EE noticed the vent tube waving and EE's ears began popping and the door on the #5044 haul truck slung open while EE brought EE's truck to a stop. A planned blast took place and the haul route was affected. There were no injuries observed. EE has had multiple doctor visits to diagnose issues with eye and ear discomfort and to explore if there was any damage present.

Employee was holding hydraulic manifold block when it slipped from the pry bar and smashed employee's finger between block and fender. Employee was taken to emergency room and received approximately 12 sutures.

Employee was loading bolts onto 3116 bolter when he lost his footing and fell on right shoulder and elbow. Employee is treating with orthopedic physician and he was given a cortizone injection on 9-18-08.

EE was charging on 103EPL using 0284 charging rig. While charging holes, EE noticed smoke coming from the comprssor. EE shut off rig and turned off master switch. Fire was too big, so he left area to go page phone to call foreman. Both Buick and Casteel mines were evacuated. MSHA was called and notified of fire within 5 minutes.

Employee stepped off of 0320 980 loader and twisted right ankle on slick rock. He was taken to company doctor and referred to orthopedic specialist. He was restricted to limited duty and began losing time 07-25-07.

Hoistman reported brake failed brake test. After troubleshooting and testing, it was decided to replace the brake blocks. Hoist was put back in service on 6/15/07 at 7:30 pm.

Compressed air line to shotcrete nozzle appeared to be clogged when air was turned on. The employee noticed air line started to come off nipple, he tried to shut the compressor off & the line came off & squirted accelerant in his eye. EE was taken to the company Dr. & prescribed an antibiotic.

Main breaker in sub-station tripped. When they first tried to reset breaker, it wouldn't stay in. Hoist went down at 9am. Msha was notified at 9:30am. Hoist was down and came back up at 3:30pm

While driving to Casteel with road marked as an escape route. Road had been shot out and not properly barricaded, allowing Ee to drive off of an 8' bluff in chest deep water. EE hurt leg and knee. He was taken to county Dr and to an orthopedic specialist. He is now off work beginning 7/3/06 waiting for knee surgery.

Employee was getting on cage when door of cage went through shoe right behind steel toe hitting left foot. Foot instantly began to swell. Employee was taken to company dr. on 05-13-06 and was diagnosed with a broken toe. Employee is off work.

Employee was scaling heading when he heard a loud boom. He realized machine was on fire and got out. He received burns to face, hands and right thigh. Employee was taken to ER and transported on to St. Louis. Employee remains off work.

EE was operating a mechanical scaler when he heard a loud boom. Scaler caught on fire and mine was evacuated at that time. It is beleived that this was an electrical flash fire & self-extinguised in less that 30 minutes. We notified MSHA at the time because it was thought we had a reportable mine fire. Upon further investigation, it revealed it probably was not.

EMPLOYEE WAS CARRYING A BOX OF POWDER TO CHARGE BLUFF HOES WHEN HE TRIPPED ON ROCK, FELL FORWARD & CAUGHT HIMSELF. WHEN HE FELL HE CUT PALM ON RT. HAND. EE WAS TAKEN TO E.R. WHERE HE HAD 3 SUTURES MAKING THIS REPORTABLE.

Employee was climbing off grader to hang anfo. While climbing off, his foot slipped on slick tire causing him to fall to ground hitting his hand and hip. Employee was taken to Salem ER where he received a splint to immobilize his thumb.

Employee was going out to look at piece of equipment when he stepped on rock twisting his right knee. He was taken to co. dr on 9-27-05. Due to results of MRI, He was seen by specialist on 10-20-05. He had surgery on 11-11-05 to repair the meniscus tear to right knee. He return to work next scheduled shift.

Employee was placing hook to get boulder out. Boulder moved, hook went forward then back while employee had hold of hook pulling his shoulder and arm. Employee has been treated for tingling and pain in left shoulder and arm. Employee had surgery on 3/14/06.

Employee was charging a swing in 103W. When he finished he reached across the basket to turn his air valve off and a piece of loose fell from face approximately 3 to 4 feet above him struck him on the arm. Cut to left hand required 12 sutures.

EMPLOYEE WAS CLIMBING OVER BOULDER PILE WHEN HE LOST HIS FOOTING AND FELL. HE FELL ON HIS HANDS AND CUT HIS RIGHT HAND ON A JAGGED EDGE OF A BOULDER. HE WAS TAKEN TO THE DOCTOR AND RECEIVED EIGHT SUTURES.

EE WAS HAND MINING IN 82 SOUT STOPE WHEN SMALL PIECE OF LOOSE OR ROCK CAME OVER TOP OF GLASSES LANDING IN HIS RIGHT EYE. EE WAS TAKEN TO COMPANY DR. ON 7-25-02, TO REMOVE FOREIGN OBJECT. DR. A PPLIED ANTIBIOTIC CREAM ON THE EYE MAKING THIS A REPORTABLE INJURY.

WHILE GETTING SHOTCRETE SCREEN OFF A STACK, ONE HUNG ON HIS SHIRT. AS HE TURNED TO FREE HIS SHIRT HE FELT A PAIN IN HIS RIGHT SHOULDER. ON 8/19 HE UNDERWENT SURGERY TO REPAIR AN ANTERIOR LABRA L TEAR IN THE RIGHT SHOULDER AT WHICH TIME LOST WORKDAYS BEGAN.

CONNECTION ON COMMUTATING POLE BURNED LOOSE ON DC HOIST MOTOR. CONNECTION WAS REPAIRED. HOIST SWITCHED TO ALTERNATE AC DRIVE WHILE REPAIRS WERE MADE. HOIST WAS SWITCHED BACK TO DC DRIVE AT 2 :30 PM. EXCEPT FOR SWITCH OVER TIME, HOIST WAS ALWAYS AVAILABLE.

EE WAS ASSISTING CHANGING A TIRE ON A JUMBO. AS THE NEW TIRE TO BE MOUNTED STARTED TO FALL, EE STRAINED HIS LOWER BACK TRYING TO PREVENT THE TIRE/WHEEL FROM FALLING OVER. AFTER THE 3RD DR VISI T ON 5-22-02 FOR THE STRAINED BACK, THE DR PRESCRIBED THERAPY FOR 4 WEEKS. THE EE IS STILL WORKING WITH NO RESTRICTIONS.
