Employee was climbing down an 8 foot A-frame ladder. The employee lost his balance and fell about 4 feet to the shop floor.

Employee was driving a stockpile truck, crossed ditch in road jarring neck area. Employee reported at that time but did not want medical treatment. Became medical case in June 9 when employee went to doctor and started physical therapy.

EE GOT OUT OF TRUCK CAB TO CHECK THE DISTANCE ON THE BLIND SIDE OF TRUCK. WHILE WALKING ON HOOD OF TRUCK TO THE FAR SIDE, HE FELL OFF THE TRUCK HITTING THE GROUND.

EMPLOYEE WAS REMOVING A COVER ON THE SPOKANE CRUSHER. WHEN HE WAS LIFTING THE COVER, HIS HANDS SLIPPED, CAUSING HIM TO STUMBLE BACKWARDS, AND FALLING AGAINST THE WALKWAY HANDRAIL.
EMPLOYEE WAS EXITING THE CAB OF THE TRUCK AND INJURED FROM THE TOP STEP TO THE GROUND, APPROX 3 FOOT. WHEN HE LANDED ON THE GROUND, HE JARRED HIS BACK.
EE WAS WALKING DOWN A HILL, AND HE DID NOT REALIZE IT WAS ICY AND HE SLIPPED AND FELL CATCHING HIMSELF WITH HIS RIGHT ARM. *NOTE* THIS DID NOT BECOME REPORTABLE TILL 9-14-01.
