While clearing out full mechanicals on K22, PA had some CKD flush out through the port they were poking through. The dust contact on the PA's skin caused some irritation; later, the PA was diagnosed with second-degree burns in the neck region.

The employee was offloading EC2 into the storage silo when a band clamp broke off, causing the hose to come loose from the connection and blow dust on the employee's face. As a result, the employee did get CKD dust in left eye.
EE was moving bricks and when one fell at a time, while trying to accommodate the other bricks, the little finger on EE's right hand got caught in the middle of the bricks.
On July 12, 2025, at approximately 7:00 a.m., the powerhouse operator noticed that a fire in the powerhouse had started in a switch gear, causing damage in the electrical panels. The incident is under investigation.
The employee was adding hydraulic fluid to a haul truck up on the quarry dust cell. Upon completion of the job, the employee rose up from a crouching position and immediately felt a sharp pain in their lower back.
Employee suffered chemical burns after work poking on 5 reject dust containing CKD. EE had initial skin irritation and didn't report the event. After the injury worsened on June 9th, they reported to their supervisor. On June 12th, the medical report stated it was a second-degree burn.
PA was called to push stone into the feeders at the primary pile, after 30 minutes of pushing, the dozer went over the edge and nose first into the feeder. The employee called on the radio for help/they remained in the dozer while the fire department performed emergency rope rescue. MSHA was notified immediately.
Two employees used a sledgehammer and a pry bar to remove a headliner from the ball mill. When the sledgehammer struck the liner, a sliver of the liner's edge came off and struck the employee holding the pry bar in the left arm.

The crew was installing an axial blower using two manual chain hoists. While lifting the filter housing, a gasket came loose underneath the suspended part. As an employee tried to adjust the gasket with their hands, one of the chains moved and the blower fell and crushed the employee's left index finger, causing an injury.
The storekeeper was moving idlers in the storage area. One piece of equipment moved suddenly and pinched the storekeeper's fingers.
The worker was transporting buckets with wet clinker and suffered superficial chemical burns due to the contact with the caustic material.
A Production Support was cleaning the rail scales and stepped on the railroad track rolling an ankle causing a sprain of the Left ankle unspecified ligament.
While vertically hoisting a mill brow liner, employee did not follow direction and unclipped cable hoist to maneuver liner. Liner moved and pinched finger between another liner.
The employee was moving oil drums and noticed a bulge starting (At this time EE didn't inform the mine operator) After 13 days EE went to a doctor where EE was diagnosed with a hernia that needed surgery. The mine operator was informed about the need for surgery on 05/13/2024.
The employee disconnected the air hose from the lance to inspect the ball valve. While attempting to inspect the ball of the ball valve the air compressed air in the lance gave way then air and debris shot under their safety glasses and into their eye.
The employee was walking in the plant to perform tank level reading when employee slipped on the uneven muddy ground (caused by weather conditions) lost footing and twisted an ankle. Medical advice was given for cold therapy and elevation. The next day employee felt some pain and was referred for more exams when employee was diagnosed with a Left fibula fracture.

Some dusty material came down from above out of a leaking feed chute that fed the press and hit the employee's eyes causing corneal abrasion. The employee needs to stay in a clean environment for one week.
The Mechanic was welding up a side box and segment together for the secondary crusher, in the shop on the workbench. EE had to reposition the box to get a proper angle of weld and when rolling down the part EE ended up pinching right hand little finger between the box and workbench. EE had leather gloves at the time of the incident. The injury was Finger Laceration.
The Millwright was uninstalling a section of the collection line, and the section the EE was pulling off slid around. EE tried to stop it with hand, and due to the size of the piece the steel cut through EE's glove and lacerated EE's hand.
The employee was closing a feed gate on a cement silo, When EE hit the gate with a hammer to close the rest of the way a spurt of cement hit EE in the face and got behind EE's safety glasses and into EE's left eye.

While working in the pug factory, the system became clogged The employee tried to disconnect using the pressure washer when the accumulated dust returned to EE. EE lost balance and fell, causing an ankle injury.
Root cause under investigation. This machine is owned and maintained by CAT who took it to their shop to investigate. Their initial review of operating codes showed nothing related to the operation or how it was being used. So likely a leak on the hydraulics, DEF, or fuel ignited by the manifold.
EE reported was walking in the plant and noted that EE's left knee gave out while walking, causing the sole of EE's shoe to slip approximately half an inch.
An employee reported that left knee was starting to hurt. The employee stated that earlier employee was kneeling on the concrete at the primary crusher chiller room cutting some material for 10 min. Upon returning to break area employee's left knee started hurting. Upon looking at the knee employee noticed it was red and swollen.
Screw conveyor was not working and needed to be repaired. There were two employees working on the job. They went to lift up the lid on top of the screw when one of the employees lost their grip causing the lid to fall on the other employee's left pinky finger causing a laceration.

The employee put a socket on a nut to remove bolt, turned to get impact wrench. Socket vibrated of nut falling and striking employee on the hand.
Employee was exiting a skid loader and used the top edge of the bucket for foot placement while climbing down. Right foot slipped and caused fall forward resulting in the area just above the knee to come into contact with the edge of the skid loader bucket and foot slipped backwards off the bucket causing overextension of the knee. Recordable 11/26/2021.
Employee states that while adding dry ice left foot slipped into a hole causing employee to do the splits. Employee complains of pain in the groin area and right lower abdomen.
Small gas (fuel) explosion in kiln during startup caused by control system logic issues that resulted in excess gas in the kiln.
Employee was walking in the parking lot to a vehicle and stepped in a pothole. EE then stumbled and tried to catch self with EE's left arm. EE hit EE's head and neck on the concrete.
Employee was removing a section of chain from its box to repair a syngyp feeder. The employee proceeded to unroll the section of chain and had their right index finger pinched between two of the tabs that are welded to the side of the chain links.
Employee was using a wrench to loosen a take up feeder bolt when employee slipped and fell forward. Employee's tooth was chipped when employee's mouth made contact with the wrench.
Miner was loading a large chain in bed of pick up truck and had thumb pinched in between chain links causing a laceration.

Incident caused by an excess volume of natural gas that entered the kiln during the relighting process and then ignited in a delayed manner. This higher-than-normal volume of natural gas then ignited and caused a pressure wave to propagate through the kiln damaging the boiler housing and kiln firing hood.
Employee was power washing. EE was about 3/4 of the way done when EE hit a steam pocket in the hot dust that caused some material to blow back at EE. EE turned head and some dust got behind EE's face shield and safety glasses then into EE's eye.
Employee was descending stairs from an elevated platform when the employee's right foot slipped off the ladder rung and employee reached out with left arm and grabbed a bar hurting shoulder.
Using hoist to lift 1/4"" thick plate approx. 6' x 8' slightly to drag it across floor. Rigging and clamps used to lift one side. The side attached to the hoist by the rigging and clamps was approx. 3 ft up when a clamp came off, plate swung and employee was struck in lower left left leg causing puncture/laceration requiring stitches.
While taking retaining rings off, another employee was air arcing on the other side of the kiln. A spark came up over the kiln and landed on the employee's back between harness and jacket. Causing a burn to the injured employee's back.
Employee was pressure washing a utility vehicle and felt a burning sensation in the upper back resulting in a strain and muscle spasm.

Employee reports EE was driving 10 mph across railroad tracks and didn't realize it was dug out and as the truck went over the tracks, the truck suddenly stopped, seatbelt didn't lock up, causing EE's head to hit the rear view mirror and EE's back to lock up.
Technician was working on the vac truck crew and was picking up material in the raw grind. Employee was holding the handle on the suction nozzle and pinched pinky finger between the handle of the nozzle and a cross member of the stair structure at the discharge of the mill due to the nozzle suddenly surging forward due to vacuum suction.
While re-bricking the kiln our employee was getting a sledge hammer to help drive shims to complete the job. When EE was returning EE slipped and dropped the hammer on EE's finger causing a cut. EE received stitches to the right middle finger and was released from care.
Employee was using a gunning machine to complete work on the nose required for restart of the kiln. The hose plugged at the discharge end and caused back pressure which created a failure in the hose causing the line to rupture at the pump. The failure in the hose blew monolithic at the employee and resulted in EE's PPE being blown from EE's face and material to enter EE's eye.
Employee was installing an SNCR well into Kiln 22 from a 60' manlift under the roof of the kiln. The Employee had hands under both sides of the flange on the well and while lowering it they pinched the right ring finger between the flange and the manlift basket railing causing a laceration to their knuckle.
Pump Operator was standing on the deck cleaning the mixer while the engine was running. The grate that had been opened to gain access to the mixer was propped up beyond 90 degrees. Due to the vibration of the unit, the grate fell and struck the operator in the hard hat and ear, cutting them above the ear.
The vehicle employee was a passenger in got stuck on an icy patch on relatively flat ground. Employee got out to push the vehicle out and while pushing felt a ""pop"" in the right calf. Their calf was in pain and unable to bear their full weight. Medical diagnosis was ""strained calf muscle"".
Death at mine site - natural causes- not mining related, suspected heart attack. Employee was eating lunch in the contractor parking lot. When another employee went to pick employee up it looked like employee fell asleep. Employee opened the door of the injured employee. Injured employee was unresponsive. CPR was started and 911 called.

Brick rig shifted in kiln while rolling it to new position. Kiln stopped, rig was leveled and wheels chocked. Crew climbed up to adjust shifted pieces. While on rig, it rolled up kiln to the right (facing upkiln) and overturned. Employee's leg got caught and was injured, later found to be broken. Other 3 employees on the rig at the time of the incident were uninjured.
Employee was jackhammering debris when the chisel point dropped through the grate and caused the jackhammer to jerk back.
Employee was sanding the walkway due to slippery conditions on the walkway. EE was using a bucket and slowly dumping the sand in front of them when their feet slipped and they went down onto their elbow. Diagnosis for shoulder injury confirmed on July 26th.
The employee was using a poking bar to poke the frozen chunks down in the feeder, on a forward stroke the employee caught the ring finger on left hand on the middle bar of the hand rail pinching finger between the poking bar and the handrail. Employee received two stitches.

Employee was walking past the tail end of 13 belt when ee saw the sample port door was open. EE stepped up on the platform to close the door. After the door was closed ee stepped off the platform and rolled left ankle on a large piece of clinker. Ankle had a large amount of swelling. On 5/31, EE went to doctor who ordered ee out of work in order to allow healing.
Transporter was in the process of loading CKD into trailer pup tanker using a manually operated valve when an unexpected surge of material forced the valve open causing hand to impact an adjacent valve. The driver removed glove and noticed significant swelling on the back of the left hand. Mine not privileged to all personal info. 1st day of LTI starts on 1/31
Clearing debris from conveyor belt. A chunk fell off and pushed employee's knee into a guardrail. Bruised the knee

PA was positioning the portable kiln shell cooling fan. The PA grabbed the tongue of the trailer & began to spin the fan on the wheeled jack stand. The jack locking pin came out & the jack folded towards the front of the tongue causing the tongue to drop to the ground and swinging the jack hitting the PA's hand. The employee was taken to the ER where EE received stitches.
Electrician walking between the back wall of the raw grind & a collector. In EE's path was a manhole covered with a circular cover constructed from expanded metal grating. When EE stepped on the cover, it tipped, causing EE to stumble. As EE stumbled, right forearm contacted the edge of the cover resulting in a puncture wound. EE's wound required two stitches to close.
Employee got cement dust in left eye which became irritated.
There were 2 crews of 2 welding the ring on the shell at the same time. The affected EE was wearing a welding PAPR while EE was welding, but had the shield up while EE did the fitting work. As EE fitted the iron, the other crew continued to weld about 6 feet to the left. On June 10th EE woke up with eye irritation. EE sought medical attention and was suffering from welding flash.
Assisting in driving shims: hand & hammer slipped off shim - collar on shim driver hit finger & drove it into shim.
Employees were removing a coupling from a reducer shaft. As the coupling was being heated with a gas torch, a puller was being used to apply hydraulic pressure to the coupling. A piece of the coupling broke off and struck the employee that was holding the torch.

Employee was holding sheet up for another employee to drill. Other employee dropped his drill hitting injured employee on the hard hat and mouth.
Contractor cut his right thigh approximately 8 inches above the knee while cutting end off of conveyor belt
Employee was sizing piece of 3/8 hard plate w/ hydraulic shear. The employee pushed piece into the shear to position it then operated the foot control while stabilizing the piece with his hand. When the shear came down the piece lifted off the shear and pinched his middle finger on his left hand between the work piece and hand guard. Employee received 6 stitches
While knocking down a pile of clinker, a large piece fell and hit employees ankle.
Employee was installing feeder chain into the #2 Samson Feeder and it bunched up near the tail sprocket. Employee then grabbed the chain to pull the bunched up chain rearward away from the sprocket. During the process, his finger was pinched between the chain links where the apron bars get bolted on.
Parts were being off loaded from the service truck by both employee's. Employee stepped on a rock and fell. Part fell from hands of employee hitting the leg which resulted in the broken bone.
Injured employee was climbing into a bobcat and injured his knee.
A group of students were listening to a Lafarge presenter when a student fainted, collapsing and hitting her chin on the floor. She regained consciousness after a few seconds. Due to her fall, she had two lacerations on her lower face. The student was taken to seek medical care and received stitches in her chin.
Work partner pulled pipe out of tank, hit a beam. When this happened injured employee looked up and got dust particles in his left eye. Eye protection was being warn. (Corneal Abrasion).

Employee was balancing kiln ID fan. He had welded a balance weight to the side of the fan wheel and was wire wheeling the slag off his weld with a 4"" grinder when it came in contact with the edge of the fan wheel and got ripped from his grasp. The grinder hit his chest, ran up his coat and hit his chin. Causing employee to receive 1 internal stitch and 3 external stitches.
Drilling hole thru diamond plated decking, Drill bit caught as it was penetrating deck and spun the drill body twisting employee's right ring finger.

Devere Const was removing transformer for Alpena Power Co on the Lafarge site. While cutting the north side a fire started. At apprx 4:55P Devere contacted Lafarge Control reporting a fire. The Alpena City Fire Dept was also dispatched. They arrived on scene at 5PM and were on site until apprx 7PM. Fire was contained w/in transformer. MSHA call center was contacted at 5:05P

Power Outage causing LSI belt to stop. Gate Valve failed to close which caused the LSI belt to start on fire from the heat from the Kiln.
Employee grabbed a passenger elevator door with his right hand and pulled the door closed, inadvertently slamming the door shut on his left middle finger. Middle finger required stitches.
Employee was exiting raw grind building when he slipped and fell twisting his ankle. On 1-16-12 employee went to the doctors and found he fractured his leg.

While exiting drive house, wheelbarrow rolled back and caught his pinky on his left hand between the concrete drive base and wheelbarrow. Employee lacerated finger and had a fracture between 1st & 2nd joint.
Employee was walking past the SW side of Feed silo #2 to take a routine boiler water sample when he slipped and fell. Employee was taken for X-Rays where it was determined that he had single broken rib on the left side.

Employee was opening slide gate on bottom of cement silo #3 storage. A steel bar is used as a cantilever. Bar slipped off gate and pinched finger against top of silo feeder. Employee cut the end of middle finger on left hand and received stitches.
While lifting up a 20' 2X6 to be nailed, he felt his shoulder pop
Employee was dislodging chunk from feeder. There is a cross bar on the feeder and employee was using this as leverage. The poker slipped off and at the same time his feet slipped causing left arm tripping the stop-cord. This tripped a pneumatic gate to prevent spills. When the gate closed his right hand ended up pinched between the gate and cross bar.
While installing a saddle on drag line-lowering saddle to floor caught his finger under the saddle causing a laceration to left finger.

#75 screw was plugged. To unplug crew was planning to dump screw. As we were trying to restart screw dust silo above flushed through the restart screw. To stop flush we were attempting to close a quickopen door, during the attempt dust blew upward into faces. Employee received dust into left eye and immediately went to eyewash station to clean out eyes.
Our technician was tightening a bolt on a torque head convertor. The wrench slipped off the bolt and he hit his hand on the convertor cover cutting the middle finger on his left hand.
The hopper door was opened by an employee. The injuried employee had a piece of plywood he was going to prop the door open. Employee was going to go up the ladder when he looked up and dust fell. The dust landed on his right eyebrow and behind his safety glasses. Employee went to washroom and flushed his eye. Employee went to Doctor, was treated and put on light duty for five days.
Employee welding on overhead collector pipe in raw grind. Felt burn near right eye. Took off mask and wiped spark away, washed eye with saline. Incident reported to supervisor. Days later eye became irritated and employee sought medical treatment. Doctor removed residue from small piece of metal and employee given prescription for medication.
Mobile mechanic was installing new handrails on portable steps. While he was tightening bottom support on handrail, the wrench slipped off bolt causing his hand to come back, striking chin. This caused two chipped teeth and one fractured tooth. Dentist repaired the two chipped teeth.
2 guys were using 15 ton hydraulic pin pusher to remove WH78 drag chain. The injured was holding a hammer handle and the other millwright was applying pressure with the manual hydraulic pump. Replaceable end of hydraulic ram shattered striking injured on right side of face shattering lower jaw and fracturing upper jaw. He also suffered a three inch laceration on the right cheek.

EE was driving mill liner out of finish mill. He struck liner with hammer & felt something twinge just below his knee on inside of left leg.He noticed a sliver was gone from liner, looked down,& saw that the sliver had gone through his pant leg & sliced skin just below knee.Medical treatment requiring 3 stitches.
Employee was using cutting torch when wind shifted and a spark flew up into his left ear, causing burn. At that time, employee shut off cutting torch and flushed ear with water. Incident reported to supervisor 4/28/08 when employee sought medical treatment. Referred to specialist in May for treatment.
Inspecting iron ore tank, removed door of discharge chute. Placed door back on brackets, then reached down to retrieve keeper bar. Not aware door was not fully hooked on upper framework. Door came loose and fell approx 2 ft and struck right hand. Second finger, one stitch; third finger end bone broken, five stitches.

Employee was replacing a quick opening door in Raw Grind. Door slipped pinching his right thumb between door and door frame. Stitches required to close wound.
Employee was welding on quarry drill when he felt a flash in his right eye while striking a welding arc. He notified his supervisor and applied eye drops for flash. A couple of days later eye was still irritated and employee sought medical treatment. Doctor removed piece of foreign material from eye, installed a contact lens and gave him a prescription for eye drops.
Employee was working with another employee changing out tires on Bobcat loader. Lug nuts were removed on right front wheel. Fellow employee left to retrieve another tool. Employee proceeded to remove tire from hub. When the tire (approx 100 lbs) became free, it dropped and his right index finger was cut on one of the threaded lug bolts. Laceration required 15 sutures.
Employee was on inspection walking down stairs to K19 clinker cooler drag tunnel when he stepped on 18"" of hot kiln feed dust. He immediately removed boot and sock and found top layer of skin above his right ankle peeled off. Applied burn spray, bandaged wound and sought medical treatment. Employee sustained 2nd degree burns to right ankle.
Employee was performing inspection on 11061 conveyor belt in tunnel. There was a roller not working and his hand got drawn into conveyor. Employee received cuts to back of his left hand from wrist area to small finger. Surgery performed.
Demoing steel. Cutting angle from beam. Finger pinched between angle & beam.
Working to pull a roof cover off so they could lower a boiler tube down. When taking the cover off he had to slide it by a pipe and that caused it to hit another cover pinching his finger.
Employee was seal passing and grinding a seam on the kiln.

Cleaning rubble while laying underneath a beltline. Water soaked through clothes causing minor burns to his skin.
While cutting off bolts on old purline with torches, employee was bending over doing this work. When employee stood upright, his back started hurting.
Lifting a beam with chain falls and come a long when one of the lifting lugs broke causing the beam to swing striking him in the chest.
Employee clamped piece of sealtite in vise and held loose end with pair of pliers in order to cut with hacksaw. Hacksaw bound as he neared end of cut and the blade jumped up and cut his left hand between third & fourth fingers. Required 3 stitches to close wound.
During lubrication of wire being installed in electrical conduit employee slipped in front of electrical pull box, reached with left hand and grabbed edge of box. This caused lacerations to hand resulting in 7 stitches and surgical repair of tendon. Root causes: not wearing required gloves; standing on slippery wet surface; and use of improper wire guard on box.
Loading scrap metal into bucket of payloader, while throwing a piece into bucket, he caught finger between payloader bucket and metal.

While welding on a chute, the chute fell from the welding bench. It fell on his left little finger causing a laceration/partial amputation to tip of little finger.
Employee had been cutting 2x6's in band saw. After he had cut 70, he turned and his glove was caught by band saw blade pulling his finger into the saw blade. Injury required 7 stitches. Employee did not lose time from work.
Employee was moving 5 gallon buckets of caulk when the handle from one of them released causing it to fall on his foot.
WHILE PUTTING ON FACIA. HE WAS DRILLING PILOT HOLES WITH CORDLESS DRILL WHEN A PIECE OF METAL WENT INTO HIS EYE
Employee was driving haul truck when he felt something enter his right eye. He received first aid treatment at plant and was taken to local hospital. A piece of metal embedded in eye was found and removed.
EE WAS TAPPING A HOT SLEEVE OFF A SHAFT WITH A HAMMER. WHEN IT CAME OFF, IT LANDED ON HIS LEFT THUMB.
While crawling on top of drill derrick to grease drill, employee slipped on drill deck and twisted left knee. Employee did not seek medical attention at that time. He later sought medical attention and received physical therapy. Employee did not lose any time from work.
Employee working 6 pm to 6 am (2/15/05). Employee closed knife gate and proceeded to open cleanout cap between gates. As he removed the cap, pressure from material flow blew out a stream of dust that struck employee in chest and traveled up to his face, getting behind his safety glasses. Empoyee sought medical treatment and was given a prescription.
INSTALLING WEDGES INSIDE A COOLER AS HE WAS COMING DOWN A 6'LADDER TO GET MORE WEDGES, HE STEPPED OFF & HIS FOOT CAUGHT THE SIDE OF A GRATE & HE TWISTED HIS ANKLE.
WHILE TRYING TO LIFT AN ELECTRIC JACK HAMMER UP ON AN ELEVATED SLAB, HE SLIPPED ON A SNOW COVERED INSULATING BLANKET. HE FELL FORWARD & HIT HIS CHEST ON A PIECE OF REBAR STICKING OUT OF THE CONCRETE. THE REBAR HAD BEEN EXPOSED WHILE REMOVING THE CURB. THIS CAUSED A PUNCTURE WOUND.

LIFTING IRON & PULLED MUSCLE (GROIN). EE FILLED OUT AN ACCIDENT REPORT DATED 11/3/04 THAT STATED THAT HE PULLED A GROIN MUSCLE AND THAT HE NEEDED HERNIA SURGERY. HE WORKED UP UNTIL 12/17/04 & HAD SURGERY ON 12/20/04. TENTATIVE TIME OFF FOR RECOVERY WILL BE 14 DAYS. WE ARE DISPUTING THIS CLAIM WHICH IS WHY THE FORM DID NOT GET FILLED OUT IN A TIMELY MANNER.
EE was grinding welds with electric grinder, windy conditions caused particles to go under shield and into right eye.
Employee was using utility knife to cut conveyor belting that had lip on each edge. While he was cutting the edge using down pressure, the momentum carried the knife blade through the rubber and struck employee's right leg (thigh). Cut on thigh required four stitches.
Employee walking to office from job site (kiln area) stepped on crack in roadway and twisted right ankle. Employee sought medical treatment 3/29. Later referred to orthopaedic doctor 6/29/04
Employee was checking oil level in kiln when he lost his balance and fell against hand railing, hitting his right side and upper torso. Employee was placed on restricted duty.

Employee was adjusting rolls on coal pulverizer using one-inch impact gun to make adjustments. He was holding impact gun away from his body while performing work when he felt a pull in upper and lower right side of back. Employee placed on restricted duty. This is a pre-existing condition that reoccurs occasionally.
Employee exited east door of #1 feed silo, slipped and fell on left side and fractured his left elbow. He had a previous left elbow fracture on 12/28/03 and was on restricted duty (22 days) until 2/16/04. Employee did not report this incident until 3/23/04. He was again placed on light duty effective 3/26/04.
EE WAS GRINDING ON DOOR OF FINISH GRIND COLLECTOR WHEN FOREIGN MATERIAL ENTERED EE'S EYE. THE EE REPORTED INCIDENT TO HIS SUPERVISOR ON 2-5-04 AND WAS SENT TO THE HOSPITAL FOR EVALU ATION. TWO PIECES OF METAL WERE FOUND IN HIS RIGHT EYE.

WHILE WALKING THROUGH DOORWAY WITH RAISED THRESHOLD, EE TRIPPED & FELL FORWARD ONTO A DOWNWARD RAMP. HE LANDED ON HIS LEFT WRIST & ELBOW. FALL RESULTED IN FRACTURE OF LEFT RADIAL HEAD & FRACTU RE OF LEFT WRIST TRIQUETRUM. EE WAS RELEASED TO RETURN TO LIGHT DUTY WORK.
A PERSON WELDING ABOVE WHERE HE WAS WORKING WELDING ON THE #14 HAMMERMILL. A PIECE OF SLAG FELL GOING DOWN THE BACK OF HIS SHIRT CAUSING A 2ND DEGREE BURN ON HIS LOWER BACK.
SKID STEER OPERATING AT HIGH RPM CAUSED A QUICK, JERKY BUCKET MOTION. PERSONNEL WERE TOO CLOSE TO OPERATING EQUIPMENT. LACK OF COMPLETE COMMUNICATION BETWEEN OPERATOR AND OTHER PEOPLE. IMPROPE R EQUIPMENT CHOICE - FORK LIFT WAS BETTER CHOICE AND STANDING BY. FOOT FRACTURE MAY REQUIRE SURGERY TO PIN BONE. KNEE WILL BE FURTHER EVALUATED IN NEAR FUTURE TO DETERMINE MEDICAL PLAN.
EMPLOYEE WAS ENTERING FINISH GRIND RECLAIM AREA WHEN HE SLIPPED ON LOOSE CLINKER AND TWISTED HIS LEFT KNEE WHILE FALLING TO THE GROUND. EMPLOYEE SOUGHT MEDICAL TREATMENT THE NEXT DAY & WAS GIV EN A KNEE IMMOBILIZER & PLACED ON LIGHT DUTY FOR THREE WEEKS.
EMPLOYEE WAS CHANGING VESSEL COVERS ON TOP OF AIRSLIDE. HE STEPPED OFF GRATING APPROX 18"" ABOVE AIRSLIDE ONTO THE AIRSLIDE (UNEVEN SURFACE) AND ROLLED HIS FOOT. EE WAS GIVEN AN AIRCAST TO WEAR INSIDE HIS WORKBOOT AND RETURNED TO WORK.
EMPLOYEE STEPPED DOWN OFF SCRAPER ONTO FROZEN, UNEVEN GROUND AND TWISTED HIS ANKLE. EMPLOYEE WAS PLACED ON RESTRICTED DUTY FOR EIGHT DAYS.

INJURED WAS WALKING ON WEST SIDE OF RAW GRIND WHEN HE SLIPPED ON COMPACTED SNOW AND FELL RESULTING IN SORE HIP AND LOWER BACK. EMPLOYEE HAS HISTORY OF BACK PROBLEMS. HE RECEIVED WORK RESTRICTI ONS DUE TO THIS INCIDENT.
EE WAS SLUGGING SHELL HARDWARE AND HIT HAND WITH A HAMMER. EE WAS UNABLE TO SUE IMPACT WRENCH BECAUSE THE AIRLINES WERE FROZEN.

INJURED WAS USING STATIONARY GRINDER TO GRIND OFF EDGE ON GUSSET WHEN METAL WAS PULLED INTO GRINDING WHEEL CAUSING INJURED'S RIGHT THUMB TO BE CAUGHT BETWEEN METAL AND GRINDING WHEEL. TOOL REST WAS ADJUSTED PROPERLY AND DID NOT MOVE.
