EMPLOYEE WAS WORKING IN ZINC TUNNEL IN THE MILL MAKING SURE THE SUMP WAS GETTING ENOUGH WATER WHEN A 12' LONG 2-1/2"" IN DIAMETER PIPE FELL AND HIT EMPLOYEE IN THE BACK OF HARD HAT / NECK / SHOULDERS.
EMPLOYEE WAS WALKING DOWN THE STEPS TO THE 3020 BELT LINE AND THERE WAS WET MUCK ON STEPS, SLIPPED AND CAUGHT SELF BEFORE FALLING. HURT RIGHT KNEE.
EMPLOYEE WAS TAKING BOLTS OUT OF THE BOTTOM OF THE TROMMEL SCREEN. EMPLOYEE HAD TO LAY DOWN WITH THE 3/4"" IMPACT WHICH WAS IN RIGHT HAND AND SLIPPED OFF OF BOLT WHICH EXTENDED RIGHT ARM AND HEARD A POP IN EE'S RIGHT SHOULDER.
CARPAL TUNNEL SYNDROME IN LEFT AND RIGHT WRIST, AND LEFT ELBOW.
EMPLOYEE WAS WALKING THROUGH THE NORTH SIDE OF THE PB FILTER VACUUM PUMP AND SLIPPED IN DRY FLOCCULANT THAT HAD FALLEN FROM FLOOR ABOVE THROUGH THE GRATING.

Employee was squatting and standing while working on a piece of mobile equipment. EE noticed discomfort in their left knee on 7/24/23. EE continued to work and the knee progressively got worse over the next 48hrs. EE reported the injury on 7/26/23.
EE CUTTING A SAPLING WITH HAND TRIMMERS WHEN A BRANCH BROKE OFF AND MOVED UP UNDER SAFETY GLASSES AND STRUCK EE IN RIGHT EYE. EE WAS TAKEN FOR MEDICAL EVALUATION. THE LIMB LACERATED THE CORNEA, THE EE WAS GIVEN AN ANTIBIOTIC EYE DROP AND CONTACT LENS FOR PROTECTION WHILE HEALING AND WAS RELEASED BACK TO FULL DUTY WITH NO RESTRICTIONS.

A sub-contractor hired by Rail-Veyor was listening to a hazard training presentation when they moved backwards and was un-aware of a mounted electrical motor framing in which they tripped over, resulting in hitting hard hat on the wall and falling to the floor causing a lower back injury.
EE STOOD UP TO STEP FORWARD AND RIGHT KNEE HYPER-EXTENDED. EE WAS PLACED ON RESTRICTED DUTY AFTER MEDICAL EVALUATION.
EE diagnosed with bilateral carpal tunnel syndrome on 4/26/2021. Treatment / surgery is pending at this time.
EE used jack to lift up forklift for service. EE putting blocks under forklift to secure load when the jack mechanically failed and smashed pinky finger on right hand between the board and forklift. EE received 12 sutures and was released back to full duty.
EE DIAGNOSED WITH BILATERAL CARPAL TUNNEL SYNDROME ON 4/8/2021. TREATMENT / SURGERY IS TENTATIVELY SCHEDULED FOR 5/20/2021 FOR THE FIRST HAND, OTHER HAND AT A LATER DATE. AT THIS TIME EE REMAINS FULL DUTY. FOLLOWING SURGERY EE WILL RETURN TO WORK ON RESTRICTED DUTY.
EE diagnosed with carpal tunnel syndrome in left hand/wrist. EE will have surgery on 2/11/2021 and be released to restricted duty following surgery.

EE attempting to loosen a frozen clevise. EE hit clevise with hammer and clevis came loose and hit EE on right forearm. Resulting in fracture to right ulna. EE placed on restricted duty after medical evaluation.

EE FELT PAIN IN LOW BACK WHILE BENDING OVER TO PLACE A WHEEL CHOCK UNDER WHEEL OF LOADER. EE PLACED ON RESTRICTED DUTY AFTER MEDICAL TREATMENT ON 5/1/2019.
EE stepped on drain in floor and fell injuring right shoulder. EE placed on restricted duty on 4/29/2019. Treatment is ongoing at this time.
EE diagnosed with bilateral carpal tunnel syndrome on 1/31/2019. Treatment and/or surgery is pending at this time.

EE using a scaling bar to pry open a valve, bar slipped and pinched EE left pinky finger between bar and thickener tank lacerating finger requiring 6 sutures. EE released back to full duty following medical treatment.
EE reported on 7/24/2018 that on 7/20/2018 EE was shoveling concentrate out of the bed of a concentrate truck feeling pain in left arm and chest area. EE was sent for medical evaluation on 8/14/2018 and released with no restrictions. Follow up medical evaluation on 8/17/2018 EE was placed on restricted duty.
EE diagnosed with occupational carpal tunnel syndrome on 7/16/2018. Treatment and surgery dates are pending at this time. EE should be released to restricted duty following surgeries.
EE lifting a tire from the side of a truck bed when the tire slipped overextending left arm straining left bicep. EE placed on restricted duty.
EE diagnosed with Carpal tunnel syndrome on 5/29/2018. Conservative treatment was requested by physician at this time.
EE diagnosed with Occupational Carpal Tunnel Syndrome on 3.9.18. Surgery dates are pending at this time. After surgery, EE will be released to restricted duty.

EE hooking up bottle rack on crane, another EE bumped the crane control to go up a little bit and caught EE left index finger between crane hook and bottle rack. EE was taken to ER and received 9 sutures in left index finger. EE released to restricted duty.
Contractor came on site to update Ethernet switch. Contractor was hit in head by an Ethernet switch requiring medical treatment. Contractor received 5 sutures in back of head and returned to finish job. This is not our employee and not required to have an MSHA ID number.
Cleaning a tank and upon completion the laborer-miner was pulling a shop-vac out of the tank. It became snagged on the rim and the worker attempted to pull harder to release the vacuum from the edge of the tank, it did not release and EE strained lower back.
EE pulling on a water hose to wash off entrance to truck wash and felt pain in low back area. EE went to local clinic on 3.22.17 and was placed on restricted duty at that time.

EE reattaching a guard when the lifting hook came loose and hit EE in the chin. While exiting ladder, EE missed a step and fell to the ground. EE placed on restricted duty for knee pain.
EE slipped on pavement at truck scale and strained hamstring in left leg. EE placed on restricted duty.

EE lost footing while exiting loader and fell on the ground, feeling pain in low back. EE placed on restricted duty. Follow up appointment following week placed EE back to full duty.
EE operating a scaler, scaling the face in a downward direction. The boom slid off the face causing the scaler to bounce and EE hit head on cab glass. EE received laceration requiring staples and placed on restricted duty.
EE diagnosed with Carpal Tunnel Syndrome. Surgery dates are pending at this time. EE is not on any restrictions until after first surgery is completed.
EE felt sting while washing out hopper on sweeper truck. Unknown insect, no medical treatment necessary.
EE was lifting a pump and felt pain in lower back. EE was placed on light duty while seeking treatment.
EE was hanging a utility line when debris fell into his eye. EE received a prescription eye drop.

EE was taken from the site by ambulance for personal health issue, undetermined by ER. Possible stroke, possible blood clot, possible ear infection. ER released him to follow up with personal Dr.

Alleged Carpal Tunnel Syndrome. Remains on full duty at this time and has not yet been determined as work related (3.31.15).
EE was moving office furniture when the leg broke on the desk tipping it. When it tipped the glass top slid sideways, when he reached to grab the glass it lacerated his left index finger requiring stitches.
EE alleged carpal tunnel syndrome. Dr diagnosed EE with carpal tunnel and set up surgery for 1/15/2015. EE will be released to Light duty following procedure on 1/15/2015.
EE was changing a pop off valve on the SO2 tank. The gas leaked around the rupture disc assembly. The EE received only first aid treatment at that time. On 11-4-14 EE still having irritation in eye, EE taken to eye dr. Eye dr. prescribed eye drop to alleviate discomfort and returned to work.
EE slipped in shower. Restricted duty for concussion symptoms
Employee was carrying a piece of plate steel with assistance from a co-worker. They lost their grip and dropped the plate. It came down on the tip EE's right ring finger. He received 1st aid treatment and returned to work. He was not taken to the doctor until 8/26 because he did not make himself available for treatment. Finger broken and light duty status.
EE tripped and fell down 3 steps. EE strained shoulder, back, and ankle.
EE was moving a water hose and lost his footing falling. EE received fracture to left middle finger.
EE fell from step on a 4' step ladder - felt pain in low back. Restricted duty for 13 days.
EE was turning off water hose. Torn tendon in left thumb requiring surgical procedure to repair.
EE slipped on snow covered ice, felt pain in right shoulder, EE had surgery 5.8.14.

EE got debris in eye while grinding. Doctor removed debris from eye and gave him a prescription eye drop.
EE was using a pry bar to pry a rock out of tail pulley when the bar slipped and caught left pinky finger, causing laceration and fracture of left pinky finger.
Employee was walking behind a powder rig and slipped in loose muck falling, he tried to catch himself causing a laceration on right hand.
Employee was exiting loader and lost his grip and fell to ground injuring neck and back. Employee returned to work on light duty and is scheduled for MRI of neck and back

The employee was removing the outer insulation from new wire with a hook knife. The wire was 2"" diameter and contained 24 pair of 18 gauge wire. The wire dropped causing his hand to slip causing a laceration to the left thumb.
Running 7"" casing collar caught on can, while putting on 7"" plate caught employee hand between table and handle on plate.
EE was moving a table saw when the frame of the saw moved and pinched his finger resulting in stitches.
EE turned pump off and then loosen a line at the connection and it blew flocculent liquid under his safety glasses resulting in prescription medication for his eyes.

EE was installing an air compressor when foreign matter enter into his eyes. He was wearing safety glasses that wrap around to protect his eyes. He was given prescription medication for his eye.
EE was using a rebar to unplug a chute when his hand slipped and he cut his finger on the edge of the chute requiring stitches.
Employee was getting off loader when his left foot hung in the ladder step pulling left knee and muscle. Employee required surgery on June 2nd and began losing time.
EE lifted an oxygen cylinder to put on a cart when he felt pain in his lower back. He had surgery on July 29, 2011.
***Occupational Illness***Employee did not contact us, he filed with worker's compensation when he had carpal tunnel surgery on February 8th, 2011.
EE was walking when he stepped on a rock and rolled his ankle. EE was taken via ambulance to a local hospital and diagnosed with multiple fractures of the ankle.

EE slipped in parking lot, fracturing clavicle and rib on left side.
EE stepped off a step ladder in front of haul truck and lost his balance fell to the ground and hit his right rib on a rock. Diagnosed has a rib fracture.
EE was changing out a pump when the shaft slipped and caught his two left fingers between the bottom of the sleeve nut and top of the gland. The ee recieved stitches on one finger and further treatment on the other finger. The ee has not returned back to work.
Alleged carpal tunnel syndrome from repetitive use of hands/arms/elbows while doing job requirements. Employee had carpal tunnel surgery on 10/14/2010 and began losing time.
Employee lost footing getting off loader, landed on right side hip. Employee was taken to emergency room and released. Employee returned to doctor July 14th and was taken off work.
Employee alleged carpal tunnel syndrome- repetitive motion from normal job duties- hands were falling asleep. EE had carpal tunnel surgery 10/15/2010 and began losing time.
EE was drilling holes in top of fender to move auto greaser, when drill bit broke through, he lost his footing and fell from top of fender to the ground. EE was taken to ER by ambulance and was diagnosed with a heel fracture.
EE was prying away old chute from the face of #1 feeder. Plate gave way and smashed left pinky finger between plate and existing metal sides. EE was taken to company doctor and received 5 sutures, making this reportable.
EE was washing PB off of roadway in truck wash, when he stepped backwards up angle of curbing, his feet slipped and he fell. EE was taken to company doctor and after continuous treatment he was given a muscle relaxer injection on 1/15/10.

EE was climbing up right side of 930 Cat to check brake fluid. While standing on right rear tire his footing slipped and all his body weight was on left shoulder. EE was taken to company doctor and was given a cortisone injection on 11/12/09.
EE alledged carpal tunnel syndrome from apparent repetitive motion. EE treated with a specialist and surgery was recommended. Surgery for the right wrist is on 2/11/10. The left wrist will be done on 3/4/10, making this reportable.
Injured EE threw water into air, landing on another EE. The other EE lost his temper and placed injured EE in the trash bin. Injured EE hit his head, scraped left knee and had a sore neck. EE was taken to company doctor and was diagnosed with a jaw fracture, making this reportable.
EE was lifting and straining right wrist while lifting blower bagging to hang. Strain caused cyst to form and after treating with physician, EE had surgery on 2/10/10 to remove cyst.
Employee had washed hydraulic oil from getman and when getting off of getman he lost his grip and fell to ground. He was seen by comnpany dr and he was diagnosed with a torn rotator cuff. He was taken off work effective 7/28/09
EE alleged carpal tunnel syndrome. After testing and treatment with the specialist, the EE was diagnosed with carpal tunnel. EE had surgery on 10/01/09 to the left hand making this a lost time occupational illness.

Employee was stripping cable jacket and using roofing blade in utility knife to pull toward him when it slipped out and cut top of left thumb. He was taken to Dr, company's physician and received 9 sutures.
Alleged carpal tunnel from repetitive motion of normal job duties. Employee treated with specialist and had surgery on 12-18-08.
EE alledged carpal tunnel syndrome. After completing test, specialist recommended surgery. EE had surgery on 1/15/09 making this reportable.
Hood on elphinstone truck struck employee on hard hat and right side of neck cutting bridge on nose between eyebrows and a laceration to right side of neck. Employee was taken to emergency room and received approximately 8 sutures.
Employee was operating tractor when it slid off road into water ditch and hit pillar rib. When he hit pillar his left side impacted against tractor. He was taken to company dr. On 4/14/08 received results from bone scan indicating fracture of #7, 8, and 9 left ribs.
EE was closing switch on 67 south transformer and pulled right groin. He was taken to company Dr on 4-7-08 & again on 5-9-08 when he was referred to an orthopedic Dr. Orthopedic Dr recommended physical therapy which he bagan on 5-19-08 making this reportable. Addendum: EE diagnosed with hernia on 7/18/08. Addendum: EE had hernia surgery on 8-25-08 making this a lost time.

Employee slipped on wet mat inside of drill. He fell 4' to 5' to gound landing on elbow. Employee was taken to company doctor and received one stitch to the laceration on right elbow.
# 37 hoist at Buick surface went down due to loss of field excitation on generator. 5:30 am hoist was running again and put back in service. MSHA was notified when hoist was put back in service.
2 employees were welding at same time working on reclaim hopper. Apparently EE was too close & rec'd flash burns to both eyes. EE was taken to Salem ER & prescribed eye drops & an anti inflamatory prescription.
Employee was using a come-a-long and sling to line up motor and pump. When sling broke, a piece of steel stuck in right arm. Employee was taken to company doctor where the steel was removed. Employee returned to work.
Employee was attempting to take a pin out of toilet stall hinge with a pocket knife. The knife slipped and jabbed his hand.

A number of hole plugs had been driven over by drill & compacted very tight. While pulling on one with pliers, he felt a strain to his left elbow. He continued working & noticed swelling, as the swelling continued the pain set in & increased. Employee was taken to the E.R. where he was given 2 injections. One for pain & the other was an antibiotic.
Driver dumped load and turned on hose to wash out truck bed. Hose was not shut off at hozle and pressure made hose fly up and hit driver in cheek under right eye.
Two employees were attaching 8"" cement hose to surface cement hole. They were in basket of JCB to make hook up. Boom was extended out approx. 20' to reach pipe at cement hole when the basket boom tipped over & fell to ground approx. 25'. EEs were taken to Dr. & released to work. Injured EE chipped his 2 front teeth & on 10/9/06 he received 2 temp. caps making this reportable.
Two employees were attaching 8"" cement hose to surface cement hole. They were in basket of JCB to make hook up. Boom was extended out approx. 20' to reach pipe at cement hole when the basket boom tipped over & fell to ground approx. 25'. They were taken to Dr. & released to work. Injured EE continued to have pain in his rib/back area. Prescribed a cortizone shot & therapy.

Employee was pulling on cage latch pin to put door back into normal position. Latch pin jumped out of hole and smashed right ring finger after pressure released on latch. Employee was taken to Salem Emergency Room where x-rays were taken. Revealed a fracture to tip of finger.
EE WAS SCALLING A BROW WHEN LOOSE FELL ON BOOM OF SCALER BOUNCING OPERATOR INSIDE CAB. HE HIS HIS HEAD (HARD HAT) ON ROOF AND HARD HAD CUT RIGHT SIDE OF HEAD. HE ALSO FELT PAIN IN LOWER BACK. TAKEN TO DOCTOR ON 8/22/05. DUE TO RESTRICTIONS FROM DOCTOR, COULD NOT RETURN TO WORK ON 8/23/05 AND BEGAN LOOSING TIME
EE WAS HOLD BICHROMATE PLASTIC BARREL LINER IN LEFT HAND WHILE USING UTILITY KNIFE TO CUT OFF TIE WRAP CLOSURE ON BAG. HAND WAS IN WRONG POSITION AND KNIFE CUT LEFT MIDDLE FINGER AT JOINT CLOSEST TO TIP OF FINGER, EE WAS TAKEN TO SALEM ER WHERE EE RECEIVED 2 SUTURES
EE WAS WORKING A RUSTY STRAP BACK AND FORTH TRYING TO BREAK IT OFF A ROOFBOLT. EE MINING BAR WAS SLIPPING AWAY AND WHEN EE DREW HAND TO CATCH IT, EE HAND SLID ALONG THE STRAP CUTTING PALM. EE RECEIVED 3 STITCHES AND WAS PRESCRIBED AN ANTIBIOTIC TO PREVENT INFECTION
EE MISJUDGED DISTANCE WHEN STEPPING OFF OF TRACTOR RESULTING IN TWISTING ANKLE. EE WAS TAKEN TO COMPANY DOCTOR WHERE EE WAS PRESCRIBED AN AIR SPLINT THAT EE MAY USE IF ANKLE HURTS BEFORE OR WHILE WALKING. X-RAYS WERE NEGATIVE
EE was trying to change water valve on hose. When he couldn't get hose off, he took knife to cut off hose. Knife slipped and cut left thumb. EE went to hospital er and received 5 sutures. EE returned to work on 3-21-05, next scheduled work shift.

EMPLOYEE WAS HAMMERING ON A BOLT WHEN A PIECE OF SLAG OFF OF THE BOLT HIT HIS CHEEK AND EYE. EMPLOYEE WAS TREATED WITH ANTIBIOTIC. EE RTW NEXT SCHEDULED SHIFT.
Employee had been running to check on another employee in the #01 White Ford. Employee is not sure how or what he did. Knee had swelling and was getting stiff. Employee had knee surgery on 10-21-04 making this reportable.
EMPLOYEE WAS FILLING THE GETMAN WITH ANFO USING A 920 FORKLIFT AND A BULK ANFO TANK. WHILE FILLING THE GETMAN, ANOTHER EMPLOYEE OPERATING THE FORKLIFT LEANED OUT OF THE 920 TO ASK HIM A QUESTION. IN DOING SO, HE BUMPED CONTROL LEVER PINNING EMPLOYEE BETWEEN GETMAN AND ANFO TANK CAUSING A HEAD INJURY, LOSS OF FINGER ON RIGHT HAND, AND A FRACTURED PALLET.
EMPLOYEE STEPPED OUT OF CAB ONTO A LOOSE ROCK TWISTING HIS RIGHT LEG AND KNEE. HE HAS BEEN TREATED CONTINOUSLY SINCE DATE OF INJURY AND ON 4-20-05 HE BEGAN LOSING TIME FROM WORK WITH LIMITED RESTRICTIONS OF NO SQUATTING, CLIMBING AND STANDING MORE THAN 15 MINUTES EVERY HOURS.
Employee was setting on side seat of the tractor. had 1 ft of clearance on either side of tractor. tractor ran over small rock tipping tractor to one side spinning employees right hand between the tractors hand rail and the deck of the condor. cut hand requiring 5 sutures.
EE WAS PICKING UP ROCKS OFF OF FLOOR GRATING ON BOTTOM END AT 3020 CONVEYOR WHEN THE WIND BLEW UP SOME DUST IN HIS RIGHT FROM THE CONVEYOR
Employee was walking down steps from third level of crusher bldg. knee gave out and ee fell straight down onto step experiencing severe pain in right knee. ee was taken to dr and began loosing time on 9-20-04 pending mri results.
EMPLOYEE WAS DOING P.M. ON TRUCK & WAS COMING OFF STEPS ON FRONT OF TRUCK & SLIPPED ON STEP. HE FELL TO GRATING STRIKING BACK OF HEAD. HE WAS TAKEN TO E.R. & RECEIVED 4 STAPLES TO CUT ON BACK OF HEAD.
AMPHIDYNE FAILED AND WAS REPLACED. PHONE CALL WAS ATTEMPTED AT TIME OF INCIDENT BUT WAS UNSUCCESSFUL. HOIST RETURNED TO SERVICE 8:45 A.M.
Employee states while working next to concrete grout pump, the pump line plugged off causing the pump to blow out a seal. This caused the concrete grout to be blown into face and left eye.
EE WAS USING TOO MUCH FORCE INSTEAD OF TRYING TO REMOVE THE OBJECT USING ANOTHER PROCEDURE
EE got off cage underground and while walking in front of cage he stepped on small rock twisting his right knee. Employee had out-patient arthroscopy surgery on 6-16-04 making this a lost time incident.
ALLEDGED CARPAL TUNNEL SYNDROME TO LEFT HAND. APPROVED & WILL HAVE SURGERY 6/3/4 AT PHELPS COUNTY REGIONAL MEDICAL CENTER MAKING THIS A REPORTABLE ACCIDENT.

EE HAD REMOVED ALL MOUNTING BOLTS IN MOUNTING FLANGED ON HEADER ASSEMBLY. EE HAD TURNED HIS BACK WHEN UNSUPPORTED HEAD ASSEMBLY FELL OFF VACUUM PUMP STRIKING EE. EE WAS KNOCKED TO THE GROUND H URTING HIS LEFT SHOULDER, UPPER BACK/NECK AREA, HEAD & RIGHT LEG. EE WAS GIVEN MUSCLE RELAXERS & HAS STARTED PHYSICAL THERAPY.

EMPLOYEE WAS REMOVING GUARD OVER DRIVE COUPLING WHILE THE SHAFT WAS STILL COASTING. HE LIFTED GUARD; IT CAUGHT ON DRIVE COUPLING AND WAS THROWN AGAINST HIS LEFT FOREARM. THIS RESULTED IN A L ACERATION REQUIRING APPROX. 15 SUTURES. HE RETURNED TO WORK THE SAME DAY.
