12/16/25 EE cut L. index finger when they were rolling O2 tank off of cage and hit it on metal. No medical services were requested. 12/23/25 EE notified safety about cutting their finger on 12/16/25. It was swelling, tender and hard to move. EE had been using antibiotic ointment on the cut. EE seen company doctor and was issued a prescription on 12/23/25. EE is still full duty.

EMPLOYEE WAS HELPING PACK A PALLET WITH TWO SMALL CYLINDERS, TRIPPED OVER A PALLET, AND FELL ON TRUNK. PALLET FELL TO THE GROUND. THE ZIP TIE HOLDING THE CYLINDERS ON THE PALLET BROKE, ONE ROLLED OFF ON EMPLOYEES R. ANKLE AND LOWER LEG.

Piece of metal came off of bucket tooth and impacted left leg, causing a laceration and metal fragment imbedded into left leg. EE taken for medical treatment and metal fragment was removed. EE given Tetanus and antibiotic to prevent any infection and EE released back to full duty.

As EE was entering the truck cab, EE shut the door with EE's left hand from behind while facing forward going into the truck. The door caught EE's thumb causing it to split open in which EE received 10 stitches in treatment.

Fuel buggy lunged forward pinning employee against pillar. EE was taken by ambulance to University of Missouri a Columbia and remains hospitalized currently. Amputation of left leg below the knee. EE remains off work.

As a 20' section of NQ rods was being lifted up along the slide to be added to the drill string, the top of the water swivel caught the edge of a tower strut leveraging the bottom of the rods to kick up striking the miner on the left elbow and shoulder forcing the miner to the deck.

EE reported back and hip pain from operating a fuel buggy on bumpy roads. EE received a medical evaluation and was placed off work on 5/19/2020, EE currently remains off work pending treatments.

EE installing grout in hole when grout came out above hole and got into left eye. EE was taken for medical attention and was prescribed a prescription medication for irritation. EE remained full duty.

EE diagnosed with Carpal Tunnel Syndrome on 5/29/2018 after physician review and testing. Treatment is pending at this time.

EE shoveling ore in skip pocket and felt pain in lower back. EE was seen by physician and was placed on restricted duty on 4/12/18.

EE slipped going down a step into the bathroom and fell, twisting left knee. EE remains full duty but will have a surgical procedure on 3.26.18. EE is expected to return to full duty following procedure on 3.26.18.

EE got foreign material in right eye while picking up safety glasses. EE received an abrasion on eye and given a prescription eye medication for treatment. EE released back to work on full duty following medical treatment.

EE diagnosed with bilateral carpal tunnel syndrome - occupational illness on 12.7.17. EE will undergo surgery on each hand. EE will be on restricted duty following surgery. Surgery dates are pending at this time.

EE lost left footing while standing on battery box. Right knee slid down between tire and loader twisting right knee. EE received medical treatment and was placed on restricted duty until follow up appointment on 11/10/17 and was returned to full duty.

Upon inspection it was determined that a 10 foot guide beam in the vertical service shaft was loose. The decision was made to completely replace the beam and the service shaft will be taken out of service for more than 30 minutes to complete this repair.

EE WAS PULLING A ROPE ON A TROLLEY BEAM WHEN THE ROPE BROKE CAUSING EE TO TWIST RIGHT ANKLE. EE WAS PLACED ON RESTRICTED DUTY WHILE TESTING WAS PERFORMED, DIAGNOSED WITH A FRACTURE OF RIGHT ANKLE AND EE REMAINS ON RESTRICTED DUTY.

EE walking around piece of equipment underground when EE slipped. EE tried to catch self and felt pain in right shoulder. Testing is ongoing. EE placed on restricted duty while testing is in progress.

I was washing raise in wet conditions when rock slipped out from under footing. Fell forward into safety line that was attached to harness swung into rock pile on opposite side of raise. Then fell onto rock surface of raise. Broke number 11 rib on left side and small scratch on head.

EE felt pain in low back area while swinging a hammer to drive out a swing cylinder pin. EE placed on restricted duty for muscle strain and was released on 11.1.16.

EE operating a haul truck when the operators seat malfunctioned. The seat dropped down causing EE to feel pain in back. EE placed on light duty restrictions for one week.

EE stepped on a rock, as he extended his hand to catch himself, he cut his finger on the pillar. EE received stitches and released back to full duty.

EE was using a scaling bar to remove loose rock when he felt pain in low back area. EE placed on restricted duty.

#30 Hoist at Fletcher went down at 3:45 pm on 10.14.15 due to an electrical issue. MSHA was called at 3:30 pm. Electricians were called for repairs. Repairs were completed and hoist was released back into service at 5:17 pm, MSHA notified at that time.

EE felt pain in lower back when stepping out of personnel carrier. EE was taken to doctor at that time and EE was released back to work at full duty. EE went to a follow up appointment on 9.21.15 and EE was placed on restrictions at that time.

EE walking to his work area, tripped and fell on right knee. EE placed on Restricted duty on 6.10.15.

At 2:00 am on 5/27/15 #30 Hoist dumped a skip and would not change directions. Electricians were called as well as MSHA. Repairs that were made by electricians were completed at 6:00 am. The hoist was then inspected and released at 7:55 am. MSHA was called at that time and notified at time of release.

EE stepped up into a forklift, when he sat down he felt pain in left knee. EE was not placed on any restrictions. EE will have surgery on 10.15.15 and should be returned to work on restrictions at that time.

04/12/2015 at 8:25 pm the #30 Hoist was shut down due to hoist operator seeing smoke near the hoist motor. Electricians were notified, MSHA notified at 8:32 pm, mine was evacuated. Electricians found and repaired a resistor. Hoist was then inspected and put back into service and MSHA notified at 12:47 am on 04/13/2015.

On 4/5/2015 at 8:55 am #31 Hoist would not operate due to having no 110V control power. 9:00 am called underground shop to report problem and evacuate mine, and called electricians. Electricians came to troubleshoot and repairs made. Hoist was tested and put back into operation. MSHA was called back at 1:15 pm to report problem was repaired.

EE slipped on ice and fell on right side. EE taken to the company doctor on 2.27.15 and was placed on restrictive duty at that time.

3:45 am #31 Shaft began to freeze up due to ""Down Draft"" The hoist was taken out of service at that time. The mine department was contacted to reverse the ventilation to create an ""Up Draft"" to unfreeze the shaft. The shaft was inspected and released at 12:30 pm. MSHA was called back at 3:40 pm

EE grabbed the hose for the 30W oil to unwind the holes off the reel when the hose ruptured in his left hand. Treatment of injection injury was given. EE was released to restricted duty following procedure.

November 12, 2014 at approx. 8 pm, hoist motor failed. Electricians were called and found bad motor. MGset motor had to be replaced. Repairs were completed on 11/13/2014 at 8:19 pm. Hoist/shaft inspection were made and put back in service at that time.

EE felt pain in stomach while driving on bumpy road. No Doctor trip was needed at that time. EE went to Dr on 3.25.15 and was diagnosed with a hernia. Further treatment with is pending.

Employee was not wearing gloves picking up pipe and received cut on right hand.

Employee was clearing out a shaker using an eight foot pry bar to loosen the rocks from the chains. One of the rocks fell from around four feet from the shaker striking the pry bar. The bar then smashed the upper area of his right foot causing a broken metatarsal.

EE was loading a concentrate truck with a crane. The cable on the crane tipped the ladder over that she was on causing her to fall to the scale below. EE received an open fracture of the left patella.

Plant power kicked off at 7:15 am on 2-6-14. Plant power was restored at 7:20 am but hoist power remained tripped. electricians worked on problem with main breaker, Called MSHA to report hoist power outage. Employee was bending down on right knee to perform welding and felt pain in right knee. A small piece of metal was pulled out of knee by the employee. The employee developed infection in right knee.

(3-21-13)-(12:25pm) #31 hoist lost power. Electricians were called to check hoist (12:30) - called MSHA. Electrician found and replaced a burnt contact on one leg of starter disconnect. Contact was replaced and hoist was back on at (2:05)pm. (2:10pm)-called MSHA back to report hoist was ok.

6:12am power had to shut off to #30 & 31 hoist to isolate if from w-2 power cable (Barge pump-Batch plant-see pump). Called MSHA @6:20 to report off and why. Isolated hoist power and called MSHA back at 7:10 to explain completed plant repairers to w-2 cable a approx 8:50am and poser was restored. MSHA inspector was on site at time of repair

EE was walking on a turntable to see how much conc was in a can when he stepped on a metal plate. The weld broke causing the metal to split and cut the ee on his left leg he received stitches.

6/4/12 at 11:45 a.m. the Electricians were called to check out the production hoist #31 the hoist had ""kicked out"" twice since 9a.n. that morning. The hoist was downed at 11:15 to clean the generator. MSHA was called at 11:20 to report the hoist was down. MSHA was called back at 12:45 to report the hoist was back up and running fine.

While hoisting a haulage truck bed to surface a power cable that controls hoist function was damaged. Electricians were called to make repairs. This hoist went down at 9:45am. Hoist man tried to call the 1-800-746-1553 twice with no luck. He then called Fletchers Hoist man to report the problem. Fletchers Hoist man called MSHA at 10:35am. to report West Forks downed hoist.

EE was walking down a small incline when he stepped on a small rock and torn a muscle in his right knee. The ee is off work starting 12/21/11 until his surgery.

At approx 1:15am the production hoist #30 lost power. Electrician were called and found and replaced a wire that powers hoist controls. At approx 8:30 am the hoist was put back in service. MSHA was called at 1:15am and hoist was reported down. At 9:20 am MSHA was contacted that the hoist was back in service.

EE was removing a broken bolt on an outrigger on a drill when he felt pain in his right arm. He was sent to physical therapy on November 23, 2011.

A driller was drilling on the south side of a loaded face. The driller and charger thought they were working 25 feet apart when the driller accidentally hit one of the loaded holes.

Neutral safety switch was not working when co-worker started super-4 with transmission in gear. Basket went into rib and he got hand caught between basket and fork mast smashing first 3 fingers on on left hand. At approximately 3:15 P.M. employees noticed smoke and saw flames. It appears to be an electrical short in the generator but is still under investigation. MSHA was notified before problem was verified and the mine was evacuated.

Whike traveling down road, drill steel started bouncing when EE tried to hold them in place he caught his finger between the drill steel smashing his right finger. EE was taken to ER and Co. Dr. was Diagnosed with a laceration and fracture to right ring finger.

Employee was jacklegging out of high scaler when he noticed a slab of loose from back. Loose fell out of back striking him on head and shoulders. He has taken to Salem ER and later transferred to Barnes in St. Louis. He has a fracture to L-2 vertebre and cut to right fore finger requiring sutures.

EE was driving charging rig down hill, brakes failed and rig hit pillar. Force from rig hitting pillar threw EE forward into steering wheel. EE hurt left shoulder, right wrist and head. EE was taken to the ER and company doctor and was taken off work on 5/1/09 pending results of X-rays and follow-up visit.

Employee was riding on an open tractor through mine when debris went into his left eye. Employee was taken to company doctor and received anti-biotic drops. Employee was released to return to full duty.

Hoistman reported that limit switch caused hoist to go down. Electricians were called to inspect hoist. They found the hoist was out of time due to declutching. Hoist was put back in time and returned to service. MSHA was called at 7:30 am reporting hoist out of service. MSHA was called at 12:30 pm that hoist had been returned to service at 11:00 am.

While in the elevated basket of an Elliot truck, two miners were hand scaling, while mining one piece of rock another piece broke loose and came down the bar and struck miner on the left arm, causing a serious laceration to the left wrist and forearm. Forearm required surgical repair.

At approximately 10:00 a.m. on 12-01-07 the hoist motor went down and electricians were called. At approximately 10:30 a.m. MSHA was notified that hoist was down and a man was in the skip station and had a ladderway to escape if needed. Hoist was repaired and returned to service approximately 5:50 p.m. same day and MSHA was notified that hoist was back in service.

Employee was trying to pry cattle panels off of forks on pettibone. Another employee lifted the basket and employee got his middle finger of right hand caught between basket and mining bar. Employee was taken to company doctor and treated for lacerations/swelling.

While drilling a face round in an ore heading, the drill bit intersected an old bootleg that contained a live cap and anfo. The explosives exploded on impact of bit. The jumbo drill was hit with fly rock breaking the windshield and back glass and damaging the cover on the water pump. Called in immediately and investigated next day by MSHA inspector.

EE was rebuilding crusher. While holding up 1-1/2"" impact wrench, his elbow hurt & swelled. EE taken to Co. Dr. Elbow not any better and seen by dr on 3/21/07 where he was treated with cortizone injection in elbow. It was suggested if not better in 2 weeks, he was to see a orthopedic specialist.

Employee was walking across surface shop yard. Wind was blowing and blew foreign objects in right eye. He was taken to dr, eye specialist on 3/6/07. Three pieces of rust were removed from his eye. He was also given an antibiotic eye drops and eye drops for pain. He was taken to co dr and rx an oral pain medication. He was seen again 3/8/07 by specialist for a follow up

Ee was pulling/prying on a piece of loose on back, when he felt sharp pain in left shoulder. Ee was taken to ER where he was given a shot of a non-steroid anti-inflamatory for pain. EE RTW next scheduled shift. EE continued to complain of pain and was taken back to company doctor on 8/3/07, where he was prescribed some aditional anti-inflamatory pain medication.

EE was working under suspended load. Suspended load was cut loose from one end causing hood from loader to counter balance down, crushing EE's left hand. He was taken to ER where where he was diagnosed with 2 fractures of left hand. On Tues. 1-23-07, he was taken to hospital in St. Louis & & recommended surgery. He had surgery on 1-24-07 & remains off work.

Employee was loading powder in down holes. While cutting blasting wire toward body, the force he was using caused the knife to continue to his forearm causing a laceration. He was taken to salem emergency room where he received 11 sutures to inside of left forearm.

Employee was operating small Bobcat mucking shaft bottom. Rear end of bobcat came off the ground and dropped back down jarring employees neck and upper shoulder. Employee was seen by doctor and released to come back to work. Employee had neck surgery on 10/04/2010 and began losing time.

Grout mix was too thick causing hose to rupture under pressure. Grout got in left eye. Employee was taken to Salem E.R. where he was given an antibiotic. He was then taken to an eye doctor where he was given additional antibiotic.

EE reported alledged carpal tunnel syndrome on 12-19-06. He was seen by specialist on 2/14/07 that determined he needed surgery. He had surgery for the right hand on 4/12/07 and will have the left hand done on 4/26/07.

Employee was climbing down off of back fill and rocks. When rock gave away, He lost footing and caused him to twist right knee. He was taken to co. dr and after continuous treatment, He had knee surgery on august 25,06. He will return to work on next scheduled shift august 30, 06.

Employee was operating a loader when company co-worker tossed a rock at loader and it hit the right side loader window. Window shattered and glass went into the employee's palms. Employee was taken to company doctor on 3/7/06 and a fragment was removed from the left palm.

Found problems, made repairs to the contactor and returned to service at 9:00 a.m. MSHA was called and told hoist was down at 12:50 a.m. Then MSHA was called and told hoist was back in service at 9:30 a.m. same day.

Employee states that he picked up a bundle of roof bolts with a forklift, brought them to the rear of tractor, began unloading them to tractor when roof bolts slipped and fell against tractor mashing employees finger. Five stitches to close wound, no fracture.

EE scaled an area when a piece of loose fell across the end of boom, lifting cab, causing operator to be lifted up in the seat. Results were hitting his head on cross member of the cab, receiving a laceration to his forehead between left eye and hairline. ee was taken to emergency room where he received sutures.

ELECTRICIANS FOUND A PROBLEM WITH THE LINKAGE ON THE CONTACTOR AND REPAIRS WERE COMPLETED. HOISTMAN CALLED MSHA AT 2:35 TO REPORT HOIST BACK IN SERVICE.

THE ELECTRICIANS & MAINTENANCE CHANGED THE ARMATURE OUT & RETURNED THE HOIST TO SERVICE AT 8:45 PM. MSHA WAS CALLED & HOIST WAS REPORTED BACK IN SERVICE AT 9:15 PM, SAME DAY.

Employee alledged carpal tunnel syndrome from repetitive motion. EE was taken to company doctor on 5-13-05. Nerve conduction study was completed and EE had left wrist surgery on 6-30-05 and on 9-6-05 Employee had right wrist surgery. Employee remains off work.
