Employee was loading conveyor onto the transport truck grabbing the railing when EE's hand slipped falling straight backwards onto EE's left side.

Employee was placing angle iron onto a conveyor when EE caught finger between the metal. EE received a laceration to the tip of the finger that required two stitches.

Employee was cutting conveyor belt rubber with a utility knife and the knife slipped cutting EE's thigh.

EE was on the dozer and closed the door of the master switch and grabbed the door handle and EE's hand slipped off the door handle and EE fell. As EE fell EE's right leg hit the track of the dozer.

Operating loader and hit a pot hole bruising employee's sciatic nerve.

Employee was placing a chain around a rock that was stuck in a Jaw crusher. The rock shifted and pinched his hand between the rock and the sidewall of the crusher. He fractured a bone on his left hand.

Handing tools to his partner from the hopper of the conveyor while working on belt. EE turned around and fell(morning dew made the belt slippery) catching his left arm on the hook of a bungee cord.

Tearing down crusher wrapping cords, taking out the plywood for the ramp, deblocking, move conveyor and picking up. Did have safety glasses on.

Cutting off the male end of an extension cord, had extension cord on the table with his hand below the table. The utility knife slipped and caught his left thumb just above the bottom knuckle.

Climbing out of loader stepped off last step of ladder and twisted knee.

NEW OPERATING SITE - MOVING IN & SETTING UP CRUSHER. EE SLIPPED ON CLAY ON STEP WHILE WALKING OUT OF GEN SET OF CRUSHER. HE CAUGHT HIMSELF BEFORE FALLING - BACK STRAIN.

Laceration to R thigh with utility knife while cutting crusher conveyor belt.

EE WAS CLIMBING DOWN FROM CHECKING NEW BOLTS ON CRUSHER, RUNGS OF LADDER ERE COVERED WITH ROCK DUST. EE LOST FOOTING AND FELL LANDING ON BACK.
