Employee slipped while descending stairs fracturing the 5th metatarsal on the right foot.

Employee was changing the configuration of wash screen diverter gates (hinged at the bottom) by hand. While repositioning one gate, the tips of the employee's middle and ring fingers were caught between the top of the gate and the discharge chute, resulting in injury.

Employee was shoveling in a surge tunnel. While attempting to loosen some spilled material, the material broke loose suddenly, causing the employee's right hand to travel further forward than expected, contacting the outside of a conveyor frame.

Employee was walking between the service truck and assigned task area when employee twisted employee's knee. This was aggravated a few hours later while the employee was getting into the rear seat of the service truck.

Employee was moving a rock from a haul road and strained back. Evaluated as first aid only on 2/2/17, and re-evaluated as restricted work duty on 2/7/17.

Employee was standing on the ground, pulling on a ratchet. The ratchet broke and caused the employee to fall backward, twisting his ankle.

While exiting dump truck drivers hand slip and he lost balance. He Fell 3' to 4' to ground landing on back of his head. He received a Subdural Hematoma/Concussion. Taken to ER in Maryland and transferred to DE. No stay was released 11 days later to full duty

Employee was setting up a pump to pump out freshwater pond. While trying to pull the discharge hose out of a ditch, he was using both of his legs for leverage; his right leg was bent under his left leg. He then felt pain in his right knee as he pulled the hose towards him.

A worker was removing bolts to take apart a conveyor. A crane was holding the upper section of the conveyor. When the last bolt was removed the upper piece of the conveyor raised up cutting the worker's right arm.

While removing a bolt with an air impact gun the head of the bolt spun cutting through his glove and into his finger.

Employee Claims: While stepping off of a ladder he stepped on uneven ground and rolled his ankle over.

While removing belt guard, housing came loose, employee lost balance and fell to ground landing on right foot.

Employee Claims: While loosening a hydraulic line on a haul truck, the line broke free and the wrench struck him in the nose.

Employee Claims: While shoveling loose material from under a conveyor, he felt pain in his back.

Employee Claims: While he was splicing a conveyor belt he stepped across the conveyor and fell to the ground bruising his right heel and knee. The employee was not wearing the required fall protection.

Employee Claims: His foot slipped off of a ladder and he fell to the ground, landing on his feet. He felt pain in his back.

Employee Claims: While pulling angled steel off of a forklift and sliding it onto a storage rack, he felt a sharp pain in his lower left back.

While assisting contract welders reassemble a crusher, the wind blew material in his eye.

Employee Claims: While operating a front-end loader and digging from a muck pile, a rock was thrown from the ""Surestrike"" rock breaker through the right side window of the loader. The rock struck the employee on the right side of the face.

Employee Claims: While trying to clean rubber out of a belt drill bit, he swung his hand down onto a nail.

Employe Claims: while dismounting a 416 Cat Backhoe, his left foot slipped on the ground twisting his foot.

Employee reported on 3/23/06 that he pulled a muscle in his left rib cage area while lifting a tire for a skid steer loader. He refused any treatment. On 4/19/2006 employee claimed that the muscle strain was not getting any better and requested medical treatment.

Employee Claims: While cleaning off build-up in a chute to take measurements, he cut his left middle finger on a loose, torn piece of steel.

Employee Claims: While Cleaning a urinal, toilet bowl cleaner splashed in her left eye.

Employee Claims: ""From taking bumps in the loader. Sometimes the bucket of the loader hits the wall and jerks you around pretty hard. I started to feel pain in my neck/back."" We were notified by the employee that he was having health problems on 09/03/04. The employee then notified us on 10/07/04 that the health problems were work related.

Employee was working under a haul truck when a hammer fell off of the frame of the truck and struck him in the head.

Employee claims: He was operaating a R-50 Euclid, hit a pothole, and felt a pain in his back.

While crawling under a water truck to check for an antifreeze leak, the employee turn to look under the frame of the truck and felt a twist in his right knee

EMPLOYEE CLAIMS: WHILE ATTEMPTING TO REMOVE A LARGE ROCK FROM A CATWALK, THE ROCK SHIFTED AND FELL ON HIS LEG AND HAND.

WHILE INSPECTING THE PLANT, HE FELL THROUGH A HINGED OPENING IN A WORK PLATFORM.

EE CLAIMS: WHILE TIGHTENING A BOLT ON R-50 EUCLID, THE WRENCH SLIPPED CAUSING HIM TO FALL FORWARD HITTING HIS JAW ON HOSE CLAMP

WHILE CLEANING ROCK OFF OF A BELT THE ROCK CAME BACK AND HIT HIS LEFT THUMB.

EMPLOYEE CLAIMS HE WAS OPERATING A FRONT END LOADER AND WHILE LOADING STONE FELT PAIN IN HIS LOWER BACK.

WHILE STEPPING DOWN FROM A CRUSHER TO PLATFORM THE CONTRACTOR SLIPPED SIDEWAYS TWISTING HIS BACK AGGRAVATING A PRE-EXISTING MEDICAL CONDITION.

WHILE HE WAS CLEANING OFF A CONVEYOR BELT A ROCK FELL FROM A SCREEN CHUTE AND HIT HIS LEFT HAND.

WHILE CLEARING ROCKS FROM A CRUSHER A ROCK FELL AND HIT HIS RIGHT HAND.

WHILE PUTTING AWAY A DELIVERY OF FLAT STEEL HE FELT A PAIN IN HIS LOWER BACK AND LEG.

EE CLAIMS HE WAS ATTEMPTING TO RE-ATTACH A SAFETY GATE WHEN HIS FINGER WAS PINCHED UNDER THE GATE.

EE WAS IN THE PROCESS OF REMOVING THE TRANSMISSION CONTROL VALVE, HIS HAND SLIPPED ON THE SIDE OF THE MACHINED SURFACE OF THE VALVE. WHICH SLICED HIS HAND.

TWO INDIVIDUALS WERE IN SEPARATE PARTS OF THE ELECTRICAL ROOM AT THE MAIN PLANT, PREPARING TO TRACE OUT A CONTROL CIRCUIT. UPON HEARING A NOISE, ONE WENT & FOUND VICTIM LYING ON THE GROUND, BE GINING TO CONVULSE & TURN BLUE. HE CALLED 911 & BEGAN CPR. HE REPORTED THE INCIDENT AS ELECTROCUTION. MECICAL TESTS INDICATED LOW VOLTAGE SHOCK WHICH CAN ALSO BE CAUSED BY DEFIBULATION.

WALKING AROUND PLANT TO CHECK ON EQUIPMENT, WIND WAS BLOWING AND DUST/PARTICLES ENTERED UNDER HIS SAFETY GLASSES INTO HIS LEFT EYE CAUSING IRRITATION AND SWELLING.

WHILE TRYING TO DISLODGE A ROCK FROM BETWEEN THERAILS OF A CONVEYOR, THE ROCK FELL STRIKING HIM IN THE LEFT ANKLE.

THE AIR HOSE CAME OFF & STRUCK EE IN RIGHT GROINAREA CAUSING A WIP LACERATION TYPE INJURY. HE WENT TO E.R. NEEDING STITCHES.

EMPLOYEE CLAIMS WHILE REFUELING L-6 LOADER, HE LOST HIS BALANCE AND FELL BACKWARDS LANDING ON THE FUEL PUMP.

EMPLOYEE WAS SHOVELING STONE OFF OF A BELT. HIS LEFT FOOT SLIPPED & HE FELT EXTREME PAIN IN THE UPPER PART OF HIS RIGHT LEG.

EMPLOYEE CLAIMS: WHILE CHANGING BELLCRANK ON LOADER, HE STARTED THE PIN IN THE HOLE AND WHILE TRYING TO LINE THE BELLCRANK UP THE PIN FEEL OUT OF THE PIN OF THE HOLE AND HE ATTEMPTED TO CATCH IT, INJURING HIS RIGHT SHOULDER.

EE CLAIMS: HE WAS PULLING A PRY BAR TOWARD HIMSELF WHEN THE PRY BAR SLIPPED. HE FELL BACKWARD HITTING HIS HEAD & SHOULDER

WHILE WALKING AROUND NUMBER 10 AND NUMBER 11 BELT AT PLANT 2, HE FELT A PAIN IN HIW LOWER BACK.
