An employee was cutting a hole in a urethane cover with a blade to access the fill plug when the blade slipped and cut into their hand, causing a laceration between the thumb and index finger.

Employee was attempting to remove a screen from the lower deck, when the screen became stuck. The employee chipped out some frozen material and EE attempted to remove the screen again. As the screen came loose, the employee jolted backwards and hit EE's head on a crossmember in the screen. The employee decided to seek out medical attention on 12/17/25.

On 4/28/25, an employee felt a pop in EE's upper back while using a sledgehammer in the gyratory crusher. Did not seek medical treatment until 4/30/2025.

Employee was descending from being on the tracks on CAT Dozer, when employee stepped down on the dozer arm employee's foot slipped forward causing employee's leg to go down between the track frame and dozer arm causing the employee to fall backwards to the ground with employee's right leg stuck causing a fracture.

EE was changing a bulb on the R-85 haul truck. There is a rubber grommet around the lens which needed to be removed. EE pulled out EE's pocket knife to cut the rubber. While EE was putting it away EE's pinky was caught in the pivot point of the serrated portion of the blade. This resulted in a cut on the outside of the left hand pinky from the nail around the finger pad.

EE stepped back into a bad drill hole while moving the bulk truck hose and cut left lower leg.

Employee was in the process of working with a crane operator to remove a motor and motor base from the screening tower using a nylon lifting strap. When the lift began, the employee noticed the boom angle was too high and signaled the operator to boom down. The operator boomed up abruptly and snapped the nylon strap. The load dropped down and shifted, striking employee.

Employee replacing a leaking o-ring on 5CR Tramping Cylinder bleed valve. EE proceeded to remove the bolt without relieving the pressure. The bolt came loose, it shot out at an extreme velocity throwing the employee's left hand into a nearby conduit. Did not seek medical treatment until 10/22/2021.

Employee was attempting to remove a partially cut shank from a bucket via sledge hammer, a piece of the hammer chipped off lodging into employee's neck/shoulder area

EE developed left shoulder discomfort during shift. EE claimed the discomfort came from cooperating the dump lever on a Cat 773D haul truck.

The employee was throwing a line at the upper dock from the barge to a dolphin. As the employee was throwing the line, the line slipped through employee's hands. The employee's momentum followed the line and employee ended up falling/jumping onto the tires that act as bumpers on the dolphin.

Injured and co-worker were positioning a skirt board for installation when it slipped, causing injured's left thumb to be bent backward. Injured finished the shift and received treatment after EE's shift.

The employee was kneeling down on right knee while using a wash down hose underneath of S8. While using the hose, a tear developed at the connection point and began to spray the employee in the neck/chest area. When this occurred, the employee flinched and lost balance rolling back slightly causing a twisting motion in right knee.

Absorption of radiations, caustics, toxic and noxious substances

Employee was walking from the building and stopped and bent down to tie boot, at this time a raccoon came out of the bushes and struck the employee in left heel. The employee then kicked and struck the raccoon with left hand. This event resulted in punctured skin on the heel and thumb. Being treated for possible rabies exposure.

Employee tripped over a rope and fell in the water. EE did not report an injury until 12/14/2016. Workers Comp. carrier denied injury on 1/12/2017. After investigation it was approved on 1/27/2017. Vulcan was notified by letter on 5/23/2017.

On July 7, 2016 employee was performing welding tasks from approximately 8:30 am until 11:30 am. At 11:30 am the employee reported that ee was feeling bad and experienced dizziness when ee stood up. Employee was immediately taken for medical treatment and was treated for dehydration symptoms.

Employee was cutting out old chute work and hot slag went into boot. The hot slag burnt employee's right ankle. *The accident happened on 06-07-16, however it was not reported until the end of the shift on 06-09-16.

EE was driving new OEM wedge into the manufactured wedge slot of crusher to separate the top shell from the bottom shell with a #8 sledge hammer. A small chip of the wedge sheared off and lodged into EEs lower left thigh near knee cap.

Employee was in the back of the service truck accessing his cab mounted tool box. As he stepped out of the bed, he missed the work platform/step and fell directly to the ground dislocating right pinky finger.

The Employee was assisting another employee with welding a plate over a hole on the Coastal 216 Barge upper deck. The EE was wearing safety glasses and was not the employee doing the welding. The employee suffered from Superficial Injury of the Cornea (welders flash) per the doctor.

Climbing onto top of bulk truck, he tripped on a hose, fell onto nitrate bin cover, broke/dislocated left elbow.

Employee was closing the gate at the end of his shift when the gate came off its hinges and fell onto the employee's little toe.

While carrying two sample buckets down from 2 tower, about 3rd step down from top of bottom landing, employee's right knee had slight pop and sharp pain. He stopped for a brief second but someone else was coming behind him with buckets, so he continued down to the bottom.

EE was operating his Komatsu truck and did not notice that someone else has inflated his lumbar support. After driving his truck 6.5 hours, he noticed his back was bothering him a day before.

Employee was climbing back into sweeper truck on 4-18-06. He felt a pain in his left shoulder. He notified his supervisor & did not want to fill out incident report & did not want medical treatment. On 4-24-06 @ 6 AM he went to his supervisor & reported pain. Then requested medical attention.
