Accident type, without injuries

A haul truck was in use in a sand quarry when a fire started in the engine compartment. The operator immediately shut the truck off, exited the cab, dismounted the truck, and distanced self safely from the machine. The fire was extinguished by local EMS. The extensive damage to the front-end of the truck is expected to result in a complete loss of the machine.

A Relief Operator while closing a valve in the crushing area felt a pop in left elbow. Employee immediately reported the occurrence to supervisor. As the shift progressed the pain, motion, and hand grip strength worsened causing the employee to request a medical evaluation.

Employee cut forehead on a length of steel channel sticking out the bed of the pickup truck. Employee received 12 stitches.

Employee was taking a break in EE's work truck while mowing grass at the main office. EE went to wipe sweat from EE's eye when EE felt something go into EE's right eye.

Employee slipped on ice during preshift inspection of the wash house and fell forward onto right shoulder. Employee initially taken to doctor and released to full work duty. Employee returned to doctor on 02/22/18 and diagnosed with Full tear of the right rotator cuff and partial tears of other ligaments.

Employee crushed finger dislodging rock in feeder, which resulted in a fracture to the tip of EE's left pinky.

Employee stepped up on to step stool using the right foot. While bringing the left foot up onto the step stool, employee's right leg gave out, or she lost her balance. Employee fell to the left onto the floor.

BUCKET ELEVATOR BELT BROKE AND FELL. A CRANE WAS USED TO LIFT THE BELTING AND RE-LOOP IT OVER THE HEAD PULLEY INTO HOUSING. EMPLOYEE DIRECTING THE CRANE USED NYLON STRAP AS A CHOKER. WHEN THE BELT HAD BEEN LIFTED 30 FT THE NYLON STRAP PINCHED THROUGH BELT CAUSING IT TO FALL. THE EMPLOYEE GUIDING THE BELT WAS STRUCK IN THE HARD HAT AND BACK BY THE BELTING AND BUCKETS.

PERFORMING MAINTENANCE ON SAND COOLER INJ EE & COWORKER DESCENDED THE LADDER. INJ EE DECENDED THE LADDER. INJ EE MOUNTED THE LADDER THE WELD BROKE AND INJ EE FELL 16 FT ONTO HIS UPPER BACK. EE WAS TAKEN TO THE HOSPITAL FOR A EXAMINATION. EE ATTENDED SEVERAL FOLLOW UP VISITS.EE WAS REFERED TO A BACK SPECIALIST. PHYSICAL THERAPY WAS PRECRIBED.
