DRILLER WAS IN DRILL CAB PREPARING TO DRILL AN 18' DEEP ANGLE HOLE. DRILL HAD BEEN LEVELLED AND ALL JACKS HAD BEEN EXTENDED. DERRICK WAS BEING POSITIONED WHEN DRILL OVERTURNED ONTO RIGHT SIDE, TRAPPING DRILLER IN CAB.

EE was injured while washing out a dust bin when material buildup sloughed off and fell onto EE's left arm pushing it downward onto the steel opening causing a laceration to the bottom of the wrist.

Employee was using a pipe wrench to remove a drain plug. When plug became loose, employee's left pinky finger was pinched between the wrench and a drain valve to the left of the plug.
Employee injured EE's right hand while operating the CR-6 swing diverter gate at the Finishing Plant. Contact with adjacent handrail resulted in a contusion on the back of the right hand.

Employee was replacing the bucket teeth on an excavator. While using a hammer to attempt to remove a tooth, a piece of it dislodged punctured his left thigh, leaving a small metal splinter. The employee was prescribed antibiotics after an assessment at an Urgent Care facility. No time was lost from work.

EE was standing outside of the mobile maintenance shop when wind caused dust/debris to enter his left eye. The eye was flushed on site, and examined after at an urgent care center. The dust/debris resulted in a corneal abrasion. The EE was wearing all of his required PPE at the time, including safety glasses.
EE was operating the primary crusher and using the rock hammer. He leaned over to look out the window at the rock as he used the hammer. He said his back was sore as a result.
Stepped out of the truck, put foot in a hole, lost balance and hurt lower back.

EE was cleaning out the back of the pickup at behind the shop. Using his gloved hand he started to brush some trash and debris into a pile when he felt a stabbing pain in his right ring finger. X-rays revealed a sliver of glass that had to be removed by a hand specialist. Employee returned to work on restricted duty.

Employee was removing frozen material from around the base of the 4 1/4 cone crusher at the Brown's Plant. He was using a Hilti Gun to hammer the material loose. When he applied pressure, he felt a sharp pain in his back.
Employee was carrying a ladder when he raised it over the hand rails to turn a corner and felt a pop in his back.
Employee was checking VS3 for a hole in the top deck. He stepped up onto an elevated motor mount to view the top deck screens. As he was preparing to step back onto the floor, his right foot slipped off of the mount, and landed on the floor, causing all of his weight to shift onto his right foot, twisting it.
EMPLOYEE WAS CLIMBING DOWN THE LADDER OF THE CATERPILLAR 777 HAUL TRUCK TO CHECK THE OIL WHEN HIS HAND SLIPPED OFF OF THE HAND RAIL AND HE FELL 10 FEET TO THE GROUND.
Employee was lifting pads for the crane when the pad slipped out of his hand and he tried to catch it, a piece of wood lodged in his left ring finger leaving a splinter of wood. He was sent to the clinic, who in turn sent him to a hand specialist to have it removed. He received 4 stitches.
EE was operating his loader at the Asphalt #6 pile, cage mill when he struck the concrete base of the loadout tunnel with his bucket. The sudden jar must have caused a back injury because on March 2nd, he started to feel pain in his back and pain/numbness in his right arm. This report was given to us on MARCH 7, 2005.

Employee was rolling up a rope to put away into the truck, he wiped his eye with his hand and apparently there was a fiber from the rope on his hand and it got into his eye. This resulted in a corneal abrasion.
Employee claims that after working 3/4 of the day, his shoulder was hurting. He had a previous shoulder injury and we believe that the work day aggravated it. He was sent to the company physician and was put onto restrictions. This accident was not reported to me until 9/25/04.
Employee claims that he was operating his loader and ran across a drainage ditch causing the loader to bounce and he felt pain in his lower back.

EE WAS LYING ON HIS STOMACH REMOVING THE 3RD SCREEN PANEL ON THE NORTH SIDE OF THE V52B SCREEN. HE WAS PULLING THE SCREEN.
EMPLOYEE WAS CLIMBING OUT OF PRIMARY CRUSHER AND WAS STEPPING ON WELDER PARTS. PARTS SHIFTED AND HE TWISTED HIS RIGHT GROIN/LEG.
REPORTED THAT THE 980C LOADER SHIFTER WAS HARD TO USE AND IT CAUSED HIM PAIN IN HIS LEFT SHOULDER.
EE WAS SHOVELING MATERIAL FROM UNDER GRIT CONVEYOR WHEN HE TRIED TO PRY SOME FROZEN MATERIAL BY PRESSING DOWN ON THE SHOVEL HANDLE. HE FELT PAIN IN HIS LOWER BACK.
