Over-exertion in pulling or pushing objects

Employee reported pain with shoulder after adjusting a gate at the secondary plant. It was determined on 11/19/2024 by an Orthopedic Doctor that the employee needed physical therapy.

The employee was overhead welding on the water truck and felt a pop in EE's right shoulder. Initially EE did not think anything of it, but later in the shift EE felt another pop in the right shoulder while assisting another employee lift a screen.

Struck against stationary object

Employee was shoveling material from under a conveyor belt and struck a hand on the plants structure. This contact resulted in a bruised fingernail and a slight fracture of the bone on the tip of a finger.

Over-exertion in lifting objects

While helping stem shot - Employee was scooping Crushed Rock into bucket for stemming - ee felt sharp pain in side was able to finish work for the day. Told me that it still hurt and I told ee to go to the doctor that evening. Employee is able to come back to work with restrictions. Strain of muscle and tendon of front wall of Thorax.

While putting material in QC pans, EE was picking them up when the material shifted causing the pans to tilt and slide across my fingers cutting three fingers requiring 5 stitches.

Fall to the walkway or working surface

Employee exited the service truck to set the wheel chocks. The employee opened the door put his left leg out and then stepped out with the right leg. Employee slipped forward causing the employee to fall and injure his right calf.

Employee working on electrical service, when re-energizing MCC after fix, arc flash occurred.

Driller came forward on October 28, 2013 and reported carpel tunnel and went back to work. He has now come forward again and is scheduled for surgery on 2/5/2015.

Miners were replacing the cable that controls the extension and retraction of the telescoping end of a stacker conveyor. The telescoping end of the stacker moved causing tension on the cable. A miner's foot was caught in the cable resulting in a broken leg. The miner was airlifted to a local hospital. The miner's left foot was amputated just above the ankle.
