EE's working to clean the mainframe arm on primary crusher. During the lift, the rigging assembly appears to have failed, striking the EE who was located in the line of fire.

Fall from machine

Multiple employees were conducting maintenance on the Sandvik Crusher at Tower 1. Two employees were working on the platform approximately 3 feet above the ground surrounding the crusher, one employee lost footing due to loose material and fell. Plant management and safety were immediately notified, Core Health was contacted, and the employee went to a local clinic for monitoring.

Injured person was setting tamp-in steel anchors in holes drilled into cement. Hammer deflected off of setting tool and struck the individual's thumb. The blunt force trauma caused a laceration requiring stitches.

Employee was working under the 7 ft crusher using a porta power jack, this became dislodged and struck EE in the lip causing 6 stitches to be administered at the hospital. It was found that the foot of the jack failed.

EE was struck by rock on left side of face while standing on catwalk

Over-exertion in pulling or pushing objects

EE was removing the chock blocks from the haul truck. As EE pulled out the first block from under the tire EE felt a pain in neck and shoulder. EE was taken for medical treatment. The ground was frosty but the chock was not frozen in place. The chock is a 27 pound weight no defects. EE is on modified duties until the doctor releases EE back to full duties.

AT THE TIME OF THIS REPORT WE BELIEVE A LARGE VOLUME OF ROCK HAS BURST FROM ABOVE THE PERIMETER HIGH WALL. THE BURST MATERIAL & RESULTANT SLIDE EXTENDS INTO THE TREE LINE ABOVE THE QUARRY RIM. NO INJURIES HAVE BEEN EXPERIENCED AND NO EQUIPMENT DAMAGED AS A RESULT OF THIS INCIDENT. FURTHER INFORMATION WILL BE AVAILABLE ON INSPECTION AT 12-2-2005

EMPLOYEE WAS ON A SIT DOWN CREEPER WORKING ON AN ENGINE, HE LOST HIS BALANCE. AS HE WAS FALLING HE HE GRABBED THE BODY OF THE TRUCK WITH HIS LEFT HAND. HE SUSTAINED AN INJURY TO HIS LEFT PINKY FINGER.

EE STARTED HIS HISFT AT 1:30 PM. HE PROCEEDED UP TWO HY-PASS WAS HEADING TOWARDS THE QUARRY. TRAVELING UP A QUARTER OF THE WAD THE 50 TON F TRUCK CAT 31-071 SUDDENLY SWORED THE BEAM PUSHING T HE HEAR DOWN THE SLOPE. THE CAB OF THE TRUCK LANDED APPROX. 20 FT OFF TWO HERE.
