Was opening a band on a box of product w/utility knife when it slipped and cut EE's pointer knuckle.

Caught in, under or between a moving and a stationary object

Two millwrights were trying to remove a metal plate from the conveyor by inserting pry bars. As the part broke free, EE's hand was pulled with the pry bar and pinched between the bar and the conveyor roller assembly causing a fracture of the distal phalanx of the left little finger.

(1st day of Treatment 1/13/23) Bending over to tie in the shot and felt low back give out.

While cleaning material out from under Conveyor S4 employee pinched the tip of right pinky finger between the handle of the shovel & the leg of the conveyor causing a small cut that required glue to close.

While changing a bit on the DTH drill a supplier was unscrewing the chuck and bit from the bottom of the hammer, the bit slipped and while trying to stabilize it the supplier's small finger moved in between the chuck and the top shoulder of the bit. The chuck came down on the smallest finger of the left hand causing a laceration under the glove.

Employee was attempting to pull a section of belt back together with a chain puller for a splice. Because the belt was under tension the handle pulled hard, due to slack in the belt it very quickly went much easier causing the handle to loosen, when that happened it slammed down and pinched right thumb between the handle and the chain. This broke the nail and cut the thumb.

The employee was blowing the chutes on the first floor screens. As employee was doing this task EE had piece a dirt get in EE's eye. EE was wearing all PPE including safety glasses.

Employee was shoveling near the tail section of D7A conveyor, the shovel made contact with a steel shim under the structural channel of the conveyor, the sudden/abrupt stop caused the shovel to twist to the left. This twisting motion combined with the force applied caused the employees knee to ""give out"" or buckle in a sideways motion.

Employee was attempting to remove snow and ice from between the wheels of a radial stacker. The employee was working from an elevated position due to built up material (snow/ice/sand mix), this awkward position likely caused an overexertion and led to the employee's back ""locking up.""

Employee was operating a haul truck in adverse weather conditions (extreme freeze/thaw) that caused the haul roads to become rougher than normal. The employee experienced lower back pain.

Miners were working to clear the jammed primary gyratory crusher. 2 miners were working to remove stone from the bowl, one miner was working farther down in the bowl using an air hammer to break up a large rock when suddenly another rock became dislodged and pinned the miner's feet down, several rocks and finer material slid down as well entrapping the employee below the knees.

Miner was carrying a piece of debris when ee tripped over an outstretched hose and dropped down to one knee.

Employee sustained a hernia as a result of lifting a machine component weighing 100 lbs.

Dozer operator was ripping on the quarry floor with a CAT D-10 when they hit a large rock, due to the nature of the ripped material the dozer was on uneven ground and the employee jarred their neck and mid back due to the side to side motion. The employee was diagnosed with a neck strain and thoracic back pain, they were prescribed medication and given modified work duty.

Operator was loading a production haul truck with a large wheel loader. As EE backed away from the truck (bucket still in the air), EE drove over a basketball size rock that had unknowingly rolled behind loader. Right after EE drove over the rock, the employee experienced pain in back.

While chipping material build in chute, a portion of the material fell in front of the employee, causing them fall backward and striking head/helmet agianst the opposite wall. Employee lost conscious for a minute. EE's work partner helped EE exited the chute under their own power. EE and partner reported immediately to their supervisor and first aid was provided

Employee slipped on the ice and noticed soreness in shoulder and was instructed to administer self-care. Employee was placed on restricted duty after MRI on 2/23/16. During the week of February 22nd all hourly employees were laid off, this information was received 2/26/16 and employee returned to work on 2/29/16 on restricted duty.

Employee was flipping a gate in front of a screen when his finger became pinched. Resulting injury was a fractured thumb.

Fall from machine

Employee was cleaning loader windows when several towels blew out of his hand. As he tried to grab the towels he came into contact with the emergency gate on the side railing which caused the latch to disengage. The employee fell through the gate to the ground resulting in a knee injury. Employee was on home health care until 10/8/15 when he was prescribed a pain killer.

Employee was lifting a tool box onto a truck (with the assistance of another employee) and felt a spasm in his back. Employee was on home health care until 9/28/15 when a muscle relaxer and pain killer were prescribed.

While haul truck operator (injured party) was being loaded a large boulder that was in the bucket of the loader hit the empty box and severely rocked the truck causing a neck strain. Re-classified from First Aid to Medical on 10/31/14 due to professional care by chiropractor.

While dismounting a work platform the employee's glove caught on the ladder's safety chain hook. He had not noticed his glove had snagged on the hook and when he stepped down the sudden unexpected jerk strained his left shoulder. This injury became a lost time Incident as of 12/7/15 due to surgery, prior to that employee was not restricted.

THE EMPLOYEE WAS DRIVEN TO CLINIC FOR MEDICAL ATTENTION IN WHICH HE RECEIVED 6 STITCHES. NO PRESCRIPTION WAS GIVEN AND THERE WAS NO LOST TIME.

On the following Monday, the employee was placed on modified light duty until the soreness can be investigated further. There was no specific event that occurred to cause the soreness.

Employee operating Front End Loader. Digging to create a floor in New Development, hit a hard spot that was not noticeable causing the Loader to stop abruptly, jarring the employees neck. He did home health care for neck until 7/29/13 when conditions worsened. On 7/29/13 the employee was put on restrictive duty and given a prescription.

Back strain while operating drill. Sitting in twisted position for 2 hrs - didn't realize seat swiveled. Found release knob on side of seat & rest of shift the seat swiveled. However, some back discomfort at end of shift. 2 days later, he reported increased back pain, sought medical attn & was prescribed a Rx & light duty.

While employee was walking around the pickup at the work site location he slipped in the fresh wet snow and fell to the ground. He didn't hit anything on his way down. But did feel his Left ankle twist during the fall.

Employee was welding on the boat-loader catwalk, in the shop. He was wearing his welding helmet & safety glasses. He raised his welding helmet while grinding the slag off the weld when a piece of foreign material got past his safety glasses and into his eye.

EE states while working on a bank with a loader and eating peanuts he jarred himself and bite his bottom lip. Cut on lip was found to be substantial and 8 stitches were required to close the wound. EE also complained of being sore on his right side and arm from being jarred against the arm rest. X-ray showed that he had fractured a rib.

Employee was pushing a cart in a tunnel when his right foot slipped in some mud, causing him to abruptly shift his weight onto his left hip. On 09/01/2011, employee sought treatment for discomfort in his lower back and hip. On 11/04/2011, an Independent Medical Exam found that the incident occurring on 04/06/2011 exasperated the employee's pre-existing osteoarthritis.

Employee was stepping over a water pipe, tripped, fell, and landed on some stones causing a minor abrasion and a bruise. On 12/21/2010, the employee notified the plant manager that he had sought treatment on 12/09/2010. On 12/21/2010 our company doctor found the employee's injury to be infected. He received a tetanus shot, prescription medication, and a follow-up 12/29/2010.

Emp. was loading haul trucks - he had just dumped a bucket in a haul truck and was backing away, lowering the hoist at the same time, the hoist did not respond so employee moved the lever forward and the bucket dropped and struck the ground. Emp. did work that full shift and worked next two scheduled shifts - he then went to the doctor on 5/17 - and the doctor took him off work.

Employee was checking pump in pumphouse, when unexpected 40 mph+/- wind blew the pumphouse door shut onto employee's fingers while they were exposed in the door jam pinch point.
