Fall to the walkway or working surface

Employee parked their personal vehicle in the scale house parking lot and slipped on ice and fell backwards as they were exiting their vehicle. Icy conditions contributed to injury.

Employee was performing belt Maintenance and lost EE's balance while on a 6ft step ladder and fell backwards to the ground. Right leg Fibula, Tibia, and ankle fractures.

Employee was stemming holes and when employee took a scoop of rock employee hit something hard in the stemming pile and hurt shoulder. I dug through the stemming pile and found a large rock.

THE PLANT WAS SHUT DOWN FOR MAINTENANCE THE EE WAS MANUALLY TURNING A PULLEY WHEN HIS FINGER GOT CAUGHT BETWEEN THE BELT & THE SHIVE

EMPLOYEE WAS PREPARING TO SERVICE THE 980 LOADER TO GET TO THE ENGINE YOU HAVE TO PULL OPEN FENDERS. IN PULLING ON ONE OF THE FENDERS HE HURT HIS BACK. 2 WEEKS LATER HE COULD NOT WORK.

THE EMPLOYEE WAS SHOVELING. HE LAID THE SHOVEL ON THE GROUND TO CLIMB OUT OF THE HOLE. HE TRIPPED AND FELL. HIS HAND HIT THE SHOVEL.

EE WAS UNPLUGGING THE CRUSHER. REMOVING THE ROCKS BY HAND AND ONE ROLLED OVER HIS FOOT.
