Employee was squatting and standing while working on a piece of mobile equipment. EE noticed discomfort in their left knee on 7/24/23. EE continued to work and the knee progressively got worse over the next 48hrs. EE reported the injury on 7/26/23.

THE LIMB LACERATED THE CORNEA, THE EE WAS GIVEN AN ANTIBIOTIC EYE DROP AND CONTACT LENS FOR PROTECTION WHILE HEALING AND WAS RELEASED BACK TO FULL DUTY WITH NO RESTRICTIONS.

A sub-contractor hired by Rail-Veyor was listening to a hazard training presentation when they moved backwards and was un-aware of a mounted electrical motor framing in which they tripped over, resulting in hitting hard hat on the wall and falling to the floor causing a lower back injury.

EE used jack to lift up forklift for service. EE putting blocks under forklift to secure load when the jack mechanically failed and smashed pinky finger on right hand between the board and forklift. EE received 12 sutures and was released back to full duty.

EE using a scaling bar to pry open a valve, bar slipped and pinched EE left pinky finger between bar and thickener tank lacerating finger requiring 6 sutures. EE released back to full duty following medical treatment.

EE reported on 7/24/2018 that on 7/20/2018 EE was shoveling concentrate out of the bed of a concentrate truck feeling pain in left arm and chest area. EE was sent for medical evaluation on 8/14/2018 and released with no restrictions. Follow up medical evaluation on 8/17/2018 EE was placed on restricted duty.

EE hooking up bottle rack on crane, another EE bumped the crane control to go up a little bit and caught EE left index finger between crane hook and bottle rack. EE was taken to ER and received 9 sutures in left index finger. EE released to restricted duty.

Contractor came on site to update Ethernet switch. Contractor was hit in head by an Ethernet switch requiring medical treatment. Contractor received 5 sutures in back of head and returned to finish job. This is not our employee and not required to have an MSHA ID number.

Cleaning a tank and upon completion the laborer-miner was pulling a shop-vac out of the tank. It became snagged on the rim and the worker attempted to pull harder to release the vacuum from the edge of the tank, it did not release and EE strained lower back.

EE pulling on a water hose to wash off entrance to truck wash and felt pain in low back area. EE went to local clinic on 3.22.17 and was placed on restricted duty at that time.

EE operating a scaler, scaling the face in a downward direction. The boom slid off the face causing the scaler to bounce and EE hit head on cab glass. EE received laceration requiring staples and placed on restricted duty.

EE exiting drill, lost footing and hit right thumb on handrail. EE remains at full duty and physician is now recommending surgery to repair. Tentative surgery date is 3/24/2016. Following surgery EE will return to work under restrictive duty.

EE was moving office furniture when the leg broke on the desk tipping it. When it tipped the glass top slid sideways, when he reached to grab the glass it lacerated his left index finger requiring stitches.

EE was changing a pop off valve on the SO2 tank. The gas leaked around the rupture disc assembly. The EE received only first aid treatment at that time. On 11-4-14 EE still having irritation in eye, EE taken to eye dr. Eye dr. prescribed eye drop to alleviate discomfort and returned to work.

Employee was carrying a piece of plate steel with assistance from a co-worker. They lost their grip and dropped the plate. It came down on the tip EE's right ring finger. He received 1st aid treatment and returned to work. He was not taken to the doctor until 8/26 because he did not make himself available for treatment. Finger broken and light duty status.

The employee was removing the outer insulation from new wire with a hook knife. The wire was 2"" diameter and contained 24 pair of 18 gauge wire. The wire dropped causing his hand to slip causing a laceration to the left thumb.

EE was installing an air compressor when foreign matter enter into his eyes. He was wearing safety glasses that wrap around to protect his eyes. He was given prescription medication for his eye.

EE stepped off a step ladder in front of haul truck and lost his balance fell to the ground and hit his right rib on a rock. Diagnosed has a rib fracture.

EE was changing out a pump when the shaft slipped and caught his two left fingers between the bottom of the sleeve nut and top of the gland. The ee recieved stitches on one finger and further treatment on the other finger. The ee has not returned back to work.

EE was drilling holes in top of fender to move auto greaser, when drill bit broke through, he lost his footing and fell from top of fender to the ground. EE was taken to ER by ambulance and was diagnosed with a heel fracture.

EE was washing PB off of roadway in truck wash, when he stepped backwards up angle of curbing, his feet slipped and he fell. EE was taken to company doctor and after continuous treatment he was given a muscle relaxer injection on 1/15/10.

EE was climbing up right side of 930 Cat to check brake fluid. While standing on right rear tire his footing slipped and all his body weight was on left shoulder. EE was taken to company doctor and was given a cortisone injection on 11/12/09.

EE alledged carpal tunnel syndrome from apparent repetitive motion. EE treated with a specialist and surgery was recommended. Surgery for the right wrist is on 2/11/10. The left wrist will be done on 3/4/10, making this reportable.

Injured EE threw water into air, landing on another EE. The other EE lost his temper and placed injured EE in the trash bin. Injured EE hit his head, scraped left knee and had a sore neck. EE was taken to company doctor and was diagnosed with a jaw fracture, making this reportable.

EE was lifting and straining right wrist while lifting blower bagging to hang. Strain caused cyst to form and after treating with physician, EE had surgery on 2/10/10 to remove cyst.

Employee had washed hydraulic oil from getman and when getting off of getman he lost his grip and fell to ground. He was seen by comnpany dr and he was diagnosed with a torn rotator cuff. He was taken off work effective 7/28/09

Hood on elphinstone truck struck employee on hard hat and right side of neck cutting bridge on nose between eyebrows and a laceration to right side of neck. Employee was taken to emergency room and received approximately 8 sutures.

Employee was operating tractor when it slid off road into water ditch and hit pillar rib. When he hit pillar his left side impacted against tractor. He was taken to company dr. On 4/14/08 received results from bone scan indicating fracture of #7, 8, and 9 left ribs.

He was taken to company Dr on 4-7-08 & again on 5-9-08 when he was referred to an orthopedic Dr. Orthopedic Dr recommended physical therapy which he bagan on 5-19-08 making this reportable. Addendum: EE diagnosed with hernia on 7/18/08. Addendum: EE had hernia surgery on 8-25-08 making this a lost time.

Employee slipped on wet mat inside of drill. He fell 4' to 5' to gound landing on elbow. Employee was taken to company doctor and received one stitch to the laceration on right elbow.

# 37 hoist at Buick surface went down due to loss of field excitation on generator. 5:30 am hoist was running again and put back in service. MSHA was notified when hoist was put back in service.

2 employees were welding at same time working on reclaim hopper. Apparently EE was too close & rec'd flash burns to both eyes. EE was taken to Salem ER & prescribed eye drops & an anti inflamatory prescription.

A number of hole plugs had been driven over by drill & compacted very tight. While pulling on one with pliers, he felt a strain to his left elbow. He continued working & noticed swelling, as the swelling continued the pain set in & increased. Employee was taken to the E.R. where he was given 2 injections. One for pain & the other was an antibiotic.

Driver dumped load and turned on hose to wash out truck bed. Hose was not shut off at hozle and pressure made hose fly up and hit driver in cheek under right eye.

Two employees were attaching 8"" cement hose to surface cement hole. They were in basket of JCB to make hook up. Boom was extended out approx. 20' to reach pipe at cement hole when the basket boom tipped over & fell to ground approx. 25'. EEs were taken to Dr. & released to work. Injured EE chipped his 2 front teeth & on 10/9/06 he received 2 temp. caps making this reportable.

Two employees were attaching 8"" cement hose to surface cement hole. They were in basket of JCB to make hook up. Boom was extended out approx. 20' to reach pipe at cement hole when the basket boom tipped over & fell to ground approx. 25'. They were taken to Dr. & released to work. Injured EE continued to have pain in his rib/back area. Prescribed a cortizone shot & therapy.

Employee was pulling on cage latch pin to put door back into normal position. Latch pin jumped out of hole and smashed right ring finger after pressure released on latch. Employee was taken to Salem Emergency Room where x-rays were taken. Revealed a fracture to tip of finger.
