Employee fell while descending conveyor about 3', resulting in a 1"" cut to right elbow and 2 stitches.

Oxygen hose on cutting torch set broke and made a loud ""pop"" causing the miner's left ear to ring and left eye to see blue and red dots. Miner was taken to ER and evaluated, released to duty with restrictions until follow up apt. Possible concussion. No other physical visible injuries.

The wind blew a piece of metal into left eye while grinding. No lost time, full duty, no restrictions, prescribed antibiotic ointment

Mine worker found an apparent dead body in pit area. Notified supervisor who notified 911 service and MSHA. Authorities determined deceased was a 17 year old male and not employed by Con Agg of MO, LLC. MSHA inspector completed the investigation for MSHA.

Temperatures were near or at 100 that day. Had been performing maintenance on the air conditioning on the loader. Then, had loaded 3 trucks. Then became dizzy, had shortness of breath, loss of control of movement and almost blacked out. Due to dehydration. NOTE: Late report due to possible non work related condition.

EE WAS CLIMBING OFF OF HIS TRUCK & SLIPPED ON A STEP & FELL 2' & HURT HIS RIGHT ANKLE AND FOOT.

EE WAS POURING CONCRETE AT THE QUARRY. HE HAD RUBBER BOOTS ON BUT SOME CONCRETE GOT ON THE BACK SIDE OF HIS LEGS.

EE WAS USING A GRINDER ON THE SOUTH WALL WHEN A PIECE OF STEEL GOT IN HIS LEFT EYE.
