Fall down stairs

Employee slipped while descending stairs fracturing the 5th metatarsal on the right foot.

Caught in, under or between a moving and a stationary object

Employee was changing the configuration of wash screen diverter gates (hinged at the bottom) by hand. While repositioning one gate, the tips of the employee's middle and ring fingers were caught between the top of the gate and the discharge chute, resulting in injury.

Struck against stationary object

Employee was shoveling in a surge tunnel. While attempting to loosen some spilled material, the material broke loose suddenly, causing the employee's right hand to travel further forward than expected, contacting the outside of a conveyor frame.

Employee was walking between the service truck and assigned task area when employee twisted employee's knee. This was aggravated a few hours later while the employee was getting into the rear seat of the service truck.

Employee was moving a rock from a haul road and strained back. Evaluated as first aid only on 2/2/17, and re-evaluated as restricted work duty on 2/7/17.

Fall to the walkway or working surface

Employee was standing on the ground, pulling on a ratchet. The ratchet broke and caused the employee to fall backward, twisting his ankle.

Fall from machine

While exiting dump truck drivers hand slip and he lost balance. He Fell 3' to 4' to ground landing on back of his head. He received a Subdural Hematoma/Concussion. Taken to ER in Maryland and transferred to DE. No stay was released 11 days later to full duty

Employee was setting up a pump to pump out freshwater pond. While trying to pull the discharge hose out of a ditch, he was using both of his legs for leverage; his right leg was bent under his left leg. He then felt pain in his right knee as he pulled the hose towards him.

A worker was removing bolts to take apart a conveyor. A crane was holding the upper section of the conveyor. When the last bolt was removed the upper piece of the conveyor raised up cutting the worker's right arm.

Employee Claims: While stepping off of a ladder he stepped on uneven ground and rolled his ankle over.

Employee Claims: While he was splicing a conveyor belt he stepped across the conveyor and fell to the ground bruising his right heel and knee. The employee was not wearing the required fall protection.

Employee Claims: His foot slipped off of a ladder and he fell to the ground, landing on his feet. He felt pain in his back.

Employee Claims: While pulling angled steel off of a forklift and sliding it onto a storage rack, he felt a sharp pain in his lower left back.

Employee Claims: While operating a front-end loader and digging from a muck pile, a rock was thrown from the ""Surestrike"" rock breaker through the right side window of the loader. The rock struck the employee on the right side of the face.

Employee Claims: While trying to clean rubber out of a belt drill bit, he swung his hand down onto a nail.

Employe Claims: while dismounting a 416 Cat Backhoe, his left foot slipped on the ground twisting his foot.

Employee reported on 3/23/06 that he pulled a muscle in his left rib cage area while lifting a tire for a skid steer loader. He refused any treatment. On 4/19/2006 employee claimed that the muscle strain was not getting any better and requested medical treatment.

Employee Claims: ""From taking bumps in the loader. Sometimes the bucket of the loader hits the wall and jerks you around pretty hard. I started to feel pain in my neck/back."" We were notified by the employee that he was having health problems on 09/03/04. The employee then notified us on 10/07/04 that the health problems were work related.

Employee was working under a haul truck when a hammer fell off of the frame of the truck and struck him in the head.

TWO INDIVIDUALS WERE IN SEPARATE PARTS OF THE ELECTRICAL ROOM AT THE MAIN PLANT, PREPARING TO TRACE OUT A CONTROL CIRCUIT. UPON HEARING A NOISE, ONE WENT & FOUND VICTIM LYING ON THE GROUND, BE GINING TO CONVULSE & TURN BLUE. HE CALLED 911 & BEGAN CPR. HE REPORTED THE INCIDENT AS ELECTROCUTION. MECICAL TESTS INDICATED LOW VOLTAGE SHOCK WHICH CAN ALSO BE CAUSED BY DEFIBULATION.
