The employee was inspecting scaffolding inside the riser duct and he got some dust in his eye. The employee was wearing all necessary PPE when this occurred.
Employee was kneeling down several times during the day and felt knee pain as a result of repetitive motion. Employee went to Doctor and was diagnosed with Bursitis of the knee.
Employee was using sledge hammer to beat pins. Six days later employee's arm was still sore and the employee sought medical treatment.

On 11/05 Employee states on 11/02 he was cleaning out an air lancing tower. EE was hanging onto a long steel pipe and sliding it inside the tower. It got stuck and he started experiencing right shoulder pain.
Employee was working at the ASF site. He stepped over a barricade on to a bolt sticking out of the floor twisting his ankle. He sought treatment at Occupational Health Services and is currently on modified duty for a sprain to his foot and ankle.
Installing Air Blaster on bin, employee & supervisor went to test fire Blaster. When Blaster fired, cap on clean out port came off and hit employee on left leg. Employee said leg was a little sore but did not need medical attention. Pain continued and employee sought treatment at OHS Compcare on 1/18/2012. Employee returned with restrictions and is doing physical therapy.

Employee was on top of silo using full strength to pull on bar. Bar came loose and employee fell onto buttocks. Initially patient was applying ice and heat to the area at home. On 11/9 employee reported continued stiffness and discomfort in tail bone and chest and sought treatment at Occupational Health Services on 11/10. Employee is on modified duty and seeking physical therapy.
Employee was clamping metal in the bandsaw when his finger got pinched in the clamping vice. Employee sought treatment at Centerpoint Medical Center in Independence Center and received 12 stitches in left index finger. Employee is on modified duty with follow up care being conducted by Occupational Health Services.
Employee was lifting buckets of refractory above and over his head to another employee. Employee was using twisting motion and felt discomfort in his back. Employee had numbness thru the night and sought treatment at Occupational Health Services on 4/1. Employee returned to work on light duty.

Employee was using an impact tool and punctured his left index finger. He was wearing leather gloves and was able to control the bleeding and placed a bandaid on the finger. He left work at 5pm, but when he got home he noticed pressure in the fingernail area and sought treatment at St Lukes ER in Lee's Summit, MO. Pressure was relieved from nail,two stitches were placed in finger.
4/21 employee phoned supv saying he didn't feel well and sought treatment with personal physician. 4/22 employee notified supv that pers phys suggested company doctor for possible workers comp. Employee told supv he didn't know what happened, then reported to company doc that he suffered strain opening valve. Diagnosis-right chest strain, restricted duty.
Employee was working at the ASF barn. When unplugging material from the Baler, chili cheese dust blew into employee's left eye causing irritation. Condition continued and employee sought treatment at Occupational Health Services on 3/15/2010. Tobrex drops were prescribed for one week and employee was returned to work at full duty.
Employee was pulling grinding aid lance from mill. When lance broke free, finger was smashed between lance and airline support bracket. Employee sought treatment at OHS. Diagnosis is laceration to right ring finger. Employee is on modified duty.

Employee was helping clean out the raw mill. He went to lower a bucket to another employee and felt discomfort in his elbow. Pain was not relieved and employee south treatment at OHS on 2/23/10. He was returned to full duty and is receiving physical therapy 2x week for 3 weeks. Return to OHS scheduled for 3/9/10.
Employee was holding and inspecting a tip valve. The air cylinder retracted and caught his right thumb between the arm and the cylinder. He sought treatment at OHS Compare for avulsion of the nail and nailbed.

Employee was cleaning the tower when a chunk of cement came loose and fell striking the bar that he was using to clean. The bar then swung around and hit him in the side of the head. He is complaining of headaches and strained muscles and sought treatment at OHS on 11/17/08. He is on 10 lb lifting restrictions and sees the doctor again on 11/20/08.
In February 2009, employee claimed carpal tunnel syndrome due to repetitive nature of her job. Investigation was taking place to determine if damage was work related. Employee filed claim with the State of MO on 4/20/09. Company determined on 5/21/09 that injury will be workers comp case. Therefore 7000-1 now being filed. Employee had surgery and is on light duty.
Employee came to control room complaining of lower back pain and thought he had kidney stones. He was taken to OHS Clinic where diagnosis was lower back strain.
Employee suffered heat exhaustion and was taken to ER at Centerpoint Hospital in Independence, MO at 3:00 AM on 8/3/08. RW followed up with the company clinic, OHS, on 8/4/08 and was released to full duty.
On 3/27 shift employee was cleaning cement dust. He saw his personal physician on 4/1 due to coughing, wheezing, and spitting up and was sent by Lafarge to the Occupational Health clinic on 4/2. He was referred to pulmonologist on 4/7 and released to full duty. Company managers and 3rd party administrator, Health Bridge, determined case a medical incident and reportable on 5/2.
Employee was attempting to pull a pallet jack that had a full load and felt a sudden popping and pain in his right wrist. Pain persisted and employee sought treatment on 1/2/08 and was diagnosed with wrist strain. Employee was given rigid splint and placed on restrictions of no right hand gripping.

Alt Solid Fuel fire caused by heat buildup in 2nd shredder. Heat was caused by buildup of low density plastic in shredder screen which caused material to be over shredded. Metal was also present in 2nd shredder screen which may have provided a spark. Any of these conditions may have lead to the ASF catching fire.
While sweeping the scale, employee stepped on 1/4"" bolt protruding upward on scale. The bolt held skirting in place. Employee rocked backwards, hyperextended, and felt pain in her knee. She stated no sharp pain except when attempting to pivot her movement. Initially first aid, medical case July 6th. Employee on kneeling/squatting restrictions and MRI to be performed.
End of shift 3/14, employee lifted tool bag out of truck and felt strain in his stomach. Morning of 3/15 employee reported incident from 3/14. He indicated his stomach was still hurting and had a knot. He was sent to local clinic. Clinic physician confirmed several days later injury was in fact work related after conversations with Lafarge and specialist. Surgery 3/30 to correct.
Was removing flyash debris when a piece of brick fell from a ledge above hitting him on right forearm.

EE saw leak in inspection port on extraction air slide under cement silo.He tried to tighten the fastener on port; it came loose; cement flushed out of silo and into his eyes.Employee was wearing safety glasses w/side shields.He flushed his eyes, still experienced discomfort and sought treatment at OHS.Eyes were flushed; employee was given prescription eye ointment, tobramycin.

Welding operations, in which employee was pulling and tugging at metal sheets, in order to position them. Placed on meduim restriction by medical facility due to back strain. Estimated restriction 14 days.
Employee was performing cleaning prior to kiln startup. He saw no buildup, but when he moved to the northernmost port a puff of hot gas exited the port, struck the employee, and burned his face. Employee sought treatment at local emergency room and was diagnosed with 2nd degree burns on his face.
Temporary employee was cleaning and shoveling the cooler area when material entered his right eye. Employee was wearing safety glasses with side shields while performing his job. Sought treatment at Medical Center. Prescribed Rx eyedrops and returned to full duty the same day.

There was no specific event associated with this injury. Employee had pain in his wrist and went to his personal doctor. His physician told him it was a work related injury due to overuse. Employee saw company doctor and was diagnosed with tendonitis and placed on modified duty.
EE on top of silos changing blanks to switch silos. Used Bar/Tool to loosen knuckles that clamp blank in place. Felt resistance on third knuckle, pushed on bar with extra effort to loosen knuckle. Relaxed pressure on bar, put pressure off and on couple times to loosen. Bar slipped, EE moved forward, pinched finger on Cement Line below. Received stitches at Liberty
Employee breaking up lumps of weathered clinker being reclaimed through grated hopper. Used hammer to hit long jack hammer bit, like a chisel to break lumps, chisel in left hand, hammer in right. Shard of metal from top of jack hammer bit broke off, striking employee on inside of arm cutting vein. Antibiotics and tetanus shots administered, wound sutured.
Employee was complaining of tingling in fingers. He states it is worse when he operates heavy equipment, especially using each hand to operate the levers. He sought treatment at occupational health services. He was diagnosed with carpal tunnel syndrome.
Contractor was in the pump room running conduit from the second step of a step ladder. As he went to get off the step ladder his foot slipped off of the first step and he fell to the ground. He put his right hand out to break his fall and broke his right wrist.

EE ENTERED THE MILL BUILDING TO CHECK THE CLINKER HOPPER. HE SLIPPED ON CLINKER THAT WAS ON THE FLOOR AND STRAINED HIS BACK.
EMPLOYEE WAS WALKING DOWN FRONT STEPS OF MAIN OFFICE BUILDING. SHE STEPPED OFF THE TOP STEP AND HER ANKLE TURNED.
EMPLOYEE WAS PUSHING CONNECTORS ON TO THE TAIL LIGHTS ON THE T304 TRUCK. NO TOOLS WERE USED. EMPLOYEE EXPERIENCED REPETITIVE TRAUMA TO THE RIGHT THUMB.
TORQUE MULTIPLIER WAS PUT ONTO SOCKET AND PRY BAR HANDLE WAS USED TO EXTEND THE HANDLE TO HOLD IT FROM ROTATING. EE USED THE 1/2 DRIVE RATCHET AND THE PRY BAR BENT AS THE RATCHET WAS BEING REL EASED, SPUN AND STRUCK THE EE ON THE RIGHT THUMB.
EMPLOYEE WAS MOVING TWO CONVEYOR SCREWS-ONE TO THE STOREROOM AND ONE TO THE JUNK AREA. WHEN HE REACHED THE STOREROOM, HE TRIED TO LIFT ONE SCREW OVER THE OTHER AN DFELT A PULL IN HIS SHOULDER . PAIN PERSISTED AND EMPLOYEE SOUGHT TREATMENT ON 8-30.
EMPLOYEE WAS TIGHTENING THE COUPLING BOLTS ON THE H-PUMP. THE RATCHET HE WAS USING SLIPPED TEETH IN THE GEARS, CAUSING A SEVERE JOLT TO THE EMPLOYEE'S ELBOW AND SHOULDER. HE DECIDED TO SEE T HE COMPANY PHYSICIAN ON 8/17/00.
EMPLOYEE WAS WALKING UP THE STEPS BY THE CRUSHERWHEN HE SLIPPED ON CLINKER AND FELL, TWISTING HIS RIGHT KNEE. A KNOT DEVELOPED ON THE KNEE AND EMPLOYEE DECIDED TO SEEK TREATMENT ON 10-20-00.
EE WAS PRYING ON MOTOR TO PLACE SHIMM FOR ALIGNMENT WHEN THE RAIL BAR SLIPPED AND STRUCK HIM IN HIS LEFT THUMB.
EMPLOYEE WAS LIFTING A 5 GALLON BUCKET OF OIL TO PUT IN THE #2 FINISH MILL DISCHARGE BEARING. THE BUCKET WAS HEAVY AND HAD TO BE LIFTED OVER A 3.5 FOOT RAILING WITHOUT SPILLING. EMPLOYEE EXP ERIENCED PAIN IN HIS CHEST WALL.
EE WAS WASHING THE IMPACTOR BASEMENT WITH A FIREHOSE. HE SLIPPED ON THE STEPS & HIT HIS HIP ON THE STEPS AS HE FELL
EE WAS PUSHING COOLER DOOR CLOSED WOTH A TEN FOOT PIPE WHEN NE FELT A SHARP PAIN IN HIS RIGHT KNEE.
ON 3/13/00 EE WAS HELPING TO UNLOAD BREAKERS FROM THE BACK OF A TRUCK. HE FELT A PAIN IN HIS LEFT FOOT. ON 3/15/00 HE WAS ASSISTING IN CALIBRATING THE WEIGH FEEDERS AT THE PLANT. HE STEPPED DO WN FROM ONE LEVEL TO ANOTHER AND FELT AN EVEN SHARPER PAIN IN THE SAME ATEA OF THE FOOT. ON 3/16/00 THE EE WENT TO THE DR WHEN THE PAIN PERSISTED & HE EXPERIENCED DISCOMFORT WHEN THE FOOT TUR
ON 3-10-00 EE WAS OPERATING THE DOZER. AS HE TRIED TO GET OFF HIS PIECE OF EQUIPMENT, HE FELL ONTO HIS KNEE ON THE BLADE ARM. ON 4-11-00, THE EE WAS OPERATING THE LOADER AND AGAIN EXPERIENCED PAIN IN HIS KNEE. HE SOUGHT TREATMENT, BUT WAS RELEASED TO WORK WITH NO RESTRICTIONS. 7-12-00 THE EE AGAIN SOUGHT TREATMENT BECAUSE OF THIS SAME PROBLEM. HE IS NOT PERFORMING HIS JOB BUT WITH
EE SUFFERRED A BACK INJURY IN NOV 1998. IN FEB 2000, THE EE SUFFERRED A REOCCURRENCE OF THIS INJURY. HE PULLED AND STRAINED HIS BACK WHILE TRYING TO CLOSE THE DOOR TO THE IMPACTOR. BACK SURGER Y WAS PERFORMED ON 7-13-00: L4 TO S1 FUSION WITHINTERBODY CAGES AT THE L4-5 MOTION SEGMENT.
EE WAS CUTTING A BOLT OFF OF THE UPPER TABLE OF THE SEPARATOR. THE TABLE DROPPED DOWN AND PUNCTURED THE LEFT LEG OF THE EE, JUST ABOVE THE KNEE.
EE STRAINED HER RIGHT SHOULDER WHEN LIFTING GRINDING BOWLS OUT OF THE SPGX GRINDING MILL. THE EE WAS RETRIEVING MATERIAL FROM THREE GRINDING BOWLS.
