Employee was helping string a conveyor belt. When pulling slack out of the roll with a rope, the knot secruring it came loose, causing them to fall onto a steel beam. Employee has a small fracture to the ninth rib.

Employee was walking around a skid steer and tripped over the corner of the bucket, causing him to fall to the ground. Employee requested to be seen by physician for leg pain. Employee was seen by a doctor for pain to his upper left leg. Physician determined there was no broken bone and restricted employee to limited walking. Employee was released to full duty on 5/4.

While refueling an air track drill, a spark ignited some diesel fuel. Flames caught EE's left hand.

Climbing a ladder, slipped and fell, striking a 2x4 piece of wood. Stitches

Pulling screens, rope broke, he fell against a hand rail.

MINER ALLEGES HE SUSTAINED WORK-RELATED INJURY (CLEARING A ROCK OUT OF THE FEEDER...THREW ROCK WEIGHING 85/100 LBS.) POSSIBLE HERNIA. MINER DIDNOT REPORT INCIDENT FOR 5 OR 8 DAYS TO MANAGEMEN T PERSONNEL AND WAS TREATED BY HIS FAMILY PHYSICIAN (UNAUTHORIZED TREATMENT). MINER SIGNED OFF ON REPORTING PROCEDURE AT ORIENTATION RECEIVED UPON HIRE.
