Forklift operator, attempting to crib up a piece of steel stock in holding rack, stood on an I-beam to gain height. While standing on the edge of the I-beam, the I-beam rolled, coming to rest with the ""I"" portion across the tops of both feet. This did not meet reporting standards until employee was diagnosed with a broken metatarsal on 09/20/2013.
Employee was attempting to remove cap from fuel tank. Pressure in the tank due to clogged vent caused the cap to strike employee in the face. Laceration on lower lip required stitches to close.
Employee was replacing a light fixture, which was never properly fastened during original installation. Fixture began to fall during process of unhooking. From the ladder that he was working on, employee reached to catch the falling fixture. He suffered a torn biceps muscle. This did not become reportable until May 10 when he had surgery and was placed on restricted duty.
Initial indicators are that employee attempted to rack out a 4160 cabinet that was under load. Lower cabinet door or two door vertical cabinet remained closed, upper cabinet door struck employee in the head. PPE caused laceration to forehead, employee also sustained concussion and broken fibula in the ankle region.
Employee was pulling air lance from a 4"" diameter inspection port when he felt a burning sensation in the groin area. Employee was diagnosed with a hernia that required surgery to repair. Incident did not become reportable until 5/04/13 when he had surgery.
Employee was lifting a section of air slide. Upon lifting, he felt pain in his lower back. Employee was diagnosed with degenerative disk. This incident was to reportable until employee had surgery on 06/28/2013.
Employee was assigned the task of changing sizing screen sections. To get to the work area, stairs are utilized. To perform work, employee positioned himself in a kneeling or squatting position. Pain in knee was present during the day. Injury did not become reportable until 04/12/2013 when employee had surgery to repair damaged meniscus.
No one significant event that caused lower back pain, employee had back pain after shift that ran into overtime. Pain persisted and worsened over the next two days, when medical treatment was sought.
Employee was opening end gate of overloaded dust truck. When gate opened, dust got into eye.
Employee was removing doors from FFM2 for inspection of interior of mill. When employee lifted the door cross bar, he felt a sharp pain in his back. He was lifting the bar from a kneeling position.
Maintenance crew was repairing FK Pump. No specific movement or event caused pain, back became more sore as the day progressed. Injured has had suffered from frequent back pain, although with this incident, injured has also experienced numbness in the left outer shin and foot in the days following the heavy work. Injured did not seek medical treatment until 09/05/2012.
Employee was working in the Raw Mill area and began to feel light-headed. He asked for a co-worker to drive him to the maintenance shop. Once at the shop, he began to walk to the lunchroom to get a drink, and he briefly lost consciousness. Physician diagnosed employee with dehydration.
Employee was descending a fixed ladder, and misjudged the number of rungs to the ground. He stepped down, thinking it would be to the ground, when there was another rung to the ground. His right knee twisted as his left foot went farther than expected. This did not become reportable until 09/27/2012.
Injured employee was using a combination wrench to remove a nut and bolt. The nut was difficult to remove, and employee used a hammer to strike the wrench. When the hammer struck the tool, the wrench slipped off the nut, and employee fell into the housing of pump that he was working on, striking his wrist. Incident did not become reportable until 11/07/2012.
Employee was removing bung from steel drum of waste material to sample contents. Was using a spark resistant tool, but apparently the steel bung caused a spark when it was being removed from the steel lid. This resulted in the lid being blown off the drum striking employee. Employee was wearing gloves, full-face respirator, safety boots and coated tyvek.
Employee was replacing bin valve at packhouse, and due to over-use, strained his back. Injury did not become reportable until 10/29/2012.
Employee was removing cutter head from rock drill when the cutter head dropped, crushing the tip of his finger between the drill bit and limestone. Required 3 stitches.
Employee extended his left foot out of the operator compartment of a Case Skid Steer. At the same time he lowered the bucket catching his foot between the bucket assembly and the frame. (He had received task training in the safe operating of the machine, including the need to keep his head, arms and legs inside the operator's compartment).
Injured employee was working from a kneeling position, and another worker in the area stepped on his foot. This twisted the knee and caused damage to the joint. Injury did not become reportable until date of surgery; 5/31/2012
Employee was cleaning waste fuel drum when he struck his elbow on sharp metal where lid had been removed. (5 stitches required.)

Injured and coworker were welding a patch on the preheater tower. System running at normal heat. Employees were working 10 minutes and cooling off for 10 minutes away from heat source. Were using fan and air horn. Exhibited symptoms of heat overexposure (Outside temperature 96 degrees.)
Miner was operating rock drill on 52' highwall, bit stuck & attempt to get bit unstuck,diagaonal section of the highwall sloughed,drill steel did not break,mtrl around drill steel pulled drill down with it approx. 25' & drill rolled over onto right side. Miner received laceration to forehead, laceration to right knee & contusions to right hip from wearing the seat-belt in drill.

Employee was using pry bar to open inspection door on preheater tower. Pry bar slipped striking employee in knee. (Did not become reportable until employee started to miss work due to surgery on 2/17/10.)
While collecting cement samples, employee slipped on ice falling to the ground. It was still snowing at the time of the accident.(Did not become reportable until he started to miss time due to surgery on 3/3/11.)

Supervisory employee walked a short distance into the kiln (system shutdown and locked out) to determine if temperature had dropped sufficiently to allow brick peeler to remove built up coating. As he did so, not realizing that there was coating overhead, it fell striking him in the back and knocking him down.

As employee was removing guard from apron pan conveyor, the guard caught, causing him to lose his balance and take a couple of steps backwards. As he did so, he stepped in a pool of hot kiln feed.
Employee was climbing ladder on loader when he slipped. He held on with his right arm to prevent fall, resulting in strain to shoulder. (Did not become reportable until employee started to miss work due to surgery on 7/13/09).

As employee was investigating plugged temperature probe on preheater tower, hot cement puffed out of port entering employee's eyes (was wearing protective clothing including face shield).

Injured and coworker were moving an empty Con-Ex shipping container with two forklifts. The forklift the injured employee was operating started to tip and employee unfastened seat belt and attempted to jump free resulting in injury to his leg.
Employee was performing routine maintenance on vacuum truck, when a large hose fell from an elevated position, striking him in his left chest. (Did not seek medical treatment until 102408.)
Employee was utilizing front end loader to load waste dust into haul truck when he noticed a glow at the rear of the truck. Employee attempted to extinguish the fire with a hand held extinguisher, but was unsuccessful. Outside emergency responders extinguished the fire.
Side of dozer slipped when employee passed crest of stockpile causing him to be partially ejected from seat. Seat belt came unhooked (Seat belt was not defective, but was apparently not securely fastened.)
Employee hooked up air hose to cement silo cone and turned on air. As he was inserting safety clip into coupling, he heard air leak. As he was adjusting the coupling, it came apart, striking him in the mouth.
While moving concrete blocks from flood wall to bed of truck, employee strained his back. (Did not start missing work until 7/24/08)
Employee was climbing a stationary vendor ladder attached to equipment. He was grasping for the next rung and his hand slipped and he fell backwards 4'. His back hit against an adjacent railing.
While moving mill grinding ball hopper into position, the ball on the hopper rolled over, pinching the employees hand between handle and ball. (Required stitches)

As employee entered new cooler, he stumbled catching foot between rollers.
Three workers were handling several lengths of pipe. Pipe slipped, pinching middle finger of left hand of one of the workers, breaking the finger just below the fingernail.

Employee & his crew were disassembling a light plant above ground. Buildup of ice on roof of auxiliary building broke loose & fell approximately 30-40 feet striking EE in the head & neck, knocking him to the ground. He has two herniated disks in his neck per physician & MRI study.
Employee was standing on a 6' step ladder when the pipe wrench he was using slipped causing him to lose his balance and fall to the floor. (Was less than 5' above floor level.)

A bolt, connecting a roller guide and mounting plate to the top of the temporary construction personnel hoist, broke. The EE was struck with a glancing blow to the head and left shoulder and resulted in a fracture to the EE's left upper arm.
EE was setting a 6 foot concrete column form and when the form started to tip over, he attempted to keep it from falling and he stepped off the concrete footing and fell approximately 3 feet to the ground below and struck his left knee and right ankle on rocks and concrete rubble, which resulted in a torn meniscus in his left knee and a sprain to his right ankle.
EE was in the laydown yard, cleaning trash and moving railroad ties. EE alledged he hurt his back while moving ties by hand. EE did not seek medical help for 2 weeks. EE's doctor recommended days away from work starting 10/25/2007. EE's doctor advised he had a possible herniated disk in lower back.

A brace was being hoisted by a crane. The connector was climbing into position. The flagman for the crane was not properly positioned and did not wait for the connector to get into position. The flagman brough the brace in too low and struck the connector in the back pinching him into a beam. He fell and his safety equipment caught him.

Employee was one of five employees pulling on a 5"" concrete pump truck hose when his right foot became entangled in a horizontal rebar mat. When the other employees tugged on the hose to move it, the hose was pulled across the top of his right foot and pushed the back of his right heel up against the rebar causing a strain to the Achilles tendon of his right foot.

ON 5/26/04, EMPLOYEE WAS DIAGNOSED WITH BILATERAL CARPAL TUNNEL SYNDROME. VARIOUS ACTIVITIES (SHOVELING, JACKHAMMERING, OPERATING MOBILE EQUIPMENT & WELDING) COULD HAVE CONTRIBUTED TO THE CONDITION. EMPLOYEE IS ALSO DIABETIC WHICH COULD HAVE BEEN A CONTRIBUTING FACTOR. DID NOT BECOME REPORTABLE UNTIL EE STARTED TO MISS WORK ON 9/16/04 DUE TO SURGERY.
EMPLOYEE WAS REPLACING COVER ON CEMENT BARGE WHEN HE RAISED UP STRIKING HIS HEAD ON THE LOADING SPOUT. DID NOT EXPERIENCE PROBLEM UNTIL EVENING OF 05/04/04. DID NOT BECOME REPORTABLE UNTIL 06/03/04 WHEN HE WAS DISABLED BY DR. & STARTED TO MISS WORK.
WHILE REMOVING INSULATION FROM CONDUCTOR CABLE, THE EE CUT HIS FINGER WHEN KNIFE SLIPPED. (LACERATION REQUIRED STITCHES.)

INJURED WAS REMOVING A RAG FROM SHREDDER (LOCKED OUT) WHEN HE CUT HIS FINGER ON A PIECE OF METAL CAUGHT IN THE RAG.
EE WAS CUTTING LID ON DRUM WITH AIR CHISEL. USED HAND TO PULL UP ON LID AND METAL CUT THROUGH GLOVES.
INJURED WAS PLACING THE TROUGHING IDLER ON KBC-1A CONVEYOR BELT FRAME, BETWEEN TOP AND RETURN SIDE OF THE BELT. WHILE LOCATING THE UNIT, THE FRAME PORTION OF THE ASSEMBLY TURNED AND HIS FINGER WAS PINCHED BETWEEN TWO OF THE ROLLERS.
EMPLOYEE WAS DESCENDING FROM FRONT END LOADER WHEN HE FELT SHARP PAIN RUNNING FROM LEFT RING FINGER TO LEFT ELBOW. HAD PREVIOUSLY EXPERIENCED OCCASIONAL NUMBNESS AND TINGLING IN LEFT HAND. (DI D NOT BECOME REPORTABLE UNTIL HE HAD CARPAL TUNNEL SURGERY ON 8-14-03).
EMPLOYEE WAS REMOVING DRAG CHAIN FROM CLINKER COOLER WHEN HE FELT PAIN IN HIS SHOULDER. INJURY DID NOT BECOME REPORTABLE UNTIL 2/27/04 WHEN HE BEGAN TO MISS WORK DUE TO SURGERY.
INJURED WAS USING DRIFT PIN TO DRIVE OUT TOP PINOF HYDRAULIC LIFT CYLINDER FOR BED ON 55-TON TRUCK. AS PIN DROVE OUT, CYLINDER DROPPED DOWN CAUSING THE DRIFT PIN TO PINCH THE INJURED'S FINGER AGAINST THE MOUNT BRACKET FOR THE CYLINDER.
WHILE CUTTING HOLE IN PRECIPITATOR ROOF, INJURED STRUCK HIS LEG ON A PIECE OF SHARP METAL (REQUIRING STITCHES).
INJURED FELT STRAIN IN SHOULDER WHILE CHANGING WOBBLER BAR IN PRIMARY CRUSHER. ALSO COMPLAINED OF NUMBNESS AND TINGLING IN BOTH HANDS. DID NOT BECOME REPORTABLE UNTIL HE HAD CARPAL TUNNEL SURGERY ON 8-6-03.
INJURED AND COWORKERS WERE MOVING CABINETS WHEN HOIST CHAIN CAUGHT ANOTHER CABINET CAUSING IT TO FALL AND STRIKE INJURED EMPLOYEE. (WAS WEARING SAFETY BOOTS WITH METATARSAL PROTECTION).
EMPLOYEE WAS REACHING OVERHEAD TO TIGHTEN BOLTS ON MOBILE EQUIPMENT BEING REPAIRED. DID NOT BECOME REPORTABLE UNTIL 12/17/02 WHEN INJURED WAS DISABLED BY DOCTOR.
INJURED JAMMED HIS HAND INTO BUCKET CONTROLS WHEN LOADER BUCKET STRUCK PILE OF ROCK. DID NOT BECOME REPORTABLE UNTIL 9-16-02 WHEN HE STARTED TO MISS WORK DUE TO SURGERY.
INJURED WAS CLIMBING LADDER TO CAB OF HAUL TRUCK WHEN HE STRUCK HIS KNEE ON THE LADDER. AS HE CONTINUED UP THE LADDER, HIS KNEE TWISTED CAUSING STRAIN. DID NOT BECOME REPORTABLE UNTIL 9/9/02 W HEN HE STARTED MISSING WORK DUE TO SURGERY.
WHILE TIGHTENING PIPE ON RAC-4 COMPRESSOR, INJURED'S HAND STRUCK A SHIM UNDER THE BASE OF THE COMP0RESSOR.
INJURED WAS CLIMBING DOWN OFF OF A TANKER TRUCK AND STEPPED ONTO A PIT GRATING WHICH HAD BEEN PUSHED APART.
INJURED WAS SHOVELING CEMENT AND HARDENED CHUNKS FROM THE GRATING UNDER BIN #18 WHEN HE FELT STRAIN IN HIS SHOULDER. DID NOT BECOME REPORTABLE UNTIL HE STARTED MISSING WORK DUE TO SURGERY ON 1 /24/03.

INJURED WAS STARTING TO DRILL A PILOT HOLE, WHEN THE GLOVE ON HIS HAND BECAME ENTANGLED WITH THE DRILL CHUCK DRAWING HIS HAND AND FINGER ONTO THE DRILL BIT.
EMPLOYEE STEPPED UP ON A LADDER TO REMOVE A PIECE OF PLASTIC STICKING UP ABOVE THE DOOR ON THE IN-FEED HOPPER. HE HAS PLACED HIS RIGHT HAND ON THE LIP OF THE FRAMEWORK FOR THE SLIDING DOOR AND IT WAS PINCHED AS THE DOOR CLOSED.
EE WAS WALKING ACROSS A GRAVLED PARKING LOT WHEN SHE STEPPED IN A HOLE AND TWISTED HER KNEE.
INJURED WAS CUTTING LIDS OFF CLOSED TOP DRUMS WITH AIR CHISEL. AS HE WAS DOING SO, HE SCRAPED HIS FINGER ON SHARP EDGE OF DRUM LID. WAS NOT WEARING PROTECTIVE GLOVES PROVIDED BY THE COMPANY.
INJURED WAS DESCENDING FROM QUARRY HAUL TRUCK WHEN HE SLIPPED STRAINING HIS BACK. DID NOT SEEKMEDICAL ATTENTION UNTIL 11-19-01, AT WHICH TIME HE WAS DISABLED BYU COMPANY DOCTOR.
INJURED WAS REMOVING LIDS FROM METAL BUCKETS ANDDISPOSING OF THEM IN A TRASH HOPPER. AS HE DID SO, HE STRUCK HIS HAND ON A SHARP METAL EDGE SUSTAINING A CUT. HE HAD REMOVED PROTECTIVE GLOVES.
EMPLOYEE WAS CHANGING OUT A WATER BOTTLE IN THE WATER COOLER. (APPROX 43 POUNDS) AS HE LIFTED THE BOTTLE AND INSERTED INTO COOLER, HE NOTICED A PAIN IN THE GROIN AREA. DID NOT BECOME REPORTABL E UNTIL SURGERY ON MONDAY DEC 3, 2001.

EMPLOYEE WAS TROUBLESHOOTING THE PACKING AND LOADING PUMPMAN'S ALARM PANEL. THIS PANEL IS LOCATED IN AN ELECTRICAL ENCLOSURE AT THE VERY BOTTOM, ON THE BACK WALL OF THE ENCLOSURE. EMPLO YEE HAD TO BE ON HIS HANDS AND KNEES TO PERFORM THIS JOB. AT THE END OF THE JOB, EMPLOYEE COMPLAINED OF PAIN IN RIGHT KNEE. BECAME REPORTABLE ON9/10/01 WHEN HE STARTED TO MISS DUE TO SURGERY.
INJURED COMPLAINED OF WRIST PAIN/NUMBNESS IN BOTH HANDS. HAS BEENEMPLOYED AS LABORER FOR 11 1/2 YEARS. PERFORMS VARIED LABOR WORK (IE. SHOVELING, JACKHAMMERING, OPERATING MOBILE EQUIPMENT-BOB CATS/FORKLIFTS). PERFORMS VACATION RELIEF IN CEMENT LAB TESTING RAW MATERIALS AND FINISHED PRODUCT. INJURED WAS POWER WASHING DRUM CLAMP ARMS FOR THE HYDROPULPER. AS HE BENT OVER TO MOVE THE PARTS TO WASH THE OTHER SIDE, HE STRAINED HIS BACK.
EE WAS SECURING TARP ON ROLL OFF BOXES OF WASTE MATERIAL. HE BENT DOWN ON ONE KNEE AND WAS USING A RATCHET TO TIGHTEN DOWN TARP. THE JERKING ACTION OF TIGHTENING, CAUSED HIM TO EXPERIENCE PAIN IN HIS LOWER BACK.
THREE CABLES WERE STRUNG ABOVE THE CENTERLINE OF THE KILN TO SUPPORT SCAFFOLD BOARDS USED AS A WORK PLATFORM. THE CENTER CABLE FELL WHEN THE ATTACHMENT BROKE LOOSE FROM THE KILN SHELL. INJURED WAS ON THE SCAFFOLD WEARING A SAFETY HARNESS, BUT HAD NOT TIED IT OFF. AS ANOTHER EE ACCESSED THE SCAFFOLD IT FELL APPROXIMATELY 9 FEET.

EMPLOYEE WAS RETURNING MIXER BLADE FOR INSTALLATION WHEN BLADE SLIPPED OUT OF HIS HAND, HITTING THE TOP OF HIS FOOT. BLADES HAD BEEN REMOVED AND REPAIRED IN MAINTENANCE SHOP, THEN TRANSPORTED TO TANK IN TRUCK. AS EMPLOYEE WAS SETTING BLADE DOWN, IT SLIPPED FALLING ONTO HIS FOOT.
INJURED WAS WALKING TO PREP ROOM WHEN HE TWISTED KNEE BECAUSE OF UNEVEN GROUND. HE HAD PREVIOUSLY INJURED SAME KNEE WHEN HE TWISTED IT MOVING THE RAIL UNLOADING RAMP. DID NOT BECOME REPORTAB LE UNTIL HE STARTED LOSING TIME DUE TO SURGERY ON 8/3/00.
INJURED HAD CUT LID OFF A CLOSED TOP DRUM OF MATERIAL TO REMOVE LIQUIDS. AS HE ATTEMPTED TO TIP DRUM, HE CUT HIS THUMB ON THE SHARP EDGE.
INJURED WAS MOVING BARREL LIFTS IN FEED PREP #2 WHEN THEY BECAME STUCK ON THE FORKS OF THE FORKLIFT. HE WAS MANUALLY PULLING THEM OFF THE FORKS WHEN THEY FELL CATCHING HIS FINGER.
INJURED WAS MOVING A 55 GAL DRUM OF WASTE MATERIAL TO DIFFERENT ROW FOR STORAGE WHEN HE SPRAINED HIS KNEE. (DRUM CART WAS AVAILABLE). DID NOT BECOME REPORTABLE UNTIL HE STARTED MISSING WORK DUE TO SURGERY ON 8-3-00.

ONJURED STATES HE HAD PAIN IN BOTH WRISTS AND NUMBNEWS IN HIS FINGERS SINCE MAY OF 1999 WHEN HE WAS ASSISTING IN THE INSTALLATION OF A NEW DUST COLLECTOR SYSTEM AT #6 STOCKHOUSE. WAS NOT DIAGN OSED UNTIL 1-6-00. DID NOT BEC9OME REPORTABLE UNTIL 11-2-00 WHEN HE STARTED TO MISS WORK DUE TO WRIST SURGERY.
