Emp. was working on a belt and a piece of metal fell and hit EE's right ring finger causing a cut and a bruise.

Technician was working on a machine at a mine site. When EE stepped down off the machine, EE felt a pain in the knee.

EE was exiting the loader, missed bottom step, and had already released his one hand grip causing him to lose his balance and he fell to the ground striking his elbow and shoulder.

Using shoulder to rest pry bar on attempting to lift head pulley to place bearings, he strained his shoulder.

Employee not sure of time nor place of occurrence. He believes that backing of equipment choked and bruised his lower back.

EMPLOYEE WAS EXITING DOZER WHEN HE STEPPED ON A DIRT CLOD AND JUMPED TO THE GROUND CAUSING A SPRAINED LEFT ANKLE.

EE WAS PUTTING WATER LINE IN TO PLANT. HE ALLEGES BACK STRAIN WHILE TIGHTENING BOLTS.
