Employee (EE) and Foreman were cleaning the surge pile hopper to inspect the wear plates, and prepare for winter shutdown. EE placed ~55"" long square head shovel past clamshell gate opening, and used left foot to assist in pushing material onto the conveyor belt below. The clamshell gate unexpectedly closed, pinching the EE's left foot and fracturing left great toe.
Non-mining related airplane with 3 people on board crashed into dredge lake 5000 feet away from mining activities. Complete destruction of air plane and 3 fatalities who were pilot and passengers. Impairment impacted production due to the search and recovery of plane and passengers. Airplane passenger was killed as a result of the plane crash.

Removing steel throat bushing from Slurry pump housing. Hung up with 1/4"" to go. Used left hand to push the top to free it. Came lose tipped back on the chain hoist supporting it and pinned left elbow on pump housing, simultaneously bending left hand back so fingers touched the top forearm. Diagnosis: Closed acute non-displaced intra-articular radial styloid fracture.
Employee parked haul truck, and was dismounting at the end of shift. Employee missed the bottom rung with right foot, and landed on the ground buckling right knee resulting in a strained right calf.
While installing skirting on the tail section of a radial stacker, the employee felt a strain to his back. Medical attention was sought by the EE on 11/9/2014. On 11/10/2014, the EE informed mine management of medical attention provided over the weekend on 11/9/2014. Injury accepted 11/10/2014.

Employee sliding wear plate into place with a coworker. In the process, employee turned ankle causing a sprain.
Employee used utility knife to open 5 Gal oil container. While opening, employee cut towards himself with knife. The bucket slipped in the process and he cut his hand. Employee received stitches and returned to work.
Removing bearing from conveyor and a small fragment of metal stuck in finger of repair person.
While repairing springs on hopper gate, it wouldn't shut, so he lifted it and it slammed shut on his right hand index finger.
Employee was closing a garage door and caught his finger in a pinch point. The tip of the left middle finger was cut off.
EE WAS LOWERING COUNTERWEIGHTS ON LONG BELTS. THERE WAS STORED ENERGY IN BELT CAUSING HOIST LEVER TO SWING HITTING EE ON LEFT ARM.
WHILE LIFTING A SECTION OF PIT CONVEYER WITH CRANE SECTION CAME LOOSE FROM CRANE AND HIT EE TOO QUIKLY IN HEAD. HE WAS WEARING HIS HARD HAT.
LOADING 55 GAL BARRLES INTO LOADER BUCKET. BARREL TIPPED QUICKLY PINCHING FINGER BETWEEN BARREL AND BACK OF BUCKET.
