Employee was cutting on piece of metal when foreign body entered into eye.

Employee was lifting a plate on crusher with coworkers when it became stuck. The plate then fell onto employee's finger crushing it. Employee received a fractured finger.

Employee states he was welding a bar on the primary crusher when a piece of metal went into his right eye. Employee was wearing a welding shield but was not wearing eye protection.

Employee was removing pieces of glass from a broken window. The back of his left hand hit a piece of this glass and was cut.
