While using the access panel to enter the top deck screen of # 5, the metal door slammed shut onto EE's left thumb crushing and ripping the top of the skin and bleeding inside gloves.

Employee was working on a conveyor installing a belt saddle. As employee walked from the South side to the North side of the conveyor to deliver tools, the employee slipped in mud while crossing under the conveyor.
The employee was in the process of removing a screen bearing housing from a shaft. While attempting to get a chain mule hooked up, the housing slipped off the shaft. The bearing housing was secured and hanging by a chain fall. The bearing housing slipped off and swung to the left and caught the employees left hand between the bearing housing and the timing gear shaft.
Employee was washing along a conveyor when employee lost water pressure. Hose had become disconnected at barb. While tightening up hose clamp with screwdriver, tool slipped and impacted left hand causing injury.

Employee was operating a 70 ton haul truck in the pit. The FEL dumped the first bucket of the load into the truck; the load was full of shot rock & a 3-5K boulder was mixed in the load. The boulder caused the truck to rock resulting in the employee in the truck complaining of pain in their back & head.
EE was preparing to press a new bearing into the housing for a screen. While centering the housing, EE's fingers were pinched between the housing and the frame of the press.
Employee indicates that as EE came around a vehicle in the workshop EE tripped over EE's own foot, causing EE to fall to the floor. The employee extended EE's arms before the fall and landed on hands. The next day the employee reported sore wrists and was sent for a medical assessment - returned with report of strain.

EE was in the process of descending the ladder of an off-road haul truck during pretrip inspection and slipped on step (fall to below). EE sustained fracture to left leg (unspecified). EE stated buildup of mud on his boots caused him to lose traction with step surface.

EE was removing used 36"" trough idlers. EE was sliding the idlers to his side of the truss when it slid off of the truss slipping from his left hand pinching EE's right thumb between the idler frame and truss as he was preparing to hold the other end with his right hand. The end of his thumb was crushed as the idler fell to the ground, causing an open wound.

Injured employee was feeding a measuring rope through a 6"" condiut with the aid of an air compressor and mouse. He was using one hand to secure airline hose and one hand to feed rope. Airline slipped from his grip and struck him in the mouth. Injury resulted in 4 sutures outside and 4 sutures inside of lip.
After coming off a ladder the employee slipped in the mud (it had been raining most of the day) hit his knee on a sharp rock and cut requiring 7 stickers.
While climbing up a ladder to fuel machine, employees hand slippped off the rail causing him to fall to the ground. Employee suffered bruises to back and leg and is on restricted duty.

Employee was welding on a rock bin. Had his safety glasses hanging around his neck. Went to lift up his welding shield and a piece of slag popped and entered his left eye.

Initial reported as First Aid only. On 4/2/2008 went to a Restricted Duty case and became reportable. Had been seen by two previous Doctors and released without treatment. Was installling a new deck frame on #6 screen tried to push deck into place. He felt that he may have strained a muscle in back of left shoulder.
While installing grating onto a catwalk for a new conveyor section. Injured employee had his finger pinched between the grating and angle support member of the catwalk.

Employee was walking on muddy ground. He lost his footing and twisted his knee.
While shoveling debris from Primary Crusher, our employee slipped in mud. He caught himself with his right hand, causing pain and swelling of his thumb.
EE strained his right shoulder while shoveling debris from the Tail Pulley of a conveyor.
Employee states that while working in the Screenhouse he lost his footing on rocks at the top of the stairs between 7 & 8 screens. He twisted his right ankle and impacted the stair rail with his left knee. He says that the ankle is the most painful.
Another EE was parking 31-099. Injured EE was already parked in 31-089 and preparing to dismount. According to the other EE, he did not see 31-089, as it was in a blind spot. 31-099 collided with 31-089 causing injured EE to sustain a blunt trauma to his ribcage.
Exiting a skid-steer, the operator's foot slipped off the foot rest (between the front of the machine and the forks). He twisted his left ankle.

Employee was shoveling waste material at the tailpulley of 6 conveyor, slipped and turned his ankle and was in a lot of pain. Taken to Shadey Grove hosp. for x-rays, Bad sprain .
EE WAS IN PUGMILL CALCIUM BLDG., REACHED DOWN TO PICK UP A BAG OF CALCIUM, HIS RT. FOOT SLIPPED & HE FELL FORWARD. THIS CAUSED PAIN IN HIS LOWER BACK.

EMPLOYEE WAS GRINDING ON A VERTICAL SURFACE. THE GRINDING OPERATION WAS COMPLETE AND WHILE LOWERING GRINDER TO FINISH POSITION GRINDING WHEEL MADE CONTACT TO EMPLOYEE'S LEFT INDEX FINGER.
