMOVING THE ROCKS BY HAND AND ONE ROLLED OVER HIS FOOT.
