Forklift operator, attempting to crib up a piece of steel stock in holding rack, stood on an I-beam to gain height. While standing on the edge of the I-beam, the I-beam rolled, coming to rest with the ""I"" portion across the tops of both feet. This did not meet reporting standards until employee was diagnosed with a broken metatarsal on 09/20/2013.

Employee was attempting to remove cap from fuel tank. Pressure in the tank due to clogged vent caused the cap to strike employee in the face. Laceration on lower lip required stitches to close.
Employee was replacing a light fixture, which was never properly fastened during original installation. Fixture began to fall during process of unhooking. From the ladder that he was working on, employee reached to catch the falling fixture. He suffered a torn biceps muscle. This did not become reportable until May 10 when he had surgery and was placed on restricted duty.
Initial indicators are that employee attempted to rack out a 4160 cabinet that was under load. Lower cabinet door or two door vertical cabinet remained closed, upper cabinet door struck employee in the head. PPE caused laceration to forehead, employee also sustained concussion and broken fibula in the ankle region.
Employee was pulling air lance from a 4"" diameter inspection port when he felt a burning sensation in the groin area. Employee was diagnosed with a hernia that required surgery to repair. Incident did not become reportable until 5/04/13 when he had surgery.
Employee was lifting a section of air slide. Upon lifting, he felt pain in his lower back. Employee was diagnosed with degenerative disk. This incident was to reportable until employee had surgery on 06/28/2013.
Employee was assigned the task of changing sizing screen sections. To get to the work area, stairs are utilized. To perform work, employee positioned himself in a kneeling or squatting position. Pain in knee was present during the day. Injury did not become reportable until 04/12/2013 when employee had surgery to repair damaged meniscus.

No one significant event that caused lower back pain, employee had back pain after shift that ran into overtime. Pain persisted and worsened over the next two days, when medical treatment was sought.
Employee was opening end gate of overloaded dust truck. When gate opened, dust got into eye.
Employee was removing doors from FFM2 for inspection of interior of mill. When employee lifted the door cross bar, he felt a sharp pain in his back. He was lifting the bar from a kneeling position.
Maintenance crew was repairing FK Pump. No specific movement or event caused pain, back became more sore as the day progressed. Injured has had suffered from frequent back pain, although with this incident, injured has also experienced numbness in the left outer shin and foot in the days following the heavy work. Injured did not seek medical treatment until 09/05/2012.
Employee was working in the Raw Mill area and began to feel light-headed. He asked for a co-worker to drive him to the maintenance shop. Once at the shop, he began to walk to the lunchroom to get a drink, and he briefly lost consciousness. Physician diagnosed employee with dehydration.
Employee was descending a fixed ladder, and misjudged the number of rungs to the ground. He stepped down, thinking it would be to the ground, when there was another rung to the ground. His right knee twisted as his left foot went farther than expected. This did not become reportable until 09/27/2012.
Injured employee was using a combination wrench to remove a nut and bolt. The nut was difficult to remove, and employee used a hammer to strike the wrench. When the hammer struck the tool, the wrench slipped off the nut, and employee fell into the housing of pump that he was working on, striking his wrist. Incident did not become reportable until 11/07/2012.
Employee was removing bung from steel drum of waste material to sample contents. Was using a spark resistant tool, but apparently the steel bung caused a spark when it was being removed from the steel lid. This resulted in the lid being blown off the drum striking employee. Employee was wearing gloves, full-face respirator, safety boots and coated tyvek.
Employee was replacing bin valve at packhouse, and due to over-use, strained his back. Injury did not become reportable until 10/29/2012.
Employee was removing cutter head from rock drill when the cutter head dropped, crushing the tip of his finger between the drill bit and limestone. Required 3 stitches.
Employee extended his left foot out of the operator compartment of a Case Skid Steer. At the same time he lowered the bucket catching his foot between the bucket assembly and the frame. (He had received task training in the safe operating of the machine, including the need to keep his head, arms and legs inside the operator's compartment).

Employee was utilizing front end loader to load waste dust into haul truck when he noticed a glow at the rear of the truck. Employee attempted to extinguish the fire with a hand held extinguisher, but was unsuccessful. Outside emergency responders extinguished the fire.

Employee was standing on a 6' step ladder when the pipe wrench he was using slipped causing him to lose his balance and fall to the floor. (Was less than 5' above floor level.)

A bolt, connecting a roller guide and mounting plate to the top of the temporary construction personnel hoist, broke. The EE was struck with a glancing blow to the head and left shoulder and resulted in a fracture to the EE's left upper arm.
EE was setting a 6 foot concrete column form and when the form started to tip over, he attempted to keep it from falling and he stepped off the concrete footing and fell approximately 3 feet to the ground below and struck his left knee and right ankle on rocks and concrete rubble, which resulted in a torn meniscus in his left knee and a sprain to his right ankle.

INJURED WAS PLACING THE TROUGHING IDLER ON KBC-1A CONVEYOR BELT FRAME, BETWEEN TOP AND RETURN SIDE OF THE BELT. WHILE LOCATING THE UNIT, THE FRAME PORTION OF THE ASSEMBLY TURNED AND HIS FINGER WAS PINCHED BETWEEN TWO OF THE ROLLERS.

INJURED WAS REMOVING LIDS FROM METAL BUCKETS ANDDISPOSING OF THEM IN A TRASH HOPPER. AS HE DID SO, HE STRUCK HIS HAND ON A SHARP METAL EDGE SUSTAINING A CUT. HE HAD REMOVED PROTECTIVE GLOVES.
EMPLOYEE WAS CHANGING OUT A WATER BOTTLE IN THE WATER COOLER. (APPROX 43 POUNDS) AS HE LIFTED THE BOTTLE AND INSERTED INTO COOLER, HE NOTICED A PAIN IN THE GROIN AREA. DID NOT BECOME REPORTABL E UNTIL SURGERY ON MONDAY DEC 3, 2001.
