Employee was walking along side of tailings line measuring the height of the line. As they stepped along some brush they stepped in a hole they did not see and felt pain in their ankle. They went to the doctor and were diagnosed with a sprain and were placed on restrictions.

Employee ran into parked railcars while operating a trackmobile. EE was thrown from the seat which resulted in a fractured nose and fractured sternum. Contributing factors to the incident were inattention and the employee failed to wear the seatbelt.
2-5:Employee slipped on ice on the way to coop railcars. EE returned to normal duties after reporting the incident. Employee sought treatment on their own without notifying the Co. 4-16:Employee informed the CO. that they saw the chiro & the strain they are currently feeling in shoulder/back was due to the fall in Feb. Employee is starting physical therapy 4-25.

Employee picked up a water jug approx. quarter full and felt a strain in back. Employee went to employee's chiropractor and was placed on modified duty.
Employee was walking towards the de-watering pumps and knee buckled. EE felt pain later in the evening and went to the clinic the next day. EE was placed on modified duty for a knee strain.
Employee went to the clinic due to shoulder irritation. EE returned to work on modified duty, the doctor thinks the irritation may have been caused by an incident that occurred in 2011.

Employee was grinding with hand held grinder wearing safety glasses and face shield. The next day employee's eye felt irritated employee went to the clinic and they removed a small piece of metal, employee received ointment prescription for eye. Employee did not miss any time.
Employee was in a haul truck and hit a rut in the road, felt a strain in neck. Employee was referred to the clinic and placed on modified duty.

THIS IS NOT A UNIMIN EMPLOYEE. This report is being filed on behave of All Pro Door Company (507)546-3667. While loading equipment on their trailer, an All Pro Door EE was pushing a scissors lift forward on the trailer when his finger got pinched between the lift & trailer. He received a 3/8"" cut on his pinky finger. He received 4 stitches. AGAIN THIS IS NOT A UNIMIN EE.
Employee tried to clean shavings off of a drill bit that was in a running drill press. The drill bit then got tangled with his glove and wrapped his left hand up in the drill bit thus, dislocating his left index finger.
Employee was tightening a nut with a wrench on a screen the wrench slipped off the nut causing the employee to project forward and hit his head on the frame of the screen causing a small cut on his forehead. He was taken to the clinic and received stitches.

Employee fell while climbing down from the catwalk which resulted in a compression fracture.
Employee complained about shoulder strain. Employee went to the doctor and has to attend physical therapy.
Employee was climbing onto the Shuttle wagon, as he reached the top platform area his foot slipped and he struck his left shin on the steel edge causing a laceration which resulted in stitches.
Employee was tightening a chain and was unable to turn the ratchet with his hands and instead of trying a different ratchet he attempted to use his foot to gain power as he did this he felt a strain in his back. Employee did not report incident until the next day and he then received chiropractic treatment.
Employee was driving a lull and turned a corner too sharp tipping it onto its side, employee jumped out of the cab as it was tipping which resulted in a laceration to his head and a dislocated shoulder.
Employee lifted a bucket over his head to hand to another employee and he felt a strain in his back. Employee received chiropractic treatments which included diathermy treatment.
Employee picked up a worn belt conveyor roller off of the quarry floor and the sharp edge cut through his glove cutting his thumb which resulted in stitches.
Employee used his hand to prop up the cutting edge bolt while removing the nuts and cut his finger which resulted in stitches.
Employee was using a grinder to cut a 12"" piece of angled steel, when the angle started to move he removed his left hand from the handle of the grinder and proceeded to use his hand to hold the angle in place instead of a vise or clamp. At this point the grinder got bound up causing the grinder to jump and hit the employee on his left hand/thumb area leaving a laceration.
EE was welding and a piece of metal got into his eye.
Employee was removing a chain that was stuck on the mast of a lull. Employee pulled the chain out from between the mast and backstop and the mast swung back to the backstop catching employees fingers in between which resulted in 2 fractured fingers.

EE was separating girts and pulled one girt off of the top of the pile and slid it forward onto the forks of the fork lift. The girt bounced and hit the top of the EE's left forearm causing the laceration.
Employee strained his back on 8/15/2012. Employee had an MRI done 11/9/2012 and had a follow up appointment on 1/29/2013 which he returned with lifting restrictions and was placed on modified duty.
On 4/1/12 employee twisted his knee while climbing down the ladder on the front end loader. On 7/16/12 an MRI was positive showing a meniscus tear.
Employee complained of pain in elbows, forearms, and hands but was unable to pinpoint a specific incident. Dr. diagnosis is tendonitis. Employee placed on modified duty with lifting, gripping, squeezing, and push/pull restrictions.
On 2/13/12 employee slipped and twisted left knee. On 5/3/12 an MRI was positive showing a meniscus tear.

Attempting to swat a wasp the employee broke through a window cutting his right wrist requiring 6 stitches. Doctor assessment was employee was fit to return to pre-injury, regular work. No lost time.

Employee was pressure washing 777 haul truck in preparation for scheduled maintenance. Water stream swung across employee's right foot, cutting through the leather and allowing high pressure hot water under steel toe. This resulted in the employee sustaining 2nd degree burn on right big toe.
Empoyee was servicing mobile equipment, while on his back he was using a ratchet and felt strain in his right shoulder. Employee was seen by his doctor 9/16/08 and placed on modified duty.

EE AND A CO-WORKER SET A PLATE OF STEEL AGAISNT A SAW HORSE ON A FLAT BED OF THE MAINTENANCE TRUCK. STEEL BEGAN TO SLIDE. WHEN EE TRIED TO STOP IT, HIS FINGER GOT SMASHED BETWEEN THE STEEL AND FLATBED OF THE PICK-UP. REQUIRED 2 STITCHES/ MODIFIED DUTY.
WHILE MOVING A ROCK OFF THE HAUL ROAD, HE STEPPED ON THE BERM, SLIPPED AND FELL, FRACTURING HIS RIGHT ANKLE.
WHILE WALKING TO HAUL TRUCK HE SLIPPED ON PATCH OF ICE, FELL WITH HIS LUNCH BOX ON HIS RIGHT SIDE. THIS RESULTED IN A HAIRLINE FRACTURE TO THE RIB.
