EMPLOYEE WALKED BETWEEN TWO PALLETS HOLDING LINER PLATES TO THROW AWAY TRASH. WHILE WALKING BACK, THE EMPLOYEE CAUGHT A FOOT ON THE LINER PLATE AND FELL ON THEIR LEFT SIDE. THEY FRACTURED A RIB ON THEIR LEFT SIDE AND BRUISED THEIR LEFT SHOULDER.

EMPLOYEE WAS USING A COME-A-LONG TO PULL A DENT OUT OF A FENDER TO ATTACH A HANDRAIL, WHEN THE COME-A-LONG SLIPPED OFF AND STRUCK THE EMPLOYEE ON THE LEFT SIDE OF THE HEAD.

EE FELT PAIN AND POP IN RIGHT ELBOW WHILE SHOVELING MUCK ON GROUND. EE REMAINED ON FULL DUTY WITH NO RESTRICTIONS UNTIL SEEN BY A SPECIALIST, THE SPECIALIST THEN ADMINISTERED AN INJECTION AND PLACED EE ON RESTCITED DUTY ON 05/01/2023.

EE OPERATING SCALER DOWN ROADWAY, HIT POTHOLE IN ROADWAY, BOUNCED IN SCALER AND FELT PAIN IN LOW BACK. EE WENT FOR MEDICAL EVALUATION ON 01/03/2023 AND WAS PLACED ON RESTRICED DUTY AT THAT TIME. EE WAS RELEASED TO FULL DUTY ON 01/09/2023.

EE riding in buddy seat while task training another employee. EE operating the truck hit a bump in the road causing the EE to hit head on cab. EE placed on restricted duty following medical evaluation.

EE alleged on 9/26/22 a pain in arm after operating loader on 9/25/22. EE placed on restricted duty following medical evaluation.

EE SITTING ON GROUND REMOVING STRAP FROM AROUND BOOM SO IT COULD BE MOVED, BOOM SHIFTED AND STRUCK EE IN RIGHT KNEE. EE PLACED ON RESTRICTED DUTY AFTER MEDICAL TREATMENT.

EE smashed finger in gate on the charging rig basket when exiting basket. EE received laceration and fracture to end of bone in left pinky finger. EE received sutures and released back to full duty.

EE CLIMBING LADDER ON HAUL TRUCK TO INSPECT FIRE EXTINGUISHER FOR MONTHLY INSPECTION. EE FOOT SLIPPED ON LADDER, STRAINING LEFT ARM. EE PLACED ON RESTRICTED DUTY ON 11/3/2021.

EE placed on restricted duty following a strain in groin area from lifting gates on the cage to put it back together after hoisting equipment.

EE noticed the vent tube waving and EE's ears began popping and the door on the #5044 haul truck slung open while EE brought EE's truck to a stop. A planned blast took place and the haul route was affected. There were no injuries observed. EE has had multiple doctor visits to diagnose issues with eye and ear discomfort and to explore if there was any damage present.

EE attempting to sit on bench in men's change room and collapsed onto floor. Witness to event began emergency procedures. EE taken by ambulance to Hospital and pronounced deceased.

EE walking down stairs, fell down 2-3 steps. EE was placed on restricted duty following medical treatment on 01/26/2021. EE will begin physical therapy as well.

EMPLOYEE WAS USING A SCALING BAR TO PRY A MIDDLE DRIVELINE ON AN ST8 AND FELT PAIN IN RIGHT ELBOW. EE WAS PLACED ON RESTRICTED DUTY FOLLOWING MEDICAL TREATMENT.

EE diagnosed with bilateral cubital tunnel syndrome on 12/15/2020. Treatment and/or surgery is pending at this time.

EE turning steering wheel and felt pain in right shoulder. EE placed on restricted duty following medical treatment.

Was unhooking the drill and stepped wrong and experienced a pull in left knee which strained the knee.

Laceration on left thumb requiring sutures while splicing cable. EE released back to work with no restrictions.

EE diagnosed with Carpal Tunnel Syndrome on 2.13.2020. EE will have surgery and be released to light duty while recovering, surgery dates are pending at this time. EE remains full duty until surgery.

EE climbing up into loader and felt pain in shoulder. EE placed on restricted duty after medical attention on 2/3/2020.

EE felt pain in abdomen when lifting a 16' drill steel. EE placed on restricted duty on 8/22/2019 while testing was performed. Testing was negative for hernia and EE was released back to full duty.

EE removing a fan from a bulkhead. EE felt pain in neck and upper back from cutting through cables. EE placed on restricted duty on 8/6/2019 after medical treatment.

EE using a pry bar to move a stuck rock, pry bar slipped and mashed left ring finger. EE received 5 sutures in finger.

EE LACERATED RIGHT RING FINGER BETWEEN HOIST ROPE AND DRUM. EE RECEIVED SUTURES AND WAS PLACED ON RESTRICTED DUTY AT THAT TIME.

EE pulling on blown hyd hose and slipped and fell. EE felt pain in right shoulder and arm. EE placed on restricted duty and released following week back to full duty.

EE diagnosed with carpal tunnel syndrome 2.21.19. Treatment and/or surgery is pending at this time.

EE slipped in shower in changeroom on surface. EE received laceration on head requiring sutures. EE released back to work following medical treatment.

EE lifting cases of powder onto charging rig basket when EE hit edge of basket with box and caused pain in left shoulder. EE was placed on restricted duty on 7/5/2018.

EE stepping off tire on charging rig, stepped onto rock, rock rolled and caught toe under edge of tire bending right foot backwards, fracturing distal fibula of right leg. EE was placed on restricted duty after medical evaluation.

EE moving pipe. EE unchained the end of the pipe to move its location when the pipe swung and struck EE on side of head and chest. EE placed on restricted duty.

Received email (1/18/2018) with diagnosis of Occupational Carpal Tunnel Syndrome on 12/8/2017. Treatment is pending at this time. EE will be released with restricted duty following surgery.

EE received a laceration on left ring finger while installing belts on a shaker motor. EE received stitches and was released back to work.

EE fell from steps when exiting mechanical scaler. EE was placed on restricted duty for pain in left hand/wrist.

A crack in a fuel tank on a dump truck was being repaired by a welder when the tank exploded. The nearby driver suffered 2nd and 3rd degree burns. The driver is at St. Johns Mercy Hospital in St. Louis, MO.

EE was scaling the back when the tooth slipped and jarred the cab of the mechanical scaler. EE was placed on restricted duty on 6.17.16. Further testing is pending.

EE training another employee in the skip pocket when EE was hit in the head with a rock, causing a cervical strain. EE was placed off work pending further testing.

EE exiting drill when EE missed the step and fell to the ground. EE felt pain in lower back and was placed on restricted duty for 2 days following incident.

EE stepped off getman scaler and twisted right ankle. EE placed on restricted duty on 2/05/16 and returned to full duty on 2/12/16.

EE using a sledge hammer to drive out a cylinder pin when a piece broke off the hammer and hit employee in the right arm. EE received stitches and returned back to work with no restrictions.

EE was nipping materials at the shaft landing when EE was stung by a wasp on the left hand, pointer finger. EE received no medical treatment.

Wasp was on the bell cord, when the EE pulled the bell cord to call for the cage he was stung by the wasp. EE received no medical treatment.

EE cutting off a piece of anfo hose when the hose flipped up and anfo went under his glasses and into his eye. Eye doctor prescribed an antibiotic for his left eye. EE returned to work with no restrictions.

EE picked up a case of dynamite, walked to the face and when he bent over to set the box down he felt pain in his lower back. EE placed on restricted duty.

EE driving a truck, hit a rock in the roadway and EE bounced up in the cab hitting his head on the cab and felt pain in back. EE placed on restricted duty on 4.28.15.

Hoist went down at 9:30 pm on Saturday 3.7.15. The skip on the underwind rope did not close because of a boulder and caused a slack rope condition. Repairs were made and hoist was inspected and released at 11:15 pm.

EE driving a haul truck. EE fell asleep and ran into edge of pillar underground. EE taken to ER that day and released with no restrictions. EE taken to dr on Monday, March 9 and placed on restrictive duty.

Alleged Carpal Tunnel Syndrome. EE remains on Full Duty while undergoing conservative care treatment.

EE was swinging a sledge hammer to tighten a hitch nut on a truck when he felt sharp pain in stomach. EE was taken to company dr and placed on restricted duty until seen by a surgeon. Surgery was scheduled by surgeon for 11.20.14. EE ""should"" be able to return to work on restricted duty the following Monday.

EE was operating a grader, when put into reverse the grader jerked and EE felt pain in neck. EE received restricted duty per company Dr.

Rock fell from back and hit employee on hard hat.

EE underwent Carpal Tunnel Surgery on 8/21/14 - Occupational Illness

EE alleged carpal tunnel on 5/13/2014. EE treated with conservative care until dr recommended surgery. Surgery is not yet scheduled. EE will have light duty restrictions after surgery.

EE was opening a box with a box cutter when the box cutter slipped, cutting employees left thumb, requiring stitches.

Employee was hammering a bearing when a piece broke off and penetrated his left forearm.

EE went to doctor and was diagnosed with carpal tunnel in his wrist. The date of the occurrence is unknown.

EE was drove haul truck over a boulder in the road and injured his back, hip and shoulder. He was given a second injection in his back on August 11, 2011 making this a MSHA Reportable.

EE was cleaning out bottom drill holes when a rock fell and cut him on his left leg resulting in stitches.

EE was using 10' mining bar to clear ore bin, rock fell onto bar causing bar to twist left hand and wrist. EE had surgery on 08/19/2010 making this a Lost Time Accident.

EE was lifting a personnal tool box when he felt pain in center of stomache. EE was taken to company doctor and on 6/5/09 he was diagnosed with a small herina. EE was released to RTW by company phyisian. ---(Surgery was on 8/6/09 making this a lost time-Misc Mail 8/10/09 #2)

Employee was holding hydraulic manifold block when it slipped from the pry bar and smashed employee's finger between block and fender. Employee was taken to emergency room and received approximately 12 sutures.

Employee was loading bolts onto 3116 bolter when he lost his footing and fell on right shoulder and elbow. Employee is treating with orthopedic physician and he was given a cortizone injection on 9-18-08.

EE was charging on 103EPL using 0284 charging rig. While charging holes, EE noticed smoke coming from the comprssor. EE shut off rig and turned off master switch. Fire was too big, so he left area to go page phone to call foreman. Both Buick and Casteel mines were evacuated. MSHA was called and notified of fire within 5 minutes.

Employee stepped off of 0320 980 loader and twisted right ankle on slick rock. He was taken to company doctor and referred to orthopedic specialist. He was restricted to limited duty and began losing time 07-25-07.

Hoistman reported brake failed brake test. After troubleshooting and testing, it was decided to replace the brake blocks. Hoist was put back in service on 6/15/07 at 7:30 pm.

Employee alledged repetitive motion in both arms and hands. He treated with a specialist and had surgery to the right hand on 9/6/07. He will have left hand surgery on 9/20/07.

Compressed air line to shotcrete nozzle appeared to be clogged when air was turned on. The employee noticed air line started to come off nipple, he tried to shut the compressor off & the line came off & squirted accelerant in his eye. EE was taken to the company Dr. & prescribed an antibiotic.

Main breaker in sub-station tripped. When they first tried to reset breaker, it wouldn't stay in. Hoist went down at 9am. Msha was notified at 9:30am. Hoist was down and came back up at 3:30pm

While driving to Casteel with road marked as an escape route. Road had been shot out and not properly barricaded, allowing Ee to drive off of an 8' bluff in chest deep water. EE hurt leg and knee. He was taken to county Dr and to an orthopedic specialist. He is now off work beginning 7/3/06 waiting for knee surgery.

Employee was getting on cage when door of cage went through shoe right behind steel toe hitting left foot. Foot instantly began to swell. Employee was taken to company dr. on 05-13-06 and was diagnosed with a broken toe. Employee is off work.

Employee was scaling heading when he heard a loud boom. He realized machine was on fire and got out. He received burns to face, hands and right thigh. Employee was taken to ER and transported on to St. Louis. Employee remains off work.

EE was operating a mechanical scaler when he heard a loud boom. Scaler caught on fire and mine was evacuated at that time. It is beleived that this was an electrical flash fire & self-extinguised in less that 30 minutes. We notified MSHA at the time because it was thought we had a reportable mine fire. Upon further investigation, it revealed it probably was not.

EMPLOYEE WAS CARRYING A BOX OF POWDER TO CHARGE BLUFF HOES WHEN HE TRIPPED ON ROCK, FELL FORWARD & CAUGHT HIMSELF. WHEN HE FELL HE CUT PALM ON RT. HAND. EE WAS TAKEN TO E.R. WHERE HE HAD 3 SUTURES MAKING THIS REPORTABLE.

Employee was climbing off grader to hang anfo. While climbing off, his foot slipped on slick tire causing him to fall to ground hitting his hand and hip. Employee was taken to Salem ER where he received a splint to immobilize his thumb.

Employee was going out to look at piece of equipment when he stepped on rock twisting his right knee. He was taken to co. dr on 9-27-05. Due to results of MRI, He was seen by specialist on 10-20-05. He had surgery on 11-11-05 to repair the meniscus tear to right knee. He return to work next scheduled shift.

Employee was placing hook to get boulder out. Boulder moved, hook went forward then back while employee had hold of hook pulling his shoulder and arm. Employee has been treated for tingling and pain in left shoulder and arm. Employee had surgery on 3/14/06.

Alleged carpal tunnel due to repetitive motion. Employee was seen by company doctor on 03-23-05 and was prescribed a wrist splint to use at home. X-ray of wrist and hand were taken.

Employee was getting a dipper of rock to load truck. He hit a hole in road, causing him to hit his elbow on the window frame. Employee did not go to dr. until 6-22-05 where he was given a steroid injection.

EMPLOYEE WAS FINISHING MAINTENANCE JOB ON GETMAN SCALER. HOOD HAD A BURR ON IT & WHEN HE SHUT THE HOOD IT SLIPPED, CAUSING A LACERATION TO HIS RT. HAND REQUIRING 4 SUTURES.

Employee was charging a swing in 103W. When he finished he reached across the basket to turn his air valve off and a piece of loose fell from face approximately 3 to 4 feet above him struck him on the arm. Cut to left hand required 12 sutures.

EMPLOYEE WAS CLIMBING OVER BOULDER PILE WHEN HE LOST HIS FOOTING AND FELL. HE FELL ON HIS HANDS AND CUT HIS RIGHT HAND ON A JAGGED EDGE OF A BOULDER. HE WAS TAKEN TO THE DOCTOR AND RECEIVED EIGHT SUTURES.

Alleged carpal tunnel left and right wrists. Surgery was performed on the right wrist on 08-04-04. The left wrist will be done on 08-18-04.

EMPLOYEE WAS CLEANING OUT SUPER 4 BASKET. WHEN HE TRIED TO MOVE A ROLL OF SCREEN WIRE OUT OF THE BASKET, HE FELT PAIN IN THE LOWER LEFT SIDE OF HIS BACK. EMPLOYEE WAS DIAGNOSED WITH HERNIATED DISC AND WAS GIVEN AN INJECTION ON 6/20/04. HE HAD BACK SURGERY ON 8/9/04 MAKING THIS A LOST TIME INCIDENT.

EE WAS HAND MINING IN 82 SOUT STOPE WHEN SMALL PIECE OF LOOSE OR ROCK CAME OVER TOP OF GLASSES LANDING IN HIS RIGHT EYE. EE WAS TAKEN TO COMPANY DR. ON 7-25-02, TO REMOVE FOREIGN OBJECT. DR. A PPLIED ANTIBIOTIC CREAM ON THE EYE MAKING THIS A REPORTABLE INJURY.

WHILE GETTING SHOTCRETE SCREEN OFF A STACK, ONE HUNG ON HIS SHIRT. AS HE TURNED TO FREE HIS SHIRT HE FELT A PAIN IN HIS RIGHT SHOULDER. ON 8/19 HE UNDERWENT SURGERY TO REPAIR AN ANTERIOR LABRA L TEAR IN THE RIGHT SHOULDER AT WHICH TIME LOST WORKDAYS BEGAN.

CONNECTION ON COMMUTATING POLE BURNED LOOSE ON DC HOIST MOTOR. CONNECTION WAS REPAIRED. HOIST SWITCHED TO ALTERNATE AC DRIVE WHILE REPAIRS WERE MADE. HOIST WAS SWITCHED BACK TO DC DRIVE AT 2 :30 PM. EXCEPT FOR SWITCH OVER TIME, HOIST WAS ALWAYS AVAILABLE.

EE WAS ASSISTING CHANGING A TIRE ON A JUMBO. AS THE NEW TIRE TO BE MOUNTED STARTED TO FALL, EE STRAINED HIS LOWER BACK TRYING TO PREVENT THE TIRE/WHEEL FROM FALLING OVER. AFTER THE 3RD DR VISI T ON 5-22-02 FOR THE STRAINED BACK, THE DR PRESCRIBED THERAPY FOR 4 WEEKS. THE EE IS STILL WORKING WITH NO RESTRICTIONS.

EMPLOYEE WAS SCALING, FELT PAIN IN CHEST AND STOPPED TO REST. TOOK EMPLOYEE TO SHOP AND THEN ON TO DOCTOR'S OFFICE. TREATED FOR HEART ATTACK, SENT TO HOSPITAL, THEN ON TO ANOTHER HOSPITAL IN S T LOUIS. HEART ATTACK FROM BLOCKAGE - TWO STENTS PUT IN AT HOSPITAL.

HE SLIPPED GETTING OUT OF A SCALER AND SPRAINED HIS BACK.
