Employee (EE) and Foreman were cleaning the surge pile hopper to inspect the wear plates, and prepare for winter shutdown. EE placed ~55"" long square head shovel past clamshell gate opening, and used left foot to assist in pushing material onto the conveyor belt below. The clamshell gate unexpectedly closed, pinching the EE's left foot and fracturing left great toe.

Removing steel throat bushing from Slurry pump housing. Hung up with 1/4"" to go. Used left hand to push the top to free it. Came lose tipped back on the chain hoist supporting it and pinned left elbow on pump housing, simultaneously bending left hand back so fingers touched the top forearm. Diagnosis: Closed acute non-displaced intra-articular radial styloid fracture.

Employee parked haul truck, and was dismounting at the end of shift. Employee missed the bottom rung with right foot, and landed on the ground buckling right knee resulting in a strained right calf.
