BEFORE EXITING THE TRAIN, TRAIN CARS THAT CAME OFF OF THE TRAIN HIT THE BACK CAUSING THE EMPLOYEE TO FALL FORWARD IN THE TRAIN AND HIT LEFT LEG AND LEFT CHEEKBONE.

Employee was working in J21 and located in an Elliot basket boom that was in an elevated position. They were using a slate bar to scale down loose material on the rib. As they were prying loose a rock on the rib, they felt pain in their abdominal area and then continued working the remainder of the shift.

EMPLOYEE REPORTED ISSUE ON 07/15/2024 BUT DID NOT GO TO THE DOCTOR. EMPLOYEE THEN CONTACTED REPRESENTATIVE AT DOE RUN ON 09/23/2024 ABOUT HAVING ISSUES. EMPLOYEE DID NOT GO ON LIGHT DUTY UNTIL AFTER EE'S SURGERY ON 06/04/2025. EMPLOYEE STATES EE WAS MUCKING AND HAULING OUT OF A ROUGH AREA IN THE MINE. AFTER THIS THE EMPLOYEE STARTED HAVING LOWER BACK ISSUES.

Employee was operating a stand up ditch witch skid steer. The stand-up ditch witch caught a hose on the wall in the tunnel on the back door cover plate on the hydraulic side, which pulled the machine sideways and propped the door like it was running over a rock, and when it did it caught the right thumb on the hand rest, there for partially amputating the right thumb

Over-exertion in lifting objects

A rock rolled into the walkway of the dump operator's shack. Injured employee moved rock (per the accident report the size of a volleyball) out of walkway and felt pain in lower back.

Individual was walking around the rod table to turn off a generator. When walking around the end of the rod table they stepped on some uneven gravel and is when they rolled their right ankle.

EE HAS RECENTLY BEEN RE-EVALUATED AND TESTING PERFORMED RESULTING IN A SURGICAL RECOMMENDATION FOR A TORN ROTATOR CUFF. SURGICAL DATE IS PENDING AT THIS TIME. EE REMAINS FULL DUTY UNTIL SURGERY DATE.

Apx. 760' HPDE pipe was being pulled down an access road. Around a 100 degree bend, the pipe developed resistance from unidentified debris (presumed rock). The resistance gave way and the tension applied from pulling the pipe caused it to straighten striking and employee inspecting for holes.

EE CHANGING BOOM ROLLERS, SLIPPED ON THE BOOM ROLLER COVER AND FELL ON LEFT SHOULDER AND RIGHT KNEE. EE PLACED ON RESTRICTED DUTY ON 8/16/2021

EE twisted left knee stepping on a loose rock on the ground. EE placed on restricted duty on 07/12/2021 after requesting a medical evaluation.

EE MOVING BATTERIES ON A PALLET AND FELT PAIN IN LEFT SHOULDER. EE SOUGHT MEDICAL TREATMENT, TESTING SHOWED TEAR IN ROTATOR CUFF THAT WILL REQUIRE SX. EE REMAINS ON FULL DUTY AT THIS TIME. SX SCHEDULED 4.21.21 AND WILL BE RELEASED TO LIGHT DUTY FOLLOWING SX.

EE diagnosed with Carpal Tunnel Syndrome and Cubital Tunnel Syndrome of the left arm on 6/10/2020. EE remains full duty until surgery (scheduled for 7/9/2020) and will be released to restricted duty following surgery.

EE flushing out a ZnSo4 line with an air hose, the hose came loose and hit employee in the face. EE received a scratch on right eye. Antibiotic eye drops were prescribed to employee.

EE untangling power cable and felt pain in right shoulder. EE did not seek medical attention at the time. EE requested medical attention on 5/12/2020 and subsequently placed on restricted duty at that time. EE remains on restricted duty pending medical treatment.

EE STEPPED ON ROCK WHILE WALKING AND TWISTED RIGHT ANKLE. EE RECIEVED NO MEDICAL TREATMENT AT THAT TIME. EE REQUESTED MEDICAL TREATMENT AND WAS DEEMED WORK RELATED PER EMAIL ON 4.27.2020. EE WAS PLACED ON RESTRICTED DUTY FOLLOWING MEDICAL TREATMENT.

EE operating a walk behind loader. EE slipped off platform, while slipping off EE grabbed control and moved loader backwards pinning lower leg between loader and concrete. EE placed on restricted duty on 7/8/2019.

EE lifting 50lb bags of anfo to dump into charging rig hopper and felt pain in neck and shoulder. EE received medical treatment on 7/08/2019 and was placed on restricted duty at that time.

EE twisted right knee when exiting piece of equipment on 11/26/2018. EE remained on full duty following medical evaluation and testing. EE will receive surgical treatment on 5/24/2019 and will be released to restricted duty on 5/25/2019.

EE stepping onto train and felt pain in left knee. EE remains full duty at this time but will have surgical ""scope"" procedure on 2.1.19 and will be placed on restricted duty following procedure.

EE smashed thumb between steering cylinder and frame resulting in small chip fracture of left thumb. EE was released to full duty following medical evalulation.

EE lifting a piece of plate steel and felt pain in right elbow. EE received medical treatment and was placed on restricted duty.

Possible unconfirmed insect bite/sting. EE working on a pump and noticed a small blister spot on side of right hand. Hand/arm began swelling. EE treated at local clinic but could not confirm diagnosis of insect bite sting. EE released to Full Duty.

EE straining while lifting liners and felt pain in groin area. EE diagnosed with hernia. EE placed on restricted duty while treatment is pending.

EE was loading glue on bolter when EE hip popped out of place and back into place pinching nerve. EE was released to full duty following medical examination on 4.10.18. A follow up appointment on 5.1.18 resulted in EE placed on restricted duty at that time.

EE performing hand scaling underground with a scaling bar and felt pain in right elbow. EE was sent for medical treatment on 3/14/18 and was placed on restricted duty at that time for elbow sprain.

EE moving a steel box when the plate clamp slipped and the steel box fell onto EE right foot fracturing a bone in right foot. EE released with light duty restrictions following ortho specialist exam.

EE operating haul truck underground, bed raised while driving and stuck in back causing EE to hit steering wheel. EE was taken for medical treatment and cleared for light duty until seen by company doctor. EE was seen at company doctor on the first shift back, 11/13/17 and released to full duty.

EE lifting water lines and felt pain in abdominal area. EE taken to local clinic and sent to ER for diagnosis and treatment. EE then transferred to Mercy Hospital in Festus Mo for surgery of hernia.

EE felt pain in low back when lifting a 40 lb box of resin onto the roof bolter. EE went to the doctor on 8.14.17 and was placed on restricted duty at that time.

EE attempting to clean at #5 crusher train dump when EE lost footing and fell into open hole. EE received sutures in a laceration on bottom lip and was placed on restricted duty for knee pain.

EE was pulling on a wrench to loosen a cartridge when the cartridge broke loose causing EE to hit elbow on the fender. EE was placed on restricted duty on 6/14/2017.

EE removing rock from under coarse ore bin when rock came in cab and hit ee on the right arm and hand. EE received laceration and sutures and placed on restricted duty.

#2 Service hoist went down at 2:30 am on 8/27/16 due to a burnt coil. Electrical was called and coil was replaced. Hoist was placed back into service at 4:47 am on 8/27/16.

EE claimed a personal illness beginning on 8.22.16. EE missed time for illness. On 5.1.17 an email from the company outside attorney came claiming the illness is a possible work related illness or aggravation of an illness. EE is currently on restricted duty. Will amend when more information on case is known.

EE getting out of a Kawasaki Mule when EE stepped on a rock and twisted right knee. EE placed on restricted duty pending treatment.

EE operating a truck underground when the truck bed hit the back. The EE hit the steering wheel. Minor injuries at the time. Incident became reportable when EE was placed on restrictive duty on 8/18/16 with neck pain.

EE diagnosed with Carpal Tunnel Syndrome. Treatment is pending at this time.

EE alleged Carpal Tunnel Syndrome on 1.28.16. After rescheduling appointments multiple times, EE was diagnosed with bilateral carpal tunnel syndrome on 6.13.16. EE terminated employment on 5.15.16. Treatment is still pending at this time.

EE climbing into haul truck cab, strained right shoulder. EE was placed on restricted duty on 6/10/16.

EE performing maintenance on a drill. When exiting the drill, EE felt pain in knee stepping off the ladder. EE placed on restricted duty pending further testing.

EE felt pain in low back region while driving haul truck underground after hitting a pot hole. EE went to the physician on 4-15-16 and was placed on restricted duty.

EE diagnosed with Carpal Tunnel Syndrome and recommended surgery. EE remains at Full Duty until surgery, will then be placed on restricted duty following surgery. Surgery dates are pending at this time.

Bearing failure on generator that supplies power to amplidine to #2 Hoist, at 3:05 pm. MSHA notified at 03:17 pm. Repairs were completed and hoist returned to operation at 5:42 pm. MSHA was notified at 5:43 pm.

Electrical components failed in PLC cabinet that controlled the dynamic brake for #6 hoist motor. MSHA notified at 4:55 pm. Repairs made and hoist was back in operation at 8:00 am on 10/29/2015. MSHA notified at 8:01 am.

EE using an air hose to clean and got debris in his left eye. EE was given a prescription eye drop and released back to work with no restrictions.

EE was walking across the park underground when he tripped on a rock and felt pain in hip. EE placed on restricted duty pending further testing.

EE was driving a haul truck underground when his light belt hit the bed lever, the bed raised and hit the back suddenly stopping the truck. When the truck stopped suddenly, EE hit his mouth on the steering wheel causing a laceration. EE received stitches and was returned to work at full duty.

EE was changing a dart valve on the CU rougher, the cap on the dart actuator was not secured and flew off hitting EE in the mouth. EE broke a tooth and chipped another.

While EE was blowing dust and debris with air hose, EE got something in left eye. Dr removed small piece of metal imbedded in left eye.

EE got foreign body imbedded in left eye. Piece was removed and prescription eye drop was given to EE.

EE was walking down front steps of the office that was covered with snow when he twisted his knee. EE had surgery on April 16, 2013 making this a reportable injury.

EE was servicing locomotive when the spring loaded handle pinched his finger resulting in a fracture bone and stitches.

EE was repairing a return roller on the #36 Feeder Belt when he stood up he felt pain in his knee. He had surgery on June 12, 2012.

Tripping out drill pipe and 1st helper threw rod to 2nd helper on the rod truck. Drill pipe hit a stake on the side of truck that was left in and bounced over and landed on 2nd helpers foot causing him to fracture right big toe.

EE was removing a piece of metal when the metal cut his hands through the gloves resulting in stitches.

EE was hammering bolt, sliver of metal broke off and struck EE's left forearm. Employee received stitches making this reportable.

EE had tingling in both his left and right hands and forearms on 7/30/09. EE was diagnosed with carpal tunnel syndrome and had surgery on 7/1/2010.

EE alledged repetitive motion over a period of time to right arm, elbow and shoulder and has caused pain/carpal tunnel. EE has been being treated and on 12/23/09 he was given an injection to right shoulder and was prescribed physical therapy, making this reportable.

Employee was pulling pump coupler off front of engine when foot slipped causing him to go down on hood. When he went down on hood a bolt was sticking up and it struck left testicle. He has been treating continuously with co. dr and specialist and he was given an injection and taken off work effective 3/2/10. He will have surgery on 3/10/10.

Employee was dumping 50 lb bags of anfo in anfo pot on truck when he felt sharp pain in back of neck. He was treated by company physician and later referred to a specialist. He received injections and will do physical therapy.

Employee drove truck through chug holes and felt pain from neck between shoulder blades down back to belt line. Employee was taken to emergency room and was given an injection in neck.

Employee was walking along side of crusher and got dirt in right eye. Employee was taken to company doctor on 6-30-08 and received antibiotic eye drops.

Employee was using wire wheel to clean up some parts for a tractor when a piece of wire came off and went into his left forearm. He later felt pain and reported accident on 12-06-07 and was then taken to company doctor. A specialist was not able to remove the piece of steel. He decided to leave it in his arm as it could do more damage to remove it. Employee received 6 sutures.

EE was standing on old conveyor belt when the belt came apart & EE fell to floor. He was taken to Ellington ER on 7-8-07 & released to return to work. He was taken to co. dr. for follow-up on 7-9-07 where he was prescribed pain med. & muscle relaxer. He continued to complain of pain & was taken back to co. dr. on 7-10-07 where add'l x-rays were taken. EE was released to full duty.
