The worker was in process of adding Material to the base of the tailings dam at Brushy Creek Mine. The moxy haul truck was going down the slope of the dam when the driver lost control. As he tried to negotiate the turn near the bottom, the truck rolled over and landed on side in a 6' deep seep pond. Diver's body was found pinned under truck and in water.

EE was attempting to cut a bolt out of the hitch of a portable vent fan with a handheld hacksaw, the saw slipped out the cutting groove and cut the top of his left hand. EE received 5 stitches. No restrictions.

#34 Service Hoist went down on 6/7/15 at 4:40 am due to an electrical issue. Electricians were called as well as MSHA. Electrician's repairs were complete; hoist was inspected and placed back into service at 10:05 am. MSHA was called back to update status at 10:08 am.

#34 Service Hoist was taken out of service at 9:30 am on 12.26.15 for the normal 7 day inspection. Hoist inspection was completed at 12:00 pm with problems found during the inspection that did not allow the release of the cage. MSHA was called as well as maintenance personnel to make needed repairs. Repairs were made and hoist was re-inspected and released at 12:00 pm on 5.27.15

EE slipped while climbing down ladder. EE was treated with first aid for scrape on thigh. EE now complains of back pain. Injury became reportable on 8.4.15 when doctor placed EE on restricted duty.

At 2:15 pm on 3.28.15 the electrical power wire fell down the shaft. The electricians and utility personnel were called to make needed repairs. The shaft was inspected and cleared for use at 1:55 pm on 3.30.15.

EE getting off the cage underground fell hitting elbow on ground. Fracture of bone in elbow.

EE felt sharp pain in back area under ribs while hand scaling. Dr diagnosed a fracture to one rib. EE released to restricted duty.

EE was tightening up bolts on haul truck when he felt pain in his stomach. He was diagnosed on March 15th with a hernia and had surgery to repair the hernia the same day. The employee's next scheduled worked day is March 21st.

The old metal weigh hopper floor was attached to the skip hoist and was being pulled up out of the mine when the metal floor got wedged inside the shaft preventing the 7 employees at that current time to exit the crusher. MSHA was notified at 5:00 p.m. and the employees were hoisted out of the crusher at 8:50 p.m.

EE was conducting a presift exam on his mechanical scaler when he was walking around the scaler his foot rolled on a rock and he heard his right knee pop. EE worked until 5/6/11, when he had his knee surgery.

EE was washing sweeper truck hopper out with high pressure water hose, when a piece of dry concrete fell down on top of his hat. EE had pain in neck and extending to mid back. After treating with company doctor, EE was referred to a specialist and due to restrictions, he could not return to work on 3/22/10.

EE was remoivng a rusty bolt under the shop truck, when a piece of rust/dirt fell onto her face and under safety galsses, into her right eye. EE was taken to ER where her eye was washed out and she was prescribed antibiotice eye drops, making this reportable.

While backing remote past himself, right rear tire shot a rock out hitting the employee in the knee. After continuous treatment employee had surgery to the right knee on 11/04/09 making this a lost time.

Employee was standing in basket holding the emulshion hose when he caught the on/off valve with mine belt. When pump came on he lost hold of hose and it sprayed him in the right eye. He was taken to eye doctor and released to work.

EE was walking on uneven concrete floor, when she turned her left ankle. EE was taken to ER on 8/25/08 and on 8/27/08. She was seen by company physician and diagnosed with a fracture of the foot. She began loosing time on 8/27/08.

Employee pulled himself up on jumbo hood to work on slip rings and stated he pulled his back. He was seen by company dr. on 5/21/08 and again on 5/27/08. Dr took off work pending x-ray and wants to keep him off work until next evaluation.

EE WAS CUTTING A ROOFBOLT. WHEN THE BOLT WAS CUT INTO, THE PRESSURE ON IT CUASED IT TO SWING AROUND AND STRIKE THE EE ON THE RIGHT INDEX FINGER. EE WAS GIVEN A METAL SPLINT TO PROTECT FROM FURTHER INJURY. X-RAYS WERE NEGATIVE

EE was in northwest hopper changing a door liner when he slipped and twisted his left ankle. EE was taken to co dr. X-rays were negative but an air splint was given that he may wear to help cushion his walking for a few days.

EE was driving axle pin into John Deere tractor with ball peen hammer. A small piece of steel broke off of hammer and became embedded in right thumb. Piece of steel was surgically removed, received 3 sutures and also an antibiotic was prescribed. EE was released to return to work the same day.

EE WAS LOOKING UP AT BACK, NOT WATCHING WHERE HE WAS GOING, HIT A MUD PUDDLE AND MUD SPLASHED AROUND HIS GLASSES INTO HIS LEFT EYE. EE WENT TO EMERGENCY ROOM AND WAS PRESCRIBED ANIT-BACTERIAL OINTMENT.

AT 10:34 AM THE HOIST ENGINEER SHUT DOWN #34 MAN HOIST FOR ELECTRICAL REPAIRMEN TO PERFORM MAINTENANCE ON ELECTRICAL RESISTORS. 3 OF APPROX. 80 RESISTORS WERE CHANGED. NO ACCIDENT OR DAMAGE WA S INVOLVED. HOIST ENGINEER REPORTED BY PHONE TO MSHA AT 11:03 AM & AGAIN AT START UP TIME 12:24 PM.
