EE fell 10ft 10in while leaning out a 11-1/2 in opening between a screen and adjacent handrail trying to look at the screen springs.

ee was pulling the slack out of a wash hose when they felt a pop in their back. Incident occurred on 8/15/2025 - ee was evaluated by medical professional and released to work without restrictions. Pain was not getting better so went back to the doctor and was put off work on 9/8/2025.
Employee was rolling a piece of pipe on the welding table and it hit a piece of angle that was also on the table causing it to fall off and land on employee's left foot just behind the steel toe leaving it bruised and swollen. Employee did not seek medical attention until a later date- ee was put off work eff 3/6/25.

EE was rolling fuel hose back into hose reel when end kicked up and hit EE in the mouth resulting in chipped tooth. Treatment sought on 8/6/24. Confirmed type of treatment that was provided with dentist on 8/21.

EEs were replacing deck plate on walkway. EEs left area then one returned to get a tool. Fell through opening in walkway, appx 13' to ground level. EE sustained cut to right hand and possible injury to vertebrae.
Ee was pulling a hose to fuel/service a piece of mobile equipment when they stepped on a rock and rolled their ankle which aggravated a pre-existing injury.
EE was assisting in an off road haul truck tire change. EE leaned in to gain a better view and inadvertently placed right foot in front of the Skidsteer track. A co-worker operating the skidsteer was unaware of this and let the skidsteer roll or creep forward, causing contact with EE's foot.
Ee was assisting with lifting a bearing housing. Bearing lost balance and ee tried to catch it, but lost own balance in the process. EE tried to catch themselves by using hand railing, resulting in strain to shoulder. Was a report only until 5/1/23 when ee started to receive physical therapy for injury - company notified on 5/3/23.

Employee was removing sump pump using overhead hoist and side pull chain and shackle. Side pull chain became slack and shackle slid down overhead hoist chain pinching employees hand between thumb and pointer finger causing laceration requiring two stitches.
Employee was moving over the spray pipes on top deck of screen. When EE straightened up a ""zip tie"" that was holding a temporary light string in place went between EE's hard hat and safety glasses poking EE in the left eye. Prescription given for drops.
Employee was helping lift a piece of steel to take to the saw. When Employee lifted the steel Employee said Employee felt a pop in lower back. Took to doctor on 3/27/2018 and was given restrictions.
Employee was climbing up onto D9T dozer, foot slipped off of track grouser and caused scrape and deep bruising. The employee reported the incident on 2/19/2018. The employee sought medical attention on their own over the weekend. On Monday, employee was still in pain and was taken to the ER, where they were diagnosed with a bacterial infection.

Employee was cleaning up at the end of the shift. Employee had just rubbed their eye and felt a irritation shortly after. The employee requested to see a doctor during the next shift. A small metal filing and rust was removed from the eye. Drops were prescribed.
Employee was digging in a trench. Loose piece of dirt became dislodged and struck employee knocking ee against trench wall. Impact caused injury to torso causing bruising and possible hernia.

Employee was walking through final screening station when EE stopped to rub right eye. The employee removed glove and rubbed with right index finger. Shortly after that the eye became irritated and the employee flushed it with eyewash at the office. Further irritation resulted in employee seeking medical treatment.
EE1 and EE2 were moving HVAC window units in the Electrical store room to perform maintenance. EE1 was holding one of the units and the unit became wedged against a reel of cable. EE1 lifted that reel up over the AC unit with left arm out of their way. While doing, so ee1 felt a sharp pain in shoulder. EE1 did not see a doctor until 7/21/2016
Employee was training to operate the drill. After break, the employee was returning to the drill, walking over rough terrain, when EE felt a pain in their right calf. EE was taken to doctor for evaluation.
Employee was in the process of reading the information tag on S4 conveyor 150 hp motor. EE had brushed off tag and then leaned in closer to be able the read the tag. When EE leaned over a cloud of dust blew out of the motor fan end cap blowing into EE's face and eyes resulting in a foreign body getting lodged in EE's eye.

Employee cleaned up some stone with a shovel (less than a wheel barrow). Had sat down. When he stood up to climb some stairs had pain in lower back.
Using an air chisel to remove broken bolts. Chisel became stuck. Used another chisel to remove stuck chisel. Piece of metal struck arm resulting in medical treatment.
Placing a scale in conveyor decking. Employee rested the scale's weight (3' x 6"" diam) on 2x4's while positioning cable puller. Weight slipped off 2x4 and pinched finger between channel of the decking and the weight. Employee received sutures; no restrictions from ER paperwork. I don't have total mining experience info at time of filing this; system forces numeric entry.

NOTE: Did not have enough information to say is reportable until today. Hospital refused to provide info. Employee was shoveling sand in windy conditions, sand got into eye. Was wearing safety glasses but not goggles. Went to ER. Employee today stated he was given prescription antibiotic and corneal abrasion. No lost time/restricted duty.
Employee cannot state a specific event. He says he was walking on level ground then felt a pain in his knee. No other activity- no bending, twisting, crouching, or other vigorous activity. Says his first thoughts were that the pain was just from getting older. Later decided he would assert work related. When to walk in clinic and placed off work initially.
Employee was grinding on metal. When finished, he lifted the grinding shield he was wearing and felt dust and debris fall on his face and behind his safety glasses. He later felt scratching under the eye lid, visited a medical and optometric facility and had a minute piece of metal removed from his left eye. He was cleared to return to work on day of med visits.
Employee was opening a small consumer products box to take out a new product. When pulling the inner contents out, he brought his hand across a stray sliver of broken glass that had been stuck in the box and cut the heel of his hand. The cut was stitched and the employee was released from the emergency room without restrictions.

EE was operating drill rig and wanted to check the level of heat coming out of vent system. The cab heater was not blowing warm air. He reached up to feel the air coming out of the heater duct and his fingers contacted the squirrel type fan. The fan cut three of his fingers on his left hand near the ends. His index finger was nicked and the other two required 12 stitches.
Employee was installing 200' of 3"" conduit in a shallow trench. This involved minor shoveling, and lifting and moving conduit to glue and position it. After working for several hours, he noticed an ache in his right shin. The employee did not stop work, but wanted the ache in his leg to be documented.
On 7/1 employee welded a pin keeper on a truck when a piece of slag went down his shirt sleeve and burned his elbow in area about the size of a pencil end. The burn became infected. Notified management on 7/11. Missed work as of 7/9; returned 7/15. Use of short-cuff leather gloves instead of long welding gloves in a overhead position allowed slag to enter the shirt.
Employee was on stone carrying train, and had stopped train to dump stone from cars. He took the remote controller in hand and started to descend from train, when he stepped on broken controller strap and fell about 2 1/2 feet. He landed on his feet but the impact jarred his back and he felt pain. Sought medical care after that.
Employee exited road grader that was being used to grade road areas after a significant snowfall. Upon exiting the grader he slipped onto ice and broke a rib(s). Surface had snow/ice. He did not fall onto anything other than the ground.
Employee was attempting to pull X-bracing across floor by pulling on a chain that was connected to it. Part of the bracing was frozen to the floor. The bracing broke free from the ice and ran into employee's foot. Employee failed to anticipate release of energy and select better work method. Employee had one day of restricted work.

Employee wearing gloves, safety glasses with side shields and face shield was cutting 3"" angle iron with a 4.5"" hand held grinder equipped with a 4.5"" ""Razorblade 27"" cutting tool. It bound up and kicked back striking employee in the face (Right cheek area).
An employee was holding an idler set on a work bench. Another employee was swinging a 4# hammer to straighten out a bent part of the foot. The hammer glanced off the idler set striking the employees finger.

Come-a-long used to direct conveyor belt over hand rail. Belt snagged on hand rail and sprang free causing come-a-long to lurch and strike employee on left face.
Tail section of P2 conveyor. Falling impact idler being removed from P2 conveyor. While changing out the idler, accident victim and co-worker decided to let it drop after sliding it off the conveyor decking. They decided to drop it because it was awkward to handle and heavy. They thought it easier to let it drop.

The employee was cleaning up his work area. He picked up some heavy steel and felt pain in his right arm, by elbow. He reported the injury at the time but did not go to the Doctors.
EE was grinding overhead wearing a face shield, when he finished, he removed the shield and was attempting to ""Dust-off"" the loose debree and got some dust in his right eye.

Employee was removing a car from the train for repairs. He was in the process of uncoupling the car. When the cars seperated the air line between them broke at one end, instead of uncoupling at the connectors. The end of the hose struck employee in the elbow, causing the injury.
Employee was using a water hose to clean under a conveyor belt. As he was hosing he hit some hard-packed material, which caused the water and mud to spray back into his face. EE was wearing saftey glasses at the time of the accident.
The operator slipped and fell as he came off the grader.
The millwright was installing an idler set when it tipped over and pinched his finger.

Employee was using a hog burner to clear a frozen chunk from a chute when the chunk broke free and pinned the employees hand against the rail.
Employee was cleaning area around conveyor belt. Employee put larger stone, several inches in diameter onto belt. The stone did not go under discharge lip, rolled back down the conveyor and pinched the empoyees finger against the tail pulley gaurd.
Employee was drilling a hole in a 2"" x 2"" x 3/8"" x 10' piece of angle iron. The end of the angle iron fell off the stand it was resting on and pinched employee's finger between the angle iron and the drill press base.
EE unloaded 4 or 5 crusher anvils off a truck by hand. Equipment that should have been used was being repaired. The heavy lifting caused a hernia. HERNIA WAS SURGICALLY REPAIRED NOVEMBER, 2005.
Employee was climbing up a Cat 785B. He was wearing rubber gloves and there was dew on the railing. Employee slipped falling about 10 to 12 feet and broke his right ankle.
Employee was carrying a 3/4 ton com-a-long. When he went to adjust it on his shoulder the handle flipped up and hit him in the mouth chipping his front tooth.

The mechanic was removing the bearing cap from the traction motor on the 109 locomotive. When he lifted the cap off the jack, as he was standing in a bent over position, he felt pain in his lower back. This injury became lost time on 9/8/04.
The repairman was changing the rounding shoes on the #109 locomotive when some dust fell into his left eye and scratched his cornea. The medicine prescribed by the doctor caused an allergic reaction.
EE was fixing a hand rail on a Hitachi 20.5 Cy Shovel. When he stepped on a raised portion of the oil tank, he twisted his foot and later found out he had a small chip out of his bone.
The pipefitter was climbing a ladder in P-2 tunnel when the bottom of the ladder began to slide away from the wall. He then jumped off the ladder from a height of about 3 feet and fractured his foot.
The millwright was removing screen cloth from 402 screen in the mill bilding. as he pulled the screen cloth, it dropped and pinched his left hand between the cloth and screen side.

THE ELECTRICIAN APPRENTICE SLIPPED AS HE PICKED UP A SUCTION HOSE ON THE SIDE OF A 6'BANK. THE HOSE WEIGHED ABOUT 75 LBS. ON THE END HE WAS LIFTING. HE HURT HIS BACK AS HE SLIPPED WHILE HOLD- ING THE HOSE.
THE LOCOMOTIVE OPERATOR CLOSED THE ENGINE DOOR ON HIS FINGER AFTER CHECKING THE OIL.
THE REPAIRMAN WAS SPLICING L-6 CONVEYER BELT WHEN HE STRUCK HIS LEFT THUMB WITH A HAMMER.
THE MECHANIC TRIPPED OVER PAD-EYE ON THE REAR DECK OF THE HITACHI SHOVEL AND FELL. HE SPRAINED HIS LEFT HAND AND WRIST. ON 6-10-03 THE INJURY BECAME LOST TIME.
THE TRUCK DRIVER CLIMBED ONTO A TRAILER TO HELP UNLOAD A ROLL OF BELTING WHEN HE SLIPPED AND FELL ON HIS LEFT HAND. ON 05/21/03, A DOCTOR DETERMINED THAT HE HAD A BROKEN BONE IN HIS HAND.
THE MECHANIC WAS STANDING ON A TWO FOOT HIGH BENCH AS HE INSTALLED GREASE LINES ON THE #035 HITACHI SHOVEL, WHEN HIS FOOT SLIPPED AND HE FELL ON HIS LEFT ELBOW. ON 04/21/03, HE WENT TO THE DOC TOR WHO ASPIRATED HIS LEFT ELBOW.

THE MECHANIC HELPER HAD TO CLIMB ONTO THE 2500 HITACHI SHOVEL TO REPAIR AND FUEL THE SHOVEL. THE BOARDING LADDER COULD NOT BE USED BECAUSE THE CRAWLER TRACK WAS IN THE WAY. HE USED A STEPLADDE R TO GAIN ACCESS TO THE SHOVEL. AS HE STEPPED OFF THE LADDER AND ONTO THE WALKWAY HIS HEEL CAUGHT A FIRE EXTINGUISHER AS HE FELL FORWARD. HIS ANKLE BROKE BEFORE HE REGAINED BALANCE.
THE INJURED WAS REMOVING BOLTS FROM THE BELLY PAN OF THE 681 LOADER. HE WAS IN A CROUCHED POSITION WHEN THE IMPACT SOCKET SLIPPED OFF THE BOLT CAUSING HIM TO TWIST AND STRAIN HIS BACK.
THE INJURED AND HIS PARTNER WERE UNDER THE 109 LOCOMOTIVE TRUCK INSTALLING BEARINGS. THERE WAS A LACK OF COMMUNICATION BETWEEN THE TWO WORKERS WHICH CAUSED ONE OF THE BEARINGS TO FALL AND STRI KE THE INJURED IN THE FORHEAD. THE BEARING WEIGHED 35 LBS AND CAUSED A CUT TO THE INJURY'S FOREHEAD.

THE INJURED PUT SOME USED ANVILS ONTO A PALLET WHEN HE NOTICIED PAIN IN HIS BACK. THE ANVILS WEIGHED ABOUT 80 POUNDS.
THE INJURED WENT INTO THE MILL BUILDING TO CHECKTHE VSI CRUSHER MOTOR BECAUSE IT WAS SMOKING. HE DID NOT SEE FIRE AND THE POWER WAS OFF SO HE LEFT THE BUILDING TO LET THE SMOKE CLEAR. HE LATER FELT SICK AND WENT TO THE DOCTOR FOR SMOKE INHALATION.
THE INJURED CLIMBED ONTO R-4 CONVEYOR BELT IN THE MILL BUILDING. THE BELT HAD STOPPED AND NEEDED TO BE CLEANED OFF WITH A WASH HOSE BEFORE THE BELT COULD BE STARTED. THE INJURED LOST HIS BALAN CE AND FELL OFF THE CONVEYOR BELT TO THE FLOOR.
HE INJURED WAS WASHING DOWN S-4 CONVEYER DRIP PAN WHEN HE SLIPPED AND STRUCK HIS BACK ON A MOTOR REDUCER WHICH CAUSED A BRUISE.

THE INJURED WAS TIGHTENING A SCREW ON THE HOT SIDE OF A 480 VOLT ELECTRICAL STARTER WHEN AN ELECTRICAL EXPLOSION OCCURRED. THE ELECTRICIAN RECEIVED BURNS TO HIS FACE, HANDS, AND ARMS.
INJURED WAS GRINDING STEEL AND GOT A SPARK IN HIS RIGHT EYE.
GRINDING ON ANGLE IRON, OTHERS NEARBY WELDING, FELT SENSATION IN EYE, THOUGHT IT WAS WELDING FLASH, BUT NEXT DAY STILL HURT, WENT TO DR AND HAD STEEL REMOVED.

INJURED WAS GETTING OFF #603 SCREEN AFTER COMPLETING A JOB. WHEN HE STEPPED ONTO THE FLOOR, HE STEPPED ON A PLACE WHERE THE FLOORING WAS MISSING, TWISTING HIS ANKLE.
INJURED WAS CLIMBING DOWN THE SERVICE TRUCK AND CAUGHT HIS RING ON A WELDING BURR ON THE HANDRAIL. HE REACHED UP WITH HIS RIGHT HAND AND SLAPPED HIS LEFT HAND OFF THE BURR WHICH CAUSED HIM TO FALL TO THE GROUND. HE LANDED ON HIS RIGHT SIDE AND STRUCK HIS HEAD ON THE GROUND.
THE EMPLOYEE WAS COOKING HIS DINNER ON THE STOVE AND THE OIL IN THE PAN CAUGHT ON FIRE. AS THE INJURED PUT OUT THE FIRE, HE BURNED HIS HAND. THE DOCTOR TREATED THE BURN WITH PRESCRIPTION MED ICATION.
THE INJURED WAS REPAIRING A TIRE THAT WAS LAYING ON THE FLOOR. SHE BENT OVER AND APPLIED A SEALANT TO THE BEAD SO THE TIRE COULD BE INFLATED. WHEN SHE STOOD UP HER BACK HURT.
INJURED WAS IN L-1 TUNNEL REPLACING CONVEYOR IDLERS WHEN HE FELT PAIN IN HIS UPPER LEFT ARM. ON 7/25/00 THIS INJURY BECAME LOST TIME.
