Was opening a band on a box of product w/utility knife when it slipped and cut EE's pointer knuckle.

Two millwrights were trying to remove a metal plate from the conveyor by inserting pry bars. As the part broke free, EE's hand was pulled with the pry bar and pinched between the bar and the conveyor roller assembly causing a fracture of the distal phalanx of the left little finger.

(1st day of Treatment 1/13/23) Bending over to tie in the shot and felt low back give out.

While cleaning material out from under Conveyor S4 employee pinched the tip of right pinky finger between the handle of the shovel & the leg of the conveyor causing a small cut that required glue to close.

While changing a bit on the DTH drill a supplier was unscrewing the chuck and bit from the bottom of the hammer, the bit slipped and while trying to stabilize it the supplier's small finger moved in between the chuck and the top shoulder of the bit. The chuck came down on the smallest finger of the left hand causing a laceration under the glove.

Employee was attempting to pull a section of belt back together with a chain puller for a splice. Because the belt was under tension the handle pulled hard, due to slack in the belt it very quickly went much easier causing the handle to loosen, when that happened it slammed down and pinched right thumb between the handle and the chain. This broke the nail and cut the thumb.

The employee was blowing the chutes on the first floor screens. As employee was doing this task EE had piece a dirt get in EE's eye. EE was wearing all PPE including safety glasses.

Employee was shoveling near the tail section of D7A conveyor, the shovel made contact with a steel shim under the structural channel of the conveyor, the sudden/abrupt stop caused the shovel to twist to the left. This twisting motion combined with the force applied caused the employees knee to ""give out"" or buckle in a sideways motion.

Employee was attempting to remove snow and ice from between the wheels of a radial stacker. The employee was working from an elevated position due to built up material (snow/ice/sand mix), this awkward position likely caused an overexertion and led to the employee's back ""locking up.""

Employee was operating a haul truck in adverse weather conditions (extreme freeze/thaw) that caused the haul roads to become rougher than normal. The employee experienced lower back pain.

Miners were working to clear the jammed primary gyratory crusher. 2 miners were working to remove stone from the bowl, one miner was working farther down in the bowl using an air hammer to break up a large rock when suddenly another rock became dislodged and pinned the miner's feet down, several rocks and finer material slid down as well entrapping the employee below the knees.

Miner was carrying a piece of debris when ee tripped over an outstretched hose and dropped down to one knee.

Employee sustained a hernia as a result of lifting a machine component weighing 100 lbs.

Dozer operator was ripping on the quarry floor with a CAT D-10 when they hit a large rock, due to the nature of the ripped material the dozer was on uneven ground and the employee jarred their neck and mid back due to the side to side motion. The employee was diagnosed with a neck strain and thoracic back pain, they were prescribed medication and given modified work duty.

Operator was loading a production haul truck with a large wheel loader. As EE backed away from the truck (bucket still in the air), EE drove over a basketball size rock that had unknowingly rolled behind loader. Right after EE drove over the rock, the employee experienced pain in back.

While chipping material build in chute, a portion of the material fell in front of the employee, causing them fall backward and striking head/helmet agianst the opposite wall. Employee lost conscious for a minute. EE's work partner helped EE exited the chute under their own power. EE and partner reported immediately to their supervisor and first aid was provided

Employee slipped on the ice and noticed soreness in shoulder and was instructed to administer self-care. Employee was placed on restricted duty after MRI on 2/23/16. During the week of February 22nd all hourly employees were laid off, this information was received 2/26/16 and employee returned to work on 2/29/16 on restricted duty.

Employee was flipping a gate in front of a screen when his finger became pinched. Resulting injury was a fractured thumb.

Employee was cleaning loader windows when several towels blew out of his hand. As he tried to grab the towels he came into contact with the emergency gate on the side railing which caused the latch to disengage. The employee fell through the gate to the ground resulting in a knee injury. Employee was on home health care until 10/8/15 when he was prescribed a pain killer.

Employee was lifting a tool box onto a truck (with the assistance of another employee) and felt a spasm in his back. Employee was on home health care until 9/28/15 when a muscle relaxer and pain killer were prescribed.

While haul truck operator (injured party) was being loaded a large boulder that was in the bucket of the loader hit the empty box and severely rocked the truck causing a neck strain. Re-classified from First Aid to Medical on 10/31/14 due to professional care by chiropractor.

While dismounting a work platform the employee's glove caught on the ladder's safety chain hook. He had not noticed his glove had snagged on the hook and when he stepped down the sudden unexpected jerk strained his left shoulder. This injury became a lost time Incident as of 12/7/15 due to surgery, prior to that employee was not restricted.

WHILE USING A COME-A-LONG TO LIFT THE C-7 FEED CHUTE, ONE OF THE EYE BOLTS BEING USED, BROKE. THUS CAUSING THE SHACKLE AND/OR EYE-BOLT TO HIT ee IN THE FOREHEAD. THE EMPLOYEE WAS DRIVEN TO CLINIC FOR MEDICAL ATTENTION IN WHICH HE RECEIVED 6 STITCHES. NO PRESCRIPTION WAS GIVEN AND THERE WAS NO LOST TIME.

IN FEEDER PAN ON A 4 FT. LADDER. HE REACHED UP TO HOOK A COME-A-LONG ON SIDE OF FEEDER.HE WAS MORE THEN HALF WAY DOWN THE LADDER,HIS FOOT THEN SLIPPED OFF & INSTEAD OF FALLING OVER, THE EMPLOYEE HELD ON TO A NEARBY PIPE.WHILE SUSPENDED IN AIR, HE SPUN AROUND THUS STRETCHING SHOULDER & BICEP. POTENTIAL LOST TIME IN THE FUTURE.

The employee reported soreness in upper back while operating a loader, the soreness occurred on a Wednesday his 3rd shift. On the following Monday, the employee was placed on modified light duty until the soreness can be investigated further. There was no specific event that occurred to cause the soreness.

Employee operating Front End Loader. Digging to create a floor in New Development, hit a hard spot that was not noticeable causing the Loader to stop abruptly, jarring the employees neck. He did home health care for neck until 7/29/13 when conditions worsened. On 7/29/13 the employee was put on restrictive duty and given a prescription.

WALKING DOWN STAIRS, BEGAN TO FALL FORWARD, WHILE HANGING ON TO THE RAIL HE PULLED BACK W/HIS LEFT ARM TO CATCH HIMSELF & HEARD A POP & FELT A STRAIN IN HIS ARM. Re-notification in Dec, 2013 - 2nd opinion confirmed work related.

Back strain while operating drill. Sitting in twisted position for 2 hrs - didn't realize seat swiveled. Found release knob on side of seat & rest of shift the seat swiveled. However, some back discomfort at end of shift. 2 days later, he reported increased back pain, sought medical attn & was prescribed a Rx & light duty.

While employee was walking around the pickup at the work site location he slipped in the fresh wet snow and fell to the ground. He didn't hit anything on his way down. But did feel his Left ankle twist during the fall.

Employee was welding on the boat-loader catwalk, in the shop. He was wearing his welding helmet & safety glasses. He raised his welding helmet while grinding the slag off the weld when a piece of foreign material got past his safety glasses and into his eye.

EE states while working on a bank with a loader and eating peanuts he jarred himself and bite his bottom lip. Cut on lip was found to be substantial and 8 stitches were required to close the wound. EE also complained of being sore on his right side and arm from being jarred against the arm rest. X-ray showed that he had fractured a rib.

Employee was pushing a cart in a tunnel when his right foot slipped in some mud, causing him to abruptly shift his weight onto his left hip. On 09/01/2011, employee sought treatment for discomfort in his lower back and hip. On 11/04/2011, an Independent Medical Exam found that the incident occurring on 04/06/2011 exasperated the employee's pre-existing osteoarthritis.

Employee was stepping over a water pipe, tripped, fell, and landed on some stones causing a minor abrasion and a bruise. On 12/21/2010, the employee notified the plant manager that he had sought treatment on 12/09/2010. On 12/21/2010 our company doctor found the employee's injury to be infected. He received a tetanus shot, prescription medication, and a follow-up 12/29/2010.

Person was driving a pickup to the primary to pick up employee. He went around the corner at the ramp on the wrong side of the road. The sun was in his eyes and he ran head on into another pickup coming from the primary.

The employee was walking to a truck and slipped on the sand covered ice, catching himself and not falling to the ground.

Employee claims back/lumbar strain as he exited his bulldozer, or while operating it.

Employee was pulling 1/2"" overlay screen up the screen to the position it. He was using the screen puller tool. Strain to back. He reported it the day it happened but did not seek medical attention until 10/21.

Emp. was loading haul trucks - he had just dumped a bucket in a haul truck and was backing away, lowering the hoist at the same time, the hoist did not respond so employee moved the lever forward and the bucket dropped and struck the ground. Emp. did work that full shift and worked next two scheduled shifts - he then went to the doctor on 5/17 - and the doctor took him off work.

Employee was checking pump in pumphouse, when unexpected 40 mph+/- wind blew the pumphouse door shut onto employee's fingers while they were exposed in the door jam pinch point.

Injured was drilling wear plate and brushed chips away with hand while wearing gloves. Spiral chip from rotating drill snagged glove and cut through it, cutting middle finger on right hand. Required 9 stitches.

Employee was attempting to reach for trouble light above conveyor when his right hand was pulled into the pinch point between belt and the idler. Resulted in open wound with fractures to right hand, wrist and all five digits.

Employee slipped on car floor of Rescreen building hurting lower back and right hip.

Employee was checking fuel level on engine, placed hand on exhaust thinking it was the air box and burned palm of left hand.

EMPLOYEE SLIPPED ON ICY CATWALK BETWEEN SAND SCREWS, LACERATED RIGHT ELBOW, WENT TO E.R. AND RECEIVED 3 STITCHES.

EMPLOYEE OPERATING TRUCK, LOADER WAS LOADING STONE INTO THE TRUCK. AFTER LOADING LAST BUCKET HE BACKED AWAY & A STONE FELL FROM BUCKET, HITTING TRUCK MIRROR AND PROPELLING GLASS THROUGH THE TRUCKS OPEN WINDOW. EMPLOYEE HAD WINDOW OPEN - EYE STARTED HURTING 10 MINS AFTER INCIDENT.

EE WASA OPERATING HAUL TRUCK, WAS WEARING SEAT BELT, HE WAS LOADED WITH WASH POND SALT, LOAD WAS LOOSE AND SHIFTED TO BACK OF TRUCK BED CAUSING FRONT OF TRUCK TO COME OFF GROUND, WHEN LOAD RELEASED TRUCK'S FRONT END LANDED BACK ON THE GROUND.

EE HIT HIS THUMB WITH A HAMMER WHEN HE WAS CHANGING A FEED CHAIN LINK ON A BLAST HOLE DRILL.

EE WAS USING A 4"" GRINDER TO GRIND OFF EXTRA BOLT LENGTH FOR BELT CLAMPS ON S2 CONVEYOR. WENT HOME FOR THE DAY, EYE WAS BOTHERING HIM. HE TRIED TO WAIT IT OUT, EYE GOT WORSE. WENT TO DOCTOR AN D THEY REMOVED MATERIAL FROM HIS LEFT EYE.

EE WAS DRILLING AND BLOWING COMPRESSED AIR WHEN HE FELT A DISCOMFORT IN HIS RIGHT EYE. THIS INJURY WAS NOT REPORTED TO US - WE RECEIVED BILL & RESEARCHED IT.

EMPLOYEE WAS STEPPING UP INTO HAUL TRUCK WHEN HIS FOOT SLIPPED UNDER THE RUNG WHILE HIS OTHER FOOT WAS ON THE RUNG. HE HAD HOLD OF THE LADDER AS HIS FEET SLID UNDER THE RUNG AND HE STRAINED HIS BACK.

EE WAS OPERATING LOADER--TRAVELING ACROSS SILICA BENCH WITH A BUCKET OF STONE AND RAN OVER A LARGE ROCK. THE QUICK SID TO SID AND BUMP STRAINED HIS RIGHT SHOULDER.

EE WOKE UP THE MORNING OF 1-21-02 WITH SEVERE BACK PAIN, HE THINKS IT WAS CAUSED FROM A JARRING THAT HAPPENED WHILE RUNNING JD LOADER THE PREVIOUS WEEK. NO SPECIFIC TIME OR DATE.

EE WAS SHOVELING SPILLAGE ONTO BELT, WHILE LIFT-ING AND TURNING HE FELT A SHARP PAIN IN HIS LOWER BACK.

EE WAS WALKING AWAY FROM SOME EQUIPMENT WHEN HIS FOOT SLIPPED ON FRESH SNOW. WHEN HIS FOOT KICKED TO THE SIDE HE SAID HIS KNEE DISLOCATED & POPPED BACK INTO POSITION. DOCTOR'S DIAGNOSIS: DISLO CATION OF PATELLA.

EE WAS WALKING UP A SET OF STAIRS AND TRIPPED ON THE STEP. THE STEPS WERE FREE OF ALL DEBRIS AND EE'S HANDS WERE EMPTY. EE ENDED UP BREAKING A TOOTH AND PUNTURING THE LIP.

EE WAS WALKING WHEN HIS HEAD HIT A BEAM, HE DUCKED BUT NOT ENOUGH. HE HIT HIS HEAD PRETTY HARD RESULTING IN PAIN IN NECK AND BACK. HE DIDN'T GO TO DR. UNTIL HIS FINGERS ON RIGHT HAND STARTED GOING NUMB.

EE WAS WAHING SPILLAGE OFF THE FLOOR WITH HIGH PRESSURE HOSE, EE TRIPPED OVER THE PART OF THE HOSE THAT WAS LAYING ON THE FLOOR, EE HURT LOWER BACK.

EE WAS IN HAUL TRUCK, WHILE HIS TRUCK WAS BEING LOADED THE LOADER BUCKET HOOKED THE SIDE OF THE TRUCK BOX. AS THE LOADER WAS BACKING AWAY THE BUCKET WHICH THEN RELEASED THE TRUCK WHICH SNAPPED BACK TO LEVEL POSITON, JOSTLING EE.

EE WAS INSPECTING D-7 CONVEYOR WHILE LEARNING OVER EE PUT HIS HAND ON A CHANNEL INT HE SINTER BUILDING, IN THE DUST ON THIS CHANNEL WAS A WELDING ROD THAT PIECED THE PALM OF HIS HAND LEFT ENTE RED BY MIDDLE FINGER EXITED BY INDEX FINGER.

EE WAS REMOVING DEBRIS FROM CEMENT BLOCK BUILDING & STRAINED HIS BACK.

OPERATING #37 LOADER REPAIRING ROADS WHEN HE TURNED TO LOOK BEHIND HIM TO BACK THE LOADER UP AND THEN HE HEARD HIS SHOULDER POP.

EE WAS WALKING TO CAT TRUCK WITH LUNCH PAIL WE WERE HAVING A BAD SNOW STEAM WHEN HE SLIPPED ON PACKED SNOW/ICE AND FELL ON LEFT SIDE.

EE WAS HANGING COME-ALONG AND STEPPED ON COUNTERWEIGHT WHICH HAD MUD ON IT. HIS FOOT SLIPPED OFF & HE LANDED ON HIS KNEE ON THE EDGE OF THE COUNTERWEIGHT.

EMPLOYEE WAS ROLLING A 55 GALLON DRUM OF OIL FROM FORK LIFT ONTO BED OF COMPANY TRUCK AND THEN NEXT DAY COMPLAINED OF SEVERE LOWER BACK PAIN.

UNLOADING A BARREL OF OIL FROM PORTABLE FORKLIFT ONTO PICKUP TRUCK BED, AND BARREL TIPPED AND HE ATTEMPTED TO CATCH IT, AND STRAINED HIS BACK.

LIFTING 50 LB BOXES ONTO SHELF AT SHOULDER HEIGHT, STRAINED SHOULDER.

PULLING PUMP OUT OF DRILLED BLAST HOLE, BECAUSE THE DEWATERING PUMP FAILED, STRAINED LEFT FOREARM.

EE WAS CRANKING UP A CHUTE ON DISCHARGE HOPPER AND HE JAMMED HIS HAND BETWEEN CRANK HANDLE AND FLOOR.

LOADER OPERATOR ACCIDENTALLY HIT THE QUICK RELEASE LEVER ON LOADER BUCKET. IT DESCENDED AND STRUCK SIDE OF HAUL TRUCK, JARRING THE OPERATOR.

EE WAS USING A HAND GRINDER ON HEAD PULLEY AND FOREIGN PARTICLE ENTERED HIS EYE.

EE WAS STOOPED OVER IN TIGHT CONDITIONS, CHANGING OIL FILTER IN A COLD ENVIRONMENT. STRAINED BACK.

HE WAS LIFTING A CABLE FROM ONE SPILE TO PUT ON ANOTHER SPILE WHILE ASSISTING THE VESSEL AT THE DOCK AND HE STRAINED HIS BACK.

STRUCK ELBOW AGAINST SCREEN FRAME WHILE TRYING TO REMOVE A BROKEN BOLT. WELD LET GO CAUSING ARM TO STRIKE FRAME.

REMOVING ENGINE FROM HAUL TRUCK PULLING ON WRENCH TO LOOSEN A BOLT, STRAINED HIS WRIST

CLIMBING DOWN LADDER OF HAUL TRK, VERY ICY, LOSTGRIP & FELL 5FT TO PLATFORM, STRUCK HIS CHEST AGAINST RAILING

SHE WAS SHOVELING DIRT FROM UNDER A WATERLINE AND TWISTING/LIFTING MOTION CAUSED A MUSCLE STRAIN IN HER BACK.

HE WAS REMOVING AN ANGLE THAT HOLDS SCREENS DOWN AND WHILE GRINDING THE SURFACE A HOT SPARK FLEW INTO HIS EYE.

HE STEPPED OFF BOTTOM STEP OF STAIRS AND SLIPPED ON MUDDY SURFACE CAUSED BY RAIN.

CLIMBING LADDER TO GET IN CAB OF DOZER, FELT A KNOT IN HIS LEG, AS DAY WENT ON HIS FOOT BECAME NUMB.

CHANGING SCREENS IN THE RESCREEN BUILDING, PUSHING THE SCREEN CLOTH UP THE SCREEN DECK, FELT MUSCLE PULL IN ARM.

HE KNELT DOWN TO START WELDING SPEED BRACKET TO CONVEYOR ANBD HE KNELT ON A STONE CAUSING HIM TO REACT SUDDENLY TWISTING HIS BACK.

REPETETIVE CUMULATIVE TRAUMA. RIGHT SHOULDER FROM DAILY CHECKING BLAST HOLE DEPTHS, BY BOBBING A 100' WEIGHTED TAPE UP & DOWN IN THE BLAST HOLE. CAUSED SHOULDER SORENESS.

DRIVER NOTICED A SHARP PAIN IN HIS BACK DURING NORMAL HAUL TRUCK OPERATION.

HE WAS LIFTING A BOX OF BOLTS TO PLACE IN STORAGE RACK AND STRAINED HIS BACK.

EMPLOYEE WAS BACKING UP HAUL TRUCK FOR LOADING &HE BACKED INTO THE FACE WALL AND JOLTED THE TRUCK. LOADER OPERATOR SPOTTED HIM TOO CLOSE TOFACE WALL.

EE WAS REMOVING WEAR PLATE FROM BY-PASS CAR AT THE RESCREEN-LATER IN LUNCH ROOM FELT PAIN IN BACK WHEN HE AVOISE FROM LUNCH TABLE.

EE WAS GETTING OUT OF HIS TRUCK IN PARKING LOT AND HE SLIPPED ON ICE AND FELL.

TIGHTENING BOLTS WHILE WORKING AT THE CRUSHER AND DIRT FLEW IN HIS EYE WHILE RUNNING AIR POWEREDGUN.
