Over-exertion in pulling or pushing objects

EE was installing idler. Pushed on idler to put bolt in to hold idler from moving EE felt a pop in their right wrist. Medical treatment sought. Work restrictions issued due to sprain.

Struck by falling object

EE was moving an idler for conveyor. One end was in the air and fell on EE left foot missing the steel toe of EE's boot by an inch. Idler is 82"" long by 6"" wide.

Over-exertion in lifting objects

EEs were in the process of removing old belting. Roughly 30lbs of belting per piece. EE lifted the old belting up with assistance from another EE onto a cart to take out of tunnel. When they bent over to pick up another piece they felt a sharp pain across their lower back.

EE slipped down an icy stair way and landed on their arm, resulting in a dislocated shoulder.

EE was on the 5th floor of the mill washing when a rock (4"" + size) came down from the 6th floor striking EE on the Left Elbow

ee was removing idlers from conveyor for a belt change. when they went to move the idler to the floor, their finger got pinched/rolled in between the cans causing a laceration / split to right pinky finger.

ee was throwing scrap steel into the skip when EE reaggravated a strain in EE's shoulder that occurred the day before.

employee was pushing a welding cart across the shop. one of the wheels fell into a low spot/crack in the concrete causing the cart to tilt. Employee reactively reached out with EE's arm to try to catch it in case it fell and felt a strain in EE's shoulder. cart is appx 100 lbs.

EE was pulling the fuel hose out of the fuel truck, as EE was pulling the hose out of the fuel truck, EE was walking backwards pulling the hose and EE tripped on a couple of large rocks causing EE to fall and twist EE's ankle.

EE was trying to lift new bearing housing into place on take up. EE said EE was in an awkward position and said EE hurt EE's left wrist.

ee was spotting a haul truck, and as the ee was walking backwards, tripped over some loose stone/uneven ground and fell resulting in lacerations to hands and legs.

Over-exertion in lifting objects

Employees had a shaft in the upright position in the elevator. when they arrived their floor, one ee held the top while the other bent down to pick up the bottom end. Upon lifting, the ee felt a pop sensation in the groin area. The shaft weighed appx 100 lbs according to ee's. EE sought medical treatment on 3/6 and was put on work restrictions.

EE was lifting trays of stone for sampling when they felt a strain in their shoulder. Incident was reported on 12/10. EE sought medical treatment on 12/18 and was issued work restrictions and continued medical treatment.

EE was lifting a piece of plate steel into place with left hand - felt pop in bicep upon lifting. Suspected rupture of tendon; currently under medical evaluation.

Accident type, without injuries

Roll crusher stalled, but drive belt kept running. Friction caused it to start on fire then ignite grease that lubricates the bearings. Response procedures executed at 5:57 when fire was discovered (contact fire dept & evacuation). Extinguished at 7:18. No entrapment or injuries. Minor damage to equipment. Inspection underway.

Accident type, without injuries

contracted stripping crew reported a chip trailer had started on fire likely due to sparks from chipper igniting the chips. Operations were immediately shut down, emergency response executed and plant water trucks / local fire dept were called to assist. Fire was extinguished, all clear was given at appx 7:45 pm. NO injuries. Fire did not spread beyond top of trailer.

EE was heading to the top of the mill in the elevator to check an alarm. Upon reaching the 12th floor EE got out of the elevator and went to close the elevator door. EE was pulling the Strap on the outside of the door and EE's hand got crushed between the doors. Work restrictions issued after medical evaluation.

EE bent down to remove toilet & felt a strain in back - reported at time of incident, no treatment sought at the time. EE was treated on 8/12/24 in ER due to ongoing/increased pain. Was put off work - became reportable at that time.

Contact with hot objects or substances

EE was welding/torching when piece of hot slag/steel fell into tongue of boot and burned through boot, sock and onto skin. First Aid applied - ee sought medical treatment on 8/7 due to increasing pain. wound was infected resulting in prescribed medication and time off work.

Employee was attempting to remove an oil drain plug, and when it broke free, employee smashed pinky finger on a pipe

Over-exertion in lifting objects

EE lifted a 2 x 12 plank to move it out of the way - as EE lifted, EE heard a ""pop"" in left bicep followed by shooting pain down to fingers on left hand. EE did a report only at the time, but sought treatment a few days later and has been put on work restrictions with further medical evaluation.

EE was welding - grabbed wrong shield - received arc flash/burn to eyes

EE reported discomfort in ankle - no specific event or incident contributed to the onset of pain. Following day, EE was experiencing increased swelling/pain and wasn't feeling well. Went to ER - admitted for severe bacterial infection in ankle assumed to be attributed to something in the water at work. EE was working in wet area at time of report. Still under investigation.

Employees finger was pinched between screen deck media & frame resulting in laceration and fractured finger tip.

EE stepped off a platform (appx 12"" high) onto a rock (4-5"" diameter) which caused an ankle sprain. Lost time/work restrictions issued.

Fall from machine

Employee was working in the mine companies shop in good, dry conditions. Employee was repairing a Motor Grader and was working on the turntable, about 2 feet off the ground. When they went to get down off the turntable they missed the step and fell 2 feet to the ground, injuring their left hip.

EE knelt down on concrete to check a heater and felt sharp pain in knee cap. Reported at time of incident, but did not seek medical treatment until 11/29/23.

ee was going in for break - when EE exited the cab of the haul truck and walked across the catwalk to access the ladder, EE stepped on a large stone that was in the catwalk resulting in a sharp pain in EE's left foot. EE did not report the incident until 11/20/2023.

ee was climbing the access ladder to CAT haul truck. Gloved hand slipped causing ee to lose balance and fall to the ground. ee was on bottom rung of ladder. incident was reported on 9/29. EE sought initial medical treatment on 12/11 - evaluated on and work restrictions issued on 12/18.

Ee was working in high heat/humid conditions and experienced signs of heat exhaustion/dehydration - was treated and prescribed rest/off work for 2 days.

EE was using a utility knife to cut down boxes for recycling - thicker box required more force. Knife slipped at overlap point resulting in cut to abdomen. Cut required stitches and lifting restrictions.

EE was shoveling stone (12/8) and power washing (12/12) - activities resulted in aggravation of a previous (non-work) injury causing strain/discomfort to forearm. Work restrictions given.

Employee tripped on piece of floor plate/grate, and fell resulting in bruising to right ribs/shoulder. Placed on light duty work restrictions.

Steel plate slid out of one of the plate clamps then slid out of the other. The plate then struck EE's hand pushing it into a metal saw horse causing damage to two fingers on EE's right hand. EE was sent via ambulance to ER for treatment - xrays determined both fingers were fractured.

Employee changed head/body position resulting in sensor on welding hood becoming blocked by a beam causing the shield to clear while employee was welding resulting in arc/welders flash.

Employee was getting tools from toolbox. When turning to go back towards workstation, felt a ""pop"" in right knee. Entire body was not facing direction of travel causing knee to twist.

Injured was operating dozer when EE backed over windrow which jarred EE's back. Injured requested medical treatment on 7/13 and was given work restrictions.

Employee was fixing a leaking pipe near the gate valves. On the way out employee struck head going out of the tunnel into the pump house where the clearance is low. Requested medical attention on 06/29 when condition didn't improve.

Mobile Mechanic was walking toward the passenger side of a pickup that was there to pick up the Loader Operator at the end of the shift. As EE was walking to the passenger side window, the truck moved forward. The front wheels were turned to the right so the truck rolled forward and onto the Mobile Mechanic's foot, which pinned it and knocked EE to the ground.

Team of millwrights was putting flights back on the apron feeder. As the millwrights were attempting to slide the flight, it suddenly moved and EE's finger was pinched between two flights.

Injured reported on 05/06 that EE began experiencing back pain while operating loader on 05/05. Injury was determined to be compensable by WC carrier on 05/26.

As employee was performing maintenance checks on primary surge stacker, the boom of the stacker collapsed and fell 20'. The employee was standing near the head pulley and suffered a broken leg and vertebra in the back.

Employee was attempting to use a pipe wrench to roll a conveyor backward in order to dislodge some oversize that was wedged between the feed chute and belt. The employee slipped, resulting in a fractured ankle.

Employee was positioning a wear plate in a chute. The wear plate shifted and employee's right index finger was caught between wear plate and chute, resulting in laceration and fracture.

Employee was handling a recently removed trougher roller to remove it from the area. As they began to tip it on to its side, the can rotated and a finger was pulled in between the cans resulting in a fracture to the left index finger.

Employee was tying up vessel to be loaded, when vessel hoist operator fed too much cable to employee causing cable to slip into water and employee to be jerked resulting in strain to back.

Employee was picking up spilled stone out of walkway along the base of small HP800 crusher in mill. A stone approximately 6"" bounced out of the feeder and down to the walkway and hitting / lacerating the employee's right elbow.

Employee was making an initial pass with the horizontal boring machine to smooth the inner surface of a poured lead Babbitt. Employee tried to pull excess gasket material from mold when the cutting edge of the bore caught left ring finger, just above the knuckle causing severe tissue trauma.

Employee was using loader to load blasted material into haul trucks. After leaving for the day, EE called 5 hours later to report soreness to lower back. EE was taken to doctor the next day and diagnosed with bruised tail bone. Restriction applied prevent normal duties.

After manually clearing stone from a blocked screen, employee felt a pain in the back of the leg. After another shift with no improvement, employee went to doctor and was given physical therapy.

Employee strained elbow while opening elevator door in mill. Saw doctor day of incident. Treatment at that time was not reportable. Employee still experienced pain in elbow. Returned to doctor on 6/5/17 and was prescribe therapy and light duty for elbow.

Employee was operating a dozer on stockpile, pushing material to load a boat. Toward the end of the shift, employee reported that their back was hurting. Employee took Saturday off and went to the doctor on Monday.

Employee was attempting to retrieve a dropped tape measure with a magnet on a rope. Employee put rope in mouth to hold onto it. EE dropped the magnet and the rope jerked two teeth loose. Employee went to the dentist and teeth were reset with no permanent damage expected.

Employee was carrying a leaf spring for the P135 pick up to the front of the pit in the automotive bay of the shop. Employee went to set the leaf spring underneath the P135 pick up when EE felt a pain and burning sensation in left shoulder. Leaf spring weighed approximately 50 - 60 lbs.

Fall from ladders

Employee was washing in mill basement getting area clean for final shutdown. EE noticed an above ground flume was plugged with stone causing water and material to drop on floor. The employee got a 8ft step ladder and climbed up to 3rd step to put wash hose into clean-out. The pressure on the hose caused EE to lose balance and EE slipped off ladder.

The employee was burning a bearing off a return roller when they stopped to clean the tip. The employee turned off the acetylene to the torch and used their left hand to wipe off the tip. At the time employee was wiping the tip, they triggered the oxygen to blow out any debris. At which time glove ignited causing burns to left palm and thumb.

Employee was exiting vehicle when EE slipped on the vehicle's running board and fell to the ground. EE's right leg was caught in the cab of the truck and as EE fell EE felt a pop in knee.

Lab Tech was analyzing stone sizing in the lab, which is a repetitive motion, when she felt soreness in her right shoulder. She reported her soreness on the DOI but did not receive treatment until 7/7/15 which includes modified duty.

EE was reaching over the return belt next to the tail pulley and lifting an idler out of its channel when he felt a pain in his back. Received medical treatment and was given work restrictions 7/20/15.

Low-hazard tailing impoundment cell gave way and released water and sediment into adjacent unoccupied and wooded area. High water level in cell #2 and high winds were contributing factors. A tailings ""delta"" of sediment was created in area of release and some trees/brush in the area were impacted. There were no injuries associated with this impoundment release.

Dozer operator was cleaing around tunnel on large stone, and also later loaded a boat from the same pile. This required alot of backing and postioning of the dozer and the dozer does not provide a ""smooth"" ride. Employee felt lower back pain when he got home that day.

Employee was cutting a box when the knife slipped out of his hand. He reached for the knife handle but his hand went past the handle and came into contact with the knife blade cutting his finger. Employee said he had blasting agent on his hand which made his hand slippery he did not have gloves on but the agent would have made the gloves also slippery.

NOTE: This case was initially reported as a ""first aid"" case. Only weeks later was there an indication that employee asserted a work-related hernia. Description: Employee was attempting to open water valve. The valve was described as difficult to open and requiring exertion to open. Employee asserts he felt a small amount of pain in his groin area when opening.

Employee (mill tech) was replacing drip pans under a conveyor. He was starting to lift a section of pan (about 185 lbs) with a co-worker, when he felt a sharp pain in his right shoulder. A mechanical lifting device had not been used. Medical clinic placed him on lifting restrictions with 2-week follow-up. Employee returned to work with restrictions; no days lost.

Employee was assisting in replacement of sump motor. Banged elbow against fixed object. Medical attention was sought 11/20/2012.

Over-exertion in lifting objects

Employee reported pain to lower abdomen after holding and then dropping to the floor a ladle carrying babbitt material. Doctor's 1st Aid. He had 4 scheduled days off (8/3-8/6) following the accident. He returned and complained of pain (8/7). We sent him back to the doctor who then gave him 2 days off (8/8-8/9). Return to work with restrictions.

Employee was chipping away welding slag when he felt something in his eye. He kept on working and reported the next day that his eye was bothering him. He was taken to the doctor's office and an anti-biotic drop was placed in his eye. He was wearing safety glasses with side shields and hard hat.

Employee had pulled a conveyor belt up and over a tail pulley using a lever action chain hoist. Chain was attached to the belt with a belt clamp. Injured employee was in process of securing the belt to a tail pulley using a ""C"" clamp to hold belt in place and then remove belt clamp for splice. The belt clamp slipped and employees finger was pinched between belt and drip pan.

Employee while wearing his hard hat ducked under a flume. He stood up too soon and bumped his head on the flume causing neck strain.

There was no report of an accident or incident. The employee informed his supervisor on 03/27/2012 that he wanted a days vacation to see a doctor because his shoulder was bothering him.

Excentric being moved on a wagon. An ear of the excentric struck a stationary wagon. The stationary wagon struck employees lower leg. Employees leg was momentarily caught between the wagon and a fixed object.

800# side plate to #2 grizzly was temporarily placed and leaning against a staircase. Employee was in process of removing 2nd plate when 1st plate slid down and on top of right foot.

Inside a large personnel/freight elevator inside the Mill building. Employee was closing overhead elevator doors and left hand was caught between them.

While lifting screen cloth up and over pad eye employee felt strain in his lower left abdomen. He did not go to or request to see a doctor at this time. He worked his routine job without restrictions or lost time up to and including January 5, 2012. On the 6th he went in for surgery.

While climbing stairs on 8' platform to loading shuttle, grating failed, employee able to catch himself, so he did not fall. He did not request medical attention at this time. On January 11, 2012 he went to his doctor who gave him an injection into his shoulder and he was sent home by physician.

Struck by falling object

After cutting the end of a bolt off with a torch, the employee was hammering out the bolt, when a fragment fell into his eye. He was wearing safety glasses, the wrap around type (shide shields). The bolt was overhead.

Employee was lifting a 20 lb steel access door to the chute and felt a sharp pain in the lower back -Injury has been working with restrictions; as of 12/6/09 we are unable to accommodate his restrictions so he is now off work due to the injury.

The machinist was tightening the bolt on the dozer part that was in the boring machine. He was using a pin wrench to turn the bolt with his right arm when he felt pain and a burning sensation near the shoulder joint. Employee was not restricted and did not miss work until today, 12/2/2009, when he was scheduled for surgery.

The repairman was installing channels on L-3 conveyor when one of the channels fell and struck his left foot. It was determined on 07/31/09 that a bone in his left foot was factured.

Employee was lifting a lever that operated a flop gate to change the flow of stone from conveyor C-2 to RC-4. It got stuck and he had to lift the lever twice. During the lifting of the lever the second time, he felt something pop in the middle of his back.

While installing an idler on N-5 conveyor, the idler on it's side. While trying to square the idler with the conveyor before standing it up, one side slipped off the channel and caught the victums tip of 3rd finger on right hand.

A fire started in the idler repair shop around 6:45 p.m. after the employees had gone home for the day. No injuries occurred. The fire destroyed most of the building and burned for more than 30 minutes making it immediately reportable to MSHA. No obvious ignition source was found.

Two millwrights were removing a gear from the shaft of a brake. The gear was heated up and one millwright held a B&O hammer next to the gear while the injured employee was hitting the B&O hammer with a 12 pound sledge hammer. As the injured swung the sledge, he felt pain in is lower right groin area, causing a double hernia.

The repairman was working on L-8 spray bar pan and installing a rubber splash guard. He saw a doctor who removed a foreign piece of material from his eye. On 08/21/08, he developed a rust ring in his eye and returned to the doctor for treatment. The doctor also gave him a prescription eye medication.

Struck against stationary object

The millwright was pulling a screen cloth out of #301 screen when he struck his right elbow on #302 screen. On 10/19/07, the injured received surgery to repair a nerve in his elbow.

The boat loader was in the south dock final screening station washing spillage on the first floor with a high pressure water hose. As he shut the water hose valve off, the hose near the valve ruptured making it impossible to control the water or to hold onto the hose. The hose struck his feet causing him to fall into a steel support horse.

The conveyor and screen attendant were sent to the P-drive building to clean dirty floors. As he was by P-7A head pulley area, moving water hoses so he could do his job, his foot got caught in the hose. Suddenly, as he stepped down on the hose, his right ankle rolled to the side. The incident resulted in a fractured ankle.

The mechanic leader was removing the 40 pound hydraulic pump from the bottom storage shelf in the shop building. The injured slid the pump towards him with his left hand. Suddenly the pump rolled off the shelf and landed on his right hand thumb. His thumb was pinched and the doctor had to remove his thumb nail.

The repairman was rebuilding a chute. He had to climb down the 54 inch chute to the bottom to take a measurement. While standing on the chute taking measurements, he started to slide forward because of dirty material lying on the chute steel. His left foot toe caught the side of the chute causing his foot to twist outward away from its normal position.

The repairman was cutting steel with a torch when the steel plate fell off one end of the stand and the other end of the plate raised striking him in the head. The injured received a small cut on the side of his head requiring 2 sutures to repair.

EE arrived on site early. She was seated @ the desk in the guard house. She and another EE were trying to help an EE of one of the ships with her luggage. Injured EE bent forward in her chair and was trying to lift a piece of luggage from a sitting position. She threw her back out when she tried to stand up with the luggage, from a sitting position.

Fall from machine

The millwright was kneeling on a conveyor belt that was laying on the idler deck, as he repaired the belt. The idler deck was elevated 36 inches above the concrete floor. As the millwright tried to climb off the belt, he caught his foot and jacket on an idler. He then lost his balance which caused him to fall off the conveyor and land on the floor.
