Fall from machine

Employee was exiting the haul truck at the beginning of the shift. As employee was descending the lower ladder portion of the haul truck they misjudged the last rung of the ladder and fell backwards to the ground. This resulted in a slight strain/sprain to the employee's lower back area.

Over-exertion in wielding or throwing objects

The employee was shoveling stemming into a blast hole when they strained their lower back. This resulted in a minor muscle strain that was treated with rest and anti inflammatories.

EE was maintaining water lines and cut upper left thigh. EE was treated and returned to work without lost time.

Employee was climbing up a ladder to check on the condition of the pony belt. When he exited the area he was stung by a wasp/bee. The employee suffered an allergic reaction from the sting.

EE WAS CUTTING A BED LINER OUT OF A HAUL TRUCK. HE WAS IN A SITTING POSITION, AND WHEN HE MOVED HE FELT PAIN IN HIS LEFT KNEE. HE WENT TO A DOCTOR ABOUT THE PAIN ON 8/8/03. WE KNEW NOTHING ABO UT HIS INJURY UNTIL THAT DAY.
