Struck against stationary object

Employee banged EE's left shoulder on a trunk duck at the three-way in the plant. EE then tried to pick up a hose that was laying on the floor and EE heard a pop and felt a tingling sensation and pain going down EE's right arm.

Employee was drilling a 5/16"" hole in a 10"" I-beam with a 1/2"" drill. The drill bit caught twisting EE's left thumb.

Employee was going to turn on the clean coal filter drain pump. When they walked around 74 tail piece, the employee slipped on ice on the plant floor causing them to fall. Employee was placed in a knee brace.

Was stepping down out of the back of a truck twisting left ankle on a rock.

EE exited truck to clean the wiper blades. When EE went to get back in the truck, slipped and fell on the snow covered road and struck left elbow.

Employee was driving a steel wedge between 2 16"" steel flanges to spread apart. The steel wedge came out striking employee above left eye.

Accident type, without injuries

A trailer loaded with hay bales was discovered on fire at the end of Red Oak road, approx, 200 yards from Refuse Fill #2 (ID#1211, MD 300-172-01). There were no injuries, no property loss. Local fire department dispatched but assistance was not necessary. This event was immediately reported.

Struck by falling object

Employee went to tram the cable hoist on the 11th floor of the prep plant to bring a load in. While tramming the hoist, the mechanism broke causing the pulley to fall and strike the employee in the safety glasses causing a laceration to the left eye lid requiring 5 stitches.

Employee was in a chute pushing the plastic chute liner back in place with a porta power. When an employee above IE lost EE's grip of the ram and it fell on IE's hard hat causing IE's neck to hurt.

An EE was handing a 5 gallon bucket full of concrete into the screen that they were patching underneath. When EE let go IE caught it twisting IE's left arm/shoulder.

EE was dismounting off of 49-K screen when foot slipped causing a pain in left knee. EE continued to work without restrictions. Due to Covid-19, surgery was postponed until 7-8-2021

Fall down stairs

EE slipped and fell while walking up steps in prep plant and twisted knee. EE continued to work until an MRI could be scheduled. MRI on 5/4/20 revealed a meniscus tear and other damage and was taken off work until 5/11/20. EE returned to work full duty and is currently awaiting a surgery date. However, elective surgeries are not being conducted at this time due to COVID-19.

Employee was setting up to sandblast. EE set the sandblasting nozzle on the floor and when EE stood up EE hit left cheek on a 2"" ball valve causing EE's 2nd molar to break off. EE found out that the inside of the tooth was bad.

Employee felt a pain in lower left back and left leg while attaching a hoist hook to a basket. Injury became a lost workday injury on Sept. 1 2020.

Employee was cutting a piece of 1/2"" thick ceramic tile when the tile became wedged between the tile saw blade and the base.

Employee hurt left wrist/hand when the employee slipped and fell while carrying a sample bag of coal down a flight of steps at the Analyzer Building.

Employee was taking down some scaffolding and when lifting up to separate the two sections they smashed the right pinky between the scaffolding and unit 92 belt structure.

Employee said that EE was picking up a piece of concrete the size of a softball and right shoulder dislocated. EE was able to put it back into place by self.

Driver was hooking toe strap back to D ring on back of Dozer and sprained left wrist.

Employee was drilling a 3/8"" hole in a beam to give the torch a place to start cutting from and a piece of drill shavings went into left eye.

EE was installing floor drain boxes into the ceiling of the 8th floor of the preparation plant. Said EE fell off the scaffolding falling 5' hitting left shoulder.

EE was walking towards the thermal dryer from the shop when another worker hit EE with the 1994 f-250 truck from behind hitting EE on right back and right elbow.

EE was lifting a sieve up from inside the screen to spin it because of wear when EE heard a pop and felt pain in EE's right elbow.

Employee was lifting a 3 ton chain hoist (Roughneck Brand from northern tool + equipment -weighs 48 lbs.) out of the hole we were working when employee felt something pull on the left side of stomach area.

Employee was doing preparation plant maintenance repair and utilizing a hammer while extracting a retainer pin when EE miss-struck the pin with the hammer, and incurred a puncture wound injury to the top of left hand.

The employee was descending down a metal permanent ladder backwards and slipped on the second rung from the bottom landing on EE's feet twisting EE's right knee in the process.

Employee was taking well water samples using a 1"" bailer. When the bailer exited the well casing it stuck the employee in forehead causing a laceration that required medical treatment, (2) sutures.

Removing bad 4"" steel channel from the side of the thermal dryer to install new siding to be installed. EE was holding the opposite end that was being cut free when it was cut loose it seesawed and smashed EE's right hand thumb up against another piece of metal.

Crawling between the top and bottom belts on V-98 train 10 about conveyor to remove 2 (berthold) ash analyzers that were not in service anymore and twisted his neck wrong in the process of removing them.

Driver was getting out of truck to use bathroom at wide spot top of 10 on private road. As he was exiting truck missed bottom step & fell to ground pull hid groin muscle.

Driver was coming out on private road and when coming our at log landing and dozed off lost control of truck ran off left side of road

Struck against stationary object

He was coming down the steep incline of the catwalk from the twin silos sliding his hand down the hand railing and caught a piece of metal between his pinky and ring finger on his right hand, The metal did not cut him, it spread his knuckles apart between his fingers.

Employee found a wood board stuck in belt chute. The employee jerked on the board in an attempt to remove from the belt chute and felt a pop in his left shoulder. Employee continue to work from date of accident until January 25, 2016 when he went off work for surgery to repair left rotator cuff injury.

Snagged pant leg on a piece of metal plate in the shop and tripped while carrying a piece of 1/2""x8""x48"" flat bar and fell to the floor landing/catching himself on/with his right hand. Per phone call he twisted/sprained his hand.

Driver was inbound loaded with coal came around turn #4 on private haul road. Lost control of trailer & tractor sliding into ditch and over turned truck & trailer.

Was driving truck 2242 coming onto Mettikis scale struck pole with left front steer tire causing steering wheel to spin catching right ring finger in wheel.

Employee was helping install beam when beam slipped he had a hold of and said he felt pain in lower back - Employee said he didn't want to fill out accident report - which was done - employee went to a clinic that evening & said no medical treatment was giving & recommended he have x-ray - on other page.

I am filing this report because employee called me on the 18th of December stating he had to have emergency surgery on the 17th of December stating that he thought he had come down with the flu on the 8th of December and had to take off work until the 11th of Dec & returned back to work still feeling ill & worked 4 days until they found a hernia in his stomach.

Employee pulled muscle in upper right arm while picking up material to move. Employee did not start missing time for this accident until after being examined by a physician for arm pain and it was determined the pain was the result of November 6, 2013 accident. First day missed January 13, 2015

Employee was struck on right cheek with compress air while performing maintenance work. The employee was examined and released by a medical provider at the time of the accident with no reportable treatment. The employee has since received medical treatment (laser surgery) to removed embedded coal particles from his right cheek making this accident reportable.

Employee was sitting metal plates 3/8 x 48 1/2 x 16 inches against pallet of flock when plate fell over hitting him left leg.

Employee was cutting off bad handrail with grinder & cutting wheel when wheel got pinched causing grinder to kick back & wheel hit employee on left arm causing small laceration.

Employee was making repairs to refuse screen and while doing so the tip of his left index finger was pinched between the out of balance weights as they were being positioned on the shaft.

EE was working to clear frozen coal from feeder when large charge of coal broke loose knocked EE off balance while falling in feeder EE caught his ring finger on left hand on something sharp and cut it requiring 9 stitches.

Flash burns (electric)

Immediate notification was made when information was received that an incident occurred at the thermal dryer. There was an arc inside the electrical cabinet which resulted in a fire and injury. At the time this information was received the severity of the injury and the fire were unknown. The fire was immediately extinguished and the injury was not life threatening.

Employee was unloading a 5 gallon can of grease from the back of a pickup truck. When he slid it off the tailgate the can dropped slightly causing a jerking action straining the employees back.

EE was brushing dirt off of a truck frame when strong wind blew dirt up under his safety glasses getting dirt in his right eye. EE went to the hospital and got his eye cleaned out.

Fall from ladders

As employee was investigating a leaking pipe, the step ladder he was using kicked out from beneath him. Ladder was positioned on stable ground - all four legs. Employee fell from the fouth step, glancing off underflow box and onto floor.

EE was approaching steps at the dryer and lost his balance and tumbled down the steps hurting his left hand.

Employee was chipping rust off of steel with hammer when piece flew under safety into left eye. Accident was not reported until 2/24/11.

Employee slipped on ice covered roadway while working outside at the loading facility. Employee continued to work until May 16, 2011 when his doctor took him off awaiting knee surgery.

Person pulled back top of 54 sieve chute to allow sieve to clear when rotated. Person was standing on platform and tripped on chute and fell forward and scraped his shins on the top of chute causing injury. Person received medical treatment on 10/14/10 and was released.

Two persons were removing the drive unit on the unit 79M dust screen. The injured person was using a slate bar to pry unit back when the bar slipped and he mashed his little finger of right hand between slate bar and frame of dust screw body causing injury.

Injured person was using hammer to clean mag screw of old mag and rust build up. While doing this a piece of rust steel got into right eye. Person went to eye doctor to have it removed.

EE was cutting off screws on old sheeting when sheeting started to fall causing the grinder to be knocked into his left forearm causing a laceration. EE needed 6 stitches.

Injured person was changing pump impeller with another person. Injured had pipe wrench on impeller trying to break it loose. The pipe wrench slipped, causing person to fall backwards on his left leg and back. Person finished shift and began to miss work after MRI. Persons first day off was 4/13/10.

Injured person was inspecting center well section that had been removed from thickner and was on the ground. Person slipped on metal and lost footing, he put his hand out to catch himself and cut his left index finger on sharp piece of metal as he fell.

2 men were rotating cross flow sieve body on #80-B cross flow. While rotating the sieve injured person got his little finger on left hand caught between pipe and body of cross flow sieve causing laceration to little finger of left hand.

Person was working on electrical problem on DNR Dozer. He was getting off dozer to go to service truck and when he stepped off push arm he twisted his right ankle and fell on ground.

EE was tightening bolt on screen feed lip with right hand when torque gun slipped out of right hand falling app 2 feet hitting left hand causing a laceration which required three stitches on back of hand.

EE was restocking screen decks on shelves in the warehouse and when he was staighening decks to put on more, he struck his left elbow on end of a deck, causing a cut that need five stitches.

Injured was replacing a pump adn was manually operating. A cable come along and when the pump was lowered into position he stepped down from elevated position approx 20 inches and when he tunred to walk. He felt sharp pain in right knee (twisting motion)

Accident type, without injuries

The top of a power pole owned by Allegheny Power supplying our 138,000 volt feed caught on fire and fell to the ground. This caught some brush on their right of way catch fire. The fire burned across onto our permitted property. Poles were inspected by Allegheny Power and they cound find no problem.

Person was changing seive screen with two other persons. When he lifted the screen over a handrail to get it into the screen deck, he felt a pain in left shoulder.

Injured employee was removing old conduit. One end of conduit was broken off. As conduit was pulled free it struck the back of injured employee's right hand causing a laceration.

Injured person was changing pump barrel for unit 115B pump with two other workers. While moving parts into place to install, person felt pain in lower back and down left leg. Person began to miss work on 9/16/09 for surgery to back.

Employee was installing liners on skirt board of belt when wind blew dirt or metal into left eye. Employee didn't think much of it. Eye was irritated so he went to Dr on 3/21/08. Piece of material was found in left eye and removed. Accident was not reported until 3/24/08.

Person was guiding shaft into vertical housing when his finger was caught between the bearing housing and shaft causing injury to middle finger on left hand. The finger had crack in last bone (tip) of middle finger and laceration.

Employee was using chain hoist to lift filter barrel when shackle pulled through beam it was fastened in, hitting employee in head causing small laceration.

Person was using hand held grinder to grind off piece of steel. There was another person grinding also in same area. A particle from one of the grinders got into injured person's eye under his safety glasses causing injury. Person did not report injury until 9/19/07. Doctor put drops in eye.

Employee was relacing weirs on thickener when he stepped on pontoon boat. As it started to tip he jumped back on weir already in place hitting his legs on sharp edge of new weir causing small laceration to lower left leg & skinning at lower leg.

EE was running Micro Traxx, cleaning out ash in combustion chamber of dryer, when machine broke the ash loose causing machine to kick sideways. This caused EE to hit middle of his back on the wall of the dryer chamber.

Injured was flagging traffic when he was struck by a pickup truck. The pickup was passing loaded ash truck. Heavy snow and high wind conditions.

Person was working on changing a hydraulic valve boom. While tightening a fitting, the ladder he was standing on shifted and he grabbed onto a pipe pulling his right shoulder. Person now has to have surgery on rotator cuff and began to miss time on 5/2/07.

Person was walking down a bank that was slick and lost his footing. He fell and landed on his right arm causing injury. Person has gone to chiropractor.

Person was working on 3rd floor of plant on vibrator for Wedag changing ""V"" belts. Person tried to go to other vibrator and ran head into valve for waterline causing him to jam neck. Person went to chiropractor.

Person was cleaning inside off at chute to do some welding. While cleaning, some dirt and rust got into his right eye. Person went to ER after work and they removed a small piece of metal.

EE was walking up grade from propane tank to company truck. As he was walking up hill he felt a sharp pain in right ankle and leg. EE went to chiropractor on 5/15/06. *Person had to have surgery on right ankle and began to miss time on 2/14/07.*

EE HAD DESCENDED STEPS FROM #25 BELT TAIL AREA & WHEN HE REACHED GROUND LEVEL & STARTED WALKING TOWARDS LOAD-OUT BLDG., HE SLIPPED ON ICEY AREA & FELL. EE'S RT. ANKLE TURNED AND HE FELL ON HIS ANKLE CAUSING SPRAIN AND TWISTING.

TWO EMPLOYEES WERE CHANGING OUT A SIEVE ON THE 3RD FLOOR OF THE PREPARATION PLANT. WHEN THE TWO MEN WENT TO LIFT THE SIEVE BACK INTO PLACE, THE INJURED PERSON FELT A SHARP PAIN IN HIS ABDOMINA L AREA. HE IS NOW OFF WORK FOR HERNIA REPAIR AS OF 1/28/04.

PERSON WAS CLIMBING INTO SKIDSTEER TO OPERATE IT TO CLEAN UP SPILLAGE AT BELT TRAIL. HIS FOOT SLIPPED AND HIS RIGHT LEG WENT BETWEEN BUCKET AND FRAME OF SKIDSTEER. PERSON FELL BACK INTO BUCKET WITH HIS RIGHT LEG PINNED BETWEEN BUCKET AND FRAME CAUSING HIM TO TWIST RIGHT KNEE.

HIS LEFT HAND REACHED OUT TO TRUCK BED AS OTHER PERSON WAS CLOSING THE TAILGATE. INJURED PERSONS HAND WAS CAUGHT BETWEEN TAILGATE AND BED OF TRUCK CAUSING INJURY TO LEFT HAND. REQUIRED 2 SUTURES.
