Employee was assisting in cleaning up an area of old parts, pipes, etc. As he was carrying a piece of iron on his shoulder, he felt something on his face & neck and brushed it aside with his gloved hand. Later, after arriving home, he noticed 3 bite marks on his neck & face. He reported the incident the next day at work and went to the company doctor at that time.
The employee was performing his inspection rounds required at beginning of each shift when he felt a stinging under his left arm. He found no insect but reported the incident at 8 Pm that night to his supervisor after swelling & soreness occurred.

Employee was cutting handrails with abrasive cutoff wheel. Handrail pipe flexed and pinched the cutoff wheel causing it to shatter and debris struck the employee in the lower right forearm. The forearm was lacerated by the debris.
A coal mill explosion occurred within our coal mill. No injuries of plant personnel. Contributing factors were a diff. of about 20F between control room thermocouple and manual readings. Operational set point for coal mill exit temp. is about 180F and with this diff. there was the possibility that the temp reached 200F.
Employee was attempting to clean out vertical air slide in cement silo. When he opened the port door, cement under pressure sprayed into his eyes after knocking off safety glasses.

EE was making his routine rounds around the kiln when he felt something in his left eye. EE was wearing safety glasses so the object must have been blown in by the wind. A small speck was seen on his eye so, being it was night shift, he was sent to the hospital to have particle removed. The attending physician flushed out the eye and suggested he get some occular fluid.
Employee was helping change out division head in finish mill when weld on plate broke, causing it fall on employee.
EE was hitting the neck of the finish mill with a sledge hammer (because of a neck choke up) trying to unstop the neck. Cement falling from above got into his left eye. He was wearing safety glases at the time.
Process operator was moving wheelbarrow from underneath raw mill tail wheel. While lifting and pulling the wheelbarrow caught on frame of conveyor suddenly stopping causing muscle strain.
Mechanic was using a 1"" impact wrench with a 2 3/8"" socket. The safety pin that held socket on was a welding rod, which was hanging out of socket. The mechanic's hand (which was in glove) was slashed and cut by the rod while socket was in motion. 12 stitches were required to seal wound on right hand.

Key Temp employee contractor was en route to the cement silos to change silos. When exiting the clinker silos on the north end, he slipped on granular clinker. This caused him to lose footing and apparently twist ankle. Two days later by x-ray a doctor found a broken bone in foot.

EE WAS USING A POCKET KNIFE TO SCRAPE OFF SILICONE RUBBER ON A PLATE FOR SILO B. KNIFE SLIPPED AND CUT HAND BETWEEN THE LEFT THUMB AND FOREFINGER.
EE'S SUPERVISOR STARTED LOOKING FOR HIM WHEN HE GOT NO RESPONSE FROM THE EE WHEN CALLED ON PLANT RADIO. HE FOUND HIS FRONT END LOADER AT THE BASE OF THE CLINKER SILO SO HE WENT TO THE TOP OF T HE SILO WHERE THE EE COULD BE TAKING MEASUREMENTS. HE CLIMBED DOWN A LADDER TO CLINKER BELT WALKWAY, HE COULD SEE THE EE W/HIS RT.ARM OVER THE TOP HANDRAIL, SLUMPED OVER, HEAD DOWN.CALLED HEL

EE WAS ASKED TO SHOVEL CLINKER OUT OF KILN BASEMENT AND PUT SLINKER IN WHEELBARROW. EE REPORTED THAT HE FELT A PAIN IN HIS LOWER BACK WHEN THE WHEELBARROW WAS ABOUT HALF FULL. HE SAID HE DID N OT TWIST OR DO ANYTHING THAT HE KNOWS OF TO CAUSE THE INJURY.
EE WAS RAISING THE WASH UNIT UP & OUT OF A TANKER TRUCK. THE CHAIN HOIST CAUGHT HIS GLOVE, PULLING HIS HAND INTO THE CHAIN. HE IMMEDIATELY REVERSED THE CHAIN DIRECTION TO RELEASE HIS HAND. HE RECEIVED A LACERATION TO HIS RIGHT HAND BETWEEN HIS THUMB AND INDEX FINGER. LACERATION REQUIRED 3 STITCHES TO CLOSE.
EE WAS OPENING THE EAST SIDE PLATE ON THE FK HOPPER. WHEN THE DOOR CAME OFF, CEMENT DUSTED OUT, GETTING INTO HIS LEFT EYE. HIS EYE WAS SCRATCHED.
EE WAS ATTEMPTING TO OPEN A VALVE ON THE SLURRY TANK. HE PUT HIS WEIGHT ON THE WRENCH. HE SLIPPED AND HIS ARM GOT CAUGHT ON ANOTHER VALVE, PULLING HIS ARM BEHIND HIS BACK. HIS ARM WAS BROKEN.

WHILE EMPLOYEE WAS INSTALLING AN ELECTRICAL TRANSFORMER HIS FOOT GOT CAUGHT BETWEEN THE TROLLEY STAGE AND ELECTRICAL BOX. EMPLOYEE HAD ASKED THAT THE TROLLEY BE MOVED SO HE COULD CHECK AMPS WH ILE THE TROLLEY WAS IN OPERATION SO HE COULD CHECK THE LOAD HE HAD ON THE TRANSFORMER. HIS FOOT WAS BROKEN IN TWO DIFFERENT PLACES.
EMPLOYEE WAS LIFTING TRUCK WASHER WITH AIR HOIST WHEN HIS LEFT HAND GOT CAUGHT BETWEEN THE WASHER AND THE FALL PROTECTION CAGE. HIS HAND WAS ON THE TRUCK WASHER AND HIS LEFT MIDDLE FINGER WAS INJURED. HE WAS TAKEN TO THE DOCTOR WHERE HIS FINGER RECEIVED 14 STITCHES.
EE AND ANOTHER EE WERE ON LADDERS REMOVING A LANCE FROM A KILN BURNER PIPE, USING PORTABLE SADDLE. THE LANCE SLID OUT OF THE SADDLE, KNOCKING EE OFF THE LADDER AND BOUNCING ON HIS RIGHT LEG. I T CAUSED A BRUISE TO HIS RIGHT ANKLE. THE LANCE WEIGHED LEG. IT CAUSED A BRUISE TO HIS RIGHT ANKLE. THE LANCE WEIGHTED APPROX. 200 LB HE WENT TO THE DR AND DIAGNOSED WITH CONTUSION TO R ANKLE

THERE WAS A LEAK ON THE CLINKER CHUTE. EE HAD TO REMOVE SILO DOOR TO SHOVEL THE CLINKER IN. THE SPILLED CLINKER CAUSED POOR FOOTING. EMPLOYEE WAS PICKING THE DOOR UP OFF OF SILO A WHEN HE SLIP PED, CAUSING INJURY TO HIS LOWER BACK. HE WAS TAKEN TO THE DOCTOR WHERE HE WAS DIAGNOSED WITH A LUMBAR STRAIN.
EMPLOYEE WAS APPLYING HIS WEIGHT TO A BAR TO CLOSE A RAILCAR DOOR. THE BAR SLIPPED AND FELL TO THE GROUND. EMPLOYEE'S WEIGHT WAS STILL ON THE BAR. THE BAR FELL ON THE MIDDLE FINGER OF HIS RIGH T HAND. HIS FINGER WAS MASHED, REQUIRING STITCHES AND THE CUTTING OFF AND REPLACING OF HIS NAIL.THE TIP OF HIS FINGER WAS FRACTURED.

EMPLOYEE WAS STRIPPING WAX FROM THE FLOOR. HE POURED STRIPPER FROM CONTAINER ONTO FLOOR, BUFFED FLOOR, MOPPED THE RESIDUE. HE STARTED TO FEEL LIGHT HEADED AND DECIDED TO TAKE A BREAK. HE FELT NAUSEOUS, EXPERIENCED DIARRHEA, VOMITING, AND SWEATING.
