Struck against stationary object

Injured was reinstalling Raymond mill panels using a 2-ton overhead chain hoist. The chain hoist trolley over-traveled the end of the beam and fell, striking injured on side of head and shoulder, causing EE to strike EE's face on the mill structure.

Injured was moving a 55 gallon drum in the warehouse when the injured felt pain in knee. Incident occurred on 2/16, but not reported until 2/18 when medical attention was requested.

Injured was shoveling material out of a hopper when EE felt pain in their wrist.

Over-exertion in pulling or pushing objects

Injured reports that EE was dragging a 4"" corrugated vacuum hose when EE felt pain in EE's back. Injured did not report injury until 7/10/2024 when EE requested medical attention. Injured was given work restrictions on 7/10 which made the injury reportable.

Injured was attempting to close the rear engine door on skid steer when they smashed a finger in the door, causing a fracture to the tip of the finger.

Injured was assisting a barge movement when a rope became pinched in a barge cleat. Injured was attempting to remove the rope from the cleat when it became taught, causing a sprained wrist.

Cleaning mud off the backstop of a conveyor with a sledge hammer and sprained left wrist.

Over-exertion in pulling or pushing objects

Pulling suction hose out of barge and felt something pop in left wrist. Employee did not feel like EE needed treatment at the time. EE's wrist continued to bother EE so EE went in for initial diagnostic procedures on 6/29/2021. Employee was allowed to return to work with no restrictions. After an MRI employee was put on restricted duty as of 7/26/2021.

Employee was climbing in the loader on a snowy day and slipped on the ladder rung and fell to the ground. Employee sustained a fracture of the right hip.

Employee was trying to clear air lines that were plugged with material. EE was pulling on a hose to remove it when EE lost EE's footing and fell. When EE fell EE's left knee struck pipes that were on the ground. EE's fall resulted in a contusion on EE's shin and a possible meniscus tear in EE's left knee.

Employee suffered a strain to EE's left wrist while using a wrecking bar to check conveyor bearings and to remove built up material. The strain was caused by the repetitive twisting motion.

Employee fell down when he slipped on the ice/snow in the parking lot when he was getting out of his car to report to work. He sustained a contusion and sprain to his lower back.

Employee was done using a hand-held grinder. As he was switching hands to put it down, his index finger on his left hand was drawn in between the guard and the blade of the grinder which had not completely shut down. This caused a laceration to his finger which required stitches.

Worker's right hand was caught in a running machine which resulted in the loss of the tips of his 3 middle fingers and damage to his thumb. Worker did not follow proper lock out tag out procedure to ensure that the machine was locked out.

Employee's lower left leg was struck by a cable while moving a barge. Was first aid treatment until employee went to the doctor on 6/14. Injury is a bruise which was not drained but he was given work restrictions.

Employee was lifting port liners and a day or two later he noticed that he had a hernia. This was reported to management on 1/30/2012. Employee had been away on vacation for two weeks. Employee went to doctor on 1/30 and was diagnosed with a left direct inguinal hernia which will require surgery.

Employee climbed an extension ladder to tie it off and neglected to lock the top section of the ladder. The ladder slid down and the employee jumped off spraining his right ankle and foot.

Employee was using a rod to loosen material in a hopper. When the material broke free his arm came down and struck a metal plate that caused a laceration on his right forearm. Employee required 4 stitches to close the wound.

This injury was reported to us on 5/29/2008. Employee was repairing a pipe. When he was leaving, his right foot got caught between two pipes and he injured his right knee.

EE was installing a top to a journal when a finger on his left hand slipped under the bottom lip. His finger was cut and required stiches.

EE was using a skid steer to clean up snow in our parking lot when he struck what was left of a recently sawed off pipe with the bucket. When he hit the pipe he was thrown forward and struck the windshield with his hard hat. The main injury that he sustained was a cracked rib.

Employee was carrying a step ladder outside when he slipped on a snow covered rock and sprained his left ankle. An air cast was put on his ankle at the doctor's office.

Possible carpal tunnel. Employee is reporting that he has cramping in both wrists and hands. He feels that it is from repetitive use (shoveling, etc.) over a number of years. Even though he reported this on 11/1 he was just put on restrictions on 12/4/2006. Prior to this date there was no medical treatment involved.

Employee was running the skid steer inside the barge when he backed into the wall of the barge. He sustained injuries to his head, neck & shoulder when he struck the lap bar or the door bar.

When working at the dock ee felt a sharp pain in his left shoulder. It is suspected that he has a torn rotator cuff and he is on restricted duty.

Employee was standing on the tires of a skid steer to remove the hooks from the pick points. His right foot slipped off of the tire and he fell and struck his right elbow on the frame of the the skid steer. He sustained a laceration on his right elbow that required 4 stiches.

This was a flare up of an old inury to employee's left elbow. The elbow swelled up and was locking. This report was filed late because it was initially thought that this would be a continuation of his previous injury. Our work comp provider has now told us that this is not the case.

Employee was carrying samples down from a platform when he tripped on the stairs and sustained a puncture wound to his right lower leg. He did not require medical treatment at that time. On 3/28/05 he noticed that the wound looked like it was infected. He went to the clininc and was given a prescription for antibotics.
